1.Measures of quality assurance for the pre-analytical process in clinical laboratories

The pre-analytical process is the first key step in the total testing process of clinical laboratories,which will directly influence the accuracy and timeliness of testing results.Efficient preanalytical quality assurance system consists of scientific personnel structure,regular training and monitoring,bright and comfortable phlebotomy environment,advanced comprehensive phlebotomy facilities,strict biosafety practice in each procedure and harmonious relationship between clinicians/patients and laboratory.Besides,the application and monitoring of sound quality indicators in pre-analytical process will promote the improvement of quality and finally get the aim of assuring patients' safety.

3.The influence of pelvic autonomic nerve preservation with total mesorectal excision on urinary and sexual function in mid and low rectal cancer

Objective:To evaluate the changes of urinary and sexual function after pelvic autonomic nerve preservation with total mesorectal excision operation in patients of rectal cancer . Methods:Fifty-two patients of Dukes B stage received The PANP following TME with follow-up was complete in 38 cases while TME in 35 cases of Dukes B from January 2004 to August 2006. The urinary and sexual dysfunctions of patients in case and control group were analyzed retrospectively by case-control study. Urinary and sexual function were evaluated by means of questionnaires before and after operation.Results:The micturation dysfunction rate was 15.7%(6/38) and 40%(14/35) ,the sexual dysfunction rate was 18.4%(7/38) and 51.4%(18/35)in case and control group respectively,with the difference being significant(P=0.013,P=0.002).Conclusion:The PANP following TME for rectal cancer could reduce sexual and micturition disorder rate and improve the quality of life of the patients after operation.
4.Exploration on Compatibility between Shanyao and Top Grade Medicinals in the Shen Nong Ben Cao Jing

Shanyao (Dioscorea opposita Thunb.) is the one of the conventional herbs in the clinical practice, which is good at nourishing yin, and entering the lung, spleen and kidney meridian. Through analysis on all formulae contain-ing Shanyao in past dynasties, we found out the top 30 kinds of traditional Chinese medicine (TCM) herbs for the highest frequency in combination with Shanyao, among which there were 28 herbs recorded in the Shen Nong Ben Cao Jing (Shen Nong's Classic of the Materia Medica) and 23 herbs were considered as the top grade medicinals. This article was focused on the analysis of the compatibility meaning and rules of Shanyao and supplementing medic-inals in order to provide scientific basis for clinical formulae and daily health cultivation so as to make good use of Shanyao.
5.Prevention of complications following cerebrovascular stent implantation

OBJECTIVE:To explore the reasons,occurrence mechanisms,as well as prevention methods for complications following cerebrovascular stent implantation.METHODS:A computer-based online search was performed by the first author.The databases of Medline and CNKI were searched with year's limitation of 1989-2009.According to the inclusive and exclusive criteria,the documents were screened,collected and evaluated from the following aspects:treating progress of cerebrovascular stent implantation;category and biocompatibility of cerebrovascular stent;as well as occurrence and prevention methods for complication following cerebrovascular stent implantation.RESULTS:A series of complications,such as carotid sinus response,hyper-perfusion syndrome,cerebral anglo spasm,bleeding at puncture locus,thrombosis,restenosis,as well as cerebral ischemia,would occur following cerebrovascular stent implantation.The complications were difficult to eliminate,but should be avoided by selecting correct implanting cases,elevating operator's proficiency,enhancing understanding of complications,obeying perioperative management,as well as correctly and in-time treatment.Polymer stent,coating stent,and drug eluting stent presented superior biocompatibility to metal stent,which could prevent restenosis following implantation.CONCLUSION:Cerebrovascular stent implantation is one of the main treatments for cerebrovascular disease.Polymer stent,coating stent,and drug eluting stent exhibit excellent biocompatibility.The complications associate to stent implantation are preventable and remediable via sufficient preoperatively preparation,exactly operation and perioperative management.

7.Clinical Research on Combined Treatment with Modified Xiao-Yao Powder and Acupuncture for Diarrhea-predominant Irritable Bowel Syndrome

This study was aimed to explore the clinical efficiency of modified Xiao-Y ao (XY) powder and acupuncture in the treatment of diarrhea-predominant irritable bowel syndrome (D-IBS). A total of 90 D-IBS patients were randomly divided into the treatment group and control group, with 45 patients in each group. Patients in the treatment group received acupuncture combined with modified XY powder. Patients in the control group received oral administration of montmorillonite powder, compound glutamine enteric-coated capsules, compound azintamide enteric coated tablet. Four weeks were one treatment course. After two treatment courses, the pretreatment and posttreatment symptom score, the score of Hamilton Depression Rating Scale (HAMD)/(HAMA), and the overall efficacy were compared. The results showed that compared with pretreatment, clinical symptoms were improved in varying degrees after treatment in both groups (P < 0.05 or P < 0.01). HAMD and HAMA were significantly decreased (P < 0.05). The treatment group had obvious advantages compared to the control group (P < 0.05). The overall efficacy of the treatment group was 91.11%, which was significantly higher than the control group of 68.89%(P < 0.05). It was concluded that modified XY powder and acupuncture can quickly relieve gastrointestinal symptoms, improve anxiety and depression. It is an effective treatment method for D-IBS.
8.MRI of Brain Structure and Function in Multiple Sclerosis with Only Spinal Cord Involved

PurposeSpinal cord is one of the most frequently involved sites of multiple sclerosis (MS), which seriously affects the life quality of patients. In this paper, we investigate the application value of voxel-based morphology (VBM) and resting-state magnetic resonance imaging (RS-fMRI) in multiple sclerosis patients with single spinal cord involvement (MS-SSCI).Materials and Methods Three-dimensional T1WI data and RS-fMRI data were acquired from 20 patients with MS-SSCI and 20 normal controls, grey matter volume (GMV), changes of white matter volume (WMV), total intracranial volume (TIV) and local nuclei volume were compared between the two groups using VBM, posterior cingulate cortex (PCC) was regarded as the seed point and the functional connectivity about whole brain was compared between the two groups by using resting-state functional connectivity analysis, the relationships between MS-SSCI structure, function change parameters and expanded disability states scale (EDSS) scores were further explored.Results①Compared with the control group, GMV, WMV, TIV of MS-SSCI group were not significantly reduced, only the volume of some regions (bilateral middle temporal gyrus, left inferior temporal gyrus) showed significant atrophy (P<0.01); MS-SSCI exhibited increased functional connectivity (FC) in the left medial prefrontal cortex, left inferior temporal gyrus, left caudate nucleus and right supplementary motor area (two-sample t test, after AlphaSim correction,P<0.01, voxel size >40).②There was no significant correlation (P>0.05) between MS-SSCI structure change parameters and EDSS; while a significant correlation between EDSS scores and FC was noted in the left inferior temporal gyrus (r=0.633,P<0.05).Conclusion Both structural abnormalities and altered FC with PCC can be detected in MS-SSCI, but only functional parameters are associated with clinical abnormalities, which are more sensitive than microstructural changes.
9.CT and MRI Diagnosis of Ovarian Clear Cell Carcinoma

Purpose To retrospectively analyze and summarize the image features of rare ovarian clear cell carcinoma (OCCC), and to provide basis for clinical diagnosis. Materials and Methods CT and MRI features of 30 cases of OCCC confirmed by pathology were retrospectively analyzed. Sixteen of all the patients underwent pre- and post-contrast CT scan. And 14 patients underwent pre- and post-contrast MRI scans. Results ① CT features: unilateral mass was revealed in 15 cases and bilateral mass was in 1 case. The maximum diameter of the mass ranged from 8 to 23.7 cm, mean (12.86±3.96) cm. One mass was irregular, 4 masses revealed incomplete capsule, and 4 masses had septa in the mass. CT value of cystic part of the mass was 20-30 HU, and which of solid part was 28-53 HU. On post-contrast CT images, the septa and solid component of the mass showed marked enhancement and delay enhancement, while the cystic component showed no enhancement. ② MRI features: Unilateral mass was revealed in 13 cases and bilateral mass was in 1 case. The maximum diameter of the mass ranged from 9.2 to 30.0 cm, mean (14.03±4.72) cm. One mass was irregular, 2 masses revealed incomplete capsule, and 2 masses had septa in the mass. The cystic component showed heterogenous signal intensity on T1WI, and high signal intensity on T2WI. There was no enhancement on post-contrast images. The solid component showed iso-intensity on T1WI, high signal intensity on T2WI, and diffused restricted on diffusion-weighted images. There was markedly enhancement on post-contrast images. ③ Blood supply of the tumor: In 8 cases, the branch of enlarged ipsilateral ovarian artery fed the tumors. In other 16 cases, the masses were surrounded by enlarged ipsilateral ovarian vein. Conclusion The characteristic CT and MRI features of OCCC include: a cystic solid mass with complete capsule; the solid component projects into the cavity, which could be hypervascular and marked enhanced.
10.The Disposal Medical Wastes Sorting in Second-level Hospitals in Zunyi City

OBJECTIVE To understand the status of medical wastes sorting disposal and the use of sharp containers in the second-level hospitals. METHODS The status of medical wastes sorting disposal and the use of sharp containers in some of the second-level hospitals in Zunyi City,Guizhou Province were investigated. RESULTS It was 77.8% where the medical wastes were sorted,packed and signed,It was 55.6% and 22.2%,respectively,where the plastic infusion bottles and the glass seal bottles were sorting and placed as recycle wastes. It was 33.3% where the hospitals used sharp containers according to the requirement of the People's Republic of China(PRC). CONCLUSIONS Partial second-level hospitals lack of enough recognition to the medical wastes management,and do not perform strictly these rules; therefore,all of the hospitals should strengthen the study of medical policies and regulations,the management of medical wastes should be standardized. The hospitals should use the wrapping sack and sharp containers making of regular factories and conforming to the national requirement for decreasing the environmental pollution of medical wastes and the sharps injury in the health care workers,and the management of recycle medical waste should be strengthened.
