Objective To investigate the effect of electro-acupuncture (EA) on vascular permeability in patients undergoing open-heart surgery with CPB. Methods Thirty-two NYHA class Ⅰ or Ⅱ and ASA class Ⅱ or Ⅲ patients with BMI of 19-28 kg/m2 undergoing open heart surgery with CPB were randomly divided into 2 groups ( n = 16 each): Ⅰ control group and Ⅱ acupuncture group . The patients were premedicated with intramuscular phenobarbital 0.1 g, pethidine 50 mg and scopolamine 0.3 mg. Anesthesia was induced with midazolam, propofol, fentanyl and vecuronium and maintained with TCI of propofol and fentanyl in both groups. The patients were acupuncture points (neiguan, lieque, yunmen) were selected for stimulation with stimulator. Acupuncture stimulation was started from 30 min before anesthesia induction and continued until the end of operation. The level of sedation was monitored by BIS and maintained at 40-60 during operation. Urinary microalbumin concentration was used as a parameter of vascular permeability, and was measured before and at the end of operation. Blood samples were taken 24 h before and 24 h after operation for measurement of ALT, AST, total bilirubin, BUN and Cr. Results The two groups were comparable with respect to sex ratio, age, body weight, ASA physical status, and preoperative ejection fraction. There was no significant difference in renal and liver function between the 2 groups.Postoperative urinary microalbumin concentration was significantly lower in acupuncture group than in control group. Conclusion Acupuncture can reduce the effect of open heart surgery performed under CPB on vascular permeability.

Severe combined immunodeficiency(SCID)is a kind of severe primary immunodeficiency disease.Patients often develop symptoms after birth, which is characterized by recurrent and life-threatening infection, sometimes accompanied by varying degrees of dysplasia.According to the classification proposed by the PID expert Committee of the International Union of Immunological Societies, SCID is clinically classified by the number of lymphocytes.Early screening and intervention of SCID is significant to improve the prognosis. With the promotion of TREC and other early screening methods, the detection rate of SCID has significantly improved, but the accurate detection rate in different ethnic groups still needs further explored.Hematopoietic stem cell transplantation is a traditional therapy for radical cure of SCID.And the safety of gene therapy for SCID has been gradually guaranteed with continuous improvement during recent years. This article will review the classification, early screening methods and treatment progress of SCID.

Objective To investigate the serum level of visfatin, C-reactive protein (CRP) and monocyte chemoattractant protein-1 (MCP-1) in patients with polycystic ovarian syndrome (PCOS), and provide the basis for understanding the pathogenesis of PCOS. Meth-ods 94 patients with PCOS and 100 healthy subjects were divided into 4 subgroups according to their body mass index(BMI), including obese PCOS subgroup(n = 52), non - obese PCOS subgroup (n = 42), obese healthy subject subgroup (n = 43) and non - obese healthy subject (n = 57). Serum visfatin, CRP, MCP-1, sex hormone levels and metabolic parameters were measured by enzyme-linked immu-nosorbent assay (ELISA), automatic chemistry analyzer or chemiluminescent immunoassay. Results Serum levels of testosterone (T), lu-teiizing hormone (LH) and prolactin (PRL) in the PCOS group were significantly higher than those in healthy subjects group(P <0.05~0.01), and follicle-stimulating hormone (FSH) levels were significantly decreased in PCOS group(P<0.01). Serum levels of Visfatin, CRP, MCP-1,fasting insulin(Fins) and insulin resistance homa model (HOMA-IR) in the obese or non - obese PCOS subgroup were signif-icantly increased than that in the obese or non - obese healthy subjects subgroup respectively (P<0.01). Pearson correlation analysis showed that the visfatin, CRP and MCP-1 levels were positively related to BMI, FINS and HOMA-IR(r=0.323~0.675, P<0.01). Par-tial correlation showed that serum visfatin levels were correlated with HOMA-IR(r=0.491, P<0.01), and serum MCP-1 levels were cor-related with LH (r=0.267, P<0.05) in the PCOS group. Conclusion The patients with PCOS had higher visfatin, CRP and MCP-1 lev-els, and visfatin levels were positively correlated with insulin resistance. Obesity were involved in the chronic inflammation course in patients with PCOS.

Objective To discuss the occurrence of thrombosis in portal system of patients with cirrhotic portal hypertension after devascularization and the methods for treatment and prevention. Methods The clinical data of 113 patients with cirrhotic portal hypertension after the devascularization were collected and analyzed retrospectively and the occurrence time parts as well as the treatment and prevention methods were discussed. Results Among the 113 patients 33 of them were found with thrombosis in their portal system and the occurrence rate was 29.2%. The occurrence time of thrombosis was 2?15 days post?de?vascularization and the median time was 6 days post?operation. Among the 33 cases with thrombosis there were 19 cases of splenic vein thrombosis 10 cases of portal thrombosis and 4 cases of both of them. After the thrombolytic therapy the thrombo?sises in 30 cases disappeared. Conclusions Most of the thrombosises in portal system happen in splenic vein post?devascular?ization. Avoiding clamping the trunk of splenic vein in the operation and taking thrombolytic therapy at the early stage after the operation can effectively prevent the occurrence of thrombosis.
