1.Comparison of the effect of dynamic hip screw and anatomical locking plate fixation for the treatment of femoral intertrochanteric fractures

Objective To compare the effect of dynamic hip screw(DHS) and anatomical locking plate for treatment of femoral intertrochanteric fractures.Methods 100 patients with intertrochanteric fractures were divided into observation group and the control group.The observation group was treated with DHS fixation,the control group was treated with anatomic locking plate.The therapeutic effect,incision length,blood loss,postoperative drainage,operative time,hospitalization time and complication rate were compared between two groups.Results The excellent,good,fair,poor,and the excellent and good rate in observation group were 35 cases,9 cases,5 cases,1 case and 86%,those in the control group were 37 cases,10 cases,3 cases,0 case and 94%.There was no significant difference in the excellent and good rate between two groups (x2 =1.77,P > 0.05).Operation incision length,bleeding volume,postoperative drainage volume and operation time values of the observation group were better than those in the control group,the differences had statistical significance(t =4.44,10.91,9.52,9.73,all P < 0.05).The hospitalization time and incidence rate of postoperative complications between two groups had no statistically significant differences (P >0.05).Conclusion In the treatment of intertrochanteric fractures,DHS fixation and anatomic locking plate fixation have good effects,anatomic locking plate fixation method has the advantages of small incision,less bleeding and shorter operative time.
2.Progress of chimeric antigen receptor T cells in the treatment of hematologic malignancies:reports from the 57th American Society of Hematology annual meeting

Chimeric antigen receptor T cells (CAR-T), one of the most promising cancer immunotherapy, has attracted much attention in the 57th American Society of Hematology (ASH) annual meeting. CAR-T therapy has obtained significant effect on leukemia and lymphoma and the latest research results are also inspiring in the 57th ASH annual meeting. It is an important task that how to combine CAR-T therapy with the traditional methods of treatment and the immune checkpoint blocking antibodies and small-molecule-targeted drugs to achieve the best effect. This paper will review the progress of CAR-T in the treatment of hematologic malignancies.
3.Effects of Mifepristone on the Expression of Inflammatory Factors in Endometriosis Model Rats

OBJECTIVE:To study the effects of mifepristone on the expression of inflammatory factors in endometriosis (Ems)model rats. METHODS:SD rats were randomly divided into normal group(normal saline),model group(normal saline), mifepristone low-dose,medium-dose and high-dose groups [0.65,1.30,2.60 mg/(kg·d)],with 10 rats in each group. Except for normal group,Ems model was induced in other groups,and they were given relevant medicine intragastrically for consecutive 4 weeks. The volume of the ectopic focus was compared before and after treatment. The serum contents of TNF-α,IL-6 and IL-8 were detected by ELISA method. The phosphorylation of NF-κB p65 and the expression of COX-2 and PGE2 were detected by West-ern blot. The expression of NF-κB p65 mRNA in ectopic focus was detected by RT-PCR. RESULTS:Compared with normal group,the volume of the ectopic focus,the serum contents of TNF-α,IL-6 and IL-8,the expression of COX-2 and PGE2 protein, the phosphorylation of NF-κB p65 and the expression of NF-κB p65 mRNA in ectopic focus were increased in model group(P<0.01). Compared with model group,the volume of the ectopic focus,the serum contents of TNF-α,IL-6 and IL-8,the expression of COX-2 and PGE2 protein,the phosphorylation of NF-κB p65 and the expression of NF-κB p65 mRNA in ectopic focus were de-creased in mifepristone groups(P<0.01),in dose-dependent manner. CONCLUSIONS:Mifepristone can reduce the volume of the Ems ectopic focus,via blocking NF-κB signaling pathway and reducing the expression of inflammatory factors.
4.Progress of targeted therapy in diffuse large B-cell lymphoma: reports in the 55th ASH annual meeting

With the further study on heterogeneity and molecular mechanism of diffuse large B-cell lymphoma (DLBCL),many novel targeted drugs have been developed,which also showed clinical benefit in early-stage studies.Therapeutic targets for DLBCL mainly include the surface antigens of B lymphocyte,B cell receptor signaling and the cellular microenvironment.Several new clinical trials about targeted therapy for DLBCL had been reported in the 55th ASH annual meeting,and the results were encouraging.The advances in this field will be summarized in this paper based on the new reports in the 55th ASH annual meeting.

6.Effects of Baobaole oral liquid on neuron excitability of feeding center in anorectic rats

OBJECTIVE: To investigate the effects of Baobaole oral liquid on neuronal excitability in lateral hypothalamic area (LHA) and ventromedial hypothalamic nuclear (VMN) in anorectic rats. METHODS: The anorectic rat model was established by feeding with special prepared forage for a week, and then Baobaole oral liquid, a liquid extract of a compound traditional Chinese medicine for activating spleen, was administered once a day for 3 weeks. Finally, extracellular recording from LHA and VMN neurons in rats were made in order to characterize their responses to gastric vagal nerve stimulation and intravenous injection of glucose in the normal, untreated, and Baobaole-treated groups. RESULTS: There was no statistical difference in response characteristics of LHA neurons to gastric vagal stimulation among 3 groups. The duration of VMN neuron excitation response to gastric vagal nerve stimulation in the untreated group was significantly longer than that of the normal control group (P<0.01), while the required stimulation intensity was significantly decreased (P<0.01). Moreover, among the neurons responding to the gastric vagal stimulation in the untreated group, the number of glycemia-sensitive neurons decreased in LHA and increased in VMN (P<0.01). The gastric vagal stimulation induced neuron responses in LHA and VMN of the Baobaole-treated group were not significantly changed as compared with the normal control group (P<0.01), and neither were the intravenous injection of glucose induced responses. CONCLUSION: Baobaole oral liquid can modulate the sensitivity of LHA and VMN neurons to the peripheral signal and make the coordination between LHA and VMN neurons in order to improve the appetite of anorectic rats.
7.Preemptive analgesia therapy of dezocine on postoperative agitation in patients undergoing esophagectomy under thoracoscopy

Objective To investigate the preemptive analgesia therapy of dezocine on postoperative agitation in patients undergoing Esophagectomy under thoracoscopy.Methods Sixty cases of ASA Ⅰ,Ⅱ undergoing thoracoscopic resection of esophageal cancer patients were divided into two groups randomly:dezocine and control group,each group with 30 patients.Patients in dezocine group were intravenously injected with dezocine 0.1 mg/kg before intubation,and patients in control group just intubation as usual.The incidence of postoperative agitation in two groups were observed,and visual analog scale(VAS) score at 0 min,15 min,30 min and 45 min after extubation were registered.The time of wake up and extubatio,the change of hemodynamic were also recorded.Results The incidences of postoperative agitation in dezocine preemptive analgesia group were obviously lower than that in the control group (10% (3/30) vs.60% (19/30),P <0.01).The VAS score at 0 min,15 min,30 min and 45 min after extubation in dezocine preemptive analgesia group ((1.3 ± 1.2),(2.4 ± 1.7),(2.7 ± 1.3),(2.8 ± 1.2) points) were significantly lower than those in the control group ((3.4±1.5),(4.7±1.8),(5.0±1.6),(4.8±1.7) points) (F within group =18.6,P<0.05; F between group =20.4,P < 0.05 ; F interaction =20.0,P < 0.05 ; compared different time between two groups,all P < 0.01).Conclusion Preemptive analgesia with dezocine can obviously lessen postoperative agitation in patients undergoing Esophagectomy under thoracoscopy,and dezocine is also effective in postoperative analgesia.
8.Theory study and medical application of real-time quantitative polymerase chain reaction

BACKGROUND: Real-time fluorescence quantitative polymerase chain reaction (PCR) refers to join the fluorescence groups into PCR reacting system, and to real-time monitor entire PCR process using the fluorescence signal accumulation, finally to make the quantitative analysis of the unknown template through the standard curve. OBJECTIVE: To study the theory of real-time fluorescence quantitative PCR and to explore its applications and progress in medicine. METHODS: With "real-time fluorescence quota PCR, theorem, application" in English for the search term, PubMed database was retrieved from January 2000 to December 2008. With "real-time fluorescence quantitative PCR, principle, application" in Chinese for the search term, Wanfang Database from January 2000 to December 2008, Tsinghua Tongfang Chinese database from January 2000 to December 2008 ware was retrieved. Literatures were limited to English and Chinese languages. Cell factor and tumor resistance genes served as the evaluation index. The methodology of research on the real-time fluorescence quantitative PCR technology and medical applied research on real-time fluorescence quantitative PCR technology were included. While repetitive research and Meta analysis were excluded. RESULTS AND CONCLUSION: Because real-time fluorescence quantitative PCR technology has not only realized PCR develops from qualitative to quantitative levels, but also has strong specificity, high sensitivity, good duplication, accurate quantization, high automaticity, and entire blocking response compared with conventional PCR, thus it becomes the important tool in the molecular biology research. Real-time fluorescence quantitative PCR technique has been widely applied, such as mRNA expression, detections of DNA copy number and determination of mononucleotide polymorphism, as well as in the clinical medicine including accurate quantitative examination of mycobacterium tuberculosis, Type B and Type C hepatitis, AIDS virus, gonococcus, and chlamydia trachomatis. Its quantitative scope extremely extends, no need of gradient dilution, the specificity is stronger, overcomes the false positivity. Due to the traditional PCR technology cannot give the accurate quantization, it is greatly limited in the practical application. Therefore, the accurate quantization of the PCR product, particularly the dynamic monitoring of viral etiology, becomes the urgent need.

10.Curative efficacy of cyclophosphamide combined with VAD regimen in treatment of multiple myeloma and its effects on coagulation function

Objective To study curative efficacy of cyclophosphamide combined with VAD regimen in treatment of multiple myeloma and its effects on coagulation function.Methods 78 patients of multiple myeloma who received therapy from July 2013 to July 2016 in our hospital were selected as research objects, and divided into the control group and the observation group , the control group was treated with VAD regimen, while the observation group was treated with cyclophosphamide combined with VAD regimen.Then coagulation factor and anti-coagulation factor activity, adverse reaction, therapeutic effect after treatment between two groups were compared.Results After treatment, decreased blood coagulation factor, Protein C, protein S and antithrombin were increased between two groups , the difference was not statistically significant.Incidence of adverse reactions in observation group was less than the control group[15.38%(6/39)vs 46.15%(18/39)], the difference was statistically significant(P<0.05);The total effective rate of observation group was statistically higher than that in the control group [ 89.74%( 35/39 ) vs 58.97%( 23/39 ) ] , the difference was statistically significant(P<0.05).Conclusion Cyclophosphamide combined with VAD regimen is well for multiple myeloma, which can effectively improve the patient's condition, improve the life quality of patients, and will not cause a greater impact on blood coagulation function.
