Objective To investigate the expression, relationship, significance of cyclooxygenase_2 (COX-2) and malonic dialdehyde (MDA) after cerebral ischemic-reperfusion injury and provide basis of treatment. Methods The focal cerebral ischemic-reperfusion model was established with thread embolish of middle cerebral artery. Western blotting, barhituric acid method and neurological evaluation were used to examine the expression of COX-2, MDA in cortex and the changes of neurological function;TTC staining was used to observe the changes of cerebral infarction volume. Results COX-2 prorein expression was correclated well with the MDA(r=0.910,P＜0.01). The content of COX-2 and MDA was very low in sham operation group, they were increased significantly at I2h/R6h model group, with the increase of reperfusion time,they remarkably reached its peak at I2h/R24h,they were slightly lower at I2h/R48h, but still maintained at a high level;Compared with model group, in tetramethylpyrazine(TMP) treatment group, the content of MDA and cerebral infarction volume were markedly decreased(P＜0.01). Conclusion The expression of COX-2 and MDA increases in cerebral ischemic-reperfusion injury.It indicates they may play an important role in the mechanisms of cerebral ischemic-reperfusion injury;TMP has neuroprotective effect.

Objective To assess the risks of complications of loop colostomy and loop ileostomy.Methods The databases of Medline,Embase,Cochrane Library,Google Scholar and Wanfang were retrieved to identify the published studies comparing the advantages between loop colostomy and loop ileostomy.All the articles retrieved were published before April 15,2011.The incidence of complications was analyzed by Meta-analysis.The data were analyzed by the Z test and the heterogeneity of the data was analyzed by the Q test.Results Five randomized controlled trials and 8 non-randomized controlled trials with 1752 cases were included in the Meta-analysis.Compared with loop ileostomy,loop colostomy was associated with significantly increased risk of stoma prolapse ( OR =3.46,95％ CI:1.81-6.63,P ＜ 0.05 ).There were no significant differences in the incidences of stoma hemorrhage and stenosis,wound infection,stoma necrosis,parastoma dermatitis and hernia between patients who received loop ileostomy and those who received loop colostomy.Compared with loop ileostomy,loop colostomy was associated with increased risk of wound infection following stoma closure(OR =3.44,95％ CI:1.95-6.05,P ＜0.05).Compared with loop colostomy,loop ileostomy was associated with increased risk of postoperative bowel obstruction following stoma closure(OR =0.43,95 ％ CI:0.20-0.91,P ＜ 0.05 ).There was no significant difference in the risk of anastomotic leak between loop ileostomy and loop colostomy.Conclusion Compared with loop ileostomy,loop colostomy is associated with increased risk of stoma prolapse and wound infection after stoma closure,while a higher risk of bowel obstruction following stoma closure is observed after loop ileostomy.

Objective:To analyze the morphological characteristics of maxillary first molars,mesiobuccal roots and the incidence of second mesiobuccal(MB2)roots in Uyghur adults.Methods:1 00 Uyghur adults with full dentition were included.The morphology of maxillary first molars and root canals were examined by cone beam computerized tomography(CBCT).The prevalence of MB2 and the difference between sexes were analysed.Results:Among 200 maxillary first molars,1 54(77%)teeth were with 3 roots and 3 ca-nals,42(21 %)with 3 roots and 4 canals,2(1 %)with 3 roots and 5 canals,1 (0.5%)was with 4 roots and 6 canal,1 (0.05%) with 4 roots and 7 canal.The percentage of type Ⅰ,Ⅱ,Ⅲ,Ⅳ and Ⅴ mesiobuccal roots was 77.0,1 3.5,9.0,0 and 0.5 respec-tively.The prevalence of MB2 was 22.32% in male and 21 .2% in female(P =0.901 ).Conclusion:The prevalence of MB2 in Uy-ghur adults is about 22% and the predominant morphology of maxillary first molarsmesiobuccal roots was type I.

Objective To investigate the effects of ribs in high intensity focused ultrasound (HIFU)beam path on formation of coagulative necrosis in cattle liver in vitro in the same acoustic power. Methods The HIFU ablation was carried out in the manner of a single dot in the liver in vitro. The liver was ablated by HIFU in free field,then the ribs were placed between the focal plane and transducer,directing the beam axis through the intercostals space, lied at the rib edge and on the centre of middle rib. For each of these axial arrangements,the plane of the front surface of ribs was placed 3 cm,6 cm or 9 cm in front of the focus. After HIFU exposure,the tissue necrosis rate(TNR),the maximum length,width,depth and position of the coagulated necrosis in the liver were measured. Results ①The TNR was 100% in the free field. When the ribs were placed 3 cm in front of the focal plane,directing the beam axis through the intercostals space, the TNR was 100%. When directing the beam axis lied at the rib edge and on the centre of middle rib,the TNR decreased sharply. When the ribs were placed 6cm and 9cm in front of the focal plane, the TNR was more than 50%. ②When the ribs were placed in HIFU beam path, the volume of necrosis decreased compared with those in free field. The relationship between the ribs and beam axis, the location of ribs placed away from the focal plane different,the volume,length,width and position of the coagulated necrosis different. ③ When the ribs were close to the focal plane,unacceptable damage could been observed on the surface of ribs.When the ribs were far away from the focal plane, the ribs were safe. Conclusions The volume of necrosis decrease when the ribs are placed in HIFU beam path. The relative location of ribs and HIFU beam axis, the location of ribs away from the focal plane are relative with the volume of the coagulated necrosis. When directing the beam axis through the intercostals space, the volume of necrosis is large, and the shape of coagulated necrosis is regulated. When the ribs are closed with the focal plane,ribs are damaged probably.

Hormone replacement therapy (HRT), involving giving sex steroid hormones such as estrogen alone or with a progestogen, is widely used in postmenopausal women.HRT helps to relieve menopausal symptoms and has also been shown to prevent osteoporosis.Although most observational studies have showed that HRT can reduce the risks of cardio-cerebrovascular diseases, the subsequent randomized controlled trials were inconsistent with the results.This article reviews the relationship between HRT and stroke from drug type, route of administration, estrogen dosage, and initiation time.

Objective To investigate the influence of live combined bifidobacterium, lactobacillus and enterococcus powder assisted with nasal jejunum nutrition on laboratory index, complication rate and economical efficiency in patients with severe acute pancreatitis (SAP). Methods Fifty patients with SAP were divided into treatment group and control group by random draw method with 25 cases each. The patients of 2 groups received conventional therapy of SAP and nasal jejunum nutrition, and the patients in treatment group were treated with the above treatment combined with live combined bifidobacterium, lactobacillus and enterococcus powder. The laboratory indexes, gastrointestinal function score, complications, hospitalization time and hospitalization expenses were compared between 2 groups. Results The white blood cell, amylase, lipase, C reactive protein, interleukin (IL)-8, tumor necrosis factor (TNF)-α, lactate dehydrogenase (LDH) and gastrointestinal function score after treatment in treatment group were significantly lower than those in control group:(5.9 ± 2.1) × 109/L vs. (8.4 ± 3.1) × 109/L, (210.4 ± 47.6) U/L vs. (271.9 ± 82.2) U/L, (205.2 ± 22.3) U/L vs. (249.3 ± 34.7) U/L, (14.7 ± 0.4) mg/L vs. (35.1 ± 0.8) mg/L, (16.0 ± 4.8)μg/L vs. (36.5 ± 12.9)μg/L, (21.7 ± 5.6) ng/L vs. (43.4 ± 9.5) ng/L, (212.5 ± 95.4) U/L vs. (284.0 ± 124.6) U/L and (0.81 ± 0.24) scores vs. (1.37 ± 0.36) scores, and the total incidence of complications, hospitalization time and hospitalization expenses were significantly lower than that in control group: 12.0%(3/25) vs. 64.0%(16/25), (18.72 ± 1.90) d vs. (21.13 ± 2.35) d and (4.48 ± 0.55) × 104 yuan vs. (4.73 ± 0.78) × 104 yuan. There were statistical differences (P<0.05). Conclusions Live combined bifidobacterium, lactobacillus and enterococcus powder assisted with nasal jejunum nutrition in the treatment of SAP patients can efficiently improve the laboratory indexes, promote gastrointestinal function recovery, decrease the risk of complications and reduce the economic burden.

Objective To investigate the expression of sodium iodide symporter (NIS ) mRNA in differentiated thyroid Carcinoma (DTC)and further explore its value in clinical diagnosis and therapy of DTC. Methods The expression of NIS mRNA was detected and analyzed in 21 nodular goiter and 45 cases of DTC (including 35 cases of papillary thyroid carcinoma and 10 cases of follicular thyroid carcinoma)by using real-time fluorescent quantitative reverse transcription polymerase chain reaction (real-time RT-PCR).Results Compared with that in nodular goiter,the mRNA expression of NIS in DTC tissue was significantly decreased (P < 0.05 ). Moreover,the mRNA expression of NIS was significantly correlated with lymph node metastasis and AJCC stage, respectively.The expression of NIS mRNA in DTC with lymph node metastasis was significantly decreased compared with that in DTC without lymph node metastasis (P <0.05).In addition,the expression of NIS mRNA in Ⅱ-Ⅳstage DTC was significantly decreased compared with that in Ⅰ - Ⅱ stage DTC (P < 0.05 ).Conclusion Differential expression of NIS can provide evidence for individual 1 3 1 I therapy for DTC.

ObjectiveTo explore the association between hepatic steatosis and the liver tissue expression of HBsAg and HBcAg in chronic hepatitis B (CHB) patients.MethodsFrom January 2005 to June 2008,a total of 147 CHB patients with hepatic steatosis diagnosed by liver biopsy and other 149 CHB patients without hepatic steatosis but with similar HBV DNA titer were enrolled.The differences of HBsAg and HBcAg immunostaining and liver injury in these two groups were compared.The data were analysed using t test and chi square test.ResultsCompared with non-steatosis group,the average age and body weight index of hepatic steatosis group were higher (t values were -3.31and -6.57,both P＜0.01).The percentage of moderate to severe hepatic inflammation in liver,obvious hepatic fibrosis and the strong positive HBsAg staining was lower (30.6％ vs 15.4％ ； 26.5％vs 12.8％； 23.1 ％ vs 6.7 ％； x2=9.63,8.92,15.76； all P＜0.01),and the percentage of strong positive HBcAg staining was also in downtrend.Compared with degree F1 and F2 of liver steatosis,the percentage of HBsAg and HBcAg strong positive staining in liver tissues of degree F3 and F4 was in downtrend.ConclusionsHepatic steatosis affected the expression of HBsAg and HBcAg in liver tissue of CHB patients.As hepatic steatosis appeared and became more severe,both expression of HBsAg and HBcAg and the degree of liver injury were in downtrend.

Objective To evaluate the activation of the visual cortex in patients with primary openangle glaucoma (POAG) and to explore whether the neuronal activity corresponds with retinal nerve fiber layer (RNFL) and cup-to-disc (C/D) values.Methods Twenty-five patients and 25 gender-and agematched healthy volunteers were studied.Blood oxygenation level-dependent functional magnetic resonance imaging (BOLD-fMRI) and three-dimensional brain volume imaging (3 D BRAVO) sequences were obtained using 3 T MR imaging system.A full-screen black-white shift checkerboard was used for visual stimulus during the fMRI experiment and was performed on each eye of all subjects using a visual-acoustical system.All acquired data were postprocessed and analyzed by statistical parametric mapping (SPM).After analysis,individual activated mapping,intra-group mean activated mapping,and inter-group variant mapping were observed.The voxel number,intensity,and Montreal Neurological Institute (MNI) coordinate of the activated clusters were recorded.The Xjviewer software was utilized to obtain activated voxel numbers in occipital lobe.A Pearson correlated test was performed to test the correlation between the number of activated voxels and RNFL,C/D and Hodapp-Anderson-Parrish (HAP) clinical stage.Results Intra-group mean activated mappings of both patients and volunteers showed obvious activation in bilateral occipital lobes.As compared with healthy volunteers,the POAG patients exhibited statistically significantly decreased activation in bilateral occipital lobes,left hippocampus,and left cerebellum,along with lower mean RNFL [(71.56 ±21.54) i m versus (111.88 ± 9.96) μm] and higher C/D values (0.71 ± 0.18 versus 0.36 ± 0.08 ; t value was respectively-10.901 and 11.643,P ＜ 0.05).The number of activated voxels in the occipital lobes of POAG patients did not correlate with RNFL,C/D and HAP clinical stage of the corresponding eye (r value was respectively 0.157,-0.113 and-0.242,P ＞ 0.05).Conclusions fMRI demonstrated differences in visual cortex activation in POAG patients relative to healthy volunteers,suggesting it might be a promising complementary method for diagnosing glaucoma.However,fMRI findings did not correlate with POAG extent,as measured by RNFL and C/D values.Ophthalmological examination remains to play an important role in the evaluation of open-angle glaucoma.
