To train the talents of clinical laboratory for the demand of clinical jobs,it is necessary to make the experimental teaching reform of clinical laboratory hematology.Comprehensive experimental teaching,which centers on case,has been explored in many ways like prefessional teachers' training,cases' choose,teaching method and experimental examination.And the practice has proved that the reform of comprehensive experimental teaching can give full play to the students' initiative and creativity,foster the students' ability to analyze and diagnose hematopathy.
2.The Research and Application Effects of Systematic Anatomy Learning System Based on Android Platform

Objective To implement a systematic anatomy learning system based on android platform and evaluate the application effects of the learning system in clinical medical undergraduate students.Methods The systematic anatomy learning system was based on android development package and correlation functions were implemented using java technique.108 undergraduate students in clinical major of 2014 in Kunming Medical University were selected and randomly divided into experimental group and control group.Students of the experimental group were given the traditional teaching method and this learning system for autonomous learning,while students of the control group were given the traditional teaching method.These two teaching methods were subjectively and objectively evaluated by questionnaire survey and final examination respectively.Results The questionnaire showed that the 87.5% of students thought that it was necessary to use the learning system.The 90.74% of students in experimental group thought their learning effect were improved and the 98.15% of them thought themselves learning ability were improved.The average score of the experimental group and the control group was 83.56 ± 5.61 and 75.33 ± 4.62 respectively.The difference between the two groups was statistically significant (P<0.05).Conclusion The systematic anatomy learning system can help students to understand and grasp the knowledge points of the systematic anatomy,and it can improve the teaching effect too.It will provide research foundation for the next step of applying for patent.
3.Exploration of three-year clinical medicine talents training model based on the concept of ‘ general practice’

Chongqing Three Gorges Medical College comprehensively reformed ‘2 + 1 ’ training mode and established the new training model based on the concepts of ‘ general practice,diagnosis and treatment ability in all subjects,progressive teaching,integration between college of clinical medicine and affiliated hospital’.Meanwhile,Chongqing Three Gorges Medical College redefined objectives of talent training; reconstructed curriculum,launched progressive teaching,reformed teaching contents and methods thus promoted the teaching quality of clinical medicine in junior college and improved quality of clinical medicine talents who are willing to go and stay in hospitals at primary level and who are practical in clinical medicine.
4.Primary development and biomechanics of single vertebrae internal fixation system for thoracolumbar compression fracture

BACKGROUND:For severe thoracolumbar compression fracture (>1/3 compression), ideal therapeutic method is minimal y invasive internal fixation, which has good biomechanical functions. Moreover, bone graft is reliable. Injured vertebra reduction and bone graft stability achieved. Motor unit of spinal column and normal physiological function were retained, resulting in lessening nearby segmental degeneration.
OBJECTIVE:To discuss the design of single vertebrae internal fixation system and evaluate its biomechanical performance which apply to treat thoracolumbar compression fracture by endoscope.
METHODS:A brand-new single vertebrae internal fixation system was designed in accordance with data of anatomic measurement of adult thoracolumbar vertebra. Six fresh adult corpse specimens were prepared to produce models of L 1 compression fracture, and assigned to control group, fracture injury group, single vertebrae internal fixation system group, AF reduction internal fixation group and anterior plate internal fixation group.Three-dimensional movement range experiments were conducted separately.
RESULTS AND CONCLUSION:The biomechanical comparison showed that there was no significant difference in three-dimensional range of motion among single vertebrae internal fixation system group (anteflexion, left and right lateroflexion), AF reduction internal fixation group and anterior plate internal fixation group (P>0.05). However, range of motion significantly increased at backward extension, left and right rotation (P<0.05). Results suggested that the design of single vertebrae internal fixation system was novel and the system had good biomechanical performance at anteflexion, left and right lateroflexion. However, it needs to be improved in which lacks of stability of extension and rotation.
5.Mutation analysis of type Ⅱ hair keratin gene in a pedigree with monilethrix

Objective To investigate the mutation of type Ⅱ human hair basic keratin (hHb) gene in a family with monilethrix.Methods Scanning electron microscopy was used to observe the structure of hair shafts.With informed consent,blood samples were drawn from affected and unaffected membets in this family,as well as from 50 healthy controls.Genomic DNA was isolated from these samples.The exon 1 and exon 7 of hHb1,hHb3 and hHb6 were amplified by polymerase chain reaction (PCR).All the PCR products were sequenced directly using ABI3730 automated sequencer.DNA sequence alignment was carried out with BLAST software.Results A typical beaded appearance was observed in affected hairs by using scanning electron microscopy.There were obvious longitudinal ridges and sulcuses in hair node.and hair cuticles were irregularly shaped.Most cortex and medullary substance were absent in affected hairs of a patient.After sequence alignment,a G1289A point mutation in exon 7 of hHb6 gene,which led to a substitution of arginine for glutamide at codon 430,was detected in affected members of this family,but not in unaffected family members or 50 unrelated human controls.No mutation was observed in exon 1 or exon 7 of hHb1 and hHb3 gene or exon 1 of hHb6 gene.Conclusion The missense mutation of R430Q is a novel mutation.which may be associated with the pathogenesis of monilethrix in this pedigree.
6.Dynamic observation of EB virus DNA load before and after the treatment in patients with nasopharyngeal carcinoma and its clinical significance.

OBJECTIVE:
Dynamic observation of Epstein-Barr virus (EBV) DNA load before and after the treatment in patients with Nasopharyngeal carcinoma (NPC), predicting the incidence of distant metastasis and offering more personalised choice of therapies.
METHOD:
Fifty-four cases of patients with NPC were taken by fluorescence quantitative PCR assay of EBV DNA load before and after the treatment, all patients were followed up according to plan and carried out the progression-free survival (PFS) and overall survival (OS).
RESULT:
EBV DNA load in plasma of patients with NPC can partly reflect the clinical characteristics of patients; EBV DNA load in some patients with distant metastasis was higher than those patients with continuous remission when they were not started treatment (P < 0.05); For those patients whose EBV DNA copies were lower than 20,000 copies/mI before the treatment, the progression-free survival and overall survival rates were higher than those high expression patients, and the difference were statistically significant (PF < 0.01 and P < 0.05).
CONCLUSION
The EBV DNA load in the plasma of NPC patients can partly predict the occurrence of distant metastases before treatment.

7.The Effects of Compound Tripterygium Hypoglaucun Hutch on Photoallergic Contact Dermatitis of Murine Model

Objective To study the efficacy and mechanism of compound Tripterygium hypoglaucum Hutch (THH) on photoallergic contact dermatitis in mice. Methods The photoallergic animal model of BALB/c mice was established by using photosensitizer chlorpromazine and UVA irradiation. The therepeutic efficacy was determined by measuring the thickness and the weight of the swelling ear and the number of infiltrated mononuclear cells in the ear tissue. Immunohistochemical technique was used to detect the ICAM-1 expression on keratinocytes, fibroblasts and vascular endothelial cells. The serum level of INF-? was measured by ELISA. The tested animals were divided into 3 groups: compound THH, THH alone and normal saline. 8.Dynamic contrast-enhanced MRI and diffusion weighted imaging in evaluating the early response to chemotherapy in non-small cell lung cancer

Objective To investigate the value of dynamic contrast-enhanced(DCE)-MRI and DWI for the early assessment of curative effects in NSCLC. Methods Forty patients from September 2012 to September 2014 with NSCLC proven by pathology were examined with DCE-MRI and DWI, at one week before the first chemotherapy and one month after treatment. DCE parameters (MER, slope, WR) and ADC values of the tumors were calculated on the workstation. According to the changes of the tumor-size after treatment, all patients were divided into two groups: good response group and poor response group. Pretreatment parameters were compared between the two groups with independent-samples t test, and parameter changes before and after treatment were compared with paired-samples t Test. ROC curve of tumor response characteristic was analyzed. Pearson correlation coefficient was used to study the correlation between post-treatment parameter changes and tumor size reduction. Results Pretreatment ADC, MER and slope value were (1.15 ± 0.09) × 10-3 mm2/s, 1.13 ± 0.28, (3.76 ± 1.27)% in the good response group(15 patients), and were (1.34±0.33)×10-3mm2/s, 0.78±0.16, (2.63±0.58)%in poor response group(25 patients), respectively. There was significant difference between tumors in two groups(t=2.329, 3.152, 4.556,P=0.027, 0.032, 0.009), the lower ADC value and the higher MER/slope value was observed in the good response group. The change rates of ADC after one month treatment in the two groups were (20.43±6.40)%, (2.98±1.65)%(t=3.485,P=0.009), and slope change were (-61.80±19.21)%, (-30.80±11.16)%(t=3.280, P=0.020). Both the rising rate of ADC and decreasing rate of slope value were significant higher in good response group than the poor response group. The ROC curve was used to analyze the change rate of ADC and slope after tumors treatment, the area under the ROC curve in ADC was largest(Az=0.790), the diagnosis efficiency was highest. We found significant positive correlation between ADC, MER, slope change rate and tumor maximum diameter reduction rate after chemotherapy(r=0.637, 0.396, 0.532,P<0.05). Conclusions DCE parameters and ADC values possess important clinical value to evaluate the early effect of chemotherapy in NSCLC. ADC value is more sensitive to early lung cancer response.
9.Value of APACHEⅡfor predicting the nursing workload in EICU

Objective To investigate the predictive value of acute physiology and chronic health evaluation Ⅱ(APACHE Ⅱ) for nursing workload in emergency intensive care unit (EICU).Methods A retrospective cohort study was conducted. Critical patients except for trauma admitted to EICU of West China Hospital of Sichuan University from June 2012 to July 2016 were enrolled. APACHE Ⅱ score and therapeutic intervention scoring system (TISS-28) score were calculated in each case. The relationship between APACHE Ⅱ score and TISS-28 score was analyzed by correlation analysis and curve estimation.Results A total of 1148 patients were enrolled, including 689 male (60.0%), and age of (56±19) years old, APACHE Ⅱ score was 19 (13, 25), and TISS score was 19 (13, 25). The Spearman correlation coefficient between APACHE Ⅱ score and TISS-28 score was 0.849 (P < 0.001), and that among the linear model quadratic model, composite model, growth model, logarithm model, cubic model and exponential modelR2 were 0.669, 0.710, 0.792, 0.764, 0.765, 0.765, respectively (allP < 0.01), and the most suitable description for relationship between APACHE Ⅱ score and TISS-28 score curve was cubic model. The nursing workload was significantly increased when APACHE Ⅱ score ≥30, and it was decreased when APACHE Ⅱ score < 30.Conclusion APACHE Ⅱ was a useful score for predicting the nursing workload in EICU.
10.Low Molecular Weight Heparin's Interference with Immunocytes' Adhesion to Fibroblast-like Synovocyte (FLS) from Patients with Rheumatoid Arthritis(RA):An in Vitro Study

OBJECTIVE:To study the effect and thepossible mechanism of low molecular weight heparin(LMWH)on immunocytes' adhesion to fibroblast-like synovocytes(FLS)isolated from patients with rheumatoid arthritis(RA)and to study LMWH'S possible anti-inflammatory effect on RA.METHODS:LMWH's interference on the adhesion of peripheral blood monouclear cells(PBMC)isolated from healthy volunteers to FLS of RA patients was determined by quantitative counting using flow cytometry.The expression of CYR61 in the in vitro cultured FLS of RA patients was detected using real-time PCR technology.RESULTS:When FLS culture system was added with PBMC,PBMC were obviously found to adhere to FLS,but the number of adhered PBMC decreased after LMWH treatment,which manifested as increase of deciduous PBMC,increased more with the increase of LMWH dose.There was a high expression of CYR61 in synovium tissue in RA patients.CONCLUSION:LMWH inhibited the adheresion of PBMC to FLS from RA patients in a dose-dependent manner,which might be attributed to its competitive combination with heparin sulfate sites on CYR61.
