The mucosal tissues form the interface between the outside world and the sterile environment of the body.Innate immunity plays a critical role in the maintenance of mucosal homeostasis.Recently,a new subset of innate immune cells named NK-22 has been discovered to reside at tonsil and intestine both in human and mice.NK-22 cells share features with both natural killer (NK) cells and lymphoid tissue-inducing (LTi) cells.NK-22 cells express a variety of NK cell receptors and phenotypically resemble that of immature NK cells (iNKs).However,they are quite different from NK cells in function and development.NK-22 cells lack conventional NK cell cytotoxic function and produce IL-22 instead of IFN-?.It is currently considered that NK-22 cells are not a subset of NK cells.They develop from LTi cells and the development process depends on ROR?t and commensal microflora.NK-22 cells are involved in immune protection against the pathogen Citrobacter rodentium.The IL-22 secreted by NK-22 cells contributes to epithelial cell resistance to injury and mucosal tissue repair after microbial infection.
2.Calicecotomy Combined with Trans-renal Parenchyma Pneumatic Lithotripsy for Complicated Staghorn Renal Calculi

Objective To evaluate the efficacy of calicecotomy combined with trans-renal parenchyma pneumatic lithotripsy for complicated staghorn renal calculi.Methods The severe hydrocalycosis was incised using electrocautery,then pneumatic lithotripsy was performed and the broken stones were taken out.For those patients with stenotic entrance to renal calyces without hydrocalycosis,we stabbed into the stones with the lithotriptic pole(1 mm in diameter) through renal parenchyma and took the broken stones out of the entrance.Results The renal pedicle were not blocked in 19 cases.The operation time was 90-150 minutes,with a mean of 120 minutes.There was no blood transfusion with the blood loss ranging form 100 to 250 ml.The procedures were successful in 17 cases without residual stones after operation;intraoperative residual sand-like calculi were found in 1 case and removed by irrigation and drainage through nephrostomy tube;intraoperative missing calyceal calculi occurred in 1 case and were cleared by extracorporeal shock wave lithotripsy(ESWL).A follow-up for 10-60 months(mean,18 months)in 15 patients showed recurrence in 2 ones,and the stones were removed by ESWL.Conclusions Calicecotomy combined with trans-renal parenchyma pneumatic lithotripsy for complicated staghorn renal calculi has the advantages of less blood loss and definite efficacy.
3.Application of Gukang Gold Far-Infrared Magnetic Therapeutic Paste to Treatment of Lateral Epicondylitis

Objective To apply Gukang gold far-infrared magnetic therapeutic paste to the treatment of lateral epicondylitis.Methods 64 cases of lateral epicondylitis patients were randomly divided into treatment group and control group,each with 32 ones.The subjects from the treatment group were treated with Gukang gold far-infrared magnetic therapeutic paste,and the remained with ibuprofen sustained-release capsules.The results after one and two courses were analyzed statistically.Results After two-course treatment,the patient from the treatment group had an higher improvement rate than the others from the control group,with a P value less than 0.05.The treatment course of the treatment group was shorter than that of the control group,with a P value less than 0.05.Conclusion When compared with ibuprofen sustainedrelease capsule,Gukang gold far-infrared magnetic therapeutic paste is more effective for the treatment of lateral epicondylitis.
4.Analysis of Mmultiple Viral Antigens in Pediatric Patients Detection Result by Direct Immunofluorescence Method

Purpose] To detect respiratory virus antigen by direct immunofluorescence method and provide evidence for early diagnosis of children with viral infection of the respiratory tract disease.[Methods] Select 398 cases of respiratory tract infection in hospitalized children, pharynx in exfoliated cells in respiratory syncytial virus by direct immunofluorescence(RSV), adenovirus(ADV), influenza virus type A(IFVA),influenza virus type B(IFVB), parainfluenza virus type 1(PIV1), parainfluenza virus type 2(PIV2) and parainflue-Nza virus type 3(PIV3) of 7 common viral antigens were detected, and carried on statistics analysis.[Results] In 398 cases of children with respiratory tract infection, 91 cases were positive, the positive rate was 22.86%. The highest for respiratory syncytial virus in 37 cases(40.66%) followed by parainfluenza virus type 3 in 19 cases(20.88%), influenza virus type B in 14 cases(15.38%); The positive rates of acute tonsillitis, capillary bronchitis, pneumonia, acute bronchitis and upper respiratory were 46.67%(14/30),40.00%(20/50), 26.67%(40/150), 18.42%(7/38), 7.69%(10/130) respectively; With the growth of age, the respiratory tract virus infection rate gradually decreased(P<0.05), one year old the viral infection rate was 76.92%;it occurred in the winter season of respiratory tract infection in children .[Conclusion] The main virus in children with acute respiratory infection in the local area is RSV. Virus infection decreases with the increase of age, treatment should be careful in the use of antibiotics.
5.Effects of epidural infusion of different concentrations of lidocaine on the depth of anesthesia during combined general-epidural anesthesia

Objective To evaluate the effects of epidural infusion of different concentrations of lidocaine on the amount of propofol needed to induce anesthesia and the end-tidal isoflurane concentration needed to maintain BIS at 50-55 during operation. Methods Forty-five ASA Ⅰ-Ⅱ patients of both sexes (24 males, 21 females) aged 21-60 yrs, weighing 46-77 kg undergoing elective upper abdominal surgery were randomly divided into 3 groups with 15 patients in each group : group Ⅰ epidural normal saline ( NS); group Ⅱ epidural 1 % lidocaine (Lido) and group Ⅲ 2 % Lido. An epidural catheter was inserted at T9-10 into epidural space and advanced 3-4 cm cephalad. After a test dose of 3 ml, NS or 1% or 2% Lido 10 ml was injected into the epidural space. Anesthesia was induced with propofol infusion at 25 mg?min-1. Propofol infusion was stopped when the patients stopped responding to loud voice (OAAS/2) and the amount of propofol infused was recorded. Then fentanyl 2 ?g?kg-1 and vecuronium 0.15 mg?kg-1 were given i.v. . The patients were intubated and mechanically ventilated and PETC02 was maintained at 35 mm Hg. Anesthesia was maintained with isoflurane inhalation and epidural infusion of Lido or NS at 7 ml?h-1 . BIS was maintained at 50-55 during operation. 6.Application of ultrasound-guided percutaneous microwave ablation treatment in patients with benign thyroid ;nodules

Objective To investigate the effectiveness and safety of ultrasound-guided percutaneous microwave ablation in the treatment of benign thyroid nodules.Methods A total 456 patients (912 nodules) underwent microwave ablation in our department.Microwave ablation was carried out using microwave antenna under local anesthesia.Nodule volume,thyroid function and clinical symptoms were evaluated before treatment and at 1 ,3,6 and 12 months after the treatment to monitor changes in the volume,adverse effect and complications.Results 912 nodules were all studied with contrast-enhanced ultrasound immediately after the ablation,and all results showed “black hole”sign.The mean follow-up period after microwave ablation was (9±6)months (range,3-24 months).At 6-month follow-up,the mean volume reduction ratio of the solid nodules,mixed nodules and the cystic nodules was (61 ± 50 )%,(81 ± 33 )%,(89 ± 46 )%, respectively.The treatment was well tolerated and no major complications were observed except pain and transient voice changes (2.4%).Conclusions Microwave ablation is a safe and effective technique for the treatment of benign thyroid nodules.
7.Primary approach for ultrasound-guided percutaneous microwave ablation of papillary thyroid microcarcinoma

Objective To evaluate the feasibility and efficacy of ultrasound-guided percutaneous microwave ablation combined with hormone suppression therapy for solitary papillary thyroid microcarcinoma.Methods A total of 25 patients with 25 nodules pathologically proven solitary papillary carcinoma 3.7 to 10.0 mm in diameter without clinically apparent lymph node,or distant metastasis were treated with microwave ablation equipement.Microwaves were emitted at 40 W for 200 -400 seconds and prolonged as necessary to attain confluent ablation zones.All patients were treated with both extended ablation therapy and levothyroxine that maintain TSH levels below 0.1 mU/L.Thyroid function test,nodule volume and clinical symptom were evaluated before ablation and during 1 ,3,6,9 and 12 months after treatment according to changes in tumor size,adverse reactions,complications and metastatic tumors.Three out of 25 patients received surgical treatment and the other 22 patients were followed up with routine ultrasound and contrast enhanced ultrasound.Results All 25 tumors were completely ablated by using mutiple plane fixed melting method and no serious or permanent complications occurred.No recurrence at the treatment site or distant metastase were detected,with a mean follow-up of 1 8 months.Histological examination showed no evidence of a tumor in the treated lesions in the 3 surgery patients.Conclusions Ultrasound-guided percutaneous microwave ablation appears to be a safe and effective technique for the therapy of solitary papillary thyroid microcarcinoma.
8.Phylogenetic Analysis of the VP1 Region of Coxsackievirus A16 Strains Isolated in Anhui Province, 2014.

To study on the phylogenetic characterization of the VP1 genes of coxsackievirus A16 (CVA16) causing hand-food-mouth disease (HFMD) isolated from Anhui province in 2014. A total of 413 throat swab specimens from HFMD patients were collected during January to November, 2014 for the isolation and identification of enteroviruses using real-time RT-PCR assays. The VP1 regions of CVA16 isolates were amplified using RT-PCR and sequenced. And the phylogenetic tree was constructed among the VP1 regions of those isolates, the different genotypes and sub-genotypes of CVA16 strains. A total of 97 enteroviruses were isolated from 413 samples, the positive rate was 23.49% (97/413), including seventeen CVA16, seventy six HEV71 and four other enteroviruses. The results of the phylogenetic tree showed that 17.CVA16 strains isolated from Anhui in 2014 clustered within B1b evolution branch of B1 genotype. The nucleotide and amino acid sequence identities were 95.30%-100% and 98.70%-100% among the isolates, respectively, but within B1b branch of 17 strains formed several small transmission chains. The nucleotide acid of 17 CVA16 isolates in Anhui province were closed to the strains isolated from Yunnan, Hunan, Guangdong, Tibet and Jiangsu, especially from Hunan in 2013 and from Shenzhen of Guangdong in 2014, the identity were 96.40%-99.70%. The CVA16 strains isolated from Anhui in 2014 were all belong to genetic subtype B1b of B1 genotype was dominant, and among those isolates, several small virus transmission chains had formed with co-circulating and evolution.

9.Risk factors for healthcare-associated infection in patients undergoing craniocerebral operation

Objective To explore risk factors for healthcare-associated infection (HAI)in patients undergoing craniocerebral operation,and provide guidance for the prevention of HAI in patients in department of neurosurgery. Methods 200 patients who underwent craniocerebral operation in a department of neurosurgery from November 2013 to November 2014 were surveyed,risk factors for HAI were analyzed.Results Among 200 patients undergo-ing craniocerebral operation,81 patients developed 99 cases of HAI,HAI rate was 40.50%,HAI case rate was 49.50%;the top five HAI sites were lower respiratory tract,urinary tract,intracranial site,bloodstream,and in-testinal tract.Univariate analysis showed that patients’age ≥60,Glasgow Coma Scale (GCS)<15,intraoperative blood loss ≥800 mL,staying in intensive care unit(ICU),indwelling gastric tube,ventricular drainage,using ventilator,tracheotomy,and using H2 receptor antagonists were important risk factors for HAI in patients undergo-ing craniocerebral operation (all P <0.05).Multivariate logistic regression analysis showed that patients’age ≥60, GCS<15,staying in ICU,and using H2 receptor antagonists were independent risk factors for HAI in patients un-dergoing craniocerebral operation.Conclusion Strengthening the surveillance of HAI patients undergoing cranioce-rebral operation and realizing risk factors for HAI are helpful for taking comprehensive prevention measures and re-ducing the incidence of HAI.
10.Effect of adenosine A2A receptor agonist combined with bone marrow mesenchymal stem cells transplantation on the negative immune regulation in mice with acute liver failure

Objective To investigate the therapeutic effect and possible mechanism of adenosine A2A receptor agonist (CGS21680) combined with bone marrow mesenchymal stem cells (BMMSC) transplantation in acute liver failure (ALF).Methods Fifty male C57BL/6 mice, 6-8 weeks old, were fed with standard diet for 1 week and randomly divided into 5 groups according to random number table: healthy control group (n=6), model group (n=11), BMMSC group (n=11), CGS21680/BMMSC group (n=11) and CGS21680 group (n=11).Except healthy control group, the other mice were injected with D-GalN and lipopolysaccharide (LPS) to establish ALF model.Ten hours later, CGS21680/BMMSC group and CGS21680 group were injected intraperitoneally with adenosine A2A receptor agonist CGS21680 (2.1 mg/kg).In addition, the BMMSC group and CGS21680/BMMSC group were injected BMMSC (1×10.6) through tail vein.After 24 hours, pathological changes of liver tissue was observed by hematoxylin and eosin staining.The change of proportion of mouse splenic Treg among CD4+T lymphocytes was detected by flow cytometry.Toll-like receptor (TLR)4 and nuclear factor (NF)-κB expression levels in liver tissue were detected by real-time fluorescence quantitative polymerase chain reaction (PCR) and Western blot.One-way analysis of variance (one-way ANOVA) and SNK-q test was conducted for data analysis.Results Serum IL-6 levels were (23.67±2.97) pg/mL in healthy control group, (151.47±6.03) pg/mL in model control group, (72.10±3.74) pg/mL in BMMSC group, (53.35±2.50) pg/mL in CGS21680/BMMSC group and (84.85±3.25) pg/mL in CGS21680 group.The differences between healthy control group and the other 4 groups were all statistically significant (t=46.02, 25.51, 19.58 and 34.03, respectively, all P<0.01).Serum TNF-ɑ levels were (24.62±3.19) pg/mL in healthy control group, (102.25±2.10) pg/mL in model control group, (54.71±2.23) pg/mL in BMMSC group, (42.20±4.72) pg/mL in CGS21680/BMMSC group and (81.76±3.50) pg/mL in CGS21680 group.The differences between healthy control group and the other 4 groups were all statistically significant (t=46.49, 19.97, 7.72 and 29.57, respectively, all P<0.01).The differences of spleen Treg proportion in healthy control group were statistically significant compared with model control group, BMMSC group, CGS21680/BMMSC group and CGS21680 group (t=51.67, 12.22, 5.91 and 18.21, respectively, all P<0.01).The differences of TLR4 mRNA levels of liver tissue in healthy control group were statistically significant compared with model control group, BMMSC group, CGS21680/BMMSC group and CGS21680 group (t=26.31, 21.33, 13.24 and 27.14, respectively, all P<0.05).The differences of NF-κB mRNA level of liver tissue in healthy control group were statistically significant compared with model control group, BMMSC group, CGS21680/BMMSC group and CGS21680 group (t=16.56, 16.34, 7.83 and 13.11, respectively, all P<0.05).The differences of TLR4 protein level in liver tissue of healthy control group were statistically significant compared with model control group, BMMSC group, CGS21680/BMMSC group and CGS21680 group (t=35.60, 10.38, 6.05 and 18.02, respectively, all P<0.05).The differences of liver NF-κB protein level in the healthy control group were statistically significant compared with model control group, BMMSC group, CGS21680/BMMSC group and CGS21680 group (t=10.80, 7.30, 4.61 and 13.24, respectively, all P<0.05).Conclusions Adenosine A2A receptor agonist combined with BMMSC can significantly up-regulate the proportion of Treg cells in acute liver failure mice and inhibit the TLR4/NF-κB pathway activation, with both coordinated regulation, and further inhibit the liver inflammation.
