1.Effect of Electroacupuncture at Neiguan (PC6) and Shenmen (HT7) on Heart Rate Variability in Patients with Post-cerebral-infarction Sequelae

ObjectiveTo observe the effect of electroacupuncture at Neiguan (PC6) and Shenmen (HT7) on heart rate variability (HRV) in patients with post-cerebral-infarction sequelae.MethodEighty patients were randomized into two groups, 40 cases in each group. The control group was intervened by anti-platelet aggregation and other ordinary symptomatic treatment based on Western medicine, while the treatment group was by electroacupuncture at Neiguan and Shenmen in addition to the treatments given to the control group.ResultThere were no significant differences in comparing the HRV indexes between the two groups before intervention (P>0.05); while the HRV indexes in the treatment group were superior to that in the control group after intervention (P<0.05). HRV indexes were improved after intervention in the treatment group (P<0.05); of the indexes, only SDNN was improved after intervention in the control group (P<0.05), and the rest indexes were insignificantly changed (P>0.05).Conclusion Electroacupuncture at Neiguan and Shenmen can significantly reduce HRV in patients with post-cerebral infarction sequelae.
2.Real world clinical research of common complications and characteristics of traditional Chinese and western treatment in gastric malignant tumor inpatients.

OBJECTIVETo Understand the characteristics of the common complications in the gastric malignant tumor inpatients in our country, the related traditional Chinese and western medicine combined usage.
METHODBy using the method of frequency analysis and correlation, analysis the diagnostic and Prescribing information in HIS database of 20 national first-class ternary general hospital inpatients.
RESULTThe most common coexistent disease are malignant tumor in other parts, benign tumor. The most common complications are serous cavity effusion, lung infection, abnormal liver function. In other parts malignant tumor, more than 50% with advanced gastric malignancy; in other parts benign tumor, 86.00% located in the digestive and Urinary system, in the complications, digestive system diseases accounted for 23.80%; further according to the association rules of data mining, and calculate the combination scheme of the most commonly used for acid suppression drugs in combination with the centralizer and eliminate pathogenic traditional Chinese medicine (TCM).
CONCLUSIONIn the real world, most of the gastric malignant tumor disorder crowd is in the advanced stage, with lymph nodes and other parts of the transfer; and at the same time to incorporate the spleen and kidney disease in TCM zang-fu organs dialectical, should notice the reach mark early intervention, achieve the purpose of cure not ill; acid suppression hemostatic drugs combined the centralizer and eliminate pathogenic the three medicine combined treatment of Chinese traditional medicine is relevant.

3.Exploration and demonstration study on drug combination from clinical real world.

Drug combination is extensive in the clinical real world,which is an important part and the inherent requirements of the post-marketing evaluation of traditional Chinese medicine (TCM). The key issues and technology include multi-domain and multi-disciplinary such as the rationality, efficacy and safety evaluation of combination drug starting from clinical real world, study on component in vivo and mechanism of combination drug, the risk/benefit assessment and cost-benefit evaluation of combination drug and so on. The topic has been studied as clinical demonstration on combination therapy of variety of diseases such as coronary heart disease, stroke, insomnia, depression, hepatitis, herpes zoster, psoriasis and ectopic pregnancy. Meanwhile, multi-disciplinary dynamic innovation alliance of clinical drug combination has been presented, which can promote the academic development and improving service ability and level of TCM.

4.Taking evaluation of post-marketing as point of cut-in to promote systematic research of traditional Chinese medicine.

Research on post-marketing Chinese medicine should be the systematic study from application to mechanism. Clinical evaluation is the basis of mechanism study, we can find the clue from clinical evaluation, then make a mechanism study to find the reason, then apply the results to clinic. So it is a virtuous circle. In order to achieve it, we cannot be limited to traditional Chinese medicine, we should form multi-disciplinary team under the direction of grand science thinking, try hard to put industry-university-research institute collaboration association to use, and if necessary, explore the new model of the whole nation system. An appropriate operation mechanism is very important.

5.Study on levels of plasma apelin and its related factors in heart failure patients with different body mass index

Objective To investigate the correlations of body mass index (BMI) with plasma apelin, waist-hip ratio (WHR), fasting plasma glucose (FPG), glycosylated hemoglobin (HbAlc), biochemical indicator, blood fat and ultrasonic cardiogram (UCG) figures in patients with congestive heart failure (CHF).Methods A total of 41 elderly CHF patients (20 males and 21 females, aged 66.0±12.3 years) were divided into 3 groups according to BMI: normal group (n= 16), overweight group (n=13) and obese group (n= 12).And they were also divided into 2 subgroups: grade Ⅲ heart function group (n=22) and grade Ⅳ heart function group (n= 19).Enzyme linked immunosorbent assay was used to detect the levels of plasma apelin, and BMI, WHR, C-reactive protein (CRP), FPG, creatine kinase (CK), CK-MB, blood fat, electrolyte and UCG of all patients were detected .Results There were significant differences in waist circumference, hip circumference and WHR among the 3 groups (P<0.05 or P<0.01).But there were no significant differences in high density lipoprotein (HDL), low density lipoprotein (LDL), total cholesterol (TC), triglyceride (TG), FPG, HbAlc, CK, CK-MB, potassium, sodium, CRP, left ventricular internal diameter at end-diastole (LVIDd), left ventricular internal diameter at end-diastole (LVIDs), interventricular septal thickness at end diastole (IVSd) and left ventrieular ejection fraction (LVEF) among the 3 groups (all P> 0.05).The apelin level was higher in obese group than in normal group and in overweight group ((0.48±0.15) mg/L vs.(0.18±0.15) mg/L and (0.27±0.06) mg/L, both P<0.01].And the plasma apelin level was higher in grade Ⅳ heart function group than in grade Ⅲ heart function group [(0.35±0.16) mg/L vs.(0.26±0.13)mg/L, P<0.05].The level of plasma apelin was positively correlated with BMI, white blood cell, CK, hemoglobin and TG, and was negatively correlated with cardiac function and LVEF.The influencing factors for the plasma apelin were BMI (β=0.672, P<0.01), age (β=0.244, P<0.05) and HDL (β=-1.000, P<0.01).Conclusions The plasma apelin level is closely correlated with the development of heart failure.Cardiac dysfunction is more severer when the level of plasma apelin is higher.The high level of plasma apelin may be one of factors for the higher survival rate of the obese CHF patients.Plasma apelin level may be used as an indicator of state of illness.
6.On the Conditionality of "Disease" in Treating Different Diseases with the Same Method.

Treating different diseases with the same method is a unique original thinking in Chinese medicine, which embodies the spirit of treatment based on syndrome differentiation. We briefly reviewed the origin and development of this concept. We also reviewed that thinking of combination of disease and syndrome is its premise and foundation. We put forward the conditionality of diseases in treating different diseases with the same method by cutting-in modern biological basic researches, that is to say, one kind specific diseases or disease of one specific system. We emphasized the importance of diseases conditionality in treating different diseases with the same method, which was of great significance in studies on combination of disease and syndrome, correspondence of prescription and syndrome, and modern biological basic researches of treating different diseases with the same method.

7.Effect of congenital human CMV infection on growth of ICR embryo and cerebral cortex

Objective To investigate the effect of congenital human cytomegalovirus(HCMV) infection on the growth of embryo and cerebral cortex. Methods HCMV (6.0 log TCID50 in 1.0 ml/mice) was separately injected into the intraperitoneum of 10-week-old female mice on pre-mating, 3, 7, 15 gestation days. Cerebral cortices of fetuses were collected by laparotomy on 19th gestation-day just before delivery. The specimens were fixed with 4% buffered solution of paraformal dehyde. And then sectioned and stained with HE. Meanwhile, part of the specimens was used for viral isolation and HCMV DNA test by PCR. Results In the cerebral cortex of fetal mice, it was found that the capillary was dilated and congested, and the parenchyma was infiltrated with neutrophils, monocyte for experimental group and not for control group. The degeneration, necrosis and apoptosis of neurons co-existed. The rates of virus isolation for the fetal cerebral cortex among pre-mating, 3-, 7-, 15-gestation-day groups were 60%、62%、67%、25%,respectively (χ2=13.475,P<0.05) and the rate for 15 gestation-day group was significantly lower than others. Average weight of experimental mice for 15-gestation-day was significantly lower than control group and those for other gestation groups were not significantly different on group t-test. Conclusion HCMV can replicate in the cerebral cortex of ICR mice, infect fetal and initiate congenital infection of cerebral cortex of fetus. In addition, embryo of different gestation stages can be infected by HCMV on experiment. HCMV delays embryo growth and causes the cerebral cortex damage.
8.Cultivation strategy and path analysis on big brand Chinese medicine for small and medium-sized enterprises.

Small and medium-sized enterprises (SMEs) are important components in Chinese medicine industry. However, the lack of big brand is becoming an urgent problem which is critical to the survival of SMEs. This article discusses the concept and traits of Chinese medicine of big brand, from clinical, scientific and market value three aspects. Guided by market value, highlighting clinical value, aiming at the scientific value improvement of big brand cultivation, we put forward the key points in cultivation, aiming at obtaining branded Chinese medicine with widely recognized efficacy, good quality control system and mechanism well explained and meanwhile which can bring innovation improvement to theory of Chinese medicine. According to the characters of SMEs, we hold a view that to build multidisciplinary research union could be considered as basic path, and then, from top-level design, skill upgrading and application three stages to probe the implementation strategy.

9.Protective effect on rat retinal ganglion cells and the safety of intravitreal injected acteoside

Objective To observe the protective effect on retinal ganglion cells (RGC) and the safety of intravitreal injected acteoside in rats.Methods A total of 50 male Sprague Dawley rats with the weight of 190-210 g were used in this study.Fifteen rats were used for safety experiment of intravitreal injection of acteoside.The rats were divided into group A,B,C,control group and blank group,three rats in each group.The rats in group A,B and C were received intravitreal injection of 5 μl acteoside at the concentration of 1,2,and 5 mg/ml,respectively.Phosphate buffer solution (PBS) was injected in rats of control group.No treatment was performed for blank group.The retinal structure was examined by hematoxylin-eosin (HE) staining of retinal frozen sections at one,two and three weeks after injection.The retinal ultrastructure was examined by ultrathin section under transmission electron microscope at one and three weeks after injection.Others 35 rats were used for experiment of protective effect of acteoside on RGC.The rats were divided into operation group A and B (n=8),sham operation group C and D (n=8),and blank group (n=3).The optic nerve of rats in operation group was clamped for 10 seconds after optic nerve exposure,while the optic nerve of rats in sham operation group was exposed only.The rats in operation group A and B were received intravitreal injection with 5 μl acteoside (1 mg/ml) and 5 μl PBS respectively.The rats in sham operation group C and D were received intravitreal injection with 1 μl acteoside (1 mg/ml) and 1 μl PBS respectively.No treatment was performed for blank group.The retinal structure was examined by HE staining of retinal frozen sections at one,two and four weeks after injection.Immunohistochemistry was used to measure the expression of growth associated protein 43 (GAP-43).RGC apoptosis was assessed by the terminal deoxynucleotidyl transferase mediated dUTP-biotin nick-end labelling (TUNEL) method.Software of SPSS 13.0 was used for the data statistical analysis in this study.Results In the safety experiment of intravitreal injected acteoside,there was no abnormity of cornea,anterior chamber,lens,vitreous cavity and retina after injection.At one,two and three weeks after injection,the retina structure was normal without significant apoptosis,necrosis and inflammatory cell infiltration.The ganglion cell layer showed slightly edema; there was no obvious change of retinal ultrastructure after injection of acteoside with 5 mg/ml and 2 mg/ml,but slight change with the format of 1 mg/ml.Transmission electron microscopy showed that intravitreal injection of 5μl acteoside at the concentration of 2 or 5 mg/ml can induce significant changes of micro-structures of retina,while injections at 1mg/ml can only induce minor changes.In the experiment of protective effect of acteoside on RGC,light microscope revealed that the cell showed typical changes of apoptosis in operation group,but not in sham operation group and blank group.At the first and second week after injection,compared with the sham operation group and blank group,the RGC number was decreased in operation group.The difference of RGC numbers between operation group A and B was statistically different (F=26.206,P<0.05).The RGC numbers in operation group continues to decrease at the fourth week after injection,there was obvious difference compared with the first and second week after injection (F=17.364,P<0.05),but there was no difference of RGC numbers among sham operation intra-group and between sham operation group and blank group at all the time points.Immunohistochemistry showed that at the first week after injection,the integrated absorbance (IA) value in operation group was higher than that in other groups (F=33.466,P<0.05) ; there was no difference of IA value between operation group A and B.At the second week after injection,IA value in operation group A had slightly declined,but higher than that in operation group B (F=14.391,P<0.05).At the fourth week after injection,IA value in operation group A declined further,but also higher than that in other groups (F=4.178,P<0.05).TUNEL showed that on the first week after injection,RGC apoptosis rate in operation group was increased than that in other groups (F=15.365,P<0.05).At the second week after injection,RGC apoptosis rate in operation group was decreased,and it in operation group A was lower than that in operation group B (F=15.365,P<0.05).At the fourth week after injection,RGC apoptosis rate in operation group was decreased obviously,there was no difference compared with other groups (F =2.057,P > 0.05).There was no difference of RGC apoptosis rate between sham operation group and blank group at all the time points.Conclusion Intravitreal injection of 5 μl acteoside (1mg/ml) is safe for rat retina,and can up-regulate GAP-43 expression and inhibit RGC apoptosis in optic nerve crush rats.
10.Analysis of effect of surgical tyeatment of lumbar spinal styenosis in senile people

Objective To investigate the effect of surgical decompression on lumbar spinal stenosis in senile people. Method 175 senile patients with lumbar spinal stenosis were admitted from Jan. 1997 to Dec. 2003. Among them, the data of effect of surgical decompression in 106 cases, including 49 males and 57 females, age ranged from 70 to 82 years, all of them having a disease history of 4-10 years, and followed-up for 2-6 years after the operation. Their data were analyzed retrospectively. All the patients had been treated conservatively. 42 patients were complicated by one internal disease, and 55 complicated by at least 2 internal diseases. In all the patients continuous movement could not last longer than 15 min before operation. Lamina decompression was done in 95 patients, and a combined operation of lamina decompression, pedicle fixation and post-lateral bone grafting in 11 patients. Result All the patients survived the operation. Complications occurred in 38 cases, and the patients recovered after effective treatment. During follow-up, lumber complaint was improved in 37 cases, numbness in both legs was improved in 63 cases, and no satisfactory improvement was seen in 6 cases. In 66 cases the Oswestry score was lower than 20%, and in 34 cases it was lower than 10%. The result of surgical decompression was satisfactory in 94% of cases, and the average continuous movement was prolonged to over 30 min after operation. Conclusion Based on the effective control of complicated disease, the operative treatment of lamina decompression with fusion or no fusion played an important role in the recovery of neurological function in senile patients with lumbar stenosis.
