3.A Analysis of evaluation for Living Donor Kidney Transplantation by Academic and Ethics Committee for Organ Transplantation

Objective: To give the living-donor and recipient medical evaluation and moral evaluation pretransplant by Academic and Ethics Committee for Organ Transplantation (AECOT), ensure the transplantation proceed smoothly and the feasibility for moral principles. Methods:AECOT was formed and gave the living-donor and recipient medical evaluation, additional moral evaluation to their family, evaluated the possibility for transplantation, ensured the long-dated safety for living-donor and recognition for ethics, ensured the transplantation proceed successfully. Results: 18 cases were evaluated by AECOT, two cases had the operation refused because of the II-diabetes for one living-donor and the other living-donor refused evaluation, one case had the operation delaied because of the overweight for the living-donor. The rest 15 cases were accepted operation smoothly,both donor and recipient recovered well post transplant. Conclusion:Academic and Ethics Committee for Organ Transplantation(AECOT) is a essential agency for Living Donor Kidney Transplantation.
4.Effects of C4d deposition in peritubular capillary of patients with acute renal allograft rejection one year post-transplant on the prognosis of renal allograft

Objective To analyze C4d deposition in the patients with late acute renal allograft rejection,and explore the role of C4d in grafts survival and grafts loss. Methods Thirty-six patients clinical and pathologically diagnosed as having acute rejection more than one year post-transplant were selected. C4d was detected by immunohistochemistry in renal allograft biopsies. The effect of C4d deposition on long-term graft survival was studied. Results Among 36 recipients with late acute renal allograft rejection, 16 cases were positive for C4d (44.4 %) and 20 negative for C4d (55.6 %). Five cases experienced graft loss in C4d positive group (31.3 %), while 6 cases in C4d negative group (30.0%). There was no significant difference in the graft loss rate between C4d-positive group and C4d-negative group. Log-Rank test demonstrated there was no significant difference in graft survival between C4d-positive group and C4d-negative group. The count of the interstitial infiltrated eosinophils in renal allograft was (9.4 + 4.5) and (2.6 + 1.8) respectively in the C4d-positive group and C4dnegative group (P<0.05). Conclusion C4d deposition in peritubular capillary of the recipients with late acute renal allograft rejection might not be a prognostic marker for graft outcome.
5.Effects of pinggan xifeng decoction on activity of cytochrome C oxidase and cellular apoptosis in hippocampi of rats with cerebral hemorrhage

BACKGROUND: Cytochrome C oxidase(CCO) is the terminal enzyme in respiration chain of mitochondrion, and it plays a key role in aerobic metabolism and energy production during the process of oxidative phosphorylation. Recently, it is found that energy production of mitochondrion is closely related to the cellular apoptosis, and the changes of CCO activity is closely related to the neuronal impairment after cerebral ischemia and anoxia.OBJECTIVE: To investigate the protective mechanisn of compound pinggan xifeng decoction on the neuronal impairment in cerebral hemorrhage (CH) according to the mitochondrial energy metabolism and cellular apoptosis in neurons.DESIGN: A randomized and controlled trial based on experimental animals.SETTING: Institute of Integrated Traditional Chinese Medicine and Western Medicine, Xiangya Hospital, and Center of Telemedicine.MATERIALS: The experiment was completed in the animal laboratory(key laboratory of province) of Institute of Combination of Traditional Chinese Medicine and Western Medicine from November 2 to 9 in 2003. A total of 80 healthy male SD rats were selected from Experimental Animal Center of Xiangya School of Medicine, Public Health Ministry.METHODS: CH rat models were induced with collagenase Ⅶ, CCO activity was assayed with histochemistry combined with semi-quantification of gray scale, and the cellular apoptosis was evaluated with Tunel method.MAIN OUTCOME MEASURES: CCO activity of CH rats in lateral hippocampal CA1;lateral cellular apoptosis of CH rats.RESULTS: After 12-hour model establishment, CCO activity in CH group was decreased dramatically compared with that in sham operation group (P< 0.01), which was 52.12 ±3.75 and 26.98 ±6.32 respectively in lateral hippocampal CA1. And the cellular apoptosis in CH group was increased notably compared with that in sham operation group(P < 0.01),which was(13.56 ± 1.72)/sight and(4. 32 ± 1.04)/sight respectively.Then the two had deteriorated afterwards. After the treatment with pinggan xifeng decoction, CCO activity can be maintained, and the cellular apoptosis was reduced.CONCLUSION: Neuronal injury was closely related to the decrease of CCO activity and the cellular apoptosis in CH. Pinggan xifeng decoction could maintain CO activity of mitochondrion, improve the cellular aerobic netabolism, and reduce the cellular apoptosis, which might be one of the protective mechanisms for secondary neuronal injury in CH.
6.Preparation of alginate-chitosan-poly (lactic-co-glycolic acid) composite microsphere and its regulation of protein release.

AIMTo elevate the encapsulation efficiency, decrease the burst release and improve the release of protein entrapped in poly (lactic-co-glycolic acid) (PLGA) microspheres. The bovine serum albumin (BSA) composite microspheres of alginate-chitosan-PLGA were prepared and the release characteristics of BSA from this composite microspheres were studied.
METHODSThe much smaller calcium alginate microcapsules were first prepared by a modified emulsification method in an isopropyl alcohol-washed way and coated with chitosan, then the alginate-chitosan microcapsules were further entrapped in PLGA to form the composite microspheres. The protein concentration was determined using a BCA protein assay kit. The release profiles were changed with various formulation factors.
RESULTSThe average diameter of the composite microcapsules was about 30 microm. Comparing with 60% to 70% of the conventional PLGA microspheres, the average encapsulation efficiency was more than 80%, and the burst releases in phosphate buffer solution of the composite microspheres decreased from 40% and 50% to 25% and further to 5% in saline solution.
CONCLUSIONThe novel composite microspheres were prepared, the drug encapsulation efficiency increased and the burst release decreased. The desired release profiles could be obtained by regulating the ratios of PLG and PLA in the composite microspheres.

7.Effect of stellate ganglion block on postoperative synaptic structure in hippocampal CA3 region in aged rats

Objective To evaluate the effect of stellate ganglion block (SGB) on postoperative synaptic structure in hippocampal CA3 region in aged rats.Methods Seventy-two male Sprague-Dawley rats,aged 20-22 months,weighing 550-650 g,were randomly divided into 3 groups (n =24 each) using a random number table:control group (group C),operation group (group O) and SGB + operation group (group SGB).Group SGB received right SGB with 0.25% bupivacaine 0.15 ml.Groups O and SGB underwent 30 min of exploratory laparotomy starting from 15 min after the end of administration.Y-maze test was performed on 1 day after operation in 6 rats chosen from each group for assessment of cognitive function.The frequency of standard training and standard time were recorded.Six rats were chosen from each group on 1,3 and 7 days after operation and sacrificed and the hippocampal CA3 region was isolated for microscopic examination and for measurement of synaptic structure.Results Compared with group C,the standard time was significantly prolonged,and the frequency of standard training was increased in groups O and SGB,the width of synaptic cleft was increased,the thickness of post-synaptic density was decreased,the length of active zones was shortened,and the curvature of the synaptic interface was decreased on 1,3 and 7 days after operation in group O (P < 0.05),and no significant changes were found in each synaptic structure parameter in group SGB (P > 0.05).Compared with group O,the standard time was significantly shortened,the frequency of standard training was decreased,the width of synaptic cleft was decreased,the thickness of the post-synaptic density was increased,the length of active zones was prolonged,and the curvature of the synaptic interface was increased on 1,3 and 7 days after operation in group SGB (P < 0.05).Conclusion The mechanism by which SGB improves the postoperative cognitive dysfunction in aged rats may be related to inhibition of changes of synaptic structure in hippocampal CA3 region.
