3.Clinical effects of hyperbaric oxygenation treatment combined with microsurgical technique on peripheral nerve injuries at high altitude

Objective To explorer the clinical therapeutic effects of applying hyperbaric oxygen drenching(HOD) combined with microsurgical technique on peripheral nerve injuries induced in plateau military training.Methods Forty-one hospitalized patients(52 nerves) with peripheral nerve injuries induced in plateau military training from Jan.2000 to Dec.2007 were randomly divided into treatment group(21 patients,27 nerves injured) and control group(20 patients,25 nerves injured).Microsurgery and postoperative HOD treatment were performed to the patients in treatment group,only microsurgery was performed to the patients in the control group.The restorative procedures for injured nerves were adopted with direct end-to-end suture or sural nerve autograft according to the degree of injury.The recovery of nerve function was assessed and compared between the two groups.Results All patients were followed up for a mean period of 18 months(ranged 6-30 months) after operations.The recovery of nerve function was assessed according to the single nerve functional evaluation criteria.In treatment group,the excellent and fine results were obtained in 7 and 16 nerves,the fineness rate was 85.2%;while in control group,the excellent and fine results were obtained in 2 and 11 nerves,the fineness rate was 52.0%,a significant difference existed on the fineness rate between the two groups(?2=6.71,P
4.Gender difference in the smile aesthetic features in aged Han Chinese and its clinical significance

Objective To investigate the gender difference in the smile aesthetic features in aged Han Chinese with normal occlusion and its clinical significance.Methods Fifty-six aged Han Chinese (male:28,female:28,aged 60-66 years)with normal occlusion were included in this study.Smiling images of the resting-position and mandibular postural position were obtained in an anteriorposterior view.The CoSmileMAA1.0 software was used to evaluate the indices associated with the smile-aesthetic features,and its clinical significance was analyzed.Results Significant differences were noted in the nasal ala width,upper lip length,lip clearance,angle oris width in resting-position/ smiling position together with the changes of lip clearance,angle oris width,distance of inferior margin of upper lip to cutting edge of the maxillary incisor teeth,distance of superior border of lower lip to cutting edge of the maxillary incisor teeth,smile line ratio and type of smile (all P<0.05).When the subjects were smiling,the changes of lip clearance was greater in females than in males [(10.7±1.9)mm vs.(11.3±1.6)mm,P<0.05],the changes of angle oris width was greater in males than in females [(14.1±1.6)mm vs.(13.4±1.3)mm,P<0.05],and the smile line ratio was less in males than in females [(0.9±0.2) vs.(1.1±0.5),P<0.05],which indicate that the females had more attractive smile in aged Han Chinese with normal occlusion than males.The type of smile was mainly high smile in elderly females and median smile in elderly males [53.6% (15 cases) and 60.7% (17 cases),x2 =6.43,P<0.05].Conclusions Significant gender difference is noted in the smile-aesthetic features in aged Han Chinese with normal occlusion.Modulation of maxillary incisor teeth length and smile line ratio can contribute to the aesthetic appearance of smiling.

6.Efficacy of stereotactic radiotherapy combined with chemotherapy for local recurrent non-small-cell lung cancer after radiotherapy

Objective:To study the efficacy of stereotactic radiotherapy (SRT) combined with gemcitabine plus cisplatin (GP)-based chemotherapy on local recurrent non-small-cell lung cancer (NSCLC) after radiotherapy. Methods:Thirty-eight patients with local recurrent NSCLC were treated with SRT(3.00-4.85 Gy per fraction, five times every week, totally 8-12 fractions, total course of 2-3 weeks). Before SRT the patients were given two cycles of GP-based chemotherapy (gemcitabine 1 000 mg/m~2 d 1 and d 8;cisplatin 30 mg/m~2 d 1-3) and at the end of SRT they were given 4 cycles of GP-based chemotherapy. Results:Of the 38 patients,7 patients had complete response, and 23 patients achieved partial response. The overall immediate response rate was 78.95% (30/38). The 6-month and 12-month total survival rates were 71.05% and 10.53%,respectively. The median survival time was 7.8 months. Conclusion:SRT combined with GP-based chemotherapy had better short-term efficacy and the patients were tolerable. No severe short-term radiation-induced injury was observed. The long-term radiation-induced injury and long-term efficacy need further investigation.
7.TanshinoneⅡA inhibites migration and invasion of human gastric cancer SGC7901 cells

Background and purpose:Recently, it was reported that tanshinoneⅡA (TanⅡA) could inhibit proliferation, induce differentiation and apoptosis of human cancer cells. Previous studies also indicated that TanⅡA could inhibit the migration and invasion of osteosarcoma. However, the effects of TanⅡA on the migration and invasion of gastric cancer and the mechanism remains unclear. The aim of this study was to investigate the effect of TanⅡA on gastric cancer cell SGC7901 migration and invasion of in vitro. Methods:After different concentrations (0.5, 1, 2, and 4μg/mL) of TanⅡA treatment for 24, 48, and 72 h respectively, MTT assay were developed to detect the cell proliferation of SGC7901. The wound healing assay and 3D-transwell assay were used to observe the migration and invasion of SGC7901 cells, respectively. Expression of intercellular adhesion molecule 1 (ICAM-1), matrix metalloproteinase-2 (MMP-2), matrix metalloproteinase-9 (MMP-9), and tissue inhibitor of metalloproteinase 2 (TIMP-2) mRNA and protein were measured with real-time PCR and Western blot. Results: 1, 2, and 4 μg/mL Tan ⅡA showed a dose-and time-dependent growth inhibition on SGC7901 cells. 2μg/mL TanⅡA showed a time-dependent migration inhibition of SGC7901 cells. 1, 2, and 4μg/mL TanⅡA could inhibit the invasion of SGC7901 cells. Real-time PCR and Western blot showed a reduction in expression of ICAM-1, MMP-2, and MMP-9, as well as an increase in expression of TIMP-2 (P<0.05).Conclusion:TanⅡA inhibits human gastric cancer SGC7901 cell migration and invasion in vitro. TIMP-2 upregulation and, ICAM-1, MMP-2, MMP-9 downregulation might be one of the mechanisms of anti-tumor of TanⅡA.
8.Administration of NF-?B decoy oligodeoxynucleotides palliates ischemia-reperfusion injury of rat liver graft

Objective To investigate the protective effect and mechanisms of NF-?B decoy oli- godeoxynucleotides(ODNs)on rat liver graft following ischemia-reperfusion injury.Methods Ani- mals were randomly divided into 3 groups(n=8 in each group):control group,ischemia-reperfusion (IR)group,and decoy ODNs group,in which donor grafts were transfected with 120?g NF-?B decoy ODNs before graft procurement.Following 2 h of reperfusion,Kupffer cells(KCs)were isolated,and NF-?B biding activity was detected with electrophoretic mobility shift assay(EMSA),and TNF-?、IL- 6 mRNA expression was assayed by using RT-PCR method.Meanwhile,the liver and serum samples were collected,the histopathologic change of liver was examined by light microscope,and liver func- tion was analyzed.Results The NF-?B biding activity,TNF-?,IL-6 mRNA expression,and the ALT,TBIL levels in serum were significantly increased following 2 h of reperfusion in IR group as compared with those in control group(P<0.01).A large amount of degeneration and necrosis in hep- atocytes accompanied with obvious congestion in hepatic sinusoid occurred.However,these tested in- dexes were significantly ameliorated in decoy ODNs group as compared with those in IR group(P<0.01),and the architecture of hepatic lobules was remained.Conclusions KCs NF-?B activation fol- lowing reperfusion plays an important role in IRI in liver transplantation.Decoy strategy shows appar- ent effect on suppression of NF-?B activation,and thus inhibits the production of downstream cyto- kines,which protects the liver graft from IRI.
9.Memory alloy embracing fixator in treatment of multiple fractured ribs and flail chest

BACKGROUND: With the development of internal fixation materials, simple operation with internal fixation has become a tendency. Ni-Ti shape memory alloy embracing fixator has such advantages as slight injury, easy operation, security, reliable fixation, and better histocompatibility. The present study was to explore curative effect and postoperative results of Ti-Ni shape memory alloy embracing fixator in patients with multiple fractured ribs and flail chest. METHODS: The curative effect and long-term follow-up results were observed after internal fixation with a shape memory alloy embracing fixator in patients with multiple fractured ribs and flail chest from January 2006 to December 2009. RESULTS: All patients were cured with an average hospital stay of 10.31±3.14 days. Post-operative pain was less severe than preoperative pain (P=0.02).The rate of postoperative complications such as atelectasis, pulmonary infection, etc was 17.65%. There were fewer long-term complications and less influence on daily work and life. CONCLUSIONS: It is practical to perform an operation for fracture of multiple ribs using a Ti-Ni shape memory alloy embracing fixator. The fixator, which is less traumatic, simple, safe, and reliable, has a good-histocompatibility and fewer postoperative complications.
