1.Mesh-like titanium alloy steel plateversus wire mesh in the repair of chest wall defects:a safety study

BACKGROUND:Titanium materials have high mechanical strength, stable physical properties, good biocompatibility, and excelent corrosion resistance, which can be safely used in humans. OBJECTIVE:To compare the safety of mesh-like titanium aloy steel plateversus wire mesh in the repair of chest wal defects. METHODS:Eighteen patients with chest wal defects, 11 males and 7 females, with an age range of 19-65 years, were enroled. The 9 of 18 patients were subjected to repair with mesh-like titanium aloy steel plate, and the other 9 patients underwent wire mesh repair. At 1 week after repair, visual analog scale scores were measured in the two groups, and patient’s satisfaction on the thoracic appearance and incidence of complications were surveyed during the folow-up. RESULTS AND CONCLUSION: Al patients were folowed up for 1-7 years. During the folow-up, X-ray review displayed good thoracic stability and no fracture of titanium plate and wire mesh. Patients recovered wel from respiratory dysfunction, with unapparent chest wal deformity and high satisfaction rate of the thoracic appearance. Moreover, compared with the wire mesh group, the satisfaction rate of the thoracic appearance was significantly higher (P< 0.05), but the incidence of complications was significantly lower in the titanium plate group (P < 0.05). At 1 week after repair, the visual analog scale scores had no difference between the two groups (P> 0.05). These findings indicate that the mesh-like titanium plate has good biocompatibility and safety to effectively maintain the stability of the chest wal.
2.Diagnosis and treatment of acute superior artery embolism:analysis of 10 cases

Objective To explore the early diagnosis,surgical treatment and clinical effect of acute superior mesenteric artery embolism.Methods Base on the retrospectively study of the clinical data of 10 patients with acute superior mesenteric artery embolism,all the patients were treated with routine blood test,D -dimer,abdominal ultrasound,abdominal CT examination.At the same time,partial selective superior mesenteric artery angiography was performed.Patients with definite or highly suspected of mesenteric artery embolism,such as no absolute surgical procedure, were treated by immediate surgical fixation.Superior mesenteric artery embolism was confirmed in the surgery.Then Operation was decided according to intestinal necrosis.Fogaty catheter removal was applied to the main trunk of the superior mesenteric artery.At the same time,such as the discovery of branches of vascular embolism,the therapy using direct injection of urokinase in local embolism was performed because of difficulty in embolectomy for narrow vessels. Then anticoagulation therapy was performed after surgery.Results Among them,1 case gave up treatment from the hospital,2 cases died of multiple organ failure after operation,and 7 cases recovered well after operation.Conclusion Early diagnosis of acute superior mesenteric artery embolization combined with early surgical treatment is the key to the treatment of acute superior mesenteric artery embolism.The cure rate can be significantly improved and the mortality rate can be reduced.Fogaty catheter was applied to treat acute superior mesenteric artery embolism with the therapy using direct injection of urokinase in local embolism has the characteristics of quick effect and curative effect.
3.Cx43 and Bcl-2 expressions in myocardial tissue during the developmental periods of human fetus

Objective To study the expressions of Connexin43 (Cx43) and B-cell lymphoma/leukemia-2 (Bcl-2) proteins in the ventricular myocardium of human fetus,and explore the correlation between the expression of the two proteins and the development of fetus myocardium.MethodsThe expressions of Cx43 and Bcl-2 protein in the ventricular myocardium of human embryos were immunohistochemically detected in the second,third and fourth month of gestation,respectively,by strepto-avidin-biotin-peroxidase complex (SABC) and avidin-biotin-peroxidase complex (ABC) technics.ResultsThe positive expression of Bcl-2 protein was detected in one or two cells in the ventricular myocardium of human embryos in the second month of gestation.With the advancement of fetal age,the number of Bcl-2 positive cells increased gradually and distributed in clusters in the ventricular myocardium of human fetus in the fourth month of gestation.The cells with positive expression of Cx43 protein were detected and scattered in dots-like structure in the cytoplasm of myocardial cells in the ventricular myocardium of human embryos in the second month of gestation.With the advancement of fetal age,the distributing region of Cx43 protein extended gradually,and the Cx43 protein positively expressed in the membranes between ventricular myocardial cells.The positively expressed Cx43 protein showed a diffused distribution in a sheet form in the ventricular wall of human fetus in the fourth month of gestation.ConclusionCx43 and Bcl-2 proteins participate in the early growth and development of the ventricular myocardial cells of human fetus.
4.Expression of neurofilament protein and synaptophysin in the developing spinal cord of human embryo

Objective To investigate the neurofilament protein (NF) and synaptophysin(SYN) distribution in the developing stage of human embryonic spinal cord and their clinical significance . Methods Totally 16 of human embryos, aged from 2 to 4 months were used in this study .Immunohistochemical staining was uesd to detect the expression and distribution of NF and SYN in the spinal cord .Results At the second month stage , NF expression was weakly positive in only the marginal layer on both sides of the spinal cord .At 3-4 months, NF expression was positive or strong positive in the spinal cord center crack before and after the middle of partition and cortex .In the ependymal layer of the lateral part of the expression of NF positive nerve fibers were seen in the gray matter .From 2 to 4 month-age, all visible SYN positive expressions distributed in the spinal cord .With the increase of gestational age , SYN positive expression in the spinal cord staining intensity value was increased first , and then decreased .The number of positive values increased month by month . Conclusion NF and SYN both take part in the regulation of human embryonic spinal cord tissue growth .
5.EXPRESSION OF Bcl-2 AND BAX PROTEINS IN THE DEVELOPING ESOPHAGUS OF HUMAN EMBRYO

Objective To explore the rule of cell proliferation,apoptosis and expression significance of involved proteins as Bcl-2 and Bax in the developing esophagus of human fetus. Methods In the second through the fourth month of gestation,the expressions of Bcl-2 and Bax proteins were observed in 14 human esophagus using the ABC of immunohistochemical methods. Results In the second,the third and the fourth months of gestation,there were Bcl-2 positive cells distributed in the intermuscular and submucous nerve plexus,and the mucous epithelial cells in the human esophagus.Bax weakly positive cells were observed in the mucous epithelial cells of the human esophagus.Conclusion Bcl-2 and Bax proteins are closely related with the developing esophagus of human embryo.
6.EXPRESSION AND SIGNIFICANCE OF nNOS,PAX3 AND CX43 PROTEINS IN EARLY DEVELOPING SPINAL CORD OF HUMAN EMBRYOS

Objective To explore the rule of distribution and the expression significance of neuronal nitric oxide synthase(nNOS),paired box 3(Pax3) and connexin 43(Cx43) proteins in the early developing spinal cord of human embryos. Methods During the 5th to 16th weeks of human gestation,the expression productions of nNOS,Pax3 and Cx43 proteins were investigated in the anterior horn of spinal cord using the immunohistochemical methods. Results During the 5th to 16th weeks of human gestation,the immunohistochemical staining intensities of both nNOS and Pax3 in the anterior horn neurons of spinal cord increased from weakly to strongly positive staining.During the 5th to 12th weeks of human gestation,the immunohistochemical negative staining of Cx43 protein was observed in the anterior horn neurons of spinal cord,while the positive staining was observed from the 13th to 16th weeks.Conclusion nNOS,Pax3 and Cx43 proteins are related to the growth and development of spinal cord in human embryos.

9.Modified anterior decompression for cervical myelopathy caused by ossification of posterior longitudinal ligament

Objective To evaluate the clinical result of modified anterior decompression for the treatment of cervical myelopathy due to ossification of posterior longitudinal ligament(OPLL). Methods From April 1994 to October 2002, 42 cases of OPLL involving 134 cervical segments were operated using modified anterior decompression, among them there were 35 cases, 122 segments of the continuous type; 4 cases, 9 segments of the mixed type; 3 cases, 3 segments of the solitary type. The preoperative index of thickness (O-Index) ranged from 12.4%-78.8%, with an average of 43.2%. A slot was made in the vertebral body through anterior approach and bilateral border of the ossification was dissected from the posterior wall of vertebral body with burr-drill. Results The average follow-up was 3 years and 8 months ranging from 1 year and 6 months to 8 years. The recovery of neurologic function was evaluated according to JOA score: 28 cases with mild neurologic deficit (13-16 points) improved on average 74% postoperatively; 11 cases of the moderately damaged group (8-12 points) improved on 75%; 3 cases of the severely damaged group (

Objective To discuss the clinical effect about flyback avulsion injury of skin in foot that repaired by anterolateral thigh flap and sural neurovascular flap with their neural anastomosis.Methods 14 patients including 10 males and 4 females,from 16 to 58 years old,with an average of 31 years.The anterolateral thigh flap taking along anterolateral thigh cutaneous nerve was cut to repair wall and sole of foot,at the same time,the anterolateral thigh cutaneous nerve and the medial plantar nerve was anastomosis;the lateral plantar nerve was embedded in the anterolateral thigh flap that was used to repair sole of foot.The sural neurovascular flap was reversed to repair lateral part of foot,and the superficial peroneal nerve was sutured with sural exterior and interior cutaneous nerve.According to Swanson evaluation of clinical effect of peripheral nervous impairment,the sensation of flap was classed as S1,S2,S3,S4,S5;the area of sensory recovery was classed as R1(
