OBJECTIVE:To study the classification of deviation in drug production. METHODS:From the perspective of dom-inant and hidden deviation, combining with the theory of risk management, risk of deviation was evaluated with different risk man-agement tools to define the severity of the deviation, and it was handled by different methods. RESULTS & CONCLUSIONS:Ac-cording to the identifiability of deviation in actual management, the deviation was divided into dominance and hidden deviation. The dominant deviation can be divided into deep and shallow level. Whether the causes of deviation could be cleared and the conse-quences could be estimates were judged after the shallow level classification, and the deviation was further divided into simple and complex deviation. As for deviation complex,it could be evaluated with the tools of risk management to define the deviation severi-ty. As for hidden deviation,it needed beforehand preventing with the tools of risk management directly to define deviation levels and provide reference for preventive measures. In the process of deviation management, key point is to relay on the scientific meth-od to identify and classify deviation, and divide the influence levels. Combining the theory of risk management to select and use risk tools is the effective way to solve the problem.

Oxidation and free radicals participate in the pathological process of multiple diseases in organisms, and acupuncture shows good effect in antagonizing oxygen stress (OS). This article reviews the effect of acupuncture in antagonizing oxygen stress and the mechanism of its anti-free radical effect in various diseases. The authors hold that acupuncture not only has a chain-blocking effect, but also has preventive and repairing effects of anti-oxidation. And anti-OS action is one of the important mechanisms of acupuncture.

Objective To investigate the clinical characteristics of acute inhalational metallic nickel poisoning and its treatment effect. Methods Data including epidemiology, clinical features, laboratory tests, imaging examinations, therapy protocol as well as prognosis were collected and analyzed. Results Thoracic CT scan of all patients demonstrated as bilateral diffuse infiltration.Nickel concentration in both blood and urine were high in all patients and the concentration was positively associated with poisoning severity. All 4 patients received glucocorticoid treatment,2 of whom with acute respiratory distress syndrome were admitted to ICU to initiate mechanical ventilation and were given sodium dimercaptopropanesulfonate. All patients survived to hospital discharge. Conclusions Acute inhalational metallic nickel poisoning is uncommon and early symptoms are inconclusive. The blood and urine nickel concentration is helpful to judge the severity of the disease.Pulmonary diffuse infiltration is the main clinical feature.Glucocorticoid, chelation therapy as well as symptomatic treatment are effective therapies to relief disease severity.

Objective To study the effect of trachea intubation and mechanical ventilation on the prognosis and discharge rate of patients with successful cardiac-pulmonary resuscitation.Method The clinical data of 389 patients, who were admitted from January 2005 to February 2007,were retrospectively analyzed. The relation between trachea intubation time and discharge rate was studied.According to the time from cardiac arrest to finishing trachea intubation, patients were divided into group A (within 3 minutes,n=209) and group B (over 3 minutes,n=143);according to the time from reaching emergency medicine department to finishing trachea intubation, the rest patients were divided into group C (within 5 minutes,n=9) and group D (over 5 minutes, n=38) minutes. The discharge rate was calculated between groups. The software of SPSS 11.0 was used for statistical analysis. Results The successful rate was 9.75% (389/3988),and 59 patients were discharged, with discharge rate 1.48% (59/3988).The discharge rate of group A was 19.62% (41/209),and was significantly higher than that of group B 6.99 % (10/143) (P

Objective To investigate the role of gamma secretase inhibitor-I (GSI-I) in cell proliferation and apoptosis of human glioma cell lines U87 and U251.Methods RT-PCR and fluorescent quantitative RT-PCR (qRT-PCR) were employed to evaluate the expressions of Notch receptors and their target gene Hes-I in both U87 and U251 cells treated by GSI-I,respectively.Then,MTT assay was used to examine the effects of GSI-I on cell proliferation of the 2 glioma cells.Meanwhile, flow cytometry technique was also employed to detect the cell cycle changes and apoptosis induced by GSI-I treatment. Results The activity of Notch pathway was inhibited by GSI-I treatment through down-regulating the expression of Notch receptors target gene Hes-I in both U87 and U251 cells.Treatment with 2.5μmol/L GSI-I or above concentrations could significantly induce the cell cycle arrest of U87 and U251 cells and these effects were positively concentration-dependent.Flow cytometry technique showed that GSI-I inhibited the cell proliferation by inducing the cell cycle arrest of U87 cells at GI phase and inducing the apoptosis of U251 cells. Conclusion GSI-I can dramatically inhibit the cell proliferation and induce the apoptosis of U87 and U251 cells, providing a reliable evidence for clinical glioma treatment.

Journal of Southern Medical University 2010;30(6):1382-1384
OBJECTIVETo evaluate the efficacy of cartilage tympanoplasty in the treatment of adhesive otitis media.
METHODSFrom June to October, 2008, 18 patients with adhesive otitis media (18 ears) were treated with tragus cartilage tympanoplasty. The air-bone gap changes and the self-perceived symptomatic improvement were evaluated at 1 month and 1 year after the operation.
RESULTSAll the patients showed dry ear within 6 weeks after the operation. Tympanic membrane healing was achieved in 17 cases, and 1 case presented with a inferior-anterior fissure in the tympanic membrane. With the average preoperative air-bone gap (at 0.25, 0.5, 1.0, and 2.0 kHz) of 44.65 dB, the patients showed an obvious decrease of the air-bone gap by over 10 dB at 1 month after the operation and by over 25 dB at one year. Symptomatic improvements were achieved in these cases, including alleviated ear discomforts (3/15 cases), total tinnitus relief (1/11 cases), and alleviated tinnitus (10/11 cases). High-frequency tinnitus was noted in 1 case (1/7 cases), and the tympanic membrane appeared normal in 17 cases.
CONCLUSIONTympanic membrane reconstruction using the tragus cartilage can be feasible for treatment of secretory otitis media, but the surgical indications should be carefully controlled.

OBJECTIVETo investigate the effects of hyperbaric oxygen (HBO) treatment on the activation of astrocytes and the expression of glia-derived neurotrophic factor (GDNF) and nerve growth factor (NGF) in the brain after traumatic brain injury (TBI).
METHODS54 male SD rats were randomly divided into three groups (n = 18): sham-operated, TBI and HBO treatment groups. TBI was induced with Feeney's method, bone window was opened without strike on the brain tissue in the sham-operated group. HBO group rats received HBO treatment for 60 min in the hyperbaric chamber containing O2 100% at 3 ATA. When neurological functions were measured 48 h after TBI, rats were decapitated, the brain water content of 18 rats was measured, 18 brains were sliced for the morphological observation after Nissl staining and for the immunohistochemistry staining of astrocyte markers glial fibrillary acidic protein (GFAP), vimentin and S100, and the other 18 brains of injured side were used for Western blot analysis of GDNF and NGF.
RESULTSHBO treatment reduced the neurological deficit, brain water content and hippocampal neuronal loss. In the observed cortex and hippocampal area astrocytes were activated, the cell number of positive expression of astrocyte markers GFAP, vimentin and S100 was increased, and the expression of GDNF and NGF was elevated after TBI. However, these indices were all enhanced further after the HBO treatment.
CONCLUSIONIt is suggested that HBO may be an effective therapy for TBI and upregulation of the expression of GDNF and NGF may underly the effect of HBO.
