1.Study on the diagnostic method of microcirculation disturbance of patients with chronic hepatitis B

ObjectiveTo study the diagnostic method of microcirculation disturbance of patients with chronic hepatitis B (CHB).Methods81 patients with CHB and 5 normal volunteer were observed.Hypoxia-induciable factor was detected by immunohistochemistry stain,TXB2 and 6-Keto-PGF1α were detected by chemiluminescence and HA was detected by RIA,ultrastructure of liver was observed by transmission electron microscope (TEM).ResultsAccompany with the pathological changes of liver in patients with CHB,the strength and range of expression of HIF-1 α enhanced,HA and TXB2 of serum rised gradually( all P < 0.05 ),but 6-Keto-PGF1α of blood plasma fell- off.The electron microscope showed that erythrocyte aggregation,stenosis and blockage of sinus hepaticus turned obviously,collagen fibers deposition and basal membrane formation aggravated gradually.ConclusionAssociated detec tion with HIF-1 α、HA 、TXB2 and 6-Keto-PGF1 α can reflect the condition of microcirculation of liver perfectly.
2.Progress in the diagnosis and treatment of hepatic Kasabach Merritt syndrome

Hepatic Kasabach-Merritt syndrome (KMS) is a highly complex and potentially fatal complication of severe hepatic hemangioma. Although hepatic KMS is rare,the death rate of victims is significant because it is often misdiagnosed upon presentation and thus receives improper treatment.Presently the literature lacks a systematic review of hepatic KMS.We have summarized its pathogenesis,diagnosis, systematic treatment, prognosis and outcomes in order to increase awareness of this deadly disease.
3.Analysis of the clinical features of 83 vasovagal syncope patients

Objective To analyze the clinical features of vasovagal syncope, enhance its clinial awareness and to promote the importance of history taking in the differential diagnosis of syncope. Methods A total of 83 patients with the diagnosis of vasovagal syncope were included in the study and their data were, undergone retrospective statistical analysis. Results A 63.9% of patients had recurrent syncope. There was 73.5% of them had motivation and 80.7% of them had presyncope symptoms. Complete loss of consciousness was found in 77.1% of patients. There was 39.1% of them had concomitant symptoms and 39.1% had postsyncopal symptoms. In 54.2% of the patients the duration of syncope was less than 2 minutes. Conclusion It is important to pay attention to history taking and to anylaze the clinical features in the diagnosis of vasovagal syncope.

Objective To study the inhibitive effect and mechanism of PPAR?1 on the extracellular matrix (ECM) accumulation of mesangial cells induced by Ang Ⅱ .Methods The plasmid of PPAR?1/WT (wild type) was transfected into mesangial cells. After 48 hours of Ang Ⅱ stimulation, the gene expression of TGF-?1, PAI-1, c-fos and c-jun was examined by RT-PCR. Protein levels of p-ERK, I-?B and nucleus/cytosol ratio of NF-?B were estimated by Westen-blot. The concentrations of FN and TGF-?1 were estimated by ELISA. The activity of PPAR?1 was examined by specific PPRE binding activity. Plasmid expressing non-functional dominant negative type of mPPAR?1, pIRES2-EGFP-mPPAR?1/DN (DN), and blank plasmid, pIRES2-EGFP (Blank) were used as controls. 6.Protective effect and mechanisms of overexpressed PPAR? against fibrosis induced by high glucose in cultured mesangial cells

0 05).Among them,WT but neither DN, Blank, nor Pio significantly ameliorated the increased concentrations of TGF-a1 and fibro nectin in culture medium induced by HG. Compared with DN and Blank, WT transfect ion significantly attenuated high glucose-caused elevation in c-fos, c-jun and p-ERK expression, reduction in Ⅰ-?B level, and incretio n in nucleus/cytosol ratio of NF-?B, while Pio demonstrated similarly signific ant changes only in p-ERK and NF-?B. No significant difference of GLUT-1 mRN A level was detected between HG groups. Conclusions Overexpressed PPAR?1 can su ppress increased ECM production from glomerular mesangial cells cultured in HG c ondition. Its inhibitory effects on activation of ERK/AP-1 and NF-?B pathways are involved. The further increase of PPAR?activity may have direct anti-fibr otic effect in diabetic kidney.
7.Treatment with antisense Smad2 oligodeoxynudeotides reduces extracellular matrix secretion in rat mesangial cells cultured with high glucose

Objective To investigate the effects of antisense Smad2 oligodeoxynudeotides(ODN) on fibronetin(FN) and collagen Ⅳ(ColⅣ) secretion of rat mesangial cells cultured with high glucose, explore the action of Smad2 in the glomerulosclerosis and to find a new method to retard the progress of glomerular fibrosis. Methods 20-mer antisense, sense and random ODNs were designed and synthesized that were phosphorothioate modified to increase stability. The antisense ODN encompassed the ATG of the rat Smad2 gene. ODN was tranferred transiently into rat mesangial cells through liposome. Rat cells were treated with high glucose. mRNA and protein of Smad2 were detected by RT-PCR and cytochemistry. 8.Analysis of Three Treatment Regimens and Pharmaceutical Care in One Patient with Lipoprotein Glomeru-lopathy

Objective:To provide reference for the treatment of lipoprotein glomerulopathy and to investigate the participation of clinical pharmacists in the whole treatment course. Methods: Three different treatment regimens, including immunosuppressive thera-py, dual plasma filtration therapy and lipid-lowering combined with reducing urinary protein therapy was respectively adopted for one patient with lipoprotein glomerulopathy, and the efficacy was evaluated. Clinical pharmacists assisted physicians in deciding treatment regimen, performed pharmaceutical care, adjusted medication and analyzed the prognosis of the patient during the follow-up. Results:1. Immunosuppressive therapy was ineffective for the patient;2. The dual filtration acted quickly, while the expense was high and the disease was easy to relapse;3. The third therapy was relatively safer and more economical with long-term effect. Conclusion:Combi-nation therapy of lowering lipid and reducing urinary protein is the most suitable treatment regimen for the patient with lipoprotein glo-merulopathy. Clinical pharmacists play an important role in the whole course of treatment to assist physicians in obtaining the maximum benefit of patients.
9.Action of blocking renin-angiotensin system on protein kinase C?Ⅱ isoform in renal tissue of diabetic rats

Objective To investigate the expression and translocation of protein kinase C (PKC) ?Ⅱ isoform in renal tissue of diabetic rats and the effects of blocking renin-angiotensin system (RAS) on them. Methods Diabetic rats induced with streptozotocin were randomized to 4 groups. (1) Diabetic control without treatment. (2) Treatment with irbesartan (40 mg?kg -1 ?d -1 ). (3)Treatment with fosinopril (40 mg?kg -1 ?d -1 ) and (4) Treatment with a combination of irbesartan and fosinopril (20 mg?kg -1 ?d -1 each). Six normoglycemic rats served as normal control. OBJECTIVETo study the technical conditions of the extraction and purification of the active composition from Polygonum orientale and Crataegus pinnatifida Bge. in Hongye Xingtong soft capsules with the macroporous resin.
METHODThe orientm, isorientm and hyperoside were used as index to screen the five kinds of resins. And the technical conditions of the enrichment and purification of D101 resin selected out of above were all-round studied.
RESULTThe D101 was fit for adsorbing orientm, isorientm and hyperoside. Under the optimal conditions, the transfer rate of orientm, isorientm and hyperoside was above 91%, and the total solid was cut down by more than 60%.
CONCLUSIONThe D101 is greatly effective for the enrichment and purification of the active composition of P. orientale and C. pinnatifida Bge.
