2.Clinical study on the controlled subcutaneous analgesia with different doses of sufentanil for postoperative abdominal surgery patients

ObjectiveTo investigate the effects of controlled subcutaneous analgesia(PCSA) with sufentanil in patients underwent abdominal surgery and the occurrence of adverse reactions. Methods90 patients underwent abdominal surgery were randomly divided intothree groups with 30 cases each. Group Ⅰ :sufentanil 0. 04μg · kg-1 ·h-1 for postoperative analgesia; Group Ⅱ : sufentanil 0. 06μg · kg-1 · h-1 for postoperative analgesia; Group Ⅲ :sufentanil 0.08μg/kg · h for postoperative analgesia;PCSA started at the end of operation with catheter buried in deltoid muscle. Visual analgesia scale(VAS) ,Rasmay scale and side effects were recorded at 4,8,16,24 and 48h after operation. ResultsVAS scores decreased significantly in group Ⅱ and group Ⅲ compared with that in group Ⅰ ( all P < 0.05) ;There were no differences of Ramsay scores between groups ( all P > 0. 05) ;There were three cases of respiratory depression in group Ⅲ. ConclusionPCSA with sufentanil (0.06μg/kg) had exact analgesic effects for postoperative abdominal surgery patients with less adverse reactions.

AIM: The autologous bone marrow mesenchymal stem cell (BMSC) transplantation for treating acute myocardial infarction (AMI) via coronary artery in clinic has made remarkable and encouraging progress. The experiment aimed to study cytokine secretion in autologous BMSC transplantation for myocardial infarction dogs and its influence on angiogenesis and cardiomyocyte apoptosis. METHODS: Experiments were performed at the Animal Laboratory and the Central Laboratory, First Affiliated Hospital of Anhui Medical University between December 2005 and May 2007. 10 mL of bone marrow was sterilely aspirated from the posterosuperior iliac spine of 36 healthy mongrel dogs. BMSCs were isolated and purified by Percoll density gradient centrifugation and cultured in vitro by adherence method. After being co-cultured with 5-azacytidine for two passages, these cells were labeled by 5-bromodeoxyuridine (Brdu) for preparation. Dog models of AMI were established in a model control group and a cell transplantation group randomly. 2 mL of cell suspension of autologous BMSCs were implanted into four different regions in the acute myocardial site via topical injection in the cell transplantation group, while the same dose of DMEM was injected into the corresponding regions in the model control group. Myocardial infraction tissues were measured after transplantation. RESULTS: Flow cytometry demonstrated that the positive rate of CD34 and CD11b on third passage of BMSC cell surface was below 5%, while the positive rate of CD44 and CD105 was above 90%. Although chronic lateral instability of the ankle is a common disorder, there are not many reports and researches on it in China. This is because most patients with the condition do not seek a thorough cure of it and consequently surgeons pay little attention to it. In this article, we discuss some of the problems about its diagnosis, describe and compare several surgical procedures for reconstruction of the lateral ligament. We hope this paper can help surgeons know more about how to make an accurate diagnosis of the chronic lateral instability of the ankle and how to deal with it.

10.Economic Benefit of Interferon in Treatment of Chronic Hepatitis B Virus Infection

OBJECTIVE:To evaluate the economic burden of chronic hepatitis B(CHB)and its complications,and to e-valuate the benefits of treatment with interferon-?.METHODS:The components of the economic burden of disease included direct medical costs and non-medical costs as well as indirect economic loss per patient-year in patients with chronic hepatitis B virus infections,including chronic hepatitis B,compensated and decompensated cirrhosis,and hepatocellular carcinoma.Net cost savings due to treatment of CHB with interferon-?were estimated using clinical data from2therapeutic schemes used in hospital.RESULTS:For the patients at different stages of hepatopathy,the direct medical cost and economic burden per year were different:9000yuan,11362yuan(CHB);13865yuan,19412yuan(compensated cirehosis);25678yuan,36979yuan(decompensated cirrhosis);26501yuan,40264yuan(liver cancer),Compared with conrentional therapy against CHB,the direct medical cost and total cost were reduced following4months treatment of interferon-?after5-year follow-up.The reduced amounts were831yuan,2038yuan per person,respectively.The surrival rate rised by2.8%.CONCLUSION:CHB infections create a heavy economic burden on the society,as well as patients and their families.We have seen the potential cost-savings due to treatment with interferon-?.
