1.Cost-effectiveness Analysis of Three Therapeutic Schemes for Sudden Deafness

OBJECTIVE: To evaluate three therapeutic schemes for sudden deafness with cost-effectiveness analysis for guiding rational use of drugs. METHODS:Using pharmacoeconomic cost-effectiveness analysis to evaluate 61 patients with 62ears of sudden deafness treated by Salvia Miltiorrhiza(groups A), Salvia Miltiorrhiza and cerebroprotein hydrolysate(groups B) or cerebroprotein hydrolysate(groups C) . RESULTS: The effective rates of three groups were 73.9%, 75.0% and 80.0% respectively without significant difference among them(P > 0.05) . The average drug costs for one patient in three groups were 617.89 yuan, 2 651.29 yuan and 2 647.99 yuan respectively. The costs for 1% increase of effective rate in three groups were 8.36 yuan, 35.35 yuan and 33.10 yuan respectively. CONCLUSION: Salvia Miltiorrhiza scheme is the most rational one for sudden deafness.
2.Correlations between Dynamic Parameters of Blood Pressure and Left Ventricular Hypertrophy in Patients with Essential Hypertension

Objective To explore the correlation between left ventricular hypertrophy (LVH) and dynamic parameters of blood pressure as well as blood pressure variation in patients with essential hypertension (EH). Methods From January to September, 2013, 581 patients with EH were divided into hypertrophy group (n=161) and non-hypertrophy group (n=420) according to the left ventricular mass index (LV-MI). The clinical data, biochemical indexes and dynamic parameters of blood pressure were compared, and the multiple factors regression was used to analyze the risk factors of LVH. Results The incidence of LVH was higher in women than in men (χ2=4.836, P=0.03), the level of blood uric acid was higher in the hypertrophy group than in the non-hypertrophy group (t=-11.540, P<0.001). The clinic systolic blood pressure was higher in the non-hypertrophy group (t=-3.805, P<0.001). There was significantly difference in 24-hour systolic blood pres-sure load, systolic blood pressure drops at night and average systolic blood pressure between two groups (t>2.770, P<0.05). Multi-factor Lo-gistic regression analysis showed that gender (OR=1.674, P=0.044) and 24-hour systolic blood pressure load (OR=1.021, P=0.003) associat-ed with LVH. Conclusion The occurrence of LVH in patients with EH was closely related to the 24-hour systolic blood pressure load, and was higher in women than in men.

4.Isovolumic relaxation strain rate evaluation of myocardial function and the coronary stenosis extent in coronary heart disease

Objective To quantitatively analyze myocardial function and the degree of myocardial ischemia with strain rate imaging (SRI). Methods SRI was performed in 34 patients with left anterior descending coronary artery disease diagnosed with coronary angiography and compared with 35 healthy volunteers. Isovolumic relaxation strain rates (SRivr) of anterior wall and anterior septal were measured. Results There was no significant cut-off value for LAD<50%. A cut-off value of SRivr=-0.42 s~(-1) (sensitivity 84.85%, specificity 80.36%) for LAD 50%-74% stenosis. A cut-off value of SRivr=-0.91 s-1 (sensitivity 91.07%, specificity 89.91%) for LAD>75% stenosis. Conclusion SRivr can quantitatively differentiate LAD 50%-74% or LAD>75% stenosis. SRI can evaluate the coronary stenosis extent quantitatively.
5.Effect of autonomic nerve activity on emotion experience: an empirical study

Objective To study the effect of autonomic nerve activity on emotion experience.Methods 71 healthy males were asked to see a neutral film STICK and conduct a computer game,then evaluated emotion experience.All participants were recorded skip temperature,skin conduction,heart rate,LF and HF during baseline and game periods.Results (1) There was significant difference in fear experience among high,middle and low synchronous groups (2.64 ± 2.05,2.50 ± 2.01,4.46 ± 2.41; P< 0.01),and low synchronous group was significantly higher than high synchronous group (P < 0.01).The main effects of three periods were significant in basis of three response types of autonomic nerve activity(skin conduction:F(2.68) =76.083,P<0.01; heart rate:F(2.68) =71.692,P < 0.01),and skip temperature,skin conduction and heart rate were no significant difference among three response types.Types and periods had no significant interaction.(2) Different response modes of autonomic nervous system has different distributions in high fear and low fear groups (x2 =9.763,P < 0.01).Skip temperature,skin conduction and heart rate were no significant difference between high fear group and low fear group.Conclusion The modes of autonomic nervous system have an effect on intensity of fear experience,but not the same in skip temperature,skin conduction and heart rate.
6.Expressions and clinical significances of IGF-1R and IRS-1 in squamous cell lung cancer

Objective The aim of the study is to detect the expression of IGF -1R and IRS-1 in squa-mous cell lung cancer ( SQCLC) and to explore the role in the development of squamous cell lung cancer and clini -cal significance .Methods Specimens from 246 surgical SQCLC and 40 adjacent normal lung tissues were evalu-ated for IGF-1R and IRS-1 expression by immunohistochemistry .We explored the correlation between IGF -1R and IRS-1,their relationship with clinicopathological parameters and their impact on outcome in SQCLC pa -tients.Results SQCLC of IGF -1R positive expression rate was 54.07%,which was higher than in adjacent normal tissue(32.5%),SQCLC of IRS-1 positive expression rate was 38.21%,which was lower than in adja-cent normal tissue(70%),the pairwise difference was statistically significant (P<0.05);Expression of IGF-1R was correlated with lymph node metastasis (P<0.05),IRS-1 expression was related with degree of differentia-tion and lymph node metastasis (P<0.05).Survival of IGF -1R expression group of patients was significantly shorter than the expression of IGF -1R negative patients ,the survival of the IRS-1 expression was significantly longer than patients who negatively expressed IRS -1,the difference was statistically significant (P=<0.001 and=0.021).IGF-1R was negatively correlated with IRS -1 expression in SQCLC(r=-0.125,P<0.001). Conclusion IGF-1R,IRS-1 are involved in the occurrence of SQCLC ,the development .The IGF-1R is an independent prognostic factor in SQCLC .
7.Correlation between brachial-ankle pulse wave velocity and carotid artery plaque formation in middle-aged and elderly patients with hypertension

Objective To explore the relationship between brachial ankle pulse wave velocity (BaPWV) change and carotid artery plaque formation in middle-aged and elderly patients with essential hypertension(EH).Methods A cross sectional study was conducted in 409 patients who were admitted in Hypertension Department in our hospital from January to September 2013.Their age ranged from 35 to 75 years,with mean aged(53.2±15.0) years.Parallel carotid artery ultrasound and BaPWV examination were performed in all patients.According to whether carotid plaques were present,patients were divided into two groups:carotid plaque group and control group.And carotid plaque group was sub-grouped into normal BaPWV group (BaPWV < 1400cm/s) and increased BaPWV group(BaPWV≥1400 cm/s) according to BaPWV levels.The detection rate of carotid plaque was compared between normal BaPWV group and increased BaPWV group.The correlation between BaPWV and carotid plaques was analyzed by multivariate logistic regression analysis.Results Compared with the control group,carotid plaque group showed the prevalence of carotid plaques was increased along with the increases of age(t=11.0,P=11.0),systolic blood pressure (t=3.87,P=3.87),diastolic blood pressure(t=3.70,P=0.00),pulse pressure(t=6.13,P 6.13),total cholesterol levels(t=2.57,P=0.01).The detection rate of carotid plaque was higher in increased BaPWV group than in normal BaPWV group [62.6%(159/254) vs.43.2%(67/155),x2=14.61,P=0.00].After adjusting for age,blood pressure and other cardiovascular risk factors,the multivariate logistic regression analysis showed that increased BaPWV was an independent risk factor for carotid plaques(OR=2.06,P=0.05),and age,smoking,systolic pressure,diastolic pressure,pulse pressure,total cholesterol,low-density lipoprotein cholesterol,triglyceride levels were positively correlated with carotid plaques.Conclusions BaPWV is one of the independent impact factors for carotid plaques,which plays an important role in early diagnosis and screening for subclinical vascular lesions.
8.Role of intercellular gap junction in propofol and sevoflurane anesthesia in rats

Objective To evaluate the role of intercellular gap junction in the propofol and sevoflurane anesthesia in rats. Methods Eighty male Wistar rats weighing 210-260 g were randomly divided into 8 groups (n = 10 each): control group (group C), carbenoxolone group (group CA), propofol group (group P), different doses of carbenoxolone + propofol groups (groups CA1 + P, CA2 + P, CA3 + P), sevoflurane group (group S) and carbenoxolone + sevoflurane group (group CA + S). The animals ware anesthetized with intraperitoneal 10% chloraldurate 4 mg/kg and placed in a stereotactic apparatus to locate the lateral ventricle. In group C, after normal saline (NS) 2 μl was injected into the latersl ventricle, intraperitoneal NS 2 ml was injected. In group CA, after carbenoxolone 200 μg was injected into the lateral ventricle, intraperitoneal NS 2 ml was injected. In groups P,CA1 + P, CA2 + P and CA3 + P, NS 2 μl, and carbenoxolone 200, 300 and 400 μg were injected into the lateral ventricle respectively and then propofol 5 mg/100 g was injected intraperitoneally. Group S inhaled 1% sevoflurane (in increments of 0. 1% ) until the righting reflex was lost. Group CA + S inhaled 1% sevoflurane (in increments of 0.1% ) until the righting reflex was lost after carbenoxolono 200 μg was injected into the lateral ventricle. The time of loss of righting reflex, duration of loss of righting reflex and the sevoflurane concentration when the righting reflex disappeared were recorded. Results The loss of righting reflex did not appear in groups C and CA. Compared with group P, the time of loss of righting reflex was significantly shortened and duration of loss of righting reflex prolonged in groups CA1 + P, CA2 + P, CA3 + P ( P < 0.01 ). The time of loss of righting reflex was significandy shorter in groups CA2 + P, CA3 + P than in group CA1 + P (P < 0.05). The sevoflurane concentration when the righting reflex disappeared was significantly lower in group CA + S than in group S ( P < 0.05 ). There was no significant difference in the time of loss of righting reflex and duration of loss of righting reflex between CA + S and S groups ( P > 0.05). Conclusion Although inhibition of the function of gap junction can strengthen the anesthetic effects of propofol and sevoflurane, it is not the major mechanism.
9.Role of protein kinase C in induction of vascular endothelial growth factor secretion by isoflurane in primary cultured rat cardiomyocytes

Objective To investigate the role of protein kinase C (PKC) in induction of vascular endothelial growth factor (VEGF) secretion by isoflurane in primary cultured rat cardiomyocytes. Methods Primary cultured neonatal rat cardiomyocytes were randomly divided into 6 groups ( n = 6 each): control group (group C), 3 different concentration isoflurane groups (group Ⅰ1-3 ), PKC inhibitor calphostin C group (group P), and PKC inhibitor + isoflurane group (group PI). The cells were exposed to 0.7%, 1.4% and 2.1% isoflurane for6 h in group Ⅰ1-3 respectivly. Calphostin C was added to the culture medium with a final concentration of 50 nmol/L in group P. Calphostin C was added to the culture medium with a final concentration of 50 nmol/L, then the cells were exposed to 1.4% isoflurane for 6 h in group PI. VEGF concentrations and expression of PKC isoforms were determined by ELISA and Western blot respectively. Results Compared with group C, the VEGF concentration was significantly increased in group Ⅰ2 and Ⅰ3, and PKCε expression was down-regulated in the cytoplasm while upregulated in the cytomembrane in group Ⅰ2 ( P < 0.01 ), but no significant change was found in the parameters mentioned above in group Ⅰ2 ( P > 0.05). PKCα, PKCδ and PKCζ expression was significantly higher in the cytoplasm than in the cytomembrane in group C and Ⅰ2. VEGF concentrations were gradually increased with the increase in isoflurane concentrations ( P < 0.05). VEGF concentrations were significantly lower in group PI than in Ⅰ2 ( P <0.05) .Conclusion Isoflurane induces VEGF secretion in primary cultured rat cardiomyocytes through translocation of PKCε from the cytoplasm to the cytomembrane, suggesting that it is a mechanism of the cardioprotective effects of isoflurane.
10.Expression of homosapiens chromosome segregation gene in homogenous nasopharyngeal carcinoma tissue cells

OBJECTIVE To study the expression of CSE1L gene sifted from the whole genome expression profiling in the aRNA from homogenous human nasopharyngeal carcinoma (NPC) and non-NPC biopsy tissue samples using semi-quantitative RT-PCR (sqRT-PCR). METHODS CSE1L gene was sifted from the whole genome expression profiling from homogenous NPC and non-NPC biopsy tissue samples. RNA later RNA Stabilization Reagent was used to preserve the tissue samples harvested from the nasopharynx of NPC and non-NPC patients. The samples were microdissected to get the homogenous tissue cells and then the total RNA was isolated from them. The antisense RNA (aRNA) was amplified from the total RNA by “in vitro transcription” (IVT). CSE1L gene expression in the homogenous tumor cells of NPC and the pure epithelial pillar cells of normal nasopharyngeal tissue was investigated by sqRT-PCR. RESULTS The high quality total RNA could be harvested from the microdissected homogenous tissue cells, and then the sufficient aRNA was amplified from it. The expression of CSE1L gene was identified using these aRNA by sqRT-PCR. There were significantdifference between the average expression value of CSE1L in NPC tissue (1.740?1.105) and in non-NPC tissue (0.618?0.183; df=30, t=3.159, P=0.004). The expression of CSE1L gene in the whole expression profiling were 1.056?0.296 in NPC tissue and 0.465?0.835 in non-NPC tissue respectively (df=16, t=4.317, P=0.001) . CONCLUSION The whole genome expression profiling with sqRT-PCR could be used to sift the marker genes from biopsy tissue samples. CSE1L may be as a candidate oncogene in NPC.
