1.Analysis of 23 G and 25 G+vitrectomy for retinal detachment

?AIM: To compare the clinical effect of 23G and 25G+vitrectomy for retinal detachment.?METHODS:Forty seven patients with retinal detachment were treated with 23G vitrectomy (27 eyes in 27 cases as group A) and 25G+ vitrectomy (20 eyes in 20 cases as group B ) . The operation time and the incidence of intraoperative complications were recorded. The occurrence of retinal reposition, visual acuity, intraocular pressure ( IOP ) and complications were observed. Postoperative follow-up time of the two groups were 3d, 1wk, 3mo. The relevant records were statistically analyzed and compared.?RESULTS: The operation time of 23G group and 25G+group were 50. 21+4. 52min, 49. 15+5. 14min,respectively and there was no significant difference between the two groups (P>0. 05). The main complications were retinal hemorrhage and iatrogenic retinal hole. There were 3 eyes with retinal hemorrhage, 2 eyes with iatrogenic retinal hole in 23G group, and 1 eye with retinal hemorrhage, 1 eye with iatrogenic retinal hole in the 25G+group, and the difference was statistically significant ( P<0. 05). The postoperative visual acuity of 23G group and 25G + group were significantly improved, and the differences between the two groups were not statistically significant at different time points after operation ( P>0. 05). The number of eyes with hypotonia in 23G and 25G+group were 3 and 1 eyes respectively, the difference was statistically significant ( P < 0. 05 ). But there were no significant differences between the two groups on IOP at 1wk and 3mo after surgery (P>0. 05). At the last follow-up, the results showed that 26 eyes ( 96%) with retinal reposition in 23G group, 19 eyes (95%) in 25G+ group, the difference was not statistically significant (P>0. 05).?CONCLUSION: The clinical effect of 23G and 25G+vitrectomy for retinal detachment is similar, but 25G+vitrectomy can reduce incidence of complications and early postoperative low IOP.
2.Establishment and application of loop-mediated isothermal amplification for detecting clinical isolates of Aspergillus spp

Objective To establish a method for detecting Aspergillus spp.by Loop-mediated isothermal amplification.Methods Aspergillus spp.specific primers were designed from the relative conservation region of the published sequence of 28S rRNA genes of A.fumigatus (GenBank accession number AY660917),A.terreus (GenBank accession number AF454183),A.flavus (GenBank accession number AF454158),and A.niger (GenBank accession number AF454169).Genomic DNA were extracted from Aspergillus standard strains,clinical control strains and clinical samples,and amplified by LAMP.The amplified results could be read with the naked eye by the coloring effect of fluorescent nucleotide dye without the DNA electrophoresis.Approximately 103 each Aspergillus conidia suspension was added to the serum from healthy volunteers,and detected these simulative clinical samples bv LAMP assay.Results The LAMP and PCR assays obtained positive results for all four Aspergillus species and 8 simulative clinical samples (double samples for each Aspergillus species) including 103 conidia,but negative in the remaining 15 non-Aspergillus species,human total blood genomic DNA,30 clinical serum samples infected with non-Aspergillus and 10 healthy volunteers.The LAMP assay had a minimum detection of 0.05-0.5pg,by means of detect different levels of 500,50,5,0.5,0.05 pg template in each reaction tube.Conclusion The results confirm that LAMP is a simple,rapid,sensitive and specific method,and can be used for detection of Aspergillus strains in clinical and environmental specimens.
3.Effects of Red Cell and Plasma Protein Parameters on Early Progression and Prognosis of Cerebral Infarction

Objective To study the effect of red cell and plasma protein parameters on progression, prognosis and recurrence of cerebral infarction. Methods Clinical data from 105 patients with cerebral infarction were analyzed. The pa?tients were divided into four paired groups:progressive stroke group and complete stroke group, short-term favorable progno?sis group and short-term unfavorable prognosis group, long-term favorable prognosis group and long-term unfavorable prog?nosis group, relapsed cerebral infarction group and not relapsed group by different criterion. The red cell and plasma protein parameters were compared between groups. Results There were significantly higher mean corpuscular volume(fL:85.92± 4.50 vs 83.79±4.64,t=2.164,P<0.05), red cell distribution width(fL:13.50±2.45 vs 11.90±2.90,t=2.694,P<0.01), globu?lin(g/L:27.46±4.33 vs 24.79±4.03,t=3.029,P<0.01)and lower albumin(g/L:39.00±3.86 vs 42.89±4.45,t=4.242,P<0.01)in progressive stroke group than those of complete stroke group. The elevated red cell distribution width, reduced albu?min were the risk factors of progressive stroke. In the short-term unfavorable prognosis group, red cell distribution width was significantly higher than that in short-term favorable prognosis group(fL:13.90 ± 2.45 vs 12.00 ± 2.12,t=2.905,P<0.01). The red cell distribution width was positively correlated with mRS scores assessed 3 months and 18 months after cerebral in?farction(P<0.01). Conclusion Progressive stroke rate increases in cerebral infarction patients with elevated red cell distri?bution and reduced albumin;Red cell distribution width has a certain reference value for forecasting the prognosis of cere?bral infarction .
4.Polymorphisms of 9 short tandem repeat loci in Chinese subjects of Li nationality in Hainan Province.

OBJECTIVETo obtain the data of the genotypes and allele frequencies of 9 short tandem repeat (STR) foci (D18S1364, D12S391, D13S325, D6S1043, D2S1772, D11S2368, D22-GATA198B05, D8S1132, and D7S3048) in Chinese subjects of Li and Han nationalities in Hainan Province.
METHODSA total of 154 randomly selected unrelated subjects of Li nationality and 112 unrelated local Han subjects from Hainan Province, along with 125 Han subjects recent immigrated to Hainan were enrolled in this investigation. Venous blood samples were obtained from the subjects to determine the allele frequencies at the 9 STR loci using multiplex primer extension assay. Statistical analysis was performed to determine the observed heterozygosities (Hobs), polymorphism information content (PIC), individual identification probability (Dp), accumulated power of discrimination, and cumulative rate of exclusion. The genetic polymorphisms of the 9 STR loci was analyzed to acquire the population genetics data.
RESULTSThe genotype and frequency distributions of the 9 STR loci were consistent with Hardy-Weinberg equilibrium. The Hobs, PIC, Dp, cumulative rate of individual identification (CDP), and cumulative chance of exclusion (CCE) showed no significant difference between the 9 foci.
CONCLUSIOND18S1364, D12S391, D13S325, D6S1043, D2S1772, D11S2368, D22-GATA198B05, D8S1132, and D7S3048 STR loci in Chinese Li and Han population in Hainan Province show a high probability of personal identification without obvious difference between the populations, suggesting their value in forensic science and population genetics.

6.Setup errors for the tumors in different parts of body with TomoTherapy

Objective To investigate the distribution rules of setup errors in different locations for tomotherapy.Methods 151 patients induding 53 head and neck tumors,45 thoracic tumors,20 abdominal tumors,and 33 pelvic tumors,who accepted tomotherapy were retrospectively analyzed in this study.The planning CT images of patients were obtained in simulation,and all patients underwent megavoltage CT (MVCT) scan before radiotherapy.And the setup errors were calculated by rigid registering MVCT images to planning CT images,and setup errors on + x(left),-x(right),+ y(in),-y(out),+z(ventral),-z (dorsal)axes were analyzed respectively.Results A total of 3 281 MVCT scans were performed on 151 patients,The setup errors on +x (left),-x(right),+y(in),-y(out),+z (ventral),-z (dorsal)axes were (1.61 ± 1.21),(1.76 ±2.11),(2.26 ± 1.74),(1.83 ± 1.47),(3.24±1.76) and (1.75 ± 1.61)mm for head and neck tumors;(2.43 ±1.88),(2.55 ± 1.92),(3.06 ±2.64),(3.90 ±2.91),(6.71 ±3.46) and (2.64 ±2.77)mm for thoracic tumors;(3.67±3.06),(2.37±1.77),(3.18±1.96),(3.98±3.01),(6.74±3.25) and (1.92±2.00) mm for abdominal tumors;(2.92 ±2.13),(2.17±1.68),(3.50±2.61),(3.72±2.66),(7.18± 3.43) and (1.92 ± 1.61)mm for pelvic tumors,respectively.The setup errors were different between +z and-z with statistically significant in all tumors (t =-4.119、-5.033、-3.763、-5.057,P < 0.05).The setup errors on + z direction of patients immobilized with thermoplastic mask were smaller than those immobilized with vacuum cushions for thoracic tumors (t =-2.357,P < 0.05).Conclusions The setup errors of head and neck tumors are less than other parts tumor in tomotherapy.The patients immobilized with thermoplastic mask can reduce the setup errors for thoracic tumors.The heterogeneity of setup errors on ventral-dorsal directions for the all parts of tumors should not be ignored.

Objective To investigate the distribution of virulence-related genes in multidrug resistant Escherichia coli.Methods Seven virulence genes papA,cnf1,cnf2,cfaB,ipaB,hofQ and ompT were detected by PCR in 20 strains of multidrug resistant Escherichia coli clinically isolated,and the positive genes were further searched in 31 strains of Escherichia coli in BioCyc database whose genomies had been fully sequenced.Results Virulence genes hofQ and ompT were detected in 20 strains of Escherichia coli with a positive rate of 95.0% (19/20) and 55.0% ( 11/20),respectively.Among 31 strains of Escherichia coli in BioCyc,21 (67.7%) were positive for hofQ gene and 15 (48.4%) were positive for ompT gene.Conclusion hofQ and ompT genes are prevalent in multidrug resistant Escherichia coli.
9.Rapid laboratory identification of Mycobacterium abscessus

Objective To identify Mycobacterium abscessus rapidly with HAIN molecular assay genotype kit、gene chip and hsp65 gene sequencing,and to assess the clinical value of these three methods.Methods 13 clinical non-tuberculous Mycobacterium (NTM) of in-patient samples were collected from January2014 to January 2015,in the Department of Clinical Laboratory,Yinzhou People's Hospital,and meanwhile,these strains were identified with HAIN molecular assay genotype Mycobacterium kit and gene chip respectively.The hsp65 gene sequencing was used as the standard method to be compared with HAIN and gene chip.Results The results of HAIN kit and hsp65 gene sequencing showed that all the 13 strains were subspecies Mycobacterium abscessus,while that of gene chip showed that these strains were Mycobacteriumchelonae complex strains,and the subtypescould not be identified.Conclusion These results obtained from the HAIN molecular assay genotype mycobacterium system are in agreement with those obtained from the hsp65 gene sequencing,whereas the HAIN kit method is easier to use.
10.Efficiency of benazepril combined with wind dispelling and dampness removing chinese herbs on stage 3 chronic kidney disease with wind-dampness syndrome: a prospective study.

OBJECTIVETo observe the clinical efficiency and safety of Benazepril and wind dispelling and dampness removing Chinese herbs were singly or combined used in patients with stage 3 chronic kidney disease (CKD 3), and to provide effective integrative medicine methods for treatment of CKD 3.
METHODSThe CKD 3 was allocated to qi and yin deficiency syndrome, inner disturbance of wind and damp syndrome, stasis in Shen meridian syndrome, and inner accumulation of damp and heat syndrome. Recruited were patients of inner disturbance of wind and damp syndrome accompanied or unaccompanied with the other 3 syndrome types. In the prospective, randomized, double blind controlled study, 60 patients confirmed as primary chronic glomerulonephritis (CGN) were randomly assigned to 3 groups with a total course of treatment for 24 weeks. Patients in the Western medicine group (WM, 23 cases) took Benazepril (10 mg/d). Those in the Chinese medicine group (CM, 20 cases) received treatment by syndrome typing. Those in the combination group (17 cases) used the two methods. The therapeutic efficacy and the occurrence of adverse reactions were observed in the 3 groups.
RESULTSThe inner disturbance of wind and damp syndrome accompanied qi and yin deficiency syndrome and stasis in Shen meridian syndrome was most often seen in these patients. It accounted for 75.0% in the CM group, 60.9% in the WM group, and 82.4% in the combination group. Totally 54 patients completed this trial. Of them, there were 19 in the CM group, 19 in the WM group, and 16 in the combination group. There was no significant difference in the total effective rate of Chinese medicine syndrome among the 3 groups (84.2%, 78.9%, and 87.5%, respectively) (P>0.05). As for the total effective rate of WM, it was obviously higher in the combination group than in the WM group and the CM group (100.0%, 94.7%, and 94.7%, respectively) (P<0.05). The ratio (24 weeks of treatment/pre-treatment ratio) of serum creatinine (SCr) was obviously higher in the WM group than in the combination group (P<0.05), the estimate glomerular filtration rate (eGFR) (24 weeks of treatment/pre-treatment ratio) was obviously lower in the WM group than in the combination group (P<0.05). There was no statistical difference in SCr and eGFR between the CM group and the combination group. The mean and median of 24 h urine protein ratio (24 weeks of treatment/pre-treatment ratio) were less in the combination group than in the other two groups. The urine protein greater than 2.0 g/d occurred in no case. The experimental drug-correlated adverse reaction rate was obviously higher in the WM group than in the CM group (30.4% vs 10.0%, P<0.05).
CONCLUSIONSAs for CGN CKD 3 patients, treatment by Benazepril combined with wind dispelling and dampness removing Chinese herbs showed favorable renal protective effects. It delayed the progress of renal failure, significantly improve the overall clinical efficacy. It was an effective treatment method for CGN CKD 3 patients with good patient tolerance and less adverse reactions.
