Orthopedic Journal of China 2006;0(02):-
Fractures of the calcaneus are most common of tarsal bone fractures and usually the result of a fall from a great height.Controversies are about the appropriate classification system, treatment options, indication for surgery, surgical approaches and post operative management. The calcaneal fractures are divided into extra-articular fractures and intra-articular fractures.Sanders classification is of great importance to the treatment options and the outcome assessments. The treatment of the calcaneal fractures is challenging, but the ideal treatment for the calcaneal fractures has not been found so that further research is needed.Nonoperative treatment continues to be the preferred method for non-displaced fractures.Displaced and comminuted fractures can be treated with operation.Complications after calcaneal fractures include blisters, compartment syndrome, wound dehiscence, wound infection, pain, subtalar arthritis and so on.This article reviews the classification, score, treatment and complication of the calcaneal fractures.

Studies show the waste anesthetic gas is harmful to the person working in operating room.At present, there are some problems in traditional scavengers and the gas absorber. This paper introduces a novel scavenger forwaste anesthetic gas which makes use of atmospheric pressure and negative pressure in operating room. This setting can scavenge the exhaust gas absolutely without affection the normal work of anaesthesia apparatus.

Objective To evaluate the scavenging efficacy of a novel scavenger in removing isoflurane. Methods Forty five patients undergoing abdominal surgery were enrolled and randomly divided into three groups. The Datex-ohmeda S/5 anesthetic machine was applied to provide general anesthesia with isoflurane in group A,Datex-ohmed Aestiva/5 in group B,and Drager RA2S in Group C.After 60 minutes, the gas samples were taken at the waste gas outlet, airtake and airout of stripe tubes of anesthetic machines by Philips intellivue MP60 Anesthesia monitor to identify the concentrations of isoflurane and PCO2 both before and after the application of the scavenger. Results The novel scavenger could completely eliminate waste anesthetic gases including isoflurane and CO2.Conclusion The eliminating rate is 100% and the air quality in operating room is improved apparently.

AIM: To study the cytoskeleton of mesench ymal stem cells (MSCs), the ultrastructure and function relationship by using atomic force microscope (AFM). METHODS: The ultrastructures and morphological feature of MSCs c ultured for 1 d and 5 d were studied by AFM. RESULTS: The special structures that possess peculiar morphologi cal characteristic of MSCs such as cytoskeleton, pseudopod, microfilament etc we re identified by AFM, and these special structures are difficult to observe unde r electronic microscopy or other conventional optical microscopy. CONCLUSIONS: AFM is a powerful tool to study ultrastructures, mo rphological features, and cytoskeleton of stem cells in near physiological condi t ions. Its application prospect in cellular biology is extensive. The special cyt oskeleton and other structures of MSCs observed above may represent the structur al base of multi-differentiation potential of MSCs.

Aortic valve calcification (AVC) is a pathological process correlated with multiple disease causes and actively regulated by cardiac valve cells. In this study, porcine aortic valve myofibroblasts cultured in vitro were treated with 50 μg z L(-1) of pathological factor tumor necrosis factor α (TNF-α). Tanshinone II A (TSN) with the concentration of 50 mg x L(-1) and TNF-α were combined in incubating cells for 72 h (3 d) and 120 h (5 d). The Western blotting and Real-time PCR were adopted to detect the changes in smooth muscle α actin (α-SMA), bone morphogenetic protein 2 ( BMP2), alkaline phosphatase (ALP) in cells, and expressions of key effect proteins GSK-3β and β-catenin on Wnt/β-catenin signal pathway. According to the findings, TNF-α can significantly increase the expression of myofibroblasts α-SMA and add the transformation activity to them, with nearly no expression of BMP2, ALP and mRNA in the control group and the TSN group but significant increase in their expressions in the TNF-α group (P < 0.01), which showed osteoblast-like phenotype. Moreover, TNF-α down-regulated the expression of up-streaming regulator GSK-3β and mRNA expression (P < 0. 01) , notably increased the expression of key effect protein β-catenin, but with no significant difference in mRNA with the control group and the TSN group. The result demonstrated that TSN showed a certain inhibitory effect on TNF-α's pathological impact (P < 0.05) in a time-dependent manner. Inflammatory factor TNF-α may promote the transformation of aortic valvular myofibroblasts to osteoblast-like phenotype by activating Wnt/β-catenin signal pathway in aortic valvular myofibroblasts, so as to cause AVC. Tanshinone II A can have a preventive effect in AVC by activating GSK-3β proteins and regulating signal transduction of Wnt/β-catenin signal pathway.

Connexin43 has been shown to play a pivotal role in wound healing process. Wound repair is enhanced by acute downregulation of connexin43, by increasing proliferation and migration of keratinocyte and fibroblast. Angiogenesis is also a central feature of wound repair, but little is known about the effects of connexin43 modulation on functions of endothelial cells. We used connexin43 specific small interference RNA (siRNA) to reduce the expression of connexin43 in human umbilical vein endothelial cell (HUVEC), and investigated the effects of connexin43 downregulation on intercellular communication, viability, proliferation, migration and angiogenic activity of HUVEC. Treatment of siRNA markedly reduced the expression of connexin43 by -80% in HUVEC (P < 0.05), and decreased the intercellular communication by -65% (P < 0.05). The viability, proliferation, migration and angiogenic activity of HUVEC decreased significantly (P < 0.05), compared with that of the normal cells. The results suggest that temporally downregulation of connexin43 expression at early stage of wound to inhibit the abnormal angiogenesis characterized with leaky and inflamed blood vessels, maybe a prerequisite for coordinated normal healing process.

Chinese Journal of Gastrointestinal Surgery 2008;11(5):440-443
OBJECTIVETo evaluate the preoperative diagnosis value of 64-slice spiral CT three dimensional angiography (3D CTA) for the vascular invasion in gastric cancer.
METHODSCT images of 40 patients diagnosed as gastric cancer by endoscope, who proceeded to surgical exploration from August 2006 to December 2007,were collected. These images were rebuilt by 3D CTA to judge vascular invasion by gastric cancer in comparison with the surgical finding as standard reference.
RESULTSSuccessful 3D CTA reconstructions were performed for all these 40 patient images. Out of 40 cases, 14 cases presented vascular invasion in the 3D CTA, and 12 of 14 cases were proved to have vascular invasion in the surgery. For assessing vascular invasion with CTA, the sensitivity was 98.1% and the specificity was 96.4% respectively (Chi Square chi(2)=0.0099,P>0.05). There was no significant differences regarding vascular invasion in gastric cancer between preoperative 3D CTA assessment and surgical finding.
CONCLUSIONSixty-four-slice spiral CT 3D angiography is effective in assessing vascular invasion in gastric cancer and is also valuable in clinical application.
