1.Application of island flap pedicled with dorsal cutaneous branches of digital internal artery for severe thermos injury of fingers

Objective To investigate the therapeutic effect of island flap pedicled with dorsal cutaneous branches of digital inter-nal artery for severe thermos injury of fingers .Methods From May 2008 to May 2012 ,15 fingers in 12 cases with severe thermos injury were treated with island flap pedicled with dorsal cutaneous branches of digital internal artery .The defect size ranged from 1 .5 cm × 2 .0 cm to 2 .2 cm × 3 .0 cm ,4 fingers with anterograde flap repair ,11 fingers with retrograde flap repair .Results Partial necrosis happened at the distal end of 2 island flaps ,All the other flaps survived completely .The patients were followed up for 6-12 months with satisfactory cosmetic and functional results ,cicatricial contracture around the skin graft in 3 fingers ,The two-points discrimination distance was 9-11 mm in flaps .Conclusion These flaps have the advantages of an extended skin paddle and a versa-tile pivot point on the phalanx ,and they allow coverage of wide and distal defects .

Early diagnosis is one of the difficulties in strangulated intestinal obstruction.Serological markers for the early diagnosis of intestinal obstruction has become a hot spot resently.Studies have shown that histidine decarboxylase had a high sensitivity and specificity in the diagnosis of gastric mucosal injury of intestinal obstruction,which would be expected to become an effective serological marker for the early diagnosis of strangulated intestinal obstruction.In this review,the author will describe the advance of serum histidine decarboxylase level for diagnosis of strangulated intestinal obstruction.
4.Clinical Observation of Abdominal Acupuncture Combined with Chinese Medicine for Treatment of Qi-deficiency Functional Constipation

Objective To observe the therapeutic effect of abdominal acupuncture combined with Chinese medicine for the treatment of functional constipation. Methods Ninety-three qi-deficiency functional constipation patients were evenly randomized into western medicine group, Chinese medicine group and combination group. The western medicine group was given oral use of Dupbalac, Chinese medicine group was given modified Buzhong Yiqi decoction, and the combination group was given abdominal acupuncture and oral use of modified Buzhong Yiqi decoction. The treatment of the three groups covered 3 months. After treatment, the therapeutic effect was evaluated accord ing to the symptom scores and the examination results of gastrointestinal kinetics. Results ( 1) The total effective rate for relieving symptoms was 100.00% in combination group, 90.32% in Chinese medicine group, and 38.71% in western medicine group, and the effect was the best in the combination group ( P<0.05). ( 2) The combination group had stronger effect on relieving dyskinesis of anal sphincter, rectal sensibility, colonic transportation than Chinese medicine group and western medicine group (P<0.05). Conclusion Abdominal acupuncture combined with Chinese medicine shows certain therapeutic effect for the treatment of qi-deficiency functional constipation, and the effect is superior to that of Chinese medicine or Dupbalac.
5.Effect of amygdalin on end-plate chondrocytes apoptosis induced by IL-1βderived from rat intervertebral discs

Aim To observe the anti-apoptotic effect of different concentrations of amygdalin on the endplate chondrocytes induced by IL-1 βderived from rat inter-vertebral discs and explore the possible mechanism fur-ther.Methods Chondrocytes were obtained from endplate of one-month SD rat intervertebral discs and cultured primary chondrocytes.After identifying,they were divided into normal group,induced group and A-mygdalin 1 0 -2 mol·L -1 ,1 0 -3 mol·L -1 ,1 0 -4 mol· L -1 ,1 0 -5 mol · L -1 administration group.Then the apoptosis was detected by flow cytometry (FCM).Re-al-Time PCR was adopted to detect the mRNA expres-sion of Bax and Bcl-2.The protein expression of Bax and Bcl-2 was detected by Western blot.Results The apoptosis of the endplate chondrocytes induced by IL-1 βderived from rat intervertebral discs could be inhib-ited by amygdalin with different concentrations.Amyg-dalin could reduce the apoptotic rate analysed by FCM,down-regulate the Bax mRNA expression of Bax and up-regulate the Bcl-2 mRNA assayed by RT-PCR;compared with the induced group the differences were statistically significant (P <0.05).Besides,observa-tion of the protein expression of Bax and Bcl-2 by Western blot found that amygdalin 1 0 -4 mol · L -1 could inhibit the effect of IL-1 β,which could increase the protein expression of Bax and reduce the protein expression of Bcl-2.Conclusion Amygdalin has an effect on anti-apoptosis of the end-plate chondrocytes induced by IL-1 βderived from rat intervertebral discs and improve the degeneration of intervertebral discs.
6.Relationship of intracellular free Ca2+ concentration and calcium-activated chloride channels of pulmonary artery smooth muscle cells in rats under hypoxic conditions.

To investigate the relationship between intracellular free Ca2+ concentration ([Ca2+]i) and calcium-activated chloride (Clca) channels of pulmonary artery smooth muscle cells (PASMCs) in rats under acute and chronic hypoxic conditions, acute hypoxia-induced contraction was observed in rat pulmonary artery by using routine blood vascular perfusion in vitro. The fluorescence Ca2+ indicator Fura-2/AM was used to observe [Ca2+]i of rat PASMCs under normal and chronic hypoxic condition. The effect of Clca channels on PASMCs proliferation was assessed by MTT assay. The Clca channel blockers niflumic acid (NFA) and indaryloxyacetic acid (IAA-94) exerted inhibitory effects on acute hypoxia-evoked contractions in the pulmonary artery. Under chronic hypoxic condition, [Ca2+]i was increased. Under normoxic condition, [Ca2+]i was (123.63 +/- 18.98) nmol/L, and in hypoxic condition, [Ca2+]i was (281.75 +/- 16.48) nmol/L (P<0.01). Under normoxic condition, [Ca2+]i showed no significant change and no effect on Clca channels was observed (P> 0.05). Chronic hypoxia increased [Ca2+]i which opened Clca channels. The NFA and IAA-94 blocked the channels and decreased [Ca2+]i from (281.75 +/- 16.48) nmol/L to (117.66 +/- 15.36) nmol/L (P<0.01). MTT assay showed that under chronic hypoxic condition NFA and IAA-94 decreased the value of absorbency (A value) from 0.459 +/- 0.058 to 0.224 +/- 0.025 (P<0.01). Hypoxia increased [Ca2+]i which opened Clca channels and had a positive-feedback in [Ca2+]i. This may play an important role in hypoxic pulmonary hypertension. Under chronic hypoxic condition, Clca channel may play a part in the regulation of proliferation of PASMCs.
7.Expression and purification of Tp0319 recombinant protein of Treponema pallidum and its application in diagnosis of syphilis

Objective To clone,express,and purify Tp 0319 outer membrane protein of Treponema pallidum and to develop an indirect ELISA for diagnosing syphilis.Methods The expression plasmid PQE32/Tp 0319 was conventionally constructed.The recombinant Tp 0319 protein was produced in E.coli M15 after induction by IPTG.The Tp 0319 protein was analyzed by SDS-PAGE and Western blotting,and then purified with Ni-NTA affinity chromatography.Indirect ELISA was developed to detect the syphilis antibody in human sera.Results The recombinant plasmid PQE32/Tp 0319 was constructed successfully and the fusion protein with relative molecular weight near 30 000 Dalton was revealed by SDS-PAGE.Western blotting proved that the recombinant protein specifically reacted with anti-Tp antibodies in sera from syphilis patients.The results of the indirect ELISA indicated the sensitivity and the specificity were both 100%.The concordance of 300 sera(150 from blood donors and 150 from syphilis patients)detected in parallel by the ELISA and the TPPA was 95.3%.Conclusions The data suggest that the prepared recombinant protein Tp 0319 of Treponema pallidum has high immunoreactivity.The recombinant protein can be used to develop ELISA kit for diagnosing syphilis.
8.Effect of shenmai injection on L-type calcium current of diaphragmatic muscle in rats.

In this study, whole cell patch clamp recording technique was employed to investigate the effect of Shenmai Injection (SMI) on L-type calcium current of diaphragmatic muscle in rats. The result showed that when the diaphragmatic muscle cell was held at -80 mV and depolarized to +60 mV, 10 microl/ml, 50 microl/ml and 100 microl/ml SMI enhanced the inner peak L-type calcium current from -(6.8 +/- 0.7) pA/pF (n=7) to -(7.3 +/- 0.8) pA/pF (P>0.05, n=7), -(8.6 +/- 1.0) pA/pF (P<0.05, n=7) and -(9.4 +/- 1.2) pA/pF (P<0.05, n=7), respectively, The rates of L-type calcium current were increased by (7.34 +/- 2.37)%, (25.72 +/- 5.94)%, and (38.16 +/- 7.33)%, respectively. However, it had no significant effect on maximal activation potential and reversal potential. Our results suggested that SMI could activate the calcium channel of the diaphragmatic fibers of the rats, increase the influx of Ca2+, and enhance the contractility of diaphragmatic muscles.

9.Expression of heat stress protein 70 mRNA in patients with chronic obstructive pulmonary disease and its significance.

The effects of cigarette smoke extract (CSE) on the expression of heat stress protein 70 (Hsp70) in human bronchi smooth muscle cells were investigated in vitro, and the changes in Hsp70 mRNA in the patients with chronic obstructive pulmonary disease and their significance were explored. Human bronchi smooth muscle cells were cultured with CSE at the different concentrations. The expression of Hsp70 mRNA and Hsp70 was detected by reverse translation-polymerase chain reaction (RT-PCR) and Western blotting respectively. Levels of Hsp70 mRNA and Hsp70 in lymphocytes from 20 patients with COPD and 20 healthy smoking control subjects were measured by RT-PCR and Western blotting. The results showed the expression of both Hsp70 mRNA and Hsp70 was decreased conformably in human bronchi smooth muscle cells treated with CSE at certain concentration in vitro. The A values of the Hsp70 mRNA expression were 0.24 +/- 0.11 and 0. 42 +/- 0.13 respectively in COPD patients and healthy smoking controls with the difference being significant (P < 0.01). There was also significant difference in the A values of the Hsp70 expression between COPD patients and healthy smoking controls (20.9 +/- 9.9 vs 44.8 +/- 15.3, P < 0.01). The levels of Hsp70 mRNA had strongly positive correlation with Hsp70 protein (r = 0.85, P < 0.01). It was suggested that the expression of Hsp70 mRNA was in concordance with the expression of Hsp70, which could provide a basis on the study of Hsp70 gene regulation and Hsp70 gene in the development of COPD.

10.Application of Excimer Laser Coronary Atherectomy for Treating the Patients With Coronary Chronic Total Occlusion

Objective: To observe the safety and efifcacy of excimer laser coronary atherectomy (ELCA) for treating the patients with coronary chronic total occlusion (CTO). Methods: A total of 3 coronary CTO patients treated by ELCA in our hospital from 2015-01 to 2016-11 were analyzed. The patients received guide steel wire gone through occlusion segment under the guidance of offside coronary angiography; drug-eluting stent implantation was conducted after ELCA treatment. The operative success rate with complication was observed and the occurrence of MACE was followed-up. Results: The device performing and interventional therapy were succeed in all 3 patients. No coronary dissection, perforation, slow relfow and thrombosis were occurred during the operation; no angina, myocardial infarction, target vessel revascularization and death were observed by follow-up study. Conclusion: Application of ELCA combining drug-eluting stent implantation was safe and effective for treating the patients with coronary CTO.
