1.The different effects of gastric banding and sleeve gastrectomy on obese rat

Objective To evaluate the different effects of gastric banding (GB) and sleeve gastrectomy (SG) on diet-induced obese rat model.Methods The obese rat model was established via the diet-induced method,and then underwent SG,GB,or sham-operation (SO) procedures.The different effects of SG,GB,and SO on these rats were evaluated at the 14th,28th and 56th day after operation.Results After 7 weeks' feeding,31rats in high-diet group reached the modeling standard.The model success rate was 88.6% (31/35).GB and SG group showed significantly less cumulative food intake than SO group ( P < 0.01).There was no significant difference between GB and SG group in cumulative food intake ( P > 0.05 ).The average weight of rats in GB and SG group was (609.1±6.0) g and (591.6 ±4.3) g respectively at the 56th day after operation while it was (649.8 ± 10.0) g for SO group.The difference had statistical significance ( P < 0.01).SG group showed better improvement effects on TC,TG,FFA,LDL-c and HDL-c than GB group (P <0.05).The postoperative ghrelin level in GB and SG group all decreased constantly and it decreased slower in GB group than in SG group.At the 56th day after operation,the ghrelin level in GB and SG group reduced from (1151.0 ± 144.2) pg/ml and (1148.2±127.2) pg,/ml to (992.0+121.9) pg/ml (P<0.05) and (761.0+125.3) pg/ml (P<0.01) respectively.The leptin level in GB and SG group all showed decreasing tendency.However,the leptin level of the 2 groups presented significant difference at the 56th day after operation.Conclusions There is no significant difference between GB and SG group in postoperative cumulative food intake.However,SG has better effects on controlling weight gain and improving blood lipids than GB.The different effects of GB and SG on the level of ghrelin and leptin in rats may account for the different curative effects.
2.Effect analysis of posterior internal fixation and fusion on patients with spinal fractures

Objective To analyze the curative effect of posterior internal fixation and fusion on patients with spinal fractures, and to provide the basis for clinical application. Methods A total of one hundred and eighty?eight patients with orthopedic incision treated in the First Affiliated Hospital of Xinxiang Medical University from March 2013 to December 2016 were divided into the observation group ( 98 cases with posterior internal fixation fusion ) and the control group ( 90 cases with anterior internal fixation fusion ) . The clinical curative effects of the patients ( including Cobb angle,spinal nerve function,complications and the success rate of surgery ) , postoperative visual analogue score ( VAS ) , hospitalization time, operation time were compared. Results In the observation group,the VAS score before operation was (6. 89±6. 89) points and reduced to (2. 70±1. 33) points after operation,the pain decreased significantly after treatment (t=26. 099,P<0. 001). The VAS score of the control group lowered from (6. 85±6. 85) points to (5. 05±1. 35) points,and the pain decreased significantly after treatment ( t=10. 704, P<0. 001 ) , and the postoperative pain in the observation group was significantly reduced than that in the control group ( t=12. 016,P<0. 001) . There were significant differences between the observation group and the control group in operation time,intraoperative blood loss,incision length and hospitalization time ( ( 141. 91 ± 35. 42 ) min vs. ( 198. 82 ± 38. 61 ) min, ( 509. 72 ±113. 65) ml vs. (823. 40±108. 54) ml,(10. 25 ± 0. 84) cm vs. (18. 83±1. 58) cm,(10. 59±12) d vs. (15. 37±1. 52) d,t=10. 541,19. 315,47. 015,24. 681,P<0. 001) . The total effective rate of the observation group was 86 cases ( 97. 96%) , and ineffective rate of the observation group was 2 cases ( 2. 04%) . In the control group, the effective and ineffective rates were 75 cases ( 83. 33%) and 15 cases ( 16. 67%) , the difference in effective rate between the two groups was statistically significant (χ2 = 12. 202, P<0. 001 ) . Conclusion Posterior internal fixation fusion is effective in the treatment of spinal fracture,which can reduce the postoperative pain,shorten the hospitalization time,less complication and high success rate. It is suitable for clinical promotion.
3.In vitro and vivo studies on features of rubella virus JR_ 23 strain in the infection of central nervous system

Objective To investigate the infectious features of rubella virus (RV) JR 23 strain in central nervous system (CNS). Methods RV JR 23 strain infected human primary cerebral neural cell culture in vitro and Balb/c mice, which were given dexamethasone and cytoxan before infection, via peritoneal injection. Viral pathogenecity was observed postinfection and RV antigens were detected in human cerebral neural cells by IFA and immunohistochemical method. Cerebral tissues were observed by HE staining and ABC methods postinfection.Results JR 23 strain didn't produce cytopathic effect (CPE). The proliferation of JR 23 strain reached highest titer of 10 3TCID 50 /ml at 72 h postinfection and decreased gradually. RV antigens were positive in cerebral neural cells, especially around the nuclei. Focal cytopathic areas were observed in cerebral cortical area and so did neuron necrosis around by gliacytes formed stellitorsis, neuronophagia and glial nodule. RV antigens could be seen in all cerebral area, but most localized in cortical area. Pathological features were basically the same among the infected groups. The infection rates of de xamethasone, cytoxan group and the group without intervention were 60%、90% and 50%, repectively. Conclusion RV JR 23 strain is not cytocidal to human neural cells and the pathological lesions induced by JR 23 strain in CNS of mice are mainly focal or dotted neuron necrosis.
4.Effects of tadenanR treatment on urinary bladder outlet dysfunction in rabbits

Objective To study the protective effect of tadenanR on the bladder contractile function following partial outlet obstruction. Methods 20 New Zealand white male rabbits were divided into 4 groups of 5 rabbits each. Control group received operation but not forming obstruction model where as other groups received operation forming obstruction model.Two weeks after operation, tadenanR group received tadenanR orally at 100 mg?kg -1 ?d -1 for three weeks; Peanut oil group received vehicle only (peanut oil) and the obstruction group did not receive any drugs for three weeks. Five weeks after operation, the weight and volume of each bladder was assessed, and 3 longitudinal strips(1.0 cm?0.5 cm) of each bladder musculature were mounted in individual baths including Thyrode's buffer?atropine?ATP?Ca 2+ -free and high K+ solusions for contractile study. The ultrastructure of detrusor cells was observed with electron-microscope. Results tadenanR did not reduce the bladder weight after partial outlet obstruction but had protective effect on the capacity of bladder.tadenan*R treatment resulted in a protective effect on the contractile response to field stimulation in all kinds of solutions when contrasted with obstruction group and peanut oil group. It also reduced the enlargement of endoplactmic reticulum and mitochondrion as compared with obstruction group and peanut oil group. Conclusions tadenanR has direct effect on prostate through the inhibition of ?-FGF and it also has a protective effect on bladder function.So it is preferable in the treatment of BPH.
5.The clinical observation of duodenal catheter decompression and intubation by the jejunum ostomy duodenal decompression in the treatment of duodenal trauma

Objective To observe the clinical effect of treatment of duodenal trauma intubation duodenal decompression jejunostomy jointly by the duodenal catheter decompression.Methods 60 patients with duodenal trauma were randomly divided into two groups,30 cases were treated with simple duodenal catheter decompression as control group,30 cases treated with duodenal catheter decompression joint jejunum ostomy intubation duodenal decompression as the observation group,generally observed two groups of patients surgery and postoperative complications,in the day of surgery and postoperative 7d evaluation of patients with nutritional status.Results The surgical time and blood loss of the two groups had no statistically significant difference(P>0.05),the observation group had no case of duodenal fistula and four cases in the control group(13.33%)occumed duodenal fistula.The difference between the two groups was statistically significant(x2=4.286,P<0.05).Postoperative concurrent abdominal abscess,lung infection,wound infection had no statistically significant differences(P>0.05).After observation group 7d body mass and right upper arm muscle circumference stability was significantly higher than that in the control group(x2=7.519,9.868,both P<0.05).Conclusion Jejunostomy joint duodenal catheter decompression intubation duodenal decompression in treatment of duodenal trauma surgical decompression had significant effect,can significantly reduce the incidence of fistula,improve patients nutritional status,promote the duodenal wound healing.
6.Efficacy of dexmedetomidine used to supplement lumbar plexus combined with sciatic nerve block for total hip arthroplasty in elderly patients

Objective To evaluate the efficacy of dexmedetomidine used to supplement lumbar plexus combined with sciatic nerve block for total hip arthroplasty in the elderly patients.Methods Fifty ASA Ⅱ or Ⅲ patients,aged 80-93 yr,with a body mass index of 17-25 kg/m2,scheduled for unilateral total hip arthroplasty,were randomly divided into 2 groups (n =25 each):normal saline group (group NS) and dexmedetomidine group (group D).Lumbar plexus combined with sciatic nerve block was performed guided by a nerve stimulator in both groups.After a loading dose of dexmedetomidine 0.5 μg/kg was infused over 10 min starting from 20 min after the block,dexmedetomidine was infused at 0.2 μg· kg-1 · h-1 until the end of surgery.While the equal volume of normal saline was given instead of dexmedetomidine in group C.Propofol 20 mg or ketamine 10 mg was infused intravenously when the efficacy of the block was inadequate.The mean arterial pressure (MAP),heart rate (HR),pulse oxygen saturation (SpO2),respiratory rate (RR),side effects during operation,requirement for propofol or ketamine and doctor' s satisfaction were recorded after admission to the operating room,immediately after iv administration,at 0,10 and 30 min after the beginning of surgery,and at the end of surgery (T0-5).Results Compared with the baseline value at T0,MAP and HR were significantly increased at T2-5 in group C (P < 0.05),and no significant change was found in the hemodynamic parameters at each time point in group D (P > 0.05).Compared with group C,the MAP,HR,incidences of respiratory depression and agitation and requirement for propofol or ketamine were significantly decreased,and the doctor's satisfaction was significantly increased (P < 0.05).Conclusion Dexmedetomidine can increase the efficacy of lumbar plexus combined with sciatic nerve block when used for supplementation of total hip arthroplasty in the elderly patients,the hemodynamics is stable and side effects are fewer.
7.Transureteroscopic pneumatic lithotripsy in the treatment of ureteral calculi(report of 116 cases)

Objective To evaluate transureteroscopic pneumatic lithotripsy in the treatment of ureteral calculi. Methods 116 cases of ureteral calculi treated with ureteroscopic pneumatic lithotripsy from Dec.1999 to Feb.2002 were studied retrospectively.There were 49 male patietns and 67 female with an average age of 43,3 being upper ureteral stone,24 middle and 68 lower ureteral stone.21 ureteral stone-street. Results The stone fragmentation rate was 90%(104/116),85%(99/116) of which being fragmented on the first treatment session.The main complications were failure to insert the ureteroscope in 3 cases and ureteral perforation in 4.94 cases were follow-up for one to eighteen months and 91 cases were stone free within one month.No ureteral stricture has been observed on intravenous urogram. Conclusions Treatment of ureteral calculi by transureteroscopic pneumatic lithotripsy has been safe, effective and less invasive.
8.A study of botulinum atoxin and uitrashort wave treatment of trigeminal neuralgia

Objective To investigate clinical efficacy about treatment of trigeminal neuralgia using botulinum toxin of type A and ultrashort wave.Methods 40 patients with trigeminal neuralgia were randomly divided into the control group(n =20)and the treatment group(n =20)according to digital table method,patients in the control group were injected with lidocaine,triamcinolone acetonide and vitamins B12 injection in pain -sensitive point,the treatment group were injected botulinum toxin of type A 50 -150 u in pain -sensitive point.The two groups were in the ultrashort wave therapy.Used VAS pain scores to evaluate local pain about two groups of patients before treatment,one week after treatment,one month after treatment,three months after treatment,the clinical efficacy after treatment at different time points were compared between the two groups.The two groups of patients showed sleep improvement,the sleep status of patients were observed.Results The VAS score in the control group 1 week after treatment was (3.78 ± 1.35)poinrs,and was (7.03 ±1.07)poinrs before treatment,the difference was statistically significant(t =-3.39, P <0.05);The VAS scores at 1 month and 3 months after treatment were respectively (6.32 ±1.08)poinrs and (7.45 ±1.03)poinrs,the data had no statistical significance as compared with that before treatment (P >0.05). After treatment for 1 week,the scores of VAS in the treatment group was (5.18 ±1.03)poinrs,which was decreased compared with that before[(7.63 ±1.04)poinrs],the difference was statistically significant(t =4.23,P <0.05). After treatment for 1 month and 3 months,the VAS scores were respectively (4.01 ±1.78)poinrs and (2.95 ± 1.34)poinrs,significantly lower than that before treatment,the differences were statistically significant(t =4.97, 5.34,all P <0.05).After treatment for 1 week,differences of VAS scores in the two groups showed no statistical significance(P >0.05).After the treatment for 1 month and 3 months,VAS score differences were statistically signifi-cant(t =7.56,11.61,all P <0.05).In the control group,after treatment for 1 week,1 month and 3 months showed significant efficiency of 77.5%,33.3% and 11.1% respectively,which showed a downward trend.In the treatment group,after treatment for 1 week,1 month and 3 months,the efficiency were 57.7%,69.2% and 80.7%,which was a rising trend.And the efficiency was significantly higher than that of the control group,the differences were statistically significant(χ2 =5.16,17.99,P <0.05).Conclusion The treatment of trigeminal neuralgia using botulinum toxin of type A with ultrashort wave can significantly reduce pain in patients,the effect is better than lidocaine,triamcinolone acetonide and vitamin B12 .
9.The protective effect of somatostatin combined with growth hormone in treatment of intestinal mucosal barrier injury in rabbits with severe acute pancreatitis

Objective To investigate the protective effect of somatostatin (SS)combined with growth hormone (GH) in treatment of intestinal mucosal barrier injury in rabbits with severe acute pancreatitis (SAP), as well as its clinical significance. Methods Seventy-two rabbits were equally assigned into model group (SAP group), SS treated group (SS group) and SS combined with GH treated group (SS + GH group). SAP models were induced by retro-injection of 5% sodium taurocholate into the pancreatic duct. After modeling, all rabbits were given 5 % glucose saline daily.The rabbits in SS group and SS+GH group were continuously Given SS (3.5μg·kg-1·h-1)for 48 hours. Besides, the rabbits in SS+GH group were subcutaneously injected with 0.15 IU/kg of GH at the 1st and the 24th hours after modeling. The levels of serum amylase, serum tumor necrosis factor-α (TNF-α) and plasma diamine oxidase were measured at the 6th, 12th, 24th and 48th hours after modeling. The pathological changes of pancreatic tissue and ileal mucosa were observed. Survival rate was calculated. Data were analyzed using SPSS 16.0 software. The univariate analysis was used to compare the difference among groups. Results In SS+GH group, the levels of serum TNF-α and plasma diamine oxidase were (2. 43 ± 0. 14) pg/ml and (4. 61 ± 0. 45) U/L at the 24th hour respectively, and were (2.08±0.23) pg/ml and (3.75±0.47) U/L at the 48th hour, respectively,which were lower than those in SAP group and SS group [(2.80 0.30) pg/ml and (8.74 ± 1.77)U/L, respectively, at the 24th hour; (2. 45±0.12) pg/ml and (5. 02±0.95) U/L, respectively, at the 48th hour)]with significant difference (P<0.05). The inflammation in pancreas and ileal mucosa was alleviated, and the integrity of bowel mucosa was improved. Survival rate of SS+GH group was significantly higher than SAP group and SS group. There was no significant difference in level of serum amylase between SS+GH group and SS group. Conclusion The combination of SS with GH may enhance the function of intestinal mucosa barrier and improve the prognosis of SAP in rabbits.
10.Semi-quantitative reverse transcription-PCR assay for renal cell carcinoma-associated protein G250 mRNA

Objective To evaluate the expression of G250 mRNA in patients with renal cell carcinoma (RCC) and its clinical significance. Methods A semi-quantitative reverse transcription (RT)-PCR was developed to assay the levels of G250 mRNA obtained from 7 normal renal tissues and 33 cases of RCC.Of the 33 RCC cases,28 had clear cell carcinoma and 5 had non-clear cell carcinoma.By UICC staging, 8 cases were diagnosed with T_1 stage carcinoma,13 with T_2 and 12 with T_3.Pathological grading showed 11 cases of G_1,15 of G_2,and 7 of G_3. Results Among the 33 cases of RCC,G250 mRNA expression was present in 28 cases (84.8%).However, it was absent in all the 7 normal renal tissues.There was a significant difference between the 2 groups (P0.05);while they were 0.48?0.32,0.23?0.16 and 0.11?0.11,respectively,in RCC cases of G_1,G_2 and G_3,which showed that the expression of G250 mRNA was inversely correlated with tumor grades(P
