1.Expression of C5aR in renal tissue of rats with unilateral ureteral obstruction

Objective To observe the effect of unilateral ureter obstruction(UUO) on the expression of complement 5a receptor(C5aR) in rat renal tissue,and to investigate the roles played by C5aR in the development of renal intersititial fibrosis.Methods Thirty six male Sprague Dawley rats were randomly assigned into two groups(18 each): sham-operated group and UUO group.The left ureter of the rats in UUO group was obstructed,while in sham-operated group the left ureter was not.Six rats in each group were executed respectively on the 5th,10th and 15th day after surgery.Histological changes in renal tubular interstitium were observed with H.E.and Masson staining,and the expression of C5aR in renal tissues was detected by iminunohistochemical ELiVisionTM plus at each time point.Results Renal tubular dilatation and partial interstitial cells degeneration were found in UUO group on the 5th day after surgery.Necrosis of the interstitial cells was found in renal tubule,and the interstitial fibrosis was found on the 10th day.On the 15th day after surgery diffuse thickening and shrinkage of renal tubular basement membrane and obvious fibrosis were found(P0.05).Conclusion The C5aR expression in renal tissue increases significantly at the early stage of UUO,while the process is slowed down in the later stage.C5aR may take part in the process of renal intersititial fibrosis at the early stage.
2.Outcomes of unilateral cleft lip repair by extended Mohler surgery

Objective To study the long term effect and practical value of unilateral cleft lip repair with extended Mohler surgery based on over 10 years experiences.Methods A lot of 320 cases with unilateral cleft lip were repaired with extended Mohler surgery at average age of 5 months and 20 days.65 repaired cases were followed up and long term treatment result was evaluated.26 cases were evaluated in 10.5 to 15 years after surgery.39 cases were evaluated in 6 to 24 months after surgery.And the details of surgery were described.Results Excellent results for most of cleft patients (63 cases, 97%) were achieved, which included unobvious vertical scar symmetric to unaffected philtrum column, symmetric cupid's bow, nostrils and intermedial nasal columellae.Conclusions Extended Mohler surgery is great valuable for unilateral cleft lip repair.Appropriate indications should be stressed.
4.Application ofWeChat for Family Rehabilitation after Anterior Cruciate Ligament Reconstruction

Objective To apply the WeChat in family rehabilitation after anterior cruciate ligament (ACL) reconstruction. Methods 126 patients accepted ACL reconstruction using quadrupled semitendinosus and gracilis tendons were divided into observation group (n=69) and control group (n=57). The control group received conventional rehabilitation program, and the observation group joined the WeChat group. They were assessed with knee range of motion (ROM), knee stability test, thigh perimeter, Lysholm score and the International Knee Documentation Committee (IKDC) score 3 and 6 months after operation. Results There was a significant difference between groups in ROM of knee flexion (P<0.05) 3 and 6 months after operation, but not in ROM of knee extension (P>0.05). There was no significant difference in thigh perimeter 3 months after operation, but it was significant 6 months after operation. There was significant difference in Lysholm score and IKDC score (P<0.05), but not in the joint stability (P>0.05). Conclusion Direction of rehabilitation with the WeChat Group can significantly improve the compliance, accelerate the rehabilitation process and reduce complications in patients after ACL reconstruction.
5.Clinical analysis of liver transplant from a child of brain death to an adult

Objective To explore clinical feasibility of liver transplant from child of brain death to adult, to summarize the clinical experiences that a child of brain death transplants liver to an adult. Methods The recipient was a 39-year-old woman patient with primary hepatic carcinoma and posthepatitis cirrhosis (decompensation stage); while the donor was a 8-old-year child of brain death because of brain neoplasms. Donated liver was gained by the method of en bloc multivisceral procurement in a short time; the operative method was classic orthotopic liver transplantation. The postoperative managements included immunosuppression, prevention of infection, hepatic protection, and other relevant supports etc. Results The transplantation operative duration was 6 hours, after which not only did the recipient survive but also her body functioned well including the liver part, with no severe postoperative complications. Conclusions The technology of transplanting livers from children to adults is feasible. The key to ensure the success of transplant operation is systematic preoperative evaluation, excellent operative technique, and perfect postoperative treatment.
6.Research advances in treatment of cerebral ischemic injury by acupuncture of conception and governor vessels to promote nerve regeneration.

Cerebral ischemia is one of the most common diseases treated by acupuncture therapeutics. Recent studies indicated that acupuncture treatment by needling the conception and governor vessels had positive effects in promoting neural regeneration in patients after cerebral ischemia injury. Acupuncture intervention could continuously promote the proliferation and differentiation of the neural stem cells in the brain, obviously up-regulate expression of growth factors, accelerate angiogenesis and inhibit apoptosis. Hence, it is necessary to present an exhaustive review on the mechanisms. The present review gives a detailed description of pathological changes of cerebral ischemia and acupuncture intervention applied to the conception and governor vessels, and proposes research prospects in the future.
7.Full fascia closure with interrupted absorbable suture and layered closure with interrupted silk suture in abdominal incision:comparison of curative effects and biocompatibility

BACKGROUND:Abdominal incision healing is not only related with the patient’s own situation, but also closely related with the surgeon's suture technique, suture method, choice of stitches. <br> OBJECTIVE:To compare the absorbable sutures and silk sutures for abdominal incision. <br> METHODS:Total y 153 colorectal cancer patients, including 91 males and 62 females, aged 30-82 years, were randomly divided into observation group (n=78) and control group (n=75). An abdominal midline incision was made in al patients receiving radical surgery of colorectal cancer. The Vicryl suture and silk suture were respectively used in the observation and control groups for abdominal incision closure. Suturing time, length of hospital stay, incision infection, disruption of wound, fat liquefaction of wound and rejection were compared between two groups. <br> RESULTS AND CONCLUSION:The suturing time and length of hospital stay were less in the observation group than the control group (P<0.05). In the observation group, there were three cases of incision infection, but no incision dehiscence and rejection occurred;in the control group, there were 10 cases of incision infection, 4 cases of incision dehiscence, and 5 cases of rejection. A significant difference was found in the incision infection, dehiscence and rejection between the two groups (P<0.05). Hospitalization expenses and fat liquefaction of incision had no difference between the two groups. these findings indicate that the Vicryl plus as an absorbable suture is simple, effective and safe that can promote wound healing and reduce complications.
8.Hyoid suspension with repose system combined with UPPP in severe OSAHS patients: short-term results.

OBJECTIVE:
To explore the effect of a comprehensive surgical approach of hyoid suspension with Repose system plus uvulopalatopharyngoplasty (UPPP) on the treatment of severe obstructive sleep apnea hypopnea syndrome (OSAHS).
METHOD:
Twelve patients with severe OSAHS (AHI > 40) diagnosed by polysomnography, were determined for the obstruction in both the velo-pharyngeal and the hypopharynx by preoperative physical examination, fiberoptic pharyngo-laryngoscopy. Two cases were given continuous airway pressure via nose, seven days before operation and then were performed hyoid suspension plus UPPP. The follow up was 3 months postoperatively. The pair T test was used to compare the preoperative and postoperative results by SPSS11.0 for windows.
RESULT:
At postoperative 3 month, there were statistically significance between preoperative and postoperative measurement in all. Mean AHI decreased from preoperative 69.28 +/- 6.50 to postoperative 19.77 +/- 9.23, lowest mean oxygen saturation increased from 65.25 -/+ 3.14 to 90.17 +/- 2.86. Snoring and sleep apnea disappeared or lessened, with the disappearance or decrease of lethargy at daytime. No severe complication occurred.
CONCLUSION
Comprehensive surgical approach of hyoid suspension with Repose system plus UPPP is effective surgical approach for patients with severe OSAHS who suffered from oropharyngeal and hypopharyngeal obstruction in short- term result.

9.Effects of simultaneous nasometer and nasopharyngeal fibroscopic examination on the value of nasalization rate in cleft palate patients

Objective:To evaluate the changes of nasalance when cleft palate patients examined using nasometer and nasopharyngeal fiberscopy simultaneously.Methods:A total of 33 cleft palate patients from August 2004 to August 2010 were examined by nasopharyngeal fiber endoscopy and nasometer simultaneously. The nasalance of 33 voice samples was compared under two situations only nasometer and nasometer with nasopharyngeal fiberscopy.Results:The statistics showed that the nasalance value of 33 voice samples of 33 participants under different situations were no significant difference through paired t-test ( P>0.05). Conclusions:Nasopharyn fiber endoscopy had no influence to the results of the nasalance value under simultaneous condition. The nasometer and nasopharyn fiber endoscopy can be applied to evaluate the velopharyngeal function of cleft palate patients simultaneously.
10.Influence of glottal stops on nasalance and velopharyngeal closure status after cleft palate surgery

Objective:To investigate the influence of glottal stops for the relationship between nasalance and velopharyngeal closure status of cleft palate patients after surgery.Methods:A total of 71 patients with postoperative cleft palate speech disorders treated in the Department of Oral and Maxillofacial surgery, School of Stomatology, Peking University from August 2004 to August 2010 were included. The compensated speech evaluation of cleft palate with or without glottic occlusion was performed in 50 males and 21 females with an average age of (15.9±6.2) years old. The nasal sound meter and nasopharyngeal fiber endoscope were used to record the nasal phonation rate and the score of nasopharyngeal fiber endoscopic visual analog scale (VAS) of the patients. According to the results of glottic plug speech evaluation, the patients were divided into three groups: whole group (without glottic plug), glottic plug group and silent glottic plug group. The nasal phonation rate values and VAS values of 11 speech samples (unit vowel No. 1-3, continuous syllable No. 4-9, sentence 10-11) of the three groups were compared by Pearson analysis.Results:There were 71 patients in the whole group, 34 patients in the glottic plug group and 37 patients in the silent glottis group. There was a negative correlation between nasal phonetic rate and VAS of 11 speech samples in the whole group, however there were no significant difference ( r>-0.179 and<-0.017, P>0.017) and no significant correlation between nasal phonetic rate and VAS in glottic occlusion group ( r>-0.196 and<-0.017, P>0.05). There was a significant negative correlation between nasal phonetic rate and VAS value of all but No. 1, 3 and 11 speech samples in silent door plug group ( P<0.05), among which No. 7 speech sample was the most sensitive sample ( r=-0.557, P<0.05). Conclusions:The present study showed the glottal stops could affect the evaluation of the velopharyngeal closure function by using nasometer. Glottal stops should be considered as one important factor for the nasalance.
