Objective: To study depression of patients with Grave's disease and the therapeutic effect of Paroxetine (antidepressant). Method: 82 patients with first onset Grave's disease were collected and 52 of them had depression. The depressive patients were divided into Paroxetine and control group. All cases had the same anti-hyperthyroidism treatment. Result: 63.4% (52/82) patients with first onset Grave's disease had depression before Paroxetine treatment. After 4 weeks and 8 weeks treatment, Paroxetine group had greater decrease in FT3 and FT4, and lower scores of SDS and SAS than control group (p<0.01). Conclusion: Paroxetine does enhance the therapeutic effect of anti-hyperthyroidism, as well as improving depression of patients with first-onset Grave's disease.

Objective The function of regional myocardium of patients with myocardial infarction(MI) treated by intra-coronary stenting was determined with quantitative tissue velocity imaging(QTVI).Methods Twenty healthy controls and 24 patients with acute anterior myocardial infarction underwent QTVI with high-frame rates. The myocardial tissue velocity imaging in the apical 4-chamber view and apical 2-chamber view and apical longitude view were digitized and stored. Off-line ventricular walls regional velocity profiles along long axis were obtained, and systolic peak velocities (Vs) and early and late diastolic peak velocities (V E and V A) of 12 regional segments were measured. Results Under normal and ischemic conditions, 6 ventricular walls showed significant basal-apical myocardial velocity in the long-axis reduction in V S and V E and V A were significantly reduced in the abnormal segments in patients with MI. Conclusion QTVI can be used to analyze regional myocardial motion in the long-axis quantitatively and synchronously;intra-coronary stenting can obviously improve regional myocardial function of left ventricle, especially the diastolic function.

Objective To investigate the inhibitory effects of rosiglitazone on the synthesis of reactive oxygen species (ROS) and the expression of monocyte chemoattractant protein 1 (MCP-1) induced by high glucose in rat mesangial cells. Methods The mesangial cells were divided into six groups: control group ( C, 5.6 mmol/L glucose), mannitol group (M, 24.2 mmol/L mannitol+group C), high glucose group( H, 30 mmol/L glucose), R1 group(R1, group H+10 μmol/ L rosiglitazone), R2 group (R2, group H+20 μmol/L rosiglitazone), N-acetylcysteine (NAC) group (N, group H+5 mmol/L NAC, NAC was added 1 h before the stimulation of high glucose). The level of ROS was measured by confocal laser scanning microscopy. The mRNA and the protein expression of MCP-1 were semi-quantitatively determined with reverse transcription-polymerase chain reaction and ELISA respectively. Results No significant differences of ROS and MCP-1 were found between control group and mannitol group. The intracellular ROS induced by high DOI:10.3760/cma.j.issn. 1001-7097.2009.01.011glucose increased by 4.1-fold compared to control group (P<0.01), which was prevented by rosiglitazone (20 μmol/L) and NAC respectively. The MCP-1 mRNA expression in group R2 and group N was significantly lower than that in group H (P<0.01). The MCP-1 protein level in group H [(940.9±20.3) ng/L] was higher than that in group C [(403.0±8.1) ng/L] (P<0.01), and the expression of MCP-1 protein in group R2 [(562.5±15.3) ng/L] and group N [(539.8±8.3) ng/L] was lower than that in group H (P<0.01). Conclusion Rosiglitazone may suppress high glucose-induced MCP-1 expression by reducing the level of ROS, which may be one of the mechanisms that rosiglitazone plays a direct role in the protection of kidney.

Objective To investigate the experience of treating Budd-Chiari syndrome through orthotopic liver transplantation. Methods The clinical data of LTx performed on 9 patients with Budd-Chiari syndrome from December 2003 to April 2010 were retrospectively analyzed. We summarize the preoperative image and surgical experience, and observe the occurrence of postoperative complications and survival. Results Budd-Chiari syndrome was diagnosed in 9 patients by the preoperative abdominal CT enhancement and vascular reconstruction, and cavity venography was done to observe obstruction and sub-type of CAVA vein. All 9 patients were subjected to cadaveric liver transplantation.Eight cases accepted classic non bypass type, and one accepted living related right lobe liver transplantation. Postoperative triple immunosuppressive regimen included tacrolimus, mycophenolate mofetil, and hormone. The average follow-up periods for all these 9 patients were 32.8 months (13 to 61 months). One patient died from the tumor recurrence at 35th month after the operation. Two patients received re-transplantation for the lost of the graft. One recipient received the donor liver with medium steatosis, and the re-transplantation was performed on the12th day after the first transplantation due to the primary non function of the graft. The other one received the secondary liver transplantation at 6th month after the first transplantation due to the biliary complication and died from the liver tumor recurrence. Among all the 9 cases, seizure disorder (1 case),dysfunction of duodenal papillary muscle (1 case) and small-for-size syndrome (one case) occurred after the operation.Pulmonary infection occurred in 4 cases:3 cases due to the bacterial infection and 1 due to the fungal infection. Neither outflow obstruction nor the recurrence of the Budd-Chiari syndrome occurred in this study. The 1- and 2-year survival rate after the operation was both 100%,and 3-year survival rate post-transplantation was 88.9% (8/9).Conclusion Liver transplantation can be the ideal treatment to the Budd-Chiari syndrome based on the definite clinical diagnosis, accurate imaging evaluation and eligible modus operandi.

Objective To identify the changes of anti-HBs titers of patients with hepatitis B virus (HBV)-related diseases in the early stage (within the first week) post-liver transplantation (LT) and analyze their influencing factors.Method A total of 26 patients were enrolled in this study. They were all positive for HBsAg pre-LT and received the prophylaxis of lamivudine in combination with intravenous hepatitis B immunoglobulin (HBIG) in the first week post-LT.The titers of anti-HBs were detected daily in blood and drainage fluid every day in the first week post-LT.If the anti-HBs titers were greater than 1000 IU/L,blood and drainage were diluted, then detected again.Result The titers of anti-HBs in HBV-DNA negative groups, low HBsAg groups, and HBeAg negative groups were higher than those in the HBV-DNA positive groups, high HBsAg groups and HBeAg positive groups in the first five days post-LT.The median titer of anti-HBs in drainage fluid was 181.60 IU/L (0.00-968.50 IU/L).And the titer of anti-HBs in drainage fluid was correlated with anti-HBs titers in blood at the same time (r =0.927,P =0.000).The amount of anit-HBs calculated in drainage fluid was very high, but it fluctuated in a wide range (0.00-908.55 IU).Conclusion In the early stage post-LT,patients in high risk groups should receive higher doses of HBIG to maintain safe levels of anti-HBs,while the lower doses of HBIG are enough to the patients in low risk groups.Furthermore, the anti-HBs titers in blood aren't affected by the anti-HBs loss in drainage fluid.

Insulin sensitivity,β-cell function and serum high-sensitivity C-reactive protein (hs-CRP)levels were observed in obese and non-obese normoglycemic first-degree relatives of type 2 diabetic patients (FDR). The results showed that there existed insulin resistance,β-cell dysfunction and increased serum hs-CRP level in obese FDR of type 2 diabetic patients. Moreover, insulin resistance and increased CRP level were positively related to waist circumference.

Objective:Previous studies suggested that the-308G/A allele in the tumor necrosis factor-α(TNF-α) gene promoter (-308G/A) may be a potential risk factor for inflammatory diseases and tumor progression. However, only a few studies have focused on the-308 polymorphism of TNF-αgene with primary lung cancer in Chinese population. This study aims to evaluate the role of TNF-α-308G/A single nucleotide polymorphism (SNP) and the risk of primary lung cancer in Chinese population. Methods:A total of 250 patients and 447 healthy individuals (control group) were involved in this study. Genotyping was performed using TaqMan technology. Results:The frequencies of (GG), (A/G), and (A/G+AA) genotypes of-308G/A SNP in TNF-αgene were 183 (73.2%), 67 (26.8%), and 67 (26.8%) in the patients, and 406 (90.8%), 39 (8.7%), and 41 (9.2%) in the control group, respectively. The distribution of poly-morphism frequencies in the case group and the control group showed a statistically significant difference for the Chinese population (P<0.05). Conclusion:Results indicated that TNF-αgene polymorphism at position-308G/A is associated with susceptibility to lung cancer in Chinese Han population.
