1.Analysis of factors of impacting on the outcomes of patients with ALI/ARDS

Objective To investigate the factors influencing the outcomes of patients with ALI/ARDS.Method Data of 63 patients with ALI/ARDS in ICU,Cancer Hospital,Chinese Academy of Medical Science,from January 2005 to December 2006,were retrospectively analyzed.Patients were divided into survivor group(n=39)and non-survivor group(n=24)according to different outcomes,and equally,patients were classified in the rcspect of different causes as pulmonary origin and extra-pulmonary origin.Results The incidence of ALI/ARDS was 5.2%(63/1201)in ICU.The univariate analysis showed that the differences in the length of mechanical ventilation(P=0.028),blood creatinine level(P=0.031),oxygenation index(P=0.023),between survivor group and non-survivor group.In addition,the differences in acute physiology and chronic health evaluation Ⅱ(APACHE Ⅱ)scores and sequential organ failure assessment(SOFA)on the admission day were significant between survival group and non-survival group(P<0.001,respectively).Logistic regression analysis showed that APACHE Ⅱ scores was the only independent predictor of the mortality in patients with ALI/ARDS(P=0.015,OR:3.809,95%CI:1.295~11.203).There was no significant difference in one-year survival between pul-monary origin cause of ALI/ARDS group and exwa-pulmonary origin cause of ALI/ARDS group(63.9% vs.55.4%).There was significant difference in one-year survival between ARDS and ALI group(44.9% vs.88.9%,P<0.05).Conclusions APACHE Ⅱscores on the admission day of patients is the only independent predictor of the mortality in patients with ALI/ARDS,whereas the underlying cause of ALI/ARDS does not matter to the survival of patients.
2.Research Progress of Penetration Enhancers

Penetration enhancers are substances to improve the rate or amount of transdermal permeation which is an important factor in transdermal drug delivery systems (TDDS). Recent researches have found that some of the new penetration enhancers have a higher penetration-effect, little irritation, fewer adverse reactions, and stable properties. In this article, domestic and foreign research reports on penetration enhancers have been collected and summarized. The research progress of penetration enhancers were reviewed, with a purpose to provide a reference for reasonable selection of penetration enhancers.
3.Localization of Hepatitis B Surface Antigen Within Recombinant CHO Cells in Response to Dimethyl Sulfoxide

The intracellular hepatitis B surface antigen (HBsAg) content per cell was increased by 7.2-fold in the culture with 1.5% dimethyl sulfoxide (DMSO) compared with that in the control without DMSO, while the extracellular HBsAg production and specific productivity were only improved by 70% and 3.2-fold, respectively. Electron microscope has been employed to reveal large dilated structures within recombinant CHO cells in the presence DMSO. The dilated structures have a distribution within whole cytoplasm, and some dilated areas were engulfed in the nucleus. These large, dilated structures were not observed in the control. Immunogold labeling was used to discover the accumulated HBsAg was localized within these dilated areas, and some HBsAg-specific labels were detected in the nucleus membrane, owing to the encroachment of the dilated areas upon nucleus. The result could help to reveal the mechanism of intracellular HBsAg accumulation in the presence of DMSO.
4.An analysis of values of massage to facilitate the rehabilitation of the women in puerperium

Objective To discuss the valuable influence of the total-body massage on the facilitation of the rehabilitation for the women in puerperium.Methods Both Chinese traditional medical total-body massage and the biological information theories and its methodology were employed in dealing with 50 cases of puerperants without contraindications,who had labored naturally 12 hours before and who had received cesarean sections 48 hours before. The results were compared with the control group in which conventional preventive health care for puerperants was adopted.Results Notable differences exist in the aspect of lochia,defecation,lactation and emotions between the two groups.Conclusion Significant benefits can be obtained from the massage to facilitate the rehabilitation of the women in puerperium while promoting their quality of lives and broadening the coverage of services of the preventive health care for the puerperants.
5.Quality evaluation of bletillae rhizoma based on hemostatic biopotency.

This dissertation is to determine the biopotency of hemostat which processed in different places by establishing a bioassay method of Bletillae Rhizoma based on the thrombin time. Contrast test is the main methodology. Specifically, the reference substance of Bletillae Rhizoma is determined by comparing with the control substance of vitamin K1 using thrombin time, which is calibrated the Bletillae Rhizoma. The hemostatic biopotency is calculated by using the method of "parallel line assay method based on quantitative responses" (3.3) from different processed products. It indicates that there is a strong linear correlation between Bletillae Rhizoma and control drugs (Y = 66.332-23.913X, R2 = 0.995 3). The hemostatic biopotency of Bletillae Rhizoma from different processed products ranged between 821.93-1 187.53 U x g(-1) shown in the paper, and all of them can meet the requirements of the test. The methodology has an appropriate instrument precision (RSD 3.8%), intermediate precision (RSD 4.6%), repeatability (RSD 3.2%) and stability (RSD 3.7%). Therefore, it can be turned out that the methodology which established in the dissertation is good at determinating the hemostatic biopotency of Bletillae Rhizoma and it is reliable, simple and repeatable.

7.Retinal blood supply changes after vitrectomy combined with panretinal photocoagulation in PDR patients with anterior segment neovascularization

AIM: To investigate hemodynamic alterations of retrobulbar vessels in proliferative diabetic retinopathy ( PDR) patients with anterior segment neovascularization, before and 3mo after vitrectomy combined with panretinal photocoagulation and to explore the clinical significance.
●METHODS: Color Doppler flow imaging ( CDFl ) was used for measurement of blood flow velocities and resistive indexes ( Rl ) of the ophthalmic artery ( OA ) , short posterior ciliary arteries ( sPCA ) and central retinal artery ( CRA ) in 21 eyes of 21 PDR patients with anterior segment neovascularization. CDFl parameters were obtained before and 3mo after vitrectomy combined with panretinal photocoagulation ( PRP) .
● RESULTS: Peak systolic velocity ( PSV ) and end diastolic velocity ( EVD ) of CRA were significantly increased after surgeries, Rl were decreased significantly (P<0. 05). Parameters of sPCA and OA have no change after surgeries (P>0. 05).
●CONCLUSION: Vitrectomy combined with panretinal photocoagulation might increase the velocity of CRA, decrease Rl and improve ocular blood supply postoperatively. lt may delay or prevent the process of neovascular glaucoma.
8.Incidence and risk factors of supraventricular arrhythmias in postoperative cancer patients in intensive care unit

Objective To evaluate the incidence and to investigate risk factors of supraventricular arrhythmia (SVAs) in postoperative cancer patients in intensive care unit ( ICU ). Methods Data of 570 patients consecutively admitted to oncologic surgical ICU of Cancer Hospital of Chinese Academy of Medical Sciences from Nov. 2008 to Oct. 2009 were retrospectively collected. Univariate and multivariate logistic analysis were conducted for potential factors that influenced SAVs. Results Thirteen patients with a history of atrial fibrillation (AF) were excluded and 557 patients were eligible for the study. SVAs occurred in 72 patients ( 12. 93% ). Multivariate analysis showed four independent predictors of SVAs including age ( OR = 1. 066,95%CI: 1. 034 - 1. 099,P <0. 001 ) ,a history of coronary heart diseases ( OR = 2. 644,95% CI: 1. 459 - 4. 790,P < 0. 05), sepsis ( OR = 2. 374,95% CI: 1. 098 - 5. 135, P < 0. 05 ) and intra-thoracic procedure ( OR =2. 322,95 % CI: 1.061 - 5.084, P < 0. 05 ) . ICU length of stay, severity ( APACHE Ⅱ scores in SVAs patients) were significantly greater in patients who were not affected by SVAs ( ICU stay: [2 ( 1 ~ 77 )]vs [3 ( 1 ~ 40 )]days,P < 0. 001; APACHE Ⅱ score: [9 (0 ~ 37 )] vs [11 (3 ~ 38 )], P = 0. 001 ). Nine cases died in SVAs patients ( 12. 5% ) and 19 died in the non-SVAs patients (3.9%), with significant difference between the two groups( x2 = 9. 673, P = 0. 002). Conclusion In oncologic surgical ICU, the incidence of SVAs is high. Age,history of coronary heart diseases, sepsis and intra-thoracic procedure were independent rsik factors of SVAs. SVAs prolong ICU length of stay. SVAs is a marker of critical illness severity.

10.The Analgesia of Oxymatrine Affecting Calcium Channel and GABA Release.

OBJECTIVETo explore the analgesia of oxymatrine (OMT) affecting high voltage-dependent calcium channels (HVDCCs) and GABA release under neuropathic pain condition.
METHODSTotally 66 C57BL/6 mice were randomly divided into the sham-operation group, the model group, and the OMT group, 22 in each group. Neuropathic pain models were established by partial sciatic nerve ligation (PSNL). Hind paw plantar mechanical response threshold (MWT) was measured by up-and-down method with Von-Frey filament. mRNA expression of HVDCCs in brains and spinal cords was detected with Real-time PCR and concentration of GABA was determined using ELISA kit.
RESULTSCompared with day 0, the left hind paw MWTwas decreased on day 7, 10, and 14 in the model group (P < 0.05). Compared with the sham-operation group, the left hind paw MWT was significantly reduced in the model group on day 7 (P < 0.05). The MWT of PSNL ipsilateral hind paw was decreased on day 7 before OMT administration, when compared with day 0 (P < 0.05), and increased after OMT administration (P < 0.05). Compared with the sham-operation group, mRNA levels of Cav1.2, Cav1.3, Cav2.1, and Cav2.3 in brain tissues were increased and those of Cav2.2 were decreased significantly in the model group (P < 0.05). In spinal cord tissues, mRNA levels of Cav1.2 and Cav1.3 were increased, but those of Cav2.1, Cav2.2, and Cav2. 3 were decreased significantly in the model group, when compared with those of the sham-operation group (P < 0.05). Compared with the model group, mRNA levels of Cavl.2, Cavl.3, Cav2.1, and Cav2. 3 in brain tissues were decreased, and those of Cav2.2 were increased significantly in the OMT group (P < 0.05). In spinal cord tissues of the OMT group, mRNA levels of Cav1.3 decreased and those of Cav2.1, Cav2.2, and Cav2.3 increased significantly with statistical difference, when compared with those of the model group (P < 0.05). Compared with the sham-operation group, GABA levels in brain tissues decreased in the model group (P < 0.05). Compared with the model group, GABA levels in brain tissues increased in the OMT group (P < 0.05). There was no statistical difference in GABA levels of spinal cord tissues among these groups (P > 0.05).
CONCLUSIONSOMT had analgesic effect on neuropathic pain, which might be probably related to HVDDCs. Cav2.2 might directly affect GABA release.
