1.Efficacy analysis of decitabine based regimen for treatment of elderly patients with acute myeloid leukemia

Objective To investigate the security and efficacy in the elderly patients with acute myeloid leukemia (AML) treated by decitabine based regimen. Methods A retrospective analysis was carried out on 12 elderly patients with AML who were treated by decitabine alone or with low-dose chemotherapy in Beijing Luhe Hospital, Capital Medical University from June 2014 to December 2015. Results There were 6 patients of complete remission (CR), 5 patients of partial remission (PR), 1 patient of none remission (NR) after one course of chemotherapy. 1 case of 6 CR patients relapsed after 6 cycles of chemotherapy , others remained CR during the follow-up . Among 5 PR patients , 2 cases achieved CR , 1 patient relapsed after 2 cycles of chemotherapy and 2 patients relapsed after 3 cycles of chemotherapy. 1 patient remained NR after 2 cycles of chemotherapy and died of severe pulmonary infection. 3 of 4 complex karyotype patients had poor efficiency, and only 1 patient achieved CR but relapsed eventually. 7 patients achieved CR in patients with +8, -X or normal karyotype in the near future. The main adverse effects were bone marrow suppression and infection. All patients could tolerate. Conclusion Elderly patients with primary AML treated by decitabine alone or with low-dose chemotherapy show good effects, well tolerance and high safety, which could serve as the front-line treatment.
2.Analysis on diagnostic value of abdominal CT, B ultrasound and magnetic resonance cholangiopancreatography in bile duct stones

Objective:Comparative analysis of imaging features of abdominal CT, B ultrasound and magnetic resonance cholangiopancreatography in three different methods for detection of extrahepatic bile duct stones. Methods: A random sample of extrahepatic bile duct stones were a sample of 150 cases as the object of study, patients in the endoscopic retrograde cholangiopancreatography after diagnosis, followed by B ultrasound examination, the implementation of CT scanning and MRI imaging in 142 cases, including 78 cases of B ultrasound detection, CT detection, 118 cases underwent MRI examination. Results: The detection results were recorded and compared the 3 imaging methods. Results: The positive rate of B ultrasound 39.4%, the positive rate of CT was 53.8%, MRI positive rate was 86.4%, the positive rate of MRI was significantly higher than that of B ultrasound and CT (x2=59.725, x2=25.595; P<0.01), the detection rate of CT was significantly higher than that of B ultrasound(x2=4.231, P<0.05). Diameter less than 8 mm of extrahepatic bile duct stones with B ultrasound and positive rate of 25%, the positive rate of CT was 33.3%, MRI positive rate was 60%, the positive rate of MRI was significantly higher than that of B ultrasound and CT(x2=7.789, x2=7.789;P<0.01), the detection rate of CT with no significant differences between the B ultrasound. Conclusion:MRI of extrahepatic bile duct stones with B ultrasound and the positive rate of CT is higher, but the clinical needs according to the comprehensive evaluation of individual patients, economic status, symptoms, diagnosis method for selecting suitable.

In order to reconstruct a bone defect as a result of a complete excision of osteofibromatosis with pathological fracture of an extremity, free transplantation of double vascularized long fibula segments was performed in 4 cases of osteofibroas hyperplasia with fracture. After a follow up of 1~2years, the grafted fibula attained a good bony union, and the limbs regained normal length and function. This seems to be an ideal way to repair huge bone defects as a result of excision of bone tumor of extremities.
4.Research on innovative tutor system with discipline crossing to train young teachers

To explore innovative training model for young teachers in university,literature and practical data in university were analyzed.A new concept of research tutor system was put forward in which young teachers and tutors were linked with scientific research project and common interests.Tutors in one subject trained the young teachers from another subject.In this model,a new relationship with equality and free-talk between tutors and students was established and the advantage of talent group effect was ascended.Discipline crossing,multiple communication and interactive learning may be promoted and the number of innovative young teachers and scientific research achievements may increase.
5.Effect of gadolinium chloride hexahydrate on rat hepatocellular mitochondria du ring liver cold ischemia storage

Objective To study the effect of gadolinium chl oride hexahydrate (GDCL3) on the structure and lipid peroxide (LPO) of rat hep atocellular mitochondria during liver cold ischemia storage. Methods Forty Wistar rats were randomized into 8 gr oups ( 5 in each group). Group 1~4 served as control. The other 4 groups were p retreated with GDCL3 (7mg/kg). The ultrastructures of hepatocellular mitocho ndria were observed by electron microscope and LPO of hepatocellular mitochondri a were investigated. Results LPO o f hepatocellular mitochondria in group 1h, 2h, 3h pretreated with GDCL3 for 1、 2、 3 h were 1.0±0.3、1.7±0.2、2.0±1.0, LPO was suppressed sign ificantly compared with control group (2.2±1.0、2.8±1.0、3.5±1.0、 P<0.05). With time prclonging of liver storage, the damage of hepatocellular mitochondria ultrastructure became more severe and LPO of hepatocellular mitoch ondria increased significantly. Pretreated with GDCL3, the damage of hepatocel lular mitochondria ultrastructures were lessened and the elevation of LPO was s uppressed significantly (1 hour、 2 hours、3 hours,P<0.05). Conclusion Mitochondria damage is one of the sensitive characters of liver cold ischemia injury, GDCL3 inhibits rat hepatoc ellular injury during liver cold ischemia storage.
6.Blockade of CD40/CD154 and B7/CD28 costimulatory pathways induces alloantigen-specific anergic state

Objective To investigate the possibility of inducing anergy by blocking the CD40/CD154 and B7/CD28 costimulatory pathways and the reversal condition of anergic cells. Methods Splenocyte proliferation in primary mixed lymphocyte reaction (MLR) consisting of BALB/c as responder and C3H as stimulator was measured by the addition of different levels of anti-CD154 and anti-CD80 monocolonal antibody (mAb). To test the reversal condition of anergic cells induced by combined anti-CD154 and anti-CD80 mAbs blocking, C3H or C57BL/6J spleenocytes were irradiated, or different concentrations of recombinant mouse interleukin-2 (rmIL-2), or both C3H splenocytes and rmIL-2 were added to the anergic cells. Results The proliferation of anergic cells treated with both mAbs in the primary MLR was strongly inhibited in a dose-dependent manner. The cells proliferated in response to third party (C57BL/6J) stimulator. The cells did not respond to original (C3H) stimulator, and they also failed to proliferated in response to the addition of exogenous IL-2. Furthermore, the anergic state was reversed by both original (C3H) stimulator and the addition of exogenous rmIL-2. Conclusion The blockade of CD40/CD154 and B7/CD28 costimulatory pathways induces alloantigen-specific anergy, and the anergic state can be reversed by both antigen restimulation and the addition of exogenous IL-2.

8.Evaluation of bone structure of hip joint using three-dimensional visualization system

BACKGROUND:It remains hard to judge characterization of lesion tissues using two-dimensional sectional image at present. Medical three-dimensional reconstruction technique could obviously improve work efficiency and accuracy of medical working staff on disease diagnosis.
OBJECTIVE:To develop a three-dimensional medical visualization system can rebuild three-dimensional model of corresponding parts by reading DICOM data, and observe the appearance of affected hip joint using reconstructed models.
METHODS:A three-dimensional (3D) medical visualization system was developed using the MFC in a PC with Windows XP operating system, development environment VC++6.0, VTK 5.6 instal ation, and necessary settings were made. Precise procedures are as fol ows: ① create a drawing object; ② create a drawing window, and draw objects was added in draw window; ③ read the CT image sequences, and set the path of the read image sequence; ④ isosurface (generate triangular facets) were extracted using MC algorithm, depending on the gray, skin and bones. Data of the input image sequence were set;gray value of tissue contour was set; ⑤the establishment of a V-belt objects and data mapping object; ⑥ graphics rendering, to receive the properties of the geometric data, and a different colors and transparency of bones and skin were set; ⑦ the viewing position was set to observe the position of the object and focus; ⑧ to create a human-computer interaction function.
RESULTS AND CONCLUSION:VC++6.0 and VTK could meet the needs of medical 3D visualization systems development. Developed 3D visualization system software could perform 3D reconstruction through reading CT images of DICOM format files. Thus, the relevant parts of the reconstructed 3D model could be observed by rotating, zooming, and panning so as to visual y observe the bone structure of hip joint, fractured appearance and type. It provided a reference for relevant therapy and operation.
9.Study on the inhibitory effects and related toxicity of all-trans-retinoic acid combined with arsenic trioxide on transplanted tumor of human bladder neoplasms in nude mice

ObjectiveTo investigate the inhibitory effects and related toxicity of all-trans-retinoic acid(ATRA) combined with arsenic trioxide(As2O3) on transplanted human bladder neoplasms in nude mice.MethodsThe subcutaneously transplanted tumor models of human bladder neoplasms in nude mice were established and then it was randomly divided into four teams: Saline group, ATRA group, As2 O3 group and the combination of ATRA and As2 O3 group.Each group composed of 10 nude mice and received intratumor injection in the following 14 d.The tumor growth inhibitory rate was calculated.The pathological changes of tumor,cardiac, liver, and kidney were observed after H.E.staining.The expression of MVD (Marked with CD43) and VEGF were detected by SP of immunohistochemistry.Blood routine examination and hepatic and nephritic functional examination was carried out to evaluate the side-effects.Results The tumor growth was significantly suppressed in ATRA group(inhibitory rate was 41.82%) and As2O3 group (inhibitory rate was 43.77%) compared with saline group.When ATRA combined with As2O3 ,inhibitory effects were improved(inhibitory rate was 68.55%).The difference of growth inhibitory rate between each group was obviously significant(x2 =26.81, P <0.01).There was less expression of MVD (Marked with CD43) and VEGF in ATRA group and As2 O3 group and the combination group compared with saline group (ODVEGF = 19.23 + 2.32,20.72 + 2.01,17.16 ± 1.59,27.33 ± 2.17, respectively; ODMVD = 44.77 + 8.25,43.39 + 7.41, 30.56 + 7.71,141.12 + 8.38, respectively), and the suppressed effect in the combination group was most significant (t = 3.16,3.08,3.37, P < 0.01).WBC inhibition was seen in ATRA group and As2O3 group and the combination group, but no significantly difference among them(P >0.05).Hepatic and nephritic toxicity were not found.ConclusionsCombination of ATRA and .As2O3 had obvious synergistic effects on transplantéd tumor of human bladder neoplasm in nude mice, and moderate white blood cell inhibition and no hepatic or nephritic toxicity.
10.Special design and three-dimensional modeling of artificial femur head

BACKGROUND:It is difficult to adjust the anteversion angle of cementless hip joint. Some areas rely on the intraoperative three-dimensional navigation technique to ensure the accuracy of the anteversion angle, but its high cost limits the promotion prospects. OBJECTIVE:To design a kind of artificial femur head, which has special functions to freely adjust the anteversion angle of artificial femur head during operation and lock prosthesis handle to prevent femur prosthesis dislocation and looseness, and to intuitively display the special design concept of this kind of artificial femur head through the three-dimensional dynamic image aided by computer. METHODS:According to the design idea, AutoCAD software was used to get the design drawings for artificial femur head. The three-dimensional modeling was performed by 3DMax software in order to observe the form and degrees of verisimilitude of model. RESULTS AND CONCLUSION:The AutoCAD software was used to draw out the design drawings of artificial femur head according to the design requirements and design idea:the prosthesis handle had three lock hole channel and its basal part was for scale, and rotating the scale could change the anteversion angle of prosthesis neck;the shape of the prosthesis neck was dentate cylindrical. The 3DMax software was used to build the three-dimensional model of the artificial femur head, and the design, form and degrees of verisimilitude of the model comply with the design requirements. The whole structure of the three-dimensional model of artificial femur head is clear and the design is reasonable, which can provide a theoretical reference for further design of artificial femur head.
