1.Diagnosis and treatment experiences of 198 cases of acute gastroduodenal perforation

Objective To discuss the etiological factor,diagnosis and treatment of gastroduodenal perforation.Methods The clinical datas of 198 cases in our hospital were submitted to a retrospective study.Results There were 44 cases of gastric ulcer,146 cases of gastric ulcer and 8 cases of gastric cancer,while 192 were cured,6 died.196 cases accepted operation(188 with gastroduodenal neoplasty and 8 with subtotal gastrectomy),while 2 were treated by conservative therapy.Conclusion Peptic ulcer is the principal etiological factor of gastroduodenal perforation,and inject air into gastroinotestinal tract with a gastric tube can increase the diagnostic rate.The neoplasty still remain to be a modus operandi of gastroduodenal perforation.

As a treasure of the traditional culture of China, TCM can be developed in long-term, that need the inheritance and innovation of scholars. During the process, it is an important responsibility and obligation that the development of TCM hospital. The development and improve of the hospital scientific research are important aspects for the development of hospital. The construction of scientific research team is an important approach to promote the hospital scientific research strength. By analyzing the whole current situation of hospital scientific research team, we will discuss the measures to strengthen the construction of hospital scientific research team from several aspects. These aspects include the system of scientific research team, talented person echelon structure, management system and the team overall strength, etc. These measures are aimed to strengthen the team construction characteristics of TCM, promote the scientific research level of TCM, contribute to the development of the career and hospital.
4.The application of serum sialic acid test in glioma diagnosis

Objective:To explore the clinical value of serum sialic acid (SA) for brain glioma diagnosis.Methods:Sixty patients with brain glioma and 60 normal persons (control group) in Deqing People's Hospital from January 2016 to September 2018 were collected, and all the brain glioma patients were divided into low grade group and high grade group according to the pathological grading. The serum SA levels of all the subjects were detected by photocolorimetric method, and the results were analyzed statistically.Results:The positive rate of SA in control group, low grade group and high grade group was 1.7%(1/60), 59.1%(13/22) and 81.6%(31/38), and there was significant difference ( χ2= 6.49, P<0.05). The positive rate in high grade group was higher than that in low grade group and control group, the positive rate in low grade group was higher than that in control group, and there was significant difference ( P<0.05). The level of SA in control group, low grade group and high grade group was 50.9(46.1-58.1), 55.6(48.2-60.9), 64.8(57.9-73.9) mg/L, and there was significant difference ( H= 15.56, P<0.05). The level of SA in high grade group was higher than that in low grade group and control group, the level of SA in low grade group was higher than that in control group, and there was significant difference ( P<0.05). After operation, the level of SA was decreased. At 6 months after operation, the level of SA in low grade group and high grade group was significantly lower than that before operation [low grade group: 50.4 (48.0-59.1) mg/L vs. 55.6(48.2-60.9) mg/L; high grade group: 60.1 (46.0-62.4) mg/L vs. 64.8(57.9-73.9) mg/L] ( P<0.05). At 6 months after operation, the level of SA in high grade group was significantly higher than that in control group [60.1 (46.0-62.4) mg/L vs. 50.9(46.1-58.1) mg/L](P<0.05), but in lower grade group, there was no significant difference [50.4 (48.0-59.1) mg/L vs. 50.9(46.1-58.1) mg/L]( P>0.05). Conclusions:Serum SA can be used in auxiliary diagnosis and prognosis evaluation for brain glioma.

Objective Based on Total Quality Management (TQM) theory,our study aims to analyze the clinical Bio-Bank overall quality management implications,basic characteristics,principles,and management mechanisms,and provide theoretical basis for the clinical Bio-Bank quality construction.Methods Using theoretical and literature research methods,Bio-Bank overall quality management qualitative analysis was conducted,putting forward a framework of Bio-Bank comprehensive quality management.Results Biological sample overall quality management was defined theoretically including its connotation,concepts and basic characteristics.We also put forward an application principle and basic operation method at the application level.Conclusions Total Quality Management (TQM) is applied to the clinical Bio-Bank construction,from where,the scientific and unique management content can effectively optimize the Bio-Bank management regulation and standardization of the sample operation process in the PDCA cycle,which is critical to improve the quality of clinical Bio-Bank.
6.Influence of length of preserved ileocecum on the efficacy of laparoscopic subtotal colectomy antiperistaltic cecorectal anastomosis in the treatment of slow transit constipation.

OBJECTIVETo investigate the influence of length of preserved ileocecum on the efficacy of laparoscopic subtotal colectomy antiperistaltic cecorectal anastomosis (LSCACRA) in treating slow transit constipation (STC).
METHODSClinical data of 81 STC patients who received LSCACRA between April 2007 And December 2011 in the 150th Center Hospital of PLA were continuously collected. Patients were divided into two groups: 10 cm to 15 cm ascending colon preserved above ileocecal junction(10-15 cm group, n=41), and 2 cm to 3 cm ascending colon preserved above ileocecal junction (2-3 cm group, n=40). The Wexner constipation scale (WCS), Wexner incontinence scale(WIS), gastrointestinal quality of life index(GIQLI), abdominal pain intensity scale(NRS), abdominal pain frequency scale and abdominal bloating frequency scale in the two groups were determined and compared before and 6, 12, 24 months after operation.
RESULTSNo postoperative incontinence was found in all the patients. There were no significant differences in evacuation frequency between two groups at 6th and 12th month after surgery (all P>0.05). Two years after operation, barium enema emptying time examination revealed 2-3 cm group was (17.7±9.5) h, which was remarkably shorter than (21.2±20.7) h in 10-15 cm group (P=0.011). The WCS, GIQLI, NRS and abdominal pain frequency scale of two groups were improved obviously at 6th, 12th and 24th month after surgery (all P<0.01). Above parameters in 2-3 cm group were superior to 10-15 cm group (all P<0.01), but abdominal bloating frequency scale was not significantly different between the two groups (P>0.05). As compared with before operation, NRS in 2-3 cm group 6, 12, 24 months after operation reduced remarkably (all P<0.01), but did not improve obviously in 10-15 cm group (P>0.05).
CONCLUSIONThe shorter length of ascending colon preserved above ileocecal junction can improve the efficacy of LSCACRA in the treatment of STC and the prognosis of patients. Two to three cm length of ascending colon preserved above the ileocecal junction should be recommended.

7.SWOT analysis and countermeasures of TCM development in China against the background of artificial intelligence

With the rapid development of information technology, artificial intelligence technology (AI) and how to use it have become the focus of current researches. The application of AI in the field of TCM has shown its uniqueness. The combination of artificial intelligence technology and traditional Chinese medicine provides a new direction and idea for the development of TCM. This paper analyzes the strengths, weaknesses, opportunities and threats (SWOT) of TCM development against the background of AI technology in China by SWOT analysis method. Based on these analyses, this paper puts forward some counter measures such as protecting personal information, avoiding negative effects and medical regulations etc. In order to facilitate and guarantee the development of TCM, we should take advantage of the AI, and avoid its disadvantages.
8.Epidemiological characteristics and outbreak analysis of varicella in Huai’an City in 2013-2022

Objective To analyze the epidemiological characteristics of varicella in Huai'an City, Jiangsu Province from 2013-2022, and to provide a scientific basis for formulating effective prevention and control strategies for varicella in Huai'an City. Methods Descriptive epidemiological methods were used to analyze the data of 2013-2022 varicella cases in Huai'an City from China Disease Prevention and Control Information System and the varicella epidemic data from the public health emergency reporting management information system. Results A total of 32 325 varicella cases were reported during 2013-2022. The overall incidence rate showed an upward trend, with an obvious seasonal high incidence in winter and spring. The incidence rate in the central urban areas of all districts and counties was relatively higher. The ratio of male to female in reported cases was 1.16:1. The most cases were people aged 5-9 years old, accounting for 39.43% of the total number of cases. In terms of occupational distribution, students predominated, accounting for 54.77% of the total number of cases. From 2013 to 2022, a total of 70 public health outbreaks of varicella were reported in Huai'an, with the majority (45) occurring in primary schools, accounting for 64.29% of the total. Conclusion After the introduction of varicella into the national planned immunization program in 2023, it is recommended to increase the publicity of vaccination, carry out universal vaccination of varicella vaccine for high-risk groups such as students, and particularly increase the second dose coverage of varicella vaccine.
9.Application of Non-pharmacotherapy Treating Cervical Radiculopathy in Real-world Study

This study was aimed to analyze the application of non-pharmacotherapy in treating cervical radiculopathy (CR) in real-world,and to provide clinical reference for CR non-pharmacotherapy.The clinical real-world data of CR was extracted by using information sharing system of traditional Chinese medicine (TCM) and clinical research.Six hundred and twenty-eight inpatients and outpatients with CR were enrolled from December 2012 to July 2014 in the information system database of Wangjing Hospital.Basic characteristics of the non-pharmacotherapy groups were analyzed by statistical description method.The node degree and mutual information value were recorded for non-pharmacotherapy application of all patients by using liquorice software.Complex network diagrams were generated.The results showed that 47% of CR patients received non-pharmacotherapy (294/628),including 67 males and 227 females.The average age of patients was 49 years old,and the prevalence of the disease was the highest from 45 to 65 years old.In all patients,the usage of manipulation and cervical traction was higher,and the combination of manipulation and acupuncture was the most.Within outpatients,the proportion of cervical traction was higher,and the combination of manipulation and acupuncture was the most frequently.Within inpatients,the proportion of manipulation and cervical traction was higher,and the combination of comprehensive physical therapy and exercise therapy was the most frequently.It was concluded that non-pharmacotherapy has been commonly used in clinical treatment of CR.Cervical traction and manipulation was the widest applications.The combination treatment was in wide application.Future studies should increase the sample size of CR patients from different regions,and enhance gradually the level of evidence of clinical research for non-pharmacotherapy treating CR.
10.Clinical Application of Chinese Patent Medicine Treating Cervical Radiculopathy in Real-world Study

Objective: Analyze the clinical application rule of Chinese patent medicine in cervical radiculopathy (CR) .Method: The clinical real-world data of CR were extracted by using information sharing system of traditional Chinese medicine and clinical research. Six hundred and twenty-eight inpatients and out patients with CR were enrolled from December 2012 to July 2014 in the information system database of Wangjing Hospital. The correlation analyses and mutual information value were recorded for Chinese patent medicine therapy application of all patients by using liquorice software. Complex network diagrams were generated. Result: Yuxuebi capsule and Jingshu granule is the highest frequency in application of Chinese patent medicine. Jingshu granule and Cobamamide for injection were the highest frequency in combined application of Chinese patent medicine and western medicine. Association frequency was 822, mutual information value was 268.07. Biqi capsule and Daiwenjiu cream were the highest frequency in combined application of Chinese patent medicine and topical drugs. Association frequency was 384, mutual information value was1754.76. Conclusion: The basic treating principle for CR was promoting blood circulation and removing blood stasis. The efficacy and safety of combined Chinese patent medicine with other medicine need further research
