1.Analysis on curative effect of laparoscopic surgery in treatment of patients with gastric gastrointestinal stromal tumors with tumor diameter more than 5 cm

Objective:To investigate the feasibility,moderate-and short-term efficacies of laparoscopic surgery in the resection of the gastric gastrointestinal stromal tumors(GIST)with the tumor diameter more than 5 cm.Methods:The clinicopathological data of 26 gastric GIST patients with the diameter more than 5 cm underwent laparoscopic complete resection were retrospectively analyzed.The postoperative recovery,complications and moderate-and short-term prognosis of the patients were observed.Results:All the patients underwent laparoscopic surgery resection without open operation.According to the location and growth types of tumors,the total gastrectomy was performed in 1 case,the approximal gastrectomy was performed in 2 cases,the distal gastrectomy was performed in 2 cases,and the gastric local resection was performed in the rest 20 cases.The mean diameter of tumors was (5.94±1.28) cm,the mean operational time was (84.23±27.02) min,and the mean blood loss was (72.38±34.24) mL.The postoperative recovery time of bowel function was (2.77±0.65)d and the mean length of postoperative hospitalization time was (7.04±2.24) d.There was 1 case of intraperitoneal hemorrhage and 1 case of anastomotic stenosis;they recovered after conservative therapy.The postoperative pathology results(according to the modified NIH standard) indicated the number of moderate-risk patients was 18(69.23%),and the number of high-risk patients was 8(30.77%).A total of 12 cases accepted the adjuvant therapy with imatinib.There was 1 case of hepatic multimetastasis,and there was no local recurrence or death in the other patients.Conclusion:The moderate-and short-term efficacies of laparoscopic surgery in the gastric GIST patients with the tumor diameter more than 5 cm is safe and feasible.
2.Personality in patients with body-dysmorphic disorder

Objective To study the similarities and differences of personality in patients with body-dysmorphic disorder(BDD),obsessive-compulsive disorder(OCD) and ordinary person.As well as to provide information for clinical therapy. Methods All BDD patients( n =29),OCD controls( n =30) and normal controls( n =30) completed Self Rating Scale of Body Image (SRSBI) and Minnesota Multiphasic Personality Inventory (MMPI). Results Ten clinical scales score of MMPI in BDD were found significant higher than normal controls which including hypochondriasis,depression,hysteria, psychopathic deviate, masculinity-femininity, paranoia,psychasthenia,schizophrenia,and social introversion.Four clinical scales[(68.18±8.70),(65.44±8.73),(61.39±9.37),(60.70±12.88)] were significant higher than OCD controls[(61.09±13.29),(58.82±10.26 ),(56.23±9.58),(50.03±12.63)] which including psychopathic deviate,psychasthenia,schiaophrenia and social introversion( P <0.05). But hysteria scale was significant lower than OCD controls( P <0.01). Conclusion BDD patients have abnormal personality which is more significant than OCD patients. Significant abnormal personality are likely the psychopathology of BDD from which the clinic symptom produced.

4.Effect of p38 protein kinase on the activation of rat alveolar macrophages by lipopolysaccharide

AIM: To investigate the role of p38 protein kinase in the activation of rat alveolar macrophages(AMs) induced by lipopolysaccharide(LPS). METHODS: Nuclear protein was extracted, p38 protein kinase in nuclear extracts was analyzed by Western blot. The concentrations of TNF-? and IL-8 in supernatant were measured by radioimmunoassay. RESULTS: The concentrations of TNF-?, IL-8 in supernatant and p38 protein kinase in nuclear extracts were increased significantly induced by LPS and blocked by SB203580, a selective inhibitor of p38 protein kinase. CONCLUSION: The inductoin of TNF-? and IL-8 in alveolar macrophages by LPS may be mediated through the activation of p38 protein kinase.
5.Influence of tiotropium bromide on lung function index, quality of life and re-hospitalization risk of patients with bronchial dilation and pulmonary infection

Objective To investigate the influence of tiotropium bromide on lung function index,quality of life and re-hospitalization risk of patients with bronchial dilation and pulmonary infection.Methods 100 patients with bronchiectasis and lung infection were chosen,and they were randomly divided into control group (50 patients) with BAL used alone and observation group(50 patients) with tiotropium bromide on the basis of control group.The short-term clinical effects,the levels of FVC,FEV1 and FEV1 %,BODE index scores and QLI scores before and after treatment,and re-hospitalization rate of the two groups were compared.Results The short-term effective rate of the observation group was significantly higher than that of the control group(94.00% vs.76.00%,x2 =7.81,P < 0.05).The levels of FVC,FEV1 and FEV1% after treatment of both two groups were significantly higherthan before treatment[(2.78 ±0.57)L vs.(2.15 ± 0.33) L,(3.60 ± 0.71) L vs.(2.20 ± 0.36) L;(2.15 ± 0.42)L vs.(1.23 ±0.32)L,(2.77±0.59)L vs.(1.17 ±0.29)L;(65.25 ±11.45)% vs.(51.79 ±9.23)%,(77.46 ±15.80) % vs.(52.02 ± 9.36) %,t =2.34,2.97;2.52,3.07;2.47,3.12,all P < 0.05].After treatment,the levels of FVC,FEV1 and FEV1 % of the observation group were significantly higher than control group [(3.60 ± 0.71) L vs.(2.78 ±0.57)L;(2.77 ±0.59)L vs.(2.15 ±0.42)L;(77.46±15.80)% vs.(65.25 ± 11.45)%,t =2.26,2.44,2.30,all P <0.05].The QLI score and BODE index scores after treatment of both two groups were significantly better than before treatment[(3.37 ± 0.54) points vs.(3.95 ± 0.65) points,(2.11 ± 0.40) points vs.(3.87 ±0.61)points,(7.28 ±1.18)points vs.(5.04 ± 0.95) points,(8.69 ± 1.54) points vs.(5.13 ±0.98) points,t =2.29,2.81;2.50,2.96,all P < 0.05].After treatment,the QLI score and BODE index scores of the observation group were significantly better than the control group[(2.11 ±0.40) points vs.(3.37 ±0.54) points,(8.69 ± 1.54) points vs.(7.28 ± 1.18)points,t =2.21,2.44,all P < 0.05].The re-hospitalization rate of the observation group was significantly lower than that of the control group (16.00% vs.2.00%;28.00% vs.6.00%;x2 =8.02,9.74,all P < 0.05).Conclusion Selective cholinergic receptor blocker combined with BAL in the treatment of patients with bronchial dilation and pulmonary infection can efficiently relieve the respiratory symptoms,improve lung function,improve the quality of life and is helpful to avoid re-hospitalization occurrence.
6.Clinical significance of urinary mALB ,RBP and CysC and their combined detection in early diagnosis of type 2 diabetic nephropathy*

Objective To investigate the application value of urine microalbumin (mALB) ,retinol binding protein(RBP) and cys-tatin C(CysC) and their combined detection in early diagnosis of type 2 diabetic nephropathy(DN) .Methods Ninety-two inpatients with DN (DN group) and 90 people undergoing the physical examination(control group) in our hospital from June 2014 to Decem-ber 2015 were collected .Urine mALB ,RBP and CysC were detected in all subjects and detection results were analyzed statistically . Results The levels of urine mALB ,RBP and CysC in the DN group were significantly higher than those in the control group ,the differences all had statistical significance (P< 0 .05) .Among 3 indicators ,the positive rate of urine mALB for detecting DN was highest (94 .57% ) ,while which of 3-index combined detection was 97 .83% ,and significantly higher than that of single detection , the difference was statistically significant(P<0 .05) .The sensitivity ,specificity ,positive predictive value ,negative predictive value and Youden index of 3-index combined detection were all higher than those of single index .The ROC curve showed that AUC of u-rine mALB for diagnosing DN was 0 .732 ,the diagnostic cut-off value was 43 .58 mg/L ,AUC of urine RBP was 0 .685 ,the diagnos-tic cut-off value was 1 .47 mg/mL ,AUC of urine CysC was 0 .701 ,the diagnostic cut-off value was 1 .42 mg/L ,while AUC of com-bined detection was 0 .928 .Conclusion Urine mALB ,RBP and CysC are better indexes reflecting renal injury .Their combined de-tection will increase the positive rate ,sensitivity and specificity for diagnosing DN .So monitoring the levels of urine mALB ,RBP and CysC has an important significance to diagnosing the occurrence and development of DN early renal injury and prevention ,treat-ment and delaying progress of DN .
7.Diagnostic value of a distintegrin and metalloproteases-8 in non-small cell lung cancer

Objective To study the diagnostic values of a distintegrin and metalloproteases-8 (ADAMS) in non-small cell lung cancer(NSCLC).Methods The serum protein level of ADAM8 was assayed by ELISA in 62 NSCLC patients,27 lung benign lesions and 32 healthy people.Results The serum levels of ADAM8 of NSCLC [(456.88±143.87)ng/L] were higher than those with lung benign lesions[ (271.63±74.20) ng/L] and normal controls(253.09±72.15 ng/L) (P<0.01), but there was no significant difference between lung benign lesions and normal controls (P0.05).No significant difference was found between adenocarcinoma and squamous cell carcinoma in serum ADAM8 level,the serum levels of ADAM8 in NSCLC with stages Ⅲ-Ⅳ [ (498.80 ± 151.80) ng/L] were higher than those with stages Ⅰ -Ⅱ[(385.80±95.85) ng/L] (P <0.01 ).The diagnostic sensitivity of detection of ADAM8 for NSCLC was 77.4% and the specificity was 90.6%.Conclusion The overexpression of ADAM8 in serum of NSCLC patients indicates that ADAM8 is related with occurrence and metastasis of NSCLC;De-tection of ADAM8 could assist the diagnosis for NSCLC.

AIM: To explore the role of p38 mitogen-activated protein kinase (p38 MAPK) in the regulation of IL-4 and IL-5 expression in asthmatic rats. METHODS: T lymphocytes were isolated and purified from blood of normal or asthmatic rats. p38MAPK in nuclear extracts, mRNA and protein of IL-4 and IL-5 in supernatants were measured by Western blotting, in situ hybridization and ELISA, respectively. RESULTS: The expression of p38MAPK, the percentage of IL-4 and IL-5 mRNA positive cells, IL-4 and IL-5 protein in supernatants were significantly increased in asthmatic T lymphocytes and was blocked by SB203580, a selective inhibitor of p38MAPK. 9.Epidemiology of nonalcoholic fatty liver disease: a follow-up study for 4 years

Objective To investigate the prevalence,incidence,death causes and natural course of nonalcoholic fatty liver disease (NAFLD) as well as the risk factors of NAFLD development in population of Guangdong province.Methods On the basis of previous epidemiological survey in 2005,3545 subjects were followed up (medium 4 years) for death causes.Of which,624 subjects (re-survey group) received interview,physical examinations,biochemical tests and ultrasonography again.The natural course and the risk factors of the NAFLD were analyzed.Results Of 3543 subjects,219subjects were failed to followed up and 4 out of 467 NAFLD patients (8.6‰) died of cardiovascular and cerebrovascular diseases during follow-up period.In rest 2857 subjects,22 (7.7‰) died of cardiovascular/cerebrovascular diseases and cancers.However,there were 117 NAFLD patients in resurvey group at the baseline.The NAFLD remained in 51 (43.6%) patients,aggravated in 26 (22.2%),alleviated in 21 (18.0%) and regressed in 19 (16.2%) at the endpoint.Meanwhile 185(36.5 %) out of 507 normal subjects developed NAFLD at the endpoint.The standard prevalence of NAFLD increased from 10.5% (2005) to 22.6% (2009,P<0.05).The standard incidence was 4.9% per year with male (5.1%) being higher than female (4.7%,P<0.05).The cumulative incidence was significantly higher in urban cities (43.1%) than in rural areas (27.4%,P<0.05).Cox proportional hazards regression showed that the age (OR= 1.893),waist circumference (OR=2.512),waist-to-hip ratio (WHR,OR= 1.400),body mass index (BMI,OR=2.581) and serum cholesterol levels (OR = 1.587) were associated with the development of NAFLD (P < 0.05).Multivariate regression logistic analysis revealed that age and BMI were associated factors for NAFLD.Decreased waist circumference was prone to remission.Conclusions The higher prevalence and incidence of NAFLD were noted in population of Guangdong province.The NAFLD patients mainly died of cardiovascular/cerebrovascular diseases,but not liver disease.Obesity,aging and dyslipidemia are high risk factors for NAFLD development.
10.Clinical Observation of Abdominal Acupuncture Combined with Chinese Medicine for Treatment of Qi-deficiency Functional Constipation

Objective To observe the therapeutic effect of abdominal acupuncture combined with Chinese medicine for the treatment of functional constipation. Methods Ninety-three qi-deficiency functional constipation patients were evenly randomized into western medicine group, Chinese medicine group and combination group. The western medicine group was given oral use of Dupbalac, Chinese medicine group was given modified Buzhong Yiqi decoction, and the combination group was given abdominal acupuncture and oral use of modified Buzhong Yiqi decoction. The treatment of the three groups covered 3 months. After treatment, the therapeutic effect was evaluated accord ing to the symptom scores and the examination results of gastrointestinal kinetics. Results ( 1) The total effective rate for relieving symptoms was 100.00% in combination group, 90.32% in Chinese medicine group, and 38.71% in western medicine group, and the effect was the best in the combination group ( P<0.05). ( 2) The combination group had stronger effect on relieving dyskinesis of anal sphincter, rectal sensibility, colonic transportation than Chinese medicine group and western medicine group (P<0.05). Conclusion Abdominal acupuncture combined with Chinese medicine shows certain therapeutic effect for the treatment of qi-deficiency functional constipation, and the effect is superior to that of Chinese medicine or Dupbalac.
