1.Analysis on diagnostic value of abdominal CT, B ultrasound and magnetic resonance cholangiopancreatography in bile duct stones

Objective:Comparative analysis of imaging features of abdominal CT, B ultrasound and magnetic resonance cholangiopancreatography in three different methods for detection of extrahepatic bile duct stones. Methods: A random sample of extrahepatic bile duct stones were a sample of 150 cases as the object of study, patients in the endoscopic retrograde cholangiopancreatography after diagnosis, followed by B ultrasound examination, the implementation of CT scanning and MRI imaging in 142 cases, including 78 cases of B ultrasound detection, CT detection, 118 cases underwent MRI examination. Results: The detection results were recorded and compared the 3 imaging methods. Results: The positive rate of B ultrasound 39.4%, the positive rate of CT was 53.8%, MRI positive rate was 86.4%, the positive rate of MRI was significantly higher than that of B ultrasound and CT (x2=59.725, x2=25.595; P<0.01), the detection rate of CT was significantly higher than that of B ultrasound(x2=4.231, P<0.05). Diameter less than 8 mm of extrahepatic bile duct stones with B ultrasound and positive rate of 25%, the positive rate of CT was 33.3%, MRI positive rate was 60%, the positive rate of MRI was significantly higher than that of B ultrasound and CT(x2=7.789, x2=7.789;P<0.01), the detection rate of CT with no significant differences between the B ultrasound. Conclusion:MRI of extrahepatic bile duct stones with B ultrasound and the positive rate of CT is higher, but the clinical needs according to the comprehensive evaluation of individual patients, economic status, symptoms, diagnosis method for selecting suitable.

Detection of lipids ,lipoprotein and lipid metabolism related indicators is important in the risk assessment and prevention of atherosclerotic cardiovascular disease ( ASCVD ) .Along with the research progress,there are still defects within the established lipid biomarkers and methods ,which need continuous optimization.The ASCVD risk assessment is expected to be further improved as the new biomarkers are coming up.
3.Identification of peroneal nerve’ s safety zone and its clinical value in treatment of MIPPO for proximal tibia fracture

Objective To define the peroneal nerve’ s safety zone and study its clinical value in treatment of MIPPO for proximal tibia fracture. Methods The peroneal nerve’ s safety zone was defined according to dead body anatomy. Patients with proximal tibia fracture were divided into traditional therapy group ( control group) and peroneal nerve’ s safety zone therapy ( observation group) . The curative effects and complications were compared. Results The peroneal nerve’s safety zone of R1 was (45. 30 ± 1. 55) mm,R2 was (45. 61 ± 1. 40) mm,R3 was(45. 42 ± 1. 62) mm,angle A was(33 ± 2. 1)°,angle B was(97 ± 2. 3)°. The operating time,bleeding volume,hospitalization time and fracture time in observation group were significantly lower than those in the control group,but there was no siginificant difference in post-ther-apy infection,delayed healing and other complications. Conclusion The curative effects according to peroneal nerve’ s safety zone are better than traditional MIPPO in proximal tibia fracture.
4.Influence of flood disaster on Oncomelania snail distribution in Wuhu section of Yangtze River in 2020

Abstract: Objective To analyze the impact of the flood disaster on the distribution of Oncomelania snails in 2020 in Wuhu City, to provide scientific basis for formulating the "14th Five-Year Plan" for schistosomiasis control and precise prevention and control measures in Wuhu City. Methods Systematic sampling combined with environmental spot check was used to investigate the distribution of snails in the historical snail's environment, existing snail's environment and suspected snail's environment in Wuhu City. The collected snails were crushed and examined by microscope to understand the death and life of snails and the infection of Schistosoma japonicum, and the density of live snails and other indicators were counted. Results The historical area of snails was 14 475.24 hm2 in Wuhu City, and the existing area of snails was 4 588.72 hm2, including 4 210.32 hm2 for lake type and 378.40 hm2 for hill type snails. The average occurrence rate of live snails was 14.59%, and the average density of live snails was 0.50 snails/0.1 m2 in Wuhu City. There were 2 745 snail breeding environments, of which 491 were lake type and 2 254 are hill type, accounting for 17.89% and 82.11% respectively. The existing snail area was mainly distributed in the beaches and ditches, accounting for 92.51% and 6.29% of the existing snail area respectively. Some snails were distributed in ponds, paddy fields and other environments. Among all the historical snail habitats, the majority were class Ⅱ and class Ⅲ environments (which still have the basic conditions for snail breeding at present), with a total of 1 739 sites (blocks), accounting for 63.35% of the total environment. No schistosomiasis infected snails have been found, and the newly discovered and recovered snails cover an area of 268.21 hm2 in Wuhu City. Conclusions The distribution of snails is extensive in Wuhu City, and snails are mostly distributed in beaches, which are vulnerable to the impact of flood disasters. The spread of Oncomelania snails is found in 30 marshlands in 3 counties in this survey. It is necessary to continue to strengthen snail monitoring after disasters, and take class Ⅱ and class Ⅲ environments as key areas for snail monitoring, so as to find, identify and evaluate the risk of schistosomiasis transmission as soon as possible, to avoid or reduce the impact of flood disasters on the process of schistosomiasis control, and promote the process of schistosomiasis elimination.
5.Effects of perioperative intraperitoneal chemotherapy with 5-fluorouracil sustained release agent on advanced gastric cancer

Objective To investigate the effects of perioperative intraperitoneal chemotherapy with 5-fluorouracil (5-FU) sustained release agent on the local recurrence and survival rates of patients with advanced gastric cancer. Methods The clinical data of 100 patients with advanced gastric cancer who had been admitted to our hospital from January 2002 to December 2004 were retrospectively analyzed. All patients were divided into treatment group and control group. Only patients in the treatment group were implanted with 5-FU sustained release agent (800 mg) in abdominal cavity after D2 gastrectomy. All patients accepted chemotherapy according to the FOLFOX regimen. Results The 3-year survival rate and 3-year disease free survival rate were 45% and 28% for treatment group, which were significantly higher than 32% and 16% for control group. Conclusions Intraperi-toneal implantation of 5-FU sustained release agent during operation can decrease local recurrence rate and improve the 3-year survival rate of patients with advanced gastric cancer after radical gastrectomy.
7.Exploration of the socialized management model of medical transportation service in large-scale grade three hospital

Objective To discuss the socialized management model of medical transportation service in large-scale grade three hospital.Methods Selected a company through a competitive bidding,using dual management model of company and nursing department.The nursing department developed and implemented a unified regulatory system,introduce competition mechanism,participate incentives reform quality inspection programs,hold multi-sect oral coordination meetings regularly and also the hospital and company projeot management meetings.Results Socialization of medical transportation service not onlyreduced the problem of the hospital management cost to solve the hospital's birth control and labor,but also improved the satisfaction of patients and medical staff to the transportation members,it provided a strong support system to clinical service.Conclusions Implementation of social management in allied transportation service in hospital is feasible.The company management,nursing supervision model is a new path for transportation management.It is a guarantee of mechanism innovation,intensive training,strict management and supervision of a stable multi-party transport team to provide high-quality transportation service.

9.Clinical Pharmacist Participating in Medication Reconciliation for an Elderly Patient

OBJECTIVE:To investigate the role of medication reconciliation in pharmaceutical care provided by clinical phar-macists. METHODS:Clinical pharmacists participated in pharmaceutical care for a elderly patient,and reconciled drugs as anti-in-fective drugs,cardiovascular drugs,electrolyte:stop taking unnecessary drugs:Xuebijing injection,Kang'erxin capsule,Shedan chuanbei soft capsule,Mosapride citrate tablet;adjust the dose of Dihydrochlorothiazide tabet and Potassium chloride tablet;stop taking Non first-line drug Reserpine tablet in order to decrease the risk of drug use;stop taking Furosemide tablet,Lactulose oral solution and using Suppositories glycerol timely according to the disease outcome. RESULTS&CONCLUSIONS:The medication reconciliation can avoid repeated drug use,optimize medication plan,reduce drug variety and cost and decrease the potential ADR risk of drug use so as to guarantee safe and effective drug use.
10.Diagnostic Value of Pathogenic Detection on Acute Respiratory Infection in Children by Multi-IgM Investigation

Objective To investigate the pathogens of acute respiratory infection of children.Methods A total of 159 children with acute respiratory infection who were hospitalized in our department from August 2005 to January 2006 were involved in this study.The serum IgM antibody of 18 pathogens were detected by indirect immunofluorescence test.The 18 pathogens included respiratory syncytial virus(RSV),adenovirus(ADV),influenza A(H1N1,H3N2)and B viruses,parainfluenza viruses(PIV) type 1,2,3 and 4,coxsackie virus B1(CBV1),coxsackie virus A7(CAV7),echovirus(ECHO7),haemophilus influenzae(HI),klebsiella pneumoniae(KP),bordetella pertussis(BP),bordetella parapertussis(BPP) and legionella pneumophila serotype 1 and 12.Results The evidence of specific IgM was obtained in 103 of 159 patients(64.78%).Influenza A was found in 66 cases(64.08%),influenza B in 49 cases(47.57%),enterovirus in 26 cases(25.24%),RSV in 18 cases(17.48%),PIV in 11 cases(10.68%),and co-infection in 66 cases(64.08%),1/ 3 of them were co-infected with influenza A and B.Conclusions Viruses are the most common agents of acute respiratory infection.Influenza virus is predominant among them.
