Objective To study the changes of CDH1 gene promoter CpG island methylation and its clinical significance in patients with esophagus and stomach double primary carcinoma(ESDC).Methods The expression of CDH1 gene methylation in cancerous tissues and adjacent cancerous tissues in 18 cases of ESDC were detected using methylation-specific PCR method. Results Eighteen patients were endoscopically diagnosed as ESDC between Jan. 2007 and Sep. 2009 in the 4th Hospital Affiliated to Hebei Medical University. The positive methylation of CDH1 gene in tissues of esophageal squamous cell carcinoma (ESCC)and adjacent cancer were 66.7% and 33. 3%, respectively, with significant difference (χ2= 4. 167, P = 0. 031). Whereas the positive methylation of CDH1 gene in tissues of gastric carcinoma (GA) and adjacent cancer were 77.8% and 44.4%, respectively, without statistical difference (χ2=1.786, P= 0. 180). There was no significant difference (P=0. 500) in positive rate of CDH1 gene methylation between ESCC tissues and GA tissues in same individual with ESDC. For 18 patients with ESDC, consistent change of CDH1 methylation in tissues of two kinds of cancers was found in 16 patients with a total agreement of 88.9 % (positive agreement of 66.7 % and negative agreement of 22. 2%). Statistical analysis showed a significant correlation between two groups (P = 0. 005). Conclusion In patients with ESDC, there is a high consistency of CDH1 methylation change, between ESCC and GA,which suggests that two kinds of cancer may have similar risk factors and molecular mechanisms.

Objective To structure the evaluation index system for nursing speciality development (EISNSD) in tertiary hospitals. Methods The evaluation index system for nursing specialty developmentwas established using a Delphi method. The weight coefficient at all levels was calculated by the analytic hierarchy process. The standard value of the index was set by multiple ways including target plan, industry regulation, historical data and so on. A total of 48 departments from three tertiary hospitals in Shanghai were investigated. The reliability and validity of the evaluation indexes were analyzed with Cronbach′s alpha coefficients and Spearman correlation. Results The EISNSD was structured with five primary indexes, 14 secondary indexes and 75 third indexes. The weight coefficients of primary indicators included 0.2859 for talent echelon building, 0.3772 for nursing practice, 0.1095 for teaching level, 0.1444 for scientific research and 0.0830 for the construction of professional training base. The correlation between indexes at all levels and their indexes of higher level was from 0.583 to 0.920 (P<0.01). The reliability of all indexes at all levels was from 0.581 to 0.945 with 0.931 for the reliability of total index. Conclusions The EISNSD in tertiary hospitals is of a strong scientificity and practicability. It can provide the quantiative basis for objective evaluation and continuity management of nursing specialty development of all departments in tertiary hospitals.

With the progress of high-throughput sequencing technologies and bioinformatics, and deepening understanding of immune system, immunomics has evolved from initially deciphering gene sequences of B cell receptor(BCR)and T cell receptor(TCR)to unraveling and mapping interactions between host immune system and antigens,as well as panorama of host immune system response mechanisms, which now encompasses various research areas, such as antigen epitopeomics,immunogenomics,immunopro-teomics,antibodyomics and immunoinformatics.Based on a large amount of immunological research data, immunological databases such as ImmPort,VDJdb and IEDB have been established to accelerate discovery of new antigen epitopes and study of immune response mechanisms.Immunomics has revealed the association between immune system and diseases, promoted the development of novel vac-cines and immunotherapeutic strategies, and effectively drove the development of personalized medicine and precision medicine. In recent years, integration of immunome with exposome and fusion it with artificial intelligence will have a significant impact on compre-hensively understanding immune system's response and regulatory mechanisms to environmental factors,as well as deciphering molecular mechanisms underlying disease occurrence and progression.
