2.Therapeutic effect observation on acupuncture plus medication for post-stroke depression

Objective:To observe the clinical efficacy of acupuncture with medication for post-stroke depression.Methods:A total of 96 patients with post-stroke depression were randomized into an observation group and a control group,with 48 cases in each group.The control group was treated with oral fluoxetine hydrochloride capsules,and the observation group was treated with acupuncture on the basis of the treatment of the control group.After 6 months of treatment,the traditional Chinese medicine (TCM) symptom scores,and Hamilton depression scale (HAMD) component scores and total scores of the two groups before and after treatment were observed,and the clinical efficacy and adverse reaction rate were compared between the two groups.Results:The total effective rate was 95.8% in the observation group and 83.3% in the control group,and the difference between the two groups was statistically significant (P<0.05).After treatment,the intra-group differences in TCM symptom scores and HAMD score of both groups were statistically significant (all P<0.01).In the observation group,the scores of indifferent expression,emotional restlessness,and sentimentality in the TCM symptoms were statistically different from those in the control group (all P<0.01).The scores of desperate factor and sleep disorder factor and total score in HAMD in the observation group were significantly different from those in the control group (all P<0.01).The incidence of adverse reactions was 6.3% in the observation group and 4.2% in the control group,and the difference was not significant between the two groups (P>0.05).Conclusion:Acupuncture plus medication is an effective method for post-stroke depression,and it can further improve the patient's bad mood.
3.Application Status of Evaluation Methodology of Electronic Medical Record: Evaluation of Bibliometric Analysis.

In order to provide a reference and theoretical guidance of the evaluation of electronic medical record (EMR) and establishment of evaluation system in China, we applied a bibliometric analysis to assess the application of methodologies used at home and abroad, as well as to summarize the advantages and disadvantages of them. We systematically searched international medical databases of Ovid-MEDLINE, EBSCOhost, EI, EMBASE, PubMed, IEEE, and China's medical databases of CBM and CNKI between Jan. 1997 and Dec. 2012. We also reviewed the reference lists of articles for relevant articles. We selected some qualified papers according to the pre-established inclusion and exclusion criteria, and did information extraction and analysis to the papers. Eventually, 1 736 papers were obtained from online database and other 16 articles from manual retrieval. Thirty-five articles met the inclusion and exclusion criteria and were retrieved and assessed. In the evaluation of EMR, US counted for 54.28% in the leading place, and Canada and Japan stood side by side and ranked second with 8.58%, respectively. For the application of evaluation methodology, Information System Success Model, Technology Acceptance Model (TAM), Innovation Diffusion Model and Cost-Benefit Access Model were widely applied with 25%, 20%, 12.5% and 10%, respectively. In this paper, we summarize our study on the application of methodologies of EMR evaluation, which can provide a reference to EMR evaluation in China.

4.Structure of Wheat High Molecular Weight Glutenin Subunits and Their Role in Determining Processing Properties

The high molecular weight glutenin subunits (HMW-GS) are the main components of storage proteins of wheat,and play a critical role in determining the visco-elastic properties of gluten. There are both quantitative and qualitative effects of HMW-GS on the processing properties of wheat. Current knowledge of the molecular structures,compositions and properties of the gluten proteins of wheat is summarized in details,and the role of the HMW-GS in determining the quality of the grain for breadmaking and how their amount and composition can be manipulated leading to changes in dough mixing properties is also discussed systematically.

Reverse transcriptase-PCR experiments suggest that the two clusters of genes potentially involved in the oxidation of reduced sulfur compounds are organized as operons in strain of the acidophilic, chemolithoautotrophic bacterium Acidithiobacillus ferrooxidans ATCC 23270, the two clusters of genes including such the ORF of putative sulfate-thiosulfate-molybdate binding proteins, the ORF of putative thiosulfate: quinone oxidoreductase and the ORF of the rhodanese-like protein (P21). Bioinformatic analyses have predicted the possible promoters sequences and the possible +1 start site of transcription for the doxDA operons.
6.Prevalence and Detection Rate of Hypertension and Keshan-disease During Last Decade in Keshan-disease Epidemic Area

Background and Objective Keshan disease is clinically characterized as a dilated eardiomyopathy. We analyze the prevalence trend during last decade of hypertension and Keshan-disease in Yangzhuang village which was a Keshan-disease epidemic area.Method The survey including medical history,blood pressure and ECG were carried out every two years during the follow up 13 years.Results During follow up period,the total detection rate(hypertension:13.4 % vs Keshan-disease:10.7 %,?~2=8.555,P=0.002)and the accumulative rate of hypertension were higher than those of Keshan-disease,which was on the contrary to that before 1993,when increasing rate of Keshan-disease was higher than hypertension.Furthermore,the accumulative increasing rate of hypertension was 240.0%,which was higher than the national average level during corresponding period with no significant differences between female and male.Conclusion The detection rate of hypertension in Keshan disease epidemic area was higher than the average rate nation-wide.Whether the hypertension prevalence was re- lated to Keshan-disease needs further investigation.

Objective Detailed descriptions of the double-labeling immunofluorescence staining for fluorescence microscopy provides an ideal sample.Methods Rat liver frozen sections were used fixative were 95% alcohol,95% formaldehyde and acetone,frozen sections,with anti-CSE,Ki-67 polyclonal antibody and incubated with FITC,Cy3 fluorescence-labeled secondary fluorescently labeled secondary antibody staining,observed under a fluorescence microscope.Results Acetone fixed group visible in the proliferative phase (S phase) cells showed a red fluorescent nucleus,cytoplasmic green fluorescence.Conclusion The impact of double labeling immunofluorescence the effect of sample links and many factors,including the two most important factors are 2 and coordination with the primary antibody and select the appropriate fixative.
8.Clinical analysis of 47 cases of livedo vasculitis

Objective To assess the clinical and pathological feature of patients with livedo vasculitis.Methods Clinical data were collected from 47 patients with livedo vasculitis,and retrospectively analyzed.All the patients were classified into three groups according to treatment strategy:anticoagulation group,anticoagulation + corticosteroids group,and anticoagulation + sulfasalazine group.Results Clinically,livedo vasculitis usually began as petechia or ecchymosis,edema with distending pain,followed by spotty necrosis which subsequently evolved into vermiculated ulcers and,finally,atrophie blanche.Pathological features included fibrinoid degeneration of and thrombus formation in small vessel walls within the superficial or deep dermis,as well as a sparse lymphocytic infiltrate around the injured vessels.The average time to onset of action of administrated drugs was (9.14 ± 3.48),(5.62 ± 1.04) and (8.23 ± 2.68) days,and time to remission was (2.57 ± 1.41),(4.06 ± 1.51) and (5.64 ± 1.32) months,in the anticoagulation group,anticoagulation + corticosteroids group and anticoagulation + sulfasalazine group,respectively.Conclusion Anticoagulation in combination with anti-inflammatory therapy appears to have a more rapid onset of action in the treatment of livedo vasculitis with a reduced recurrence rate compared with anticoagulation therapy alone.
9.Effect of tolterodine on catheter-related bladder discomfort after general aneathesia operation

Objective To determine whether a prophylactic tolterodine administration before surgical operation on non-urologic patients under general aneathesia can prevent the occurrence of catheter-related bladder discomfort (CRBD) ; and to assess patients’ tolerance to the symptoms as well as the impact on related consultation work of urologic surgeons.Methods One hundred and eighty cases of non-urology patients who need general aneathesia operations were divided into 2 groups:90 cases in tolterodine group and 90 in control group.The assessment of CRBD is categorized into 4 steps and statistics for adverse events ( dry mouth,dizzyness and facial flushing) was also conducted.A record of the patients’ needs for urologic surgical consultation during their reservation of catheter was also kept.SPSS 13.0 used in the statistical analysis of data in terms of X2 examination,where the divergence P < 0.05 was regarded statisticly valid.Results 82 cases were followed up in the tolterodine group with a 24.4% CRBD occurrence,which included 7.2% shows moderate and severe symptoms,and there were also 23 cases with dry mouth ( 28.0% ),4 cases with dizzyness (4.8%),13 cases with facial flussing ( 15.8% ),and 1 case who needs further consultation (1.2%).In the 86 followed-up cases in control group,CRBD occurance rate was 54.7%,with 30.2% showed moderate and severe symptoms,plus 2 cases suffered from severe consequences.Nine cases ( 10.5%) in control group requires further consultation ( X2 =19.499,P =0.000 < 0.05 ).Conclusions A prophylactic tolterodine administration before surgery to the patients underwent general aneathesia can prevent the occurrence ofcatheter-related bladder discomfort (CRBD) and reduce the consultation work of urologic surgeons.Patients using tolterodine show a higher rate of adverse events,yet to which most patients can tolerate.
10.Construction and pathological features of a rabbit osteoarthritis model in early, middle, and later phases

BACKGROUND: Application of traditional osteoarthritis animal models was limited by long duplicated time, poor stability, different successful rate, and rough analysis of osteoarthritis.OBJECTIVE: To observe clinical and pathological features of osteoarthritis by modified Hulth modeling way and to determine the stages of ostecarthritis.METHODS: A 2-cm medial longitudinal incision was resected to expose knee joint. Anterior and posterior cruciate ligament and medial collateral ligament were cut off, and medial meniscus was fully cut to reserve articular cartilage., The injured limb was not fixed postoperatively. Animals were allowed to move freely. At one week after surgery, 800 000 U penicillin was used to avoid from infection, 30 mind, twice per day, for 12 successive weeks. The normal group was treated without any treatments. Pathological features were observed using Mankin scores under electron and optic microscope at 4, 6, 8, 10, and 12 weeks postoperatively.RESULTS AND CONCLUSION: Changes of osteoarthritis were observed in the model group at 4 and 6 weeks after operation,showing synovial hyperemia and hyperplasia, increased synovial fluid effusion, cartilage surface roughness, matrix stained tinge,and Mankin score of 3.5-3.8. Intermediate stage changes of osteoarthdUs were found in the model group at 8 weeks after operation, showing synovial hyperplasia, less synovial fluid, flssuration reaching cartilage surface, cartilage cells with tangled and uneven staining matrix, Mankins score of 8-9. Advanced osteoarthritis changes were observed in the model group at 12 weeks after operation, showing severe nodular synovial hyperplasia, less and turbid synovial fluid, osteophyte formation of serious exposure of subchondral bone, cartilage calls reducing the majority of loss of matrix staining, and Mankin score of 12-14. Electron microscopy indicated a coincidence with the histological observation of cell mutation. The rabbit model by Hulth suggested that early change of osteoarthritis occurred at 6 weeks after operation, intermediate stage at 8 weeks, and advanced stage at 12 weeks.This model could be more comprehensive response to osteoarthdtis cartilage degeneration from early compensatory hypertrophy to decompensation after the cartilage cells and matrix reduction, cartilage softened to endarterectomy missing characterized the late changes in the entire process.
