Objective To investigate the clinical significance of DNA haploid analysis for malignant degree and prognosis assessment of breast carcinoma. Methods Highclarity Colourful Pathological Analysis System-1000( HPIAS-1000) was used to analyze the DNA haploid of 120 breast carcinoma patients who had been followed up for more than 5 years. All patients were divided into three groups according to histology. 48 advanced differentiation cases, 44 middle differentiation cases and 28 low differentiation cases. Objective To investigate the effects of the two major conjugated linoleic acid(CLA) isomers on serum lipoprotein composition in fatty rats.Method Eighteen male OLETF rats were randomly divided into three groups.The control group fed with AIN76 diets,CLA groups were fed with 1% 9c,11t-CLA (9ct group) or 1%10t,12c-CLA (10tc group) containted AIN76 diets.After two weeks,serum triglyceride (TG),total cholesterol (TC),and high density lipoprotein cholesterol (HDL-c) were determined by commercial kits.On the other hand,serum lipoprotein were separated into chylomicron(CM),very low density lipoprotein(VLDL),low density lipoprotein(LDL) and HDL by HPLC according to the different particle sizes,and the TC and TG levels were measured in each lipoprotein.Results 10t,12c-CLA feeding reduced the concentrations of rat serum TG significantly,and increased the concentration of serum TC (26.1%) by increasing TC levels of the small particle size LDL and the big particle size HDL.While 9c,11t-CLA feeding increased the serum TG by 22.6%,and had no effect on the serum TC.Conclusion 10t,12c-CLA can reduce the concentration of serum TG and increase the concentration of HDL-c,but the effect on the improvement of atherosclerosis still need further investigation.

Objective To explore the effects of different infants-feeding patterns on preschoolers' physical development, intelligence development and bone mineral density (BMD). Methods A total of 953 preschoolers from seven cities and two rural areas in China were recruited by multiple stage stratified cluster sampling methods from November 2011 to April 2012. Physiology measurement was performed to get their height and weight and to calculate body mass index(BMI). BMD of distal radius was measured by ultrasound BMD scanner. The intelligence quotient was assessed by Standford-Binet Test of Intelligence(Chinese Version). The demographic factors and the feeding ways were investigated by an interviewer-administered questionnaire and all subjects were divided into three groups : exclusive breastfeeding, mixed feeding and artificial feeding group. ANOVA analysis, Chi-square test and non-parametric test were used to analyze the data. Results There were 476, 335 and 142 children in exclusive breastfeeding, mixed feeding and artificial feeding groups, respectively. The exclusive breastfeeding rate in urban areas was significantly lower than that in rural areas [47.2%(343/726) vs 58.6%(133/227), χ2=9.780, P=0.008]. The height-for-age Z score and weight-for-age Z score were significantly different among children with different feeding ways (χ2=10.728 and 12.175, P=0.005 and 0.002). The children in the exclusive breast feeding group showed lower height-for-age Z score and lower weight-for-age Z score comparing with those in the mixed feeding and artificial feeding groups (all P<0.05). The value of speed of sound of BMD was significantly different among exclusive breastfeeding, mixed feeding and artificial feeding groups [(3 640.2±142.0) vs (3 613.9±141.9) and (3 613.4±143.0) m/s, F=3.946, P=0.020], and that in the exclusive breastfeeding group was higher than in the mixed feeding group (P<0.05). No significant difference was found in BMI-for-age Z score, intelligence score and food allergy rate and food intolerance rate among the three groups(all P>0.05). Conclusions Feeding patterns during the first several months after birth may affect the weight, height and BMD in preschool-children, but not the intelligence development of these children.

Objective To probe into the trust of residents on the health centers' service offered in their community in general for providing better family doctor services.Methods A custom-made trust scale for medical service (including 23 indicators) and random sampling were called into play.An imercept survey was made to 224 residents of different gender,age and education in a community in Beijing regarding their overall trust on medical service.Results The different satisfaction over their selection of the 23 indicators among residents of different gender is not significant statistically (P＞0.05); the seven differences of the residents of different ages regarding their trust of the general medical service are significant statistically; the 11 differences of the residents of different education regarding their trust on the community health centers in general are significant statistically.Conclusion The GP-based community health centers are expected to improve the trust of residents on them in general,by means of enhancing government responsibility,government investment and the improvement of services of general practitioners.

Objective To investigate the effect of dexmedetomidine on the stress responses during wakeup test in patients undergoing cerebral functional area operation performed under propofol combined with remifentanil anesthesia.Methods Thirty-six ASA physical status Ⅰ or Ⅱ patients,undergoing cerebral functional area operation requiring wake-up test,aged 18-60 yr,weighing 50-70kg,were randomly divided into control group (group C) or dexmedetomidine group (group D) with 18 cases in each group.Dexmedetomidine 0.8 μg/kg was infused over 10 min before induction of anesthesia followed by infusion at 0.4 μg·kg-1 · h-1 in group D,while the equal volume of normal saline was infused in group C.Anesthesia was induced with target-controlled infusion of propofol and remifentanil and iv injection of cisatracurium.At 30 min prior to wake-up test,target-controlled infusion of propofol and application of mulscle relaxants were stopped,the target plasma concentration of remifentanil was decreased to 1 ng/ml,and in group D the infusion rate of dexmedetomidine was decreased to 0.1 μg·kg 1· h-1.Anesthesia time and consumption of anesthetics before wake-up test,wake-up time,and development of complications and intraoperative awareness during wake-up test were recorded.At 30 min prior to wake-up test (T1),immediately after wake-up (T2),at 5 min after wake-up (T3),and at 10 min after the anesthetic depth was deepened (T4),HR,mean arterial pressure and BIS value were recorded and blood samples were taken for determination of plasma concentrations of epinephrine (E) and norepinephrine (NE).Results Compared with group C,the consumption of propofol and remifentanil was significantly reduced before wake-up,the incidence of hypertension was decreased during wake-up test,and HR and plasma E and NE concentrations were decreased at each time point (P ＜ 0.05),and no significant difference in mean arterial pressure and BIS value was found in group D (P ＞ 0.05).Tachycardia,restlessness,bucking and awareness were not observed during wake-up test in group D.Conclusion Dexmedetomidine can inhibit the stress responses during wake-up test and raise the quality of wake-up test in patients undergoing cerebral functional area operation performed under propofol combined with remifentanil anesthesia.

Objective To assess the incidence,clinic feature,pathological characteristics and prognosis of incidental prostate cancer from specimens via radical cystoprostatectomy (RCP) for muscleinvasive bladder cancer.The postoperative erectile function in patients with NVB operation are also to be evaluated.Methods Between Jan 2002 and Jan 2015,150 male patients with bladder cancer who underwent radical cystoprostatectomy in our center were included in this study.133 patients underwent open surgery and 17 cases underwent laparoscopic operation (13 patients underwent NVB operation).The average age of 150 patients was 63 years (ranging 41 to 83 years).The average tPSA was 3 ng/ml(ranging 0.2 to 7.8 ng/ml).The digital rectal examination was negative,CT results didnt exhibit the evident of abnormality in prostate.Results Overall,incidental prostate cancer was diagnosed in 11 male patients (7.3％).The pathological diagnosis are all prostatic adenocarcinoma.In those patients,the mean age was 65.5 years (ranging 42 to 82 years).The mean tPSA at the time of surgery was 2.6 ng/ml (0.3-4.0 ng/ml).There was no statistical significance of tPSA values in the incidental prostate cancer group and non risk group (2.6vs.3.2ng/ml)(P ＞ 0.05).The preoperative enhanced CT imaging showed normal prostate in 7 cases,prostate hyperplasia in 2 cases,prostate calcification in 1 cases and bladder cancer invasion into the prostate in 1 cases.9 cases of incidental prostate cancer are low risk grade (Gleason score ≤ 6,the clinical stage ≤T2).Among 11 cases,bladder cancer histologic type in 10 cases was transitional cell carcinoma and small cell carcinoma in 1 case.According to the TNM classification,4 cases were less than pT2 stage and 7 cases were more than pT3 stage.11 patients were followed up between 6 to 24 months (mean 10 months).All patient survived at the end of study without the treatment of castration.Among 13 cases accepted NVB operation,8 cases achieve the preoperative level of erectile function to achieve.Conclusions The rate of incidental prostate cancer from RCP specimens is low,Most patients have low risk level,and there was no statistical significance of tPSA values between the incidental prostate cancer group and non risk group.

Objective To investigate the relationship between cross‐sectional area (CSA) and average CT value of posterior vertebral extensor group and bone mineral density(BMD) in postmenopausal women .Methods Totally 114 postmenopausal women being eligible for inclusion were enrolled .All the subjects ,ages range from 55 to 87 years old ,underwent quantitative computed tomography(QCT) scans from December 2015 to December 2016 in Beijing Jishuitan Hospital .The original image was transmitted to the image postprocessing workstation ,using Mindways QCT Pro software ,vertebral bone mineral density of L2-L4 were meas‐ured .CSA and average CT value of bilateral posterior vertebral muscles were measured by 2 diagnostic physicians in the L4 verte‐bral body horizontal axis CT images ,and analyzed respectively .Results Average CT values of posterior vertebral muscles between the left and the right side had a significant difference (P<0 .01) .The average CT value of bilateral posterior vertebral muscles was positively correlated with lumbar bone mineral density measured by QCT(P<0 .01) .There was no significant correlation between the CSA of posterior vertebral extensor group and BMD .Conclusion The loss of posterior vertebral muscle mass may be an impor‐tant risk factor for osteoporosis .The average CT value of posterior vertebral extensor group is more promising than CSA in the e‐valuation of the degenerative muscles .

Objective To investigate the variations of multi-segments of the Willis' circle in cases with ACA-A1 variation.Methods Retrospective comparative analysis was applied to images of 2 246 healthy cases which were excluded from abnomalities of physical and biochemistry examination and angiostenosis caused by angiosclerosis.We divided variant group and contrast group according to variation of ACA-A1 segment.Several aspects were carried to comparison:the variation between bilateral ACA-A1 segment within the variant group was studied by x2 test; the proportion of FTP and developing of PCA-P1 were put into test.Finally we use trend x2 test to compare the variation degree of ACA-A1,ipsilateral FTP formation and PCA-P1 variation.Results Variants of ACA-A1 contains 634 cases whereas normal cases were 1 612.A prominence in variation revealed in right side of ACA-A1 (x2=26.19,P＜0.01) which revealed by 429 cases in right side versus 205 cases in lefi side respectively.Parallelled with contrast group,the variant group showed immensely advantages in high proportion of FTP (x2=14.165,P＜0.01) which indicated by variant group 22.56％ (143/634)and contrast group 18.30％ (295/1 612); but crippled in PCA-P1 formation (x2=4.32,P＜0.05)which illustrated by proportions of by dysplasia and deficiency compared by variant group and contrast group was 17.35 ％ (110/634),14.21％ (229/1 612).AC A-A 1 normal,slight variation,dysplasia and deficiency together with FTP dyformation was 9.15％ (58/634),15.62％ (52/333),16.50％ (34/206),23.16％ (22/95) respectively.Within the variation group,a pattern between ACA-A1 and FTP which fluctuated simultaneously with significant statistical discrepancies (x2=51.117,P＜0.01; linear by linear x2=13.340,P＜0.01)which demonstrated by ACA-A1 normal,slight variation,dysplasia,deficiency together with FTP formation was 4.57％ (29/634),12.91％ (43/333),16.02％ (33/206),20.00％ (19/95) respectively,as far as ACA-A1 variation was concerned,The variant group were more prone to develop PCA-P1 dysplasia or deficiency (x2=45.87,P＜0.01; linear-by-linear x2=43.474,P＜0.01).Conclusions There were more multi-variation of Willis' circle in the cases with ACA-A1 variation,MRA can value the anatomy and variation of the Willis' circle.MRA can also guide the clinic diagnosis,treatment and prognosis of the cerebrovascular diseases.
