Objective To investigate the early changes of CD4 + T cells and the relevant cytokines in blood of patients with severe trauma.Methods Sixty-one consecutive patients with trauma admitted into the 97th Hospital of PLA from September 2009 to November 2011 were enrolled in this study.The exclusion criteria included:① Patients were younger than 18 or older than 75 years.②Patients received blood transfusion.③Those suffered from immune system disorders,tumors or diabetes,or recent history of virus,bacteria or parasites infections.④ Those had current or recent treatment with corticosteroids or immunosuppressive drugs.According to ISS score ≥ 16,there were 61 traumatic patients divided into mild trauma group and severe trauma group.Seventeen healthy volunteers were taken as control subjects.At admission,the peripheral venous blood samples of patients were taken to count the number of CD3+,CD4 +,CD8 + T cells by flow cytometry and to detect TNF-α,INF-γ,IL-1,IL4,IL-6 and IL-12 levels in plasma by enzyme linked immunosorbent assay,meanwhile the ratio of Th1 / Th2 type cytokine were calculated.Data were analyzed with t test and ANOVA or Kruskal-Wallis test by using SPSS version 16.0package.P value ＜ 0.05 was considered to be statistical significance.Results Compared with control group,mild trauma and severe trauma groups showed a similar decrease in number of CD3 + T cells,and severe trauma group showed a most significant decrease in number of CD4 + T cells.Severe trauma group had a lower INF-γ level compared with control group; IL-1 level in both mild trauma and severe trauma groups were lower than that in the control group; INF-γ/ IL-6 in severe group was significantly lower than that in the mild group.However,INF-γ IL-6 in mild group was higher than that in the control group.Conclusions The early stage of severe trauma exhibits a significant decrease in number of both CD3 + and CD4 +T cells,accompanied by a significant reduction in most of cytokines,and has a tendency of shift to Th2 type cytokine.Therefore,the changes of immune cells should be promptly and successively monitored after severe trauma.

Objective To find out common allergens in Rizhao by investigating the allergens in patients with allergic rhinitis.Method The allergens in 686 patients with allergic rhinitis in Rizhao were detected by skin prick test.Results 509 of 686(74.20%)cases presented positive reaction,in which,the positive rate was 70.99%in inhalation group and 30.61%in food group,the most common inhalation allergen was pteronyssinus,then was dermatophagoides farinae,cockroaches;The most common food allergen was the jellyfish,then was peaches,peanuts.Conclusion The inhaled allergens were the main risk factors for allergic rhinitis,in which,mites was the most common allergens.

OBJECTIVE To investigate the use of antibiotics in our Outpatient Department during 2006-2007,and to analyze and evaluate the trend and rationality.METHODS The data about consumption quantity and consumption sum of antibiotics in our Outpatient Department during 2006-2007 were retrieved by means of computer management software in the department of pharmacy for the analysis of the clinical use of antibiotics using the method of defined daily dose(DDD).RESULTS Fluoro quinolones,cephalosporins and plant antibiotics have dominated the first places among the antibiotics used in our hospital,and the consumption of cephalosporins kept increasing year by year.CONCLUSIONS The use of antibiotics in our hospital should be further standardized to ensure the drug using safe,efficient and economic in our hospital.

OBJECTIVE To promote clinical rational use of antibiotic prophylaxis through surveying it in Obstetrics and Gynecology Department (OG) by pharmacists. METHODS Data of 223 cases using of antibiotic prophylaxis in OG were retrospectively analyzed. According to unreasonable use of antibiotic prophylaxis,the clinical pharmacists put out reasonable measures and directed treatment in succedent 206 patients. RESULTS The rate of unreasonable use of antibiotic prophylaxis in 223 cases was 100% and rate of infection after operation was 9.87%. PurposeSpinal cord is one of the most frequently involved sites of multiple sclerosis (MS), which seriously affects the life quality of patients. In this paper, we investigate the application value of voxel-based morphology (VBM) and resting-state magnetic resonance imaging (RS-fMRI) in multiple sclerosis patients with single spinal cord involvement (MS-SSCI).Materials and Methods Three-dimensional T1WI data and RS-fMRI data were acquired from 20 patients with MS-SSCI and 20 normal controls, grey matter volume (GMV), changes of white matter volume (WMV), total intracranial volume (TIV) and local nuclei volume were compared between the two groups using VBM, posterior cingulate cortex (PCC) was regarded as the seed point and the functional connectivity about whole brain was compared between the two groups by using resting-state functional connectivity analysis, the relationships between MS-SSCI structure, function change parameters and expanded disability states scale (EDSS) scores were further explored.Results①Compared with the control group, GMV, WMV, TIV of MS-SSCI group were not significantly reduced, only the volume of some regions (bilateral middle temporal gyrus, left inferior temporal gyrus) showed significant atrophy (P<0.01); MS-SSCI exhibited increased functional connectivity (FC) in the left medial prefrontal cortex, left inferior temporal gyrus, left caudate nucleus and right supplementary motor area (two-sample t test, after AlphaSim correction,P<0.01, voxel size >40).②There was no significant correlation (P>0.05) between MS-SSCI structure change parameters and EDSS; while a significant correlation between EDSS scores and FC was noted in the left inferior temporal gyrus (r=0.633,P<0.05).Conclusion Both structural abnormalities and altered FC with PCC can be detected in MS-SSCI, but only functional parameters are associated with clinical abnormalities, which are more sensitive than microstructural changes.

Objective Comparison of three different molecular assays for the detection and molecular characterization of circulating tumor cells in breast cancer .Methods The retrospective study compared three different molecular assays to detect CTC in the peripheral blood of 30 healthy individuals and 71 benign breast disease patients and 83 early and 84 metastatic breast cancer patients .All samples were collected at the outpatient , inpatient and physical examination department of Sichuan Provincial People ′s Hospital from January 2011 to June 2014.The same cDNAs were analyzed by:singleplex RT-qPCR assay for BCL-2, multiplex RT-qPCR for BCL-2, HER-2, HMAM, and a commercially available molecular assay (AdnaTest BreastCancer ) for GA733-2, MUC-1, HER-2.The positive of CTC were compared among healthy individuals and benign breast disease patients and breast cancer patients .Chi square test was used to compare the expression of gene markers among the three groups , and the agreement of Kappa test was used to evaluate the method.Results (1) Detection rates of early breast cancer by single RT-qPCR, Adna kits and multiple RT-qPCR were 13.3%, 16.9% and 18.1%, respectively , and the detection of metastatic breast cancer were 31.0%, 42.9%and 35.7%, respectively.There were significant differences in the positive of CTC by three molecular assays between healthy individuals and benign breast disease patients and early breast cancer patients ( The test values were 4.235 and 4.301, 5.367 and 5.474, 5.894 and 6.023 respectively, P<0.05).There were no differences between benign breast disease patients and early breast cancer patients (The test values were 0.891,0.748 and 0.701 respectively,P >0.05) .There were significant differences between metastasis breast cancer patients and healthy individuals and benign breast disease patients and early breast cancer patients ( The test values were 8.429,7.553 and 7.061;10.24, 9.025 and 8.745; 9.658, 8.417 and 8.201 respectively,P<0.05).(2) In early breast cancer: The concordance between AdnaTest and single RT-qPCR was 79.5%while between AdnaTest and multiplex RT-qPCR was 77.1%.No agreement was found among them ( The test values were 1.065 and 1.871, P were 0.371 and 0.258 ) .The concordance between single RT-qPCR and multiplex RT-qPCR was 80.7%.No agreement was found between them (The test values was 2.814, P was 0.156).(3) In patients with overt metastasis:The concordance between AdnaTest and single RT-qPCR was 78.6%( The test values was 10.986).While between AdnaTest and multiplex RT-qPCR was 80.9%( The test values was 9.251 ) . Agreements were found among them ( P was 0.002 and 0.005 respectively ) .The concordance between single and multiplex RT-qPCR was 88.1%( The test values was 12.364 ) .Agreement was found between them (P was 0.001).Conclusions No correlations were found among different molecular methods to detect CTC in the early primary breast cancer , but correlations were found in the metastatic breast cancer , suggesting that different rate of CTC caused by the number of CTC and its heterogeneity should be considered to the clinical diagnosis and treatment of breast cancer while molecular method is used .

Aim To investigate the periphery analge-sic effect of myricetin on a rat model of inflammatory pain and the mechanism. Methods Rat models of in-flammatory pain were induced by complete Freund ’ s adjuvant ( CFA) injection in left hindlimb plantar cen-ter. The thermal withdrawal latency ( TWL) was meas-ured before and after CFA or myricetin treatment. Elec-trophysiological method was used to identify the effect of myricetin on the action potential frequency and the voltage dependent potassium channel currents in small DRG neurons. Results Rats with CFA injected showed thermal hyperalgesia ( P <0. 05 ) and TWL in-creased significantly after myricetin intraperitoneally in-jected ( P <0. 05 ) . Current clamp recording showed the action potential frequency of small DRG neurons in rats was inhibited by myricetin ( P<0. 01 ) and voltage calmap recording showed the inhibitory effect of myr-icetin was enhanced by calcium depended potassium channel currents ( P<0. 05 ) . Conclusion Myricetin exerts periphery analgesic effect by enhancing calcium depended potassium channel currents and inhibiting excitability of small neurons of dorsal root ganglion.

Applying multi-media combined with case teaching method to optimize pediatric dermatology teaching and clinical practice.The data of typical cases were collected by multi-media method,which can visually describe the characteristics of the cases and help students master the knowledge of pediatric dermatology by personal involvement.It can also cultivate students′ ability to analyze and solve problems as well as alleviate the conflicts between the clinical practice and lack of teaching case resources under the intense doctor-patient relationship.In conclusion,it is a teaching reform which deserves extending and applying.
