1.Hepatic artery and portal vein dual perfusion chemotherapy combined with intratumoral injection of lipiodol-ethanol for unresec-table primary hepatic carcinoma

Objective To study the therapeutic effect of hepatic artery and portal vein dual perfusion chemotherapy (AVPC) combined with intratumoral injection of lipiodol-ethanol (ITILE) for unresectable primary hepatic carcinoma (PHC). Methods 138 pathologically proved and unresectable PHC cases were divided into two groups: Group A (80 cases), treated with AVPC through hypodermic implanted drug delivery pumps. Group B (58 cases), treated with AVPC plus ITILE. Results The secondary resection rate was 2.5% in group A, while 12.1% in group B, (P<0.05); The 0.5, 1, 2 years survival rate in group A was 56.3% 45.0% and 21.2%, in group B 81.0% 61.2% and 39.6% respectively. there were significent difference between two groups in 0.5,1,2 years survival rate respectively (P<0.05); the complication occurrence rate was found no significant difference between two groups (P>0.05). Conclusions The therapeutic effect of AVPC plus ITILE for unresectable PHC is much better than that of AVPC alone.
2.Associations of obesity and peripheral blood lipid indicators with non-small cell lung cancer: a Mendelian randomization study

Objective:
To examine the causal relationships between obesity, peripheral blood lipid indicators and non-small cell lung cancer (NSCLC) using Mendelian randomization (MR) method, so as to provide the basis for developing NSCLC prevention and control strategies.
Methods:
Genetic variation data of three obesity evaluation indicators, including body mass index (BMI), body fat ratio (BFR) and waist-to-hip ratio (WHR), and seven peripheral blood lipid indicators, including triglyceride (TG), total cholesterol (TC), low density lipoprotein cholesterol (LDL-C), high density lipoprotein cholesterol (HDL-C), apolipoprotein A1 (ApoA1), apolipoprotein B (ApoB) and lipoprotein a [LP (a)] were collected through genome-wide association studies (GWAS) and related public databases. Potential causal relationships between obesity, peripheral blood lipid indicators and NSCLC were analyzed using inverse-variance weighted (IVW) method and multivariable MR analysis upon a random effect model. Heterogeneity and horizontal pleiotropy of instrumental variables were evaluated using Cochran's Q test and MR-Egger regression.
Results:
There was statistically association between BMI with NSCLC (OR=1.256, 95%CI: 1.087-1.451); there were no statistically associations between BFR, WHR, seven peripheral blood lipid indicators and NSCLC (all P>0.005). There was heterogeneity in the association between BMI, BFR, WHR, TG, HDL-C and NSCLC (all P<0.05); no horizontal pleiotropy of instrumental variables was found (all P>0.05). There was no statistically association between BMI and NSCLC after adjusting BFR (OR=1.367, 95%CI: 0.878-2.128); there was still statistically association between BMI and NSCLC after adjusting WHR and peripheral blood lipid indicators (both P<0.05).
Conclusions
The increase of BMI is associated with the increased risk of NSCLC incidence. BFR may be a potential influencing factor for the association between BMI and NSCLC.
3.Effect of Guizhi Decoction on cardiac muscular collagen reconstruction in spontaneous diabetes rats

Objective:This study was to study effect of keeping ying and weiqi in balance on cardiac muscular collagen Ⅰ,collagen Ⅲ,angiotensinⅡ in spontaneous diabetes rats,and provide new thoughts for prevention and treatment for diabetic cardiomyopathy.Methods:40 spontaneous diabetes rats were divided into four groups at random:spontaneous diabetes control group,diformin group,Guizhi Decoction group and Huanglian Jiedu Decoction group.Except control group,each group was given intragastric administration for 12weeks.Blood glucose,collagenⅠ,collagen Ⅲ,collagenⅠ/collagen Ⅲ,angiotensin Ⅱ were determined.Euzymelinked immunosorbent assay and radio-immuniy method were used.Results:Blood glucose in Huanglian Jiedu Decoction group and diformin group decreased;Compared respectively with diformin group and control group,collagenⅠdecreased obviously and collagen Ⅲ increased signifi cantly in Guizhi Decoction group,and the collagenⅠ/collagen Ⅲ decreased.Compared with diformin group and control group,cardiac muscular homogenate AT-Ⅱ contents decreased in both of Guizhi Decoction group and Huanglian Jiedu Decoction group.Conclusion:Guizhi Decoction can decrease the content of collagen Ⅰ,AT-Ⅱ and collagenⅠ/collagen Ⅲin cardiac muscular,and increase content of collagen Ⅲ,then prevent and treat cardiac muscular collagen reconstruction and myocardial fibrosis.Huanglian Jiedu Decoction can decrease the content of AT-Ⅱ,and decrease collagenⅠ/ collagen Ⅲ,collagenⅠ slightly,and can improve cardiac muscular collagen reconstruction.Keeping ying and weiqi in balance is a new thought for prevention and treatment for diabetic cardiomyopathy.
6.Mechanism of radiosensitizing effects of endostatin on H-520 human lung squamous cancer cells

Objective To investigate the mechanism of radiosensitizing effects of endostatin on H-520 human lung squamous cancer cells.Methods H-520 cells was treated with endostatin and/or radiation.Colony-forming assays were used to indicate the radiosensitising effects.Cell cycle distribution and expression of phosphor-p38-MAPK were assayed by FCM,and cyclin D1,cdk2,cdk4 and survivin mRNA leveh were assayed by RT-PCR.Phosphor-Akt was evaluated by Western-blotting.Results Combination of endostatin and irradiation inhibited the proliferation of H-520 cells.According to the colony-forming assays,the D0,Dq,D10 and SF2 values of the combination groups were much lower than those of irradiation groups.The sensitization enhancement ratio(SER)was 1.51.G2/M arrest occurred after 4 Gy irradiation.The gene expression of cyclin D1,cdk2,ckd4 and survivin and phosphor-Akt protein were down-regulated after treatment.The expression of phosphor-p38-MAPK protein was also down-regulated after treatment with 200 μg/ml endostar.Conclusions Endostatin inhibits the growth of H-520 cells and radiosensitizes the cells by induction of G0/G1 arrest,cell apoptosis and down-regulation of gene expression of cyelin D1,cdk2,cdk4 and reduces the phosphorylation of Akt and p38-MAPK.
7.Clinical analysis and intervention strategies in 67 cases of pregnancy complicated with heart failure

To explore the clinical characteristic and intervention strategies for pregnancy complicated with heart failure. The clinical data of 67 pregnant women with heart failure during January 1998 to December 2010 were collected and analyzed.The rate of heart failure in pregnancy with heart diseases was 31% (67/216).The causes of pregnancy complicated with heart failure were peripartum cardiomyopathy ( n =33,49% ) and hypertensive disorder complicating with pregnancy heart disease( n =28,42% ).Heart failure in pregnancy appeared at an average of (36.0 ± 2.7 ) weeks.The average terminating time of pregnant women with heart failure was ( 36.5 ± 2.1 ) weeks.Cesarean section was the main safe choice of delivery for pregnant women with heart failure.The average weight of newborns for pregnant women with heart failure was(2517 ±541 )g.The rate of neonatal asphyxia was 22% (11/50).Strengthening routine antenatal examination and treating pregnant complications timely can prevent and reduce the incidence of heart failure.
8.Hypofraction radiotherapy by helical tomotherapy for unresectable but confined intrahepatic hepatocellular carcinoma is efficient and safe in comparison with conventional 3-dimension conformal radiotherapy

Objective To evaluate the safety of helical tomotherapy (HT) for hepatocellular carcinoma confined to the liver,and to investigate the efficacy of HT by comparison with three-dimensional conformal radiotherapy (3DCRT).Methods In June 2011,hypofractionated HT was performed in 35 patients with unresectable hepatocellular carcinoma confined to the liver who had no distant metastasis or tumor thrombosis.The dose in each fraction increased from 2.4 Gy to 5.0 Gy with a median dose of 3.2 Gy.The median dose for the total treatment was 50.0 Gy.Within the same period,45 patients received conventionally fractionated 3DCRT with a median dose of 54.0 Gy.The adverse reactions,clinical outcomes,and survival time were compared between the two groups.The survival rate was calculated with Kaplan-Meier method,and log-rank test was used to estimate statistical significance of survival differences.Multivariate analysis of survival was carried out with Cox' s regression model.Results There were no significant differences in adversc reactions between the two groups.The objective response rate was significantly higher in the HT group than in the 3DCRT group (60% vs.33%,P=0.024).There was no significant difference in the disease control rate between the two groups (94% vs.89%,P=0.459).The HT group showed significantly higher 1-,2-,and 3-year overall survival rates than the 3DCRT group (97% vs.80%,P=0.025;81% vs.55%,P =0.008;67% vs.50%,P =0.048).The multivariate analysis indicated that tumor smaller than 8 cm,transarterial chemoembolization before radiotherapy,and HT were associated with improved survival (P=0.005;P =0.000;P=0.002).Conclusions HT shows an advantage in precise treatment of hepatocellular carcinoma confined to the liver.With acceptable toxicity,hypofractionated HT shortens the treatment duration and improves the survival in patients.
9.The effect of one-stage repair of secondary nasolabial deformity and nasal septoplasty on nasal airway resistance.

OBJECTIVE:
To assess the effect of one-stage repair of secondary nasolabial deformity and nasal septoplasty for cleft patients on nasal airway resistance (NAR).
METHOD:
Using active anterior rhinomanometry, NAR was measured in eighteen patients with cleft lip and palate who suffered form one-stage repair of secondary nasolabial deformity and septoplasty at per-and-post operation.
RESULT:
NAR was (0.664 +/- 0.200) kPa/(s x L) before operation, (0.304 +/- 0.180) kPa/(s x L) six months after operation, and (0.396 +/- 0.250) kPa/(s x L) twelve months after operation respectively. The differences are statistically significant (P < 0.01) between the NAR before and after operation. Subjective impression score of nasal patency was 7.5 +/- 1.5 before-operation, 2.1 +/- 2.0 after-operation for six months, 3.0 +/- 2.4 after-operation for twelve months. There are significant differences in the subjective impression score of nasal patency as well (P < 0.01).
CONCLUSION
Correction of septal deformities play a very important role in the operation for secondary nasolabial deformity, which can decrease NAR and improve the subjective impression of nasal patency.

10.Optimization of experimental parameters for quantitative NMR (qNMR) and its application in quantitative analysis of traditional Chinese medicines.

Quantitative NMR (qNMR) is a technology based on the principle of NMR. This technology does not need the references of the determined components, which supplies a solution for the problem of reference scarcity in the quantitative analysis of traditional Chinese medicines. Moreover, this technology has the advantages of easy operation, non-destructiveness for the determined sample, high accuracy and repeatability, in comparison with HPLC, LC-MS and GC-MS. NMR technology has achieved quantum leap in sensitivity and accuracy with the development of NMR hardware. In addition, the choice of appropriate experimental parameters of the pre-treatment and measurement procedure as well as the post-acquisition processing is also important for obtaining high-quality and reproducible NMR spectra. This review summarizes the principle of qHNMR, the various experimental parameters affecting the accuracy and the precision of qHNMR, such as signal to noise ratio, relaxation delay, pulse width, acquisition time, window function, phase correction and baseline correction, and their corresponding optimized methods. Moreover, the application of qHNMR in the fields of quantitation of single or multi-components of traditional Chinese medicines, the purity detection of references, and the quality analysis of foods has been discussed. In addition, the existing questions and the future application prospects of qNMR in natural product areas are also presented.
