1.The construction of continuing nursing management model for lung cancer chemotherapy complications based on m-health

Objective To construct the continuing nursing management model for lung cancer chemotherapy complications based on m-health. Methods Literature research, qualitative interviews and expert group meeting were used to build the continuing nursing management model for lung cancer chemotherapy complications based on m-health. Results The nursing management model of lung cancer chemotherapy complications including service objectives, service providers, service objects, service time, service flow, service platform and related personnel responsibilities were determined. Conclusions The continuing nursing management model for lung cancer chemotherapy complications based on m-health has strong scientific and practical characteristics, which is helpful for patients to timely monitor the occurrence of complications of chemotherapy, access the corresponding prevention and intervention measures.
2.HLA-peptide tetramers and adoptive immunotherapy in prevention of cytomegalovirus disease

BACKGROUND: Antiviral drugs can reduce the incidence of early-onset cytomegalovirus(CMV)disease,but are associated with strong toxicity and the development of late-onset CMV disease.In order to prevent CMV disease better,cytotoxic T lymphocytes(CTL)may play a critical role in controlling CMV reactivation.Fluorescent HLA-peptide tetramers are used to monitor the recovery of CMV CTL in recipients of allogeneic transplants.OBJECTIVE: To explore the effect of HLA-peptide tetramers and adoptive immunotherapy in treating CMV disease.METHODS: A computer-based online search of Pubmed and Wanfang databases was performed for articles related to CTL detection,application of antiviral drugs and HLA-peptide tetramers,and adoptive immunotherapy with key words"HLA-peptide tetramers,cytomegalovirus,specific CTL,adoptive immunotherapy"in English and Chinese.Repetitive articles were excluded and 29 articles were included.RESULTS AND CONCLUSION: Adoptive immunotherapy with CMVs cytotoxic T cells as preemptive therapy is a very elegant strategy; however,generation of these cells is costly and time-consuming,and therefore the therapy is not available at every transplantation center.Magnetic selection of CMVs CD8+T cells from peripheral blood of CMV-seropositive donors by using HLA-peptide tetramers may be very hopeful,which simplifies adoptive immunotherapy.
3.Ultrasound-guided versus CT-guided percutaneous biopsy for the qualitative diagnosis of ultrasonic-visual chest lesions: a comparison study

Objective To compare the puncturing hit rate,positive rate of pathological diagnosis and the incidence of complications between color Doppler ultrasound-guided and CT-guided percutaneous biopsy for the qualitative diagnosis of ultrasonic-visual chest lesions.Methods A total of 112 patients,who were encountered from January 2015 to June 2016 in authors' hospital and whose imaging materials suggested the presence of ultrasonic-visual chest lesions,were enrolled in this study.There were no bones or lung air between the thoracic skin and chest lesion to hinder imaging observation.Ultrasound-guided puncturing was employed in 52 patients (ultrasound-guided group) and CT-guided puncturing was adopted in 60 patients (CT-guided group).The puncturing hit rate,positive rate of pathological diagnosis and the incidence of complications were compared between the two groups.Results The puncturing hit rate in ultrasound-guided group was 100% (52/52),which was higher than 91.7% (55/60) in CT-guided group.The positive rate of pathological diagnosis in ultrasound-guided group was 96.2% (50/52),which was higher than 80.0% (48/60)in CT-guided group.The incidence of complications in ultrasound-guided group was 3.8% (2/52),which was lower than 18.3%(11/60) in CT-guided group.Conclusion For the qualitative diagnosis of ultrasonic-visual chest lesions,ultrasound-guided percutaneous biopsy is more reliable than CT-guided percutaneous biopsy.
4.Construction of evaluation index system for nursing speciality development in tertiary hospitals

Objective To structure the evaluation index system for nursing speciality development (EISNSD) in tertiary hospitals.Methods The evaluation index system for nursing specialty developmentwas established using a Delphi method. The weight coefficient at all levels was calculated by the analytic hierarchy process. The standard value of the index was set by multiple ways including target plan, industry regulation, historical data and so on. A total of 48 departments from three tertiary hospitals in Shanghai were investigated. The reliability and validity of the evaluation indexes were analyzed with Cronbach′s alpha coefficients and Spearman correlation.Results The EISNSD was structured with five primary indexes, 14 secondary indexes and 75 third indexes. The weight coefficients of primary indicators included 0.2859 for talent echelon building, 0.3772 for nursing practice, 0.1095 for teaching level, 0.1444 for scientific research and 0.0830 for the construction of professional training base. The correlation between indexes at all levels and their indexes of higher level was from 0.583 to 0.920 (P<0.01). The reliability of all indexes at all levels was from 0.581 to 0.945 with 0.931 for the reliability of total index.Conclusions The EISNSD in tertiary hospitals is of a strong scientificity and practicability. It can provide the quantiative basis for objective evaluation and continuity management of nursing specialty development of all departments in tertiary hospitals.
5.Effects of Multidisciplinary Team-Based Nurse-led Transitional Care on Clinical Outcomes and Quality of Life in Patients With Ankylosing Spondylitis

PURPOSE: The purpose of this study is to investigate the impact of transitional care by a nurse-led multidisciplinary team (MDT) on clinical outcomes and quality of life of patients with ankylosing spondylitis. METHODS: A randomized control study design was used. Subjects were allocated randomly to an experimental group and a control group. The experimental group received intensive transitional care by a nurse-led MDT, whereas the control group received routine nursing care. Disease activity, spinal mobility, comprehensive function, health service utilization, and quality of life were assessed at the baseline and at six months with the Bath Ankylosing Spondylitis Metrology Index, the Bath Ankylosing Spondylitis Disease Activity Index (BASDAI), the Bath Ankylosing Spondylitis Functional Index (BASFI), a health service utilization questionnaire and version 2 of the Short Form-36 health survey. RESULTS: Compared with the baseline, the BASDAI, BASFI, emergency visits, hospitalizations, hospitalization days, and bodily pain, vitality, mental health, total score, and average score of version 2 of the Short Form-36 health survey were improved in the experimental group (p < .05), whereas only bodily pain, vitality, and role-emotional were improved in the control group p < .05). At six months, the experimental group exhibited significantly more improvement on the BASDAI, BASFI, hospitalizations, all domains except Role-physical as well as total score and average score p < .05) compared with the control group. CONCLUSION: A MDT-based nurse-led transitional care improves clinical outcomes and quality of life of patients with ankylosing spondylitis. Future research should be carried out on modes of follow-up and family support.

6.Research on the correlation between parents' posttraumatic growth, mental resilience and family function in children with autism spectrum disorder

Objective:To explore the correlation between parents′ posttraumatic growth, mental resilience and family functioning in children with autism spectrum disorder.Methods:Using the convenience sampling method, 169 parents of children with autism spectrum disorders were selected. General data questionnaire, Posttraumatic Growth Inventory (PTGI), Conner-Davidson Resilience Scale (CD - RISC) and Family Assessment Device (FAD) were investigated.Results:The total PTGI score of parents of children with autism spectrum disorder was 58.79±18.92. The scores of appreciation of life, new possibilities, personal strength, relating to others and spiritual change were 19.49±6.15, 10.46±4.59, 9.05±3.32, 7.79±3.65, 12.01±3.83. The PTGI score of 37.3% parents was lower than 52, which was at a low level. Parents in the high score (> 74) group and the low score (< 52) group had statistically significant differences ( P<0.05) in the total score of CD - RISC and scores in all dimensions, as well as the differences in the scores of the four dimensions of FAD, namely problem solving, communication, role and behavior control. The total score of PTGI of parents was positively correlated with the total score of CD - RISC and the scores of its all dimensions ( r values were 0.201-0.317, P<0.05), and negatively correlated with the scores of Problem Solving, Communication and Behavioral Control of FAD ( r values were -0.233 - -0.196, P<0.05). Conclusions:The parents of children with autism spectrum disorder have an average level of posttraumatic growth. People with better mental resilience and healthier family functions have higher post-traumatic growth level.
7.Construction and application of standardized training mode for new nurses based on blended teaching

In order to enhance the practical level of new nurses and improve the training effect and efficiency of new nurses, a management system of standardized training for new nurses has been established in The Third Affiliated Hospital of Sun Yat-sen University since July 2016. A standardized training program including basic and professional training, theoretical and practical training has been developed. Based on the established mobile management system and through the combination of online and offline training, a two-year standardized training was carried out for 289 new nurses. The theoretical test and site-based comprehensive skills test for graduation examination were conducted in July 2018. The results showed that the training bases and trained nurses held a positive attitude towards the standardized training mode and the completion rate of training program in each stage was 100%. This training mode has an excellent training effect and can ensure the implementation of training programs and the improvement of training efficiency.
8.Construction of prnp gene knockout vector and its transfection in eukaryotic cell.

It is one of the frequently utilized strategies for positive-negative selection to elevate the gene targeting efficiency in somatic cells by enriching targeted colonies. Knocking out prnp in animals by gene targeting can prevent it from expressing Prion protein (Pathogenic protein of transmissible spongiform encephalopathy), which enables it to resist infection of Prion. We constructed a bovine prnp biallelic targeting vector via the positive-negative selection strategy, and transfected the linearized vector into the bovine fetal fibroblasts through electroporation. Then, we selected cells in cell culture medium with G418 under a concentration of 600 microg/mL followed by Ganciclovir (GCV) under a concentration of 200 nmol/mL. In the end, we successfully obtained 176 cell clones. All these clones were identified by means of sequencing, immunofluorescence and western blotting, respectively, confirming that there existed 9 positive cell clones. The results showed that the bovine prnp gene was successfully knocked out. Conclusively, we provide an effective way to knockout bovine prnp gene, which could serve as the basis for producing prion protein gene knockout transgenic cloned cattle.
