1.Evaluation of the viability and energy metabolism of canine pancreas graft subjected to significant warm ischemia damage

Objective To investigate the energy metabolism state and the posttransplantive function under different warm ischemia time (WIT). Methods The left lobe of pancreas was resected, and according to WIT (30, 60, 90, 120 min), the canines were randomized into 4 groups. The graft was perfused and preserved by the UW solution (0-4 ℃) cold storage method for 24 h. And then the heterotopic segmental autotransplantation of pancreas was performed, while the normal control group and no warm ischemia(NWI)group were also set up. Blood sugar (BG) and intravenous glucose tolerance test (IVGTT) K value and the plasma insulin level were examined in the first week after pancreas transplantation. The amylase level of pancreatic juice 24 h and one week after transplantation were detected either. The tissue concentrations of adenosine triphosphate (ATP) and total adenine nucleotides (TAN) were determined using high performance liquid chromatography (HPLC). The pancreas cortex was taken for morphological analysis and ultrastructure study 1 h after transplantation. Results Plasma glucose in groups of 30 min and 60 min was returned to normal 3-5 days after transplantation, while groups of 90 min and 120 min could not one week after transplantation. IVGTT K values in groups of NWI, 30 min and 60 min were more than 1, while those in groups of 90 min and 120 min were less than 1 one week after transplantation. 2.The diagnosis and treatment of 86 cases of primary duodenal tumors

Objective To investigate the diagnosis and therapeutic methods of primary tumors of duodenum(PTD).Methods Clinical data of 86 patients with primary duodenal tumors who were hospitalized in the First Affiliated Hospital of China Medical University during 26 years were analyzed retrospectively.Of all the tumors,18 cases were benign tumors and 68 cases were malignant tumors.Results The clinical(manifestations) were symptoms of bile duct obstruction,GI and nonspecific symptoms,and signs included(fever),anemia and weight loss.The correct preoperative diagnostic rate by auxiliary examinations were:(duodenal) endoscopy 92%,air barium double radiography 89%,MRCP 85%,BUS 33%,CT 51%,MRI 75%,and duodenal endoscopy combined with air barium double radiography were 95%.Tumors were mostly located in the descending portion of duodenum(71 cases).Eighteen cases of benign PTDs were resected(completely) with a 5-year survival of 92.3%.Among the 68 maligant PTD cases,35 cases underwent(pancreatoduodenectomy),and 8 cases received segmental duodenectomy,with a resection rate of 63.2% and the 5-year survival rate of 31.0%.The remaining 25 cases,in which the tumors were unresectable,were treated by bypass operations,the 5-year survival rate was 0%.Conclusions Patients with PTD usually lack specific symptoms and signs,but combination of endoscopy,GI air barium double radiography and other(examinations) can improve the preoperative positive diagnostic rate.Segmental duodenectomy and simple tumor resection are curable for primary benign tumors of duodenum;while for primary malignant tumors of(duodenum),the therapy of choice should be pancreatoduodenectomy.

4.Connection of Matrix Operation Live Telecast System with Telemedicine Education Web

5.Effects of L-arginine on the hypothermic perfusion and on preservation of isolated pancreas in canines

Objective To study the effects of L-arginine on the reperfusion injury of hypothermic perfusion and on preservation of isolated pancreas(ISP) in canines . Methods After segmental pancreatic transplantation(SPTS) model were set up,the animals were randomly divided into 3 groups:(1)ISP irregulated and preserved with Euro Colln Solution(ECS)plus L-arginine(L-arg group);(2)IPS irregulated and preserved with ECS plus N-nitro-L-arginine methy ester(L-NAME group);and(3)IPS irregulated and preserved with ECS plus normal saline(Control group).The IPS were irregulated and preserved at 0-4℃ for 24h.Serum concentration of amylase and lipase were detected. Objective To investigate changes in serum levels of antinuclear antibody(ANA), anti?double?stranded DNA(dsDNA)antibody and anti?extractable nuclear antigen(ENA)antibody before and after anti?tumor necrosis factor?α(TNF?α)therapy in psoriatic patients. Methods Clinical data obtained from 32 patients with psoriasis were analyzed retrospectively. Of the 32 patients, 13 received intravenous injection of 5 mg/Kg infliximab at week 0, 2, 6 for 3 sessions, then once every 8 weeks(infliximab group), while other 19 received subcutaneous injection of 25 mg etanercept twice every week(etanercept group). The treatments in the 2 groups both lasted more than 3 months. Serum levels of ANA, anti?dsDNA antibody and anti?ENA antibody and changes of clinical symptoms were detected and observed respectively before each treatment in the infliximab group, as well as every 3- 6 months in the etanercept group. The 75%reduction in psoriasis area and severity index(PASI75)and disease activity score of 28 joints(DAS28) were used to evaluate clinical efficacy. Serum levels of ANA, anti?dsDNA antibody and anti?ENA antibody were measured by indirect immunofluorescence(IIF)assay, Western blot analysis combined with enzyme?linked immunosorbent assay(ELISA), and Western blot analysis, respectively. Results After 3?month treatment, the 32 patients achieved clinical remission to different extents. Of 32 patients receiving anti?TNF?αtherapy, 7(21.9%)developed new autoantibodies. Concretely speaking, 4 patients in the infliximab group developed autoantibodies in 8.3 ± 5.1 months, including 3 cases positive for ANA and 3 for anti?ENA antibody. Three patients in the etanercept group developed autoantibodies in 9.0 ± 3.0 months, including 3 cases positive for ANA and 1 for anti?ENA antibody. Conclusion Partial patients with psoriasis may develop autoantibodies after anti?TNF?αtherapy.

9.A simulation study of the effects of ischemia on spiral waves in 2D human ventricular tissue.

Based on human ventricular single cell mathematical model, a two-dimensional mesh of ventricular wall tissue was constructed. Through the increasing of the concentration of extracellular K+, we simulated the propagation of spiral wave in a condition under the influence of ischemia in 2-D human ventricular tissue. The results showed that along with the increase of ischemic level and size, the instability of spiral waves increased, and under the influence of certain ischemic level and size, spiral waves broke up. Through this simulation study of the effects of ischemia on spiral waves in 2-D human ventricular tissue, we explained the corresponding mechanism of the maintenance of ventricular tachycardia and the cause of ventricular fibrillation under the influence of ischemia.

10.Correlation study between 3 T MR DTI measurements and clinical symptoms in patients with cervical spondylotic myelopathy

Objective To evaluate the correlation between diffusion tensor imaging(DTI)measurements,fiber tracking(FT)and the clinical symptoms in patients with cervical spondylotic myelopathy.Methods According to the Japanese orthopaedics association score(JOA),104 patients with cervical spondylopathy were divided into 4 groups:mild in 31 patients with 13-16 scores,moderate in 27 with 9-12 scores,severe in 25 with 5-8 scores,and serious in 21 with 0-4 scores.According to the lesion signal characters,all patients were divided into 3 groups:Group A with normal signal in both T1 WI and T2WI in 33 patients,Group B with normal signal in T1WI but high signal in T2WI in 30 patients,and Group C with low signal in T1 WI and high signal in T2WI in 41 patients.Apparent diffusion coefficient (ADC),fractional anisotropy(FA),λ1,λ2,λ3 were measured in the spinal cord at the serious pressed section,and fiber tractography was performed.The Spearman correlation analyses was used to correlate each of the DTI measurement with JOA score.Group difference was tested with one-way ANOVA method.Results High quality of DTI was acquired in all patients.The FA values in the mild,moderate,severe,and serious groups were respectively 0.69 ±0.13,0.58 ±0.03,0.46 ±0.08,and 0.37 ±0.11 and significant difference was found in different groups(F =100.59,P < 0.05)and positively correlated with JOA scores (r =0.883,P < 0.05).There was no statistical significance between JOA scores and ADC,λ1,λ2,λ3(r=0.232,0.217,0.113,0.127,P >0.05).The FA values in group A,B,and C were respectively 0.67 ±0.33,0.51 ±0.21,0.38 ±0.03,and significant difference was found among different groups(F =50.05,P < 0.05).Decrease of JOA score and high signal in T2 companied with decrease of FA value.Decrease of FA values was found associated with increase of fiber bundle damage.The ADC,λ2,λ3 but not λ1 were significantly different among the JOA groups and the group A,B,and C.Conclusions The FA values are positively correlated with clinical symptoms.Decrease of FA values is found associated with increase of fiber bundle damage.DTI can show the severity and extent of damage of spinal cord in patients with cervical spondylotic myelopathy.
