1.Value of apparent diffusion coefficient of magnetic resonance diffusion-weighted imaging for differential diagnosis of common pediatric posterior fossa tumors

Objective To investigate the value of apparent diffusion coefficient (ADC) of magnetic resonance diffusion-weighted imaging(DWI) for differential diagnosis of common pediatric posterior fossa tumors.Methods Forty-five children with posterior fossa tumors confirmed by surgery and pathology,who were hospitalized at the First Affiliated Hospital of Henan University of Traditional Chinese Medicine from January 2013 to December 2015 were analyzed by using retrospective case-control study,including 24 cases of medulloblastomas,12 cases of pilocytic astrocytomas,and 9 cases of ependymomas.All the children were examined by preoperative magnetic resonance imaging(MRI) plain scan,enhanced scan and DWI.The minimum ADC (ADCmin) values of different tumors were measured,and the receiver operating characteristic (ROC) curve was delineated.The threshold value of ADCmin,sensitivity,specificity and the accuracy for differential diagnosis of 3 tumors were obtained.Results The ADCmin value of medulloblastoma was the lowest [(0.482±0.290)×10-3 mm2/s],and the ADCmin value of pilocytic astrocytoma was the highest [(1.592±0.320)×10-3 mm2/s],while that of ependymoma was in the middle [(0.826±0.390)×10-3mm2/s].There was a significant difference in ADCmin value among 3 tumors(F=48.415,P=0.000).The threshold value of ADCmin to distinguish medulloblastoma from ependymoma was 0.672×10-3 mm2/s,the sensitivity was 97.0%,the specificity was 100.0%,and the diagnostic accuracy rate was 97.8%.The threshold value ADCmin to distinguish ependymoma from pilocytic astrocytoma was 1.058×10-3 mm2/s,the sensitivity was 95.7%,the specificity was 97.9%,and the diagnostic accuracy rate was 94.7%.Conclusions The minimum ADC value can be used as a supplementary means for conventional MRI,which is helpful for the differential diagnosis of pediatric posterior fossa tumors.
3.Clinical analysis of 31 ankylosing spondylitis patients with malignancies

Objective:To investigate the clinical features of patients with ankylosing spondylitis ( AS) and malignancies. Methods: In the study, 31 AS patients with malignancies in Peking University First Hospital from January 2006 to July 2014 were enrolled, and their clinical features were compared with AS patients without malignancies at the same time in the hospital. Then their general characteristics, clinical and imaging findings of AS, characteristics of malignancies and prognosis were retrospectively analyzed. Results:The 31 cases of AS with malignacies accounted for 11 . 8% of all AS patients admitted to the hos-pital in the same period. They comprised of 27 males (87%) and 4 females (13%) . Their mean ages at diagnosis of AS were 43 ± 17 years ( range:16-76 years) , and their mean ages at diagnosis of malig-nancy were 60 ± 12 years ( range:31-87 years) respectively. Malignancy was diagnosed after the estab-lishment of AS in 27 of our patients. Furthermore, imaging changes typical of AS were found in all the 31 patients at the diagnosis of AS, but the majority of them had never received any proper treatment for AS. The malignancies of 31 AS patients included bladder cancer (7 cases) , hematological malignancy (6 ca-ses), lung cancer (5 cases), renal cancer (5 cases), prostate cancer (2 cases), carcinoma of renal pelvis (2 cases) , breast cancer (2 cases) , gastric carcinoma (2 cases) , rectal cancer (2 cases) , neu-roendocrine carcinoma (2 cases) , colon carcinoma (1 case) , and esophagus cancer (1 case) . Conclu-sion:There is risk of malignancy in AS. Malignancy is not rare in patients with AS. Bladder cancer is the most common one followed by hematological malignancy. In consequence, close attention should be paied to the conditon of AS with malignancy in clinical practice.
4.Influence of flood disaster on Oncomelania snail distribution in Wuhu section of Yangtze River in 2020

Abstract: Objective To analyze the impact of the flood disaster on the distribution of Oncomelania snails in 2020 in Wuhu City, to provide scientific basis for formulating the "14th Five-Year Plan" for schistosomiasis control and precise prevention and control measures in Wuhu City. Methods Systematic sampling combined with environmental spot check was used to investigate the distribution of snails in the historical snail's environment, existing snail's environment and suspected snail's environment in Wuhu City. The collected snails were crushed and examined by microscope to understand the death and life of snails and the infection of Schistosoma japonicum, and the density of live snails and other indicators were counted. Results The historical area of snails was 14 475.24 hm2 in Wuhu City, and the existing area of snails was 4 588.72 hm2, including 4 210.32 hm2 for lake type and 378.40 hm2 for hill type snails. The average occurrence rate of live snails was 14.59%, and the average density of live snails was 0.50 snails/0.1 m2 in Wuhu City. There were 2 745 snail breeding environments, of which 491 were lake type and 2 254 are hill type, accounting for 17.89% and 82.11% respectively. The existing snail area was mainly distributed in the beaches and ditches, accounting for 92.51% and 6.29% of the existing snail area respectively. Some snails were distributed in ponds, paddy fields and other environments. Among all the historical snail habitats, the majority were class Ⅱ and class Ⅲ environments (which still have the basic conditions for snail breeding at present), with a total of 1 739 sites (blocks), accounting for 63.35% of the total environment. No schistosomiasis infected snails have been found, and the newly discovered and recovered snails cover an area of 268.21 hm2 in Wuhu City. Conclusions The distribution of snails is extensive in Wuhu City, and snails are mostly distributed in beaches, which are vulnerable to the impact of flood disasters. The spread of Oncomelania snails is found in 30 marshlands in 3 counties in this survey. It is necessary to continue to strengthen snail monitoring after disasters, and take class Ⅱ and class Ⅲ environments as key areas for snail monitoring, so as to find, identify and evaluate the risk of schistosomiasis transmission as soon as possible, to avoid or reduce the impact of flood disasters on the process of schistosomiasis control, and promote the process of schistosomiasis elimination.
6.A cross-lagged regression analysis between resilience, mental health and psychological stress of recruits during training

ObjectiveTo explore the resilience and mental health of recruits during training,and to investigate mutual prediction of resilience between mental health and psychological stress to provide the scientific evidence for the mental health training and psychological selection of recruits.MethodsConnor-Davidson resilience questionnaire ( CD-RISC),symptom checklist ( SCL-90 ),psychological stress self-evaluation test ( PSET),satisfaction with life scale (SWLS),positive and negative affect scale (PANAS) were applied to 650 recruits in 2011 before starting training and after two months of training.Results ① Chinese recruits had good resilience,and the CD scores of pre-and post-test were (61.60 ± 14.36) and (61.41 ± 14.55 ) respectively.②Mental health (SCL-90) of two tests were good in recruits.③ Correlation analysis showed that the same premise of phase correlation,resilience (pre-test) could significantly predict mental health (post-test) ( β =0.070,P < 0.05 ),mental health ( pretest) could not predict resilience (post-test) (β =0.015,P > 0.05 ).Resilience (pre-test) could significantly predict psychological stress (post-test) ( β =0.075,P < 0.05 ),psychological stress (pre-test) could not predict resilience (post-test) (β =0.017,P > 0.05 ).④ Compared with high resilience group,low-resilience group of recruits had higher total score of SCL-90 and psychological stress and less positive emotion,more negative emotion and lower life satisfaction.ConclusionThe recruits training have good resilience.Resilience of recruits have predictive ability to mental health,psychological stress during training.
7.Clinical Features and Prognosis of Neonatal Arrhythmias

Objective To explore the clinical features and prognosis of arrhythmia in newborn infants.Methods Eighty children with arrhythmia were diagnosed by physical examination and electrocardiogram monitoring in author's hospital from Jan.2004 to Dec.2006.Their clinical features and 24-hour dynamic electrocardiogram were analyzed at the acute stage and followed up.Results Out of 80 cases of arrhythimics,52 cases were boys and 28 cases were girls,with an average age of 4 days on diagnosis of arrhythmias.Forty-five neonates presented with supraventricular arrhythmia,accounting for 56.3%.Their clinical presentations were atypical,and the hypoxia,infection,electrolyte disturbances and metabolic disorders were the main causes of the arrhythmias.After supportive and anti-arrhythmia treatment,18 cases of arrhythmias disappeared of 25 children with premature atrial beats;10 cases of 21 children with premature ventricular beats were cured,2 cases of 4 children with ventricular tachycardia,1 case died;2 cases of 3 children with supraventricular tachycardia didn't occure;3 cases of 5 with atrio-ventricular block were cured.The prognosis was better in supraventricular arrhythmias than that in ventricular and other arrhythmias at discharge.At the follow-up of(1.2?0.7)years,there were no differences in rates of recovery between supraventricular arrhythmias and ventricular arrhythmias.Conclusions Supraventricular arrhythmia was the most common type of arrhythmias in neonates.Most of the arrhythmia in neonates might be functional and could recover without treatment.Supraventricular arrhythmia usually had better prognosis in acute period.Only a few neonates with severe arrhythmia need anti-arrhythmias treatment.

Aim To compare the advantage of different methods of terminating early pregnancy. Methods 684 women in early pregnancy(≤49 d) were allocated into three groups according to their request: Group A (medical abortion,n=241), taking (po) mifepristone 25 mg every 12 h for three days and then taking misopostol 600 ?g at 8:00 on the forth day; Group B (induced abortion, n=220), taking the routine intrauterine operation by vacuum aspirator; and Group C (medical with induced abortion, n=223), inserting a Gongshuan suppository into rectum 0.5~2 h before induced abortion operation. Some indices were compared, including the efficacy, vaginal bleeding volume and time, side effects and acceptability of the three abortion methods. Objective To evaluate the feasibility of monitoring the gene expression of VEGF165 via the diglycylcysteine (GGC) reporter gene system by reporter probe of 99Tcm-GH. Methods DNA fragments encoding GGC binding motifs were prepared by PCR and positioned at the C end of VEGF165 gene after the linearization of pcDNA3-VEGF165 plasmid. A replication-defective adenovirus vector Ad5-VEGF165GGC motif-internal ribosomal entry site(IRES) -enhanced green fluorescent protein (EGFP) (Ad5-VIE)was constructed, with a cytomegalovirus (CMV) early promoter driving the expression of VEGF165 gene,GGC motif and EGFP, under the aid of an IRFS. A replication-defective adenovirus carrying the Ad5-EGFP was used as the control. Mensenchymal stem cells (MSC) were infected with the recombinant adenovirus at a multiplicity of infection (MOI) from 0 to 100 infectious units (0,10,25,50,100). The cellular uptake of 99Tcm-GH in infected MSC were then studied at 30, 60, 90 and 120 min. VEGF165 was detected by quantitative reverse transcriptase real-time PCR (RT-PCR), Western-blot, and immunohistochemisty. EGFP was observed by RT-PCR and fluorescence microscopy. The correlation analysis was studied between the cellular uptake of 99Tcm-GH and the expression of VEGF165. SPSS 13.0 was applied for statistical analysis. Independent samples t-test, q-test and Pearson correlation coefficient were used. Results After infected with different viral titer of Ad-VIE, the cellular uptake of 99Tcm-GH increased with the increasing virus titer(r2 =0.86, P <0.05), with the peak rate (7.94 ±0.75) % at MOI = 100. In time-dependent uptake study, the cellular uptake rates increased rapidly with the time extension, and the highest uptake occurred at 120 min with the peak uptake rate (7.72 ±0.22)%. The uptake rates of 99Tcm-GH in Ad5-VIE-infected cells were significantly higher than those of Ad5 -EGFP-transfected cells at all time points (t = 15.10- 54.92, all P <0.05). The VEGF165 and EGFP mRNA levels increased with increasing virus titer, and the VEGF165 mRNA correlated well with the EGFP mRNA(r2 = 0. 99, P < 0.05). After infected with different MOI of Ad5-VIE, good relationship was found between the cellular uptake of 99Tcm-GH and the expression of VEGF165protein in MSC(r2 =0.90, P <0.05). Inmunohistochemisty showed VEGF165 protein expressed obviously at Ad5-VIE-infected MSC, and the EGFP was observed by fluorescence microscopy. Conclusions The cellular uptake of 99Tcm-GH in Ad5-VIE-infected MSC are well correlated with the expression of VEGF165 in vitro. The expression of therapeutic gene VEGF165 can be monitored by the GGC peptide expression.
