1.Effect of mechanical ventilation on 43 patients with myocardial infarction

ObjectiveTo investigate the effect of mechanical ventilation (MV) on myocardial infarction (MI).Methods43 MI patients of the treating group were treated with mechanical ventilation when PaO2<55 mmHg, SaO2<85%.26 MI patients of the control group were treated with same treatment except for inhaling O2 through nose or face mask.ResultsThe survival rate of the treating group was 34.9%, but that of the control group was 7.8%, there was a significant difference between two groups (P<0.01).ConclusionMechanical ventilation can raise the survival rate of MI patients and improve functions of failing heart.

4.Value of apparent diffusion coefficient of magnetic resonance diffusion-weighted imaging for differential diagnosis of common pediatric posterior fossa tumors

Objective To investigate the value of apparent diffusion coefficient (ADC) of magnetic resonance diffusion-weighted imaging(DWI) for differential diagnosis of common pediatric posterior fossa tumors.Methods Forty-five children with posterior fossa tumors confirmed by surgery and pathology,who were hospitalized at the First Affiliated Hospital of Henan University of Traditional Chinese Medicine from January 2013 to December 2015 were analyzed by using retrospective case-control study,including 24 cases of medulloblastomas,12 cases of pilocytic astrocytomas,and 9 cases of ependymomas.All the children were examined by preoperative magnetic resonance imaging(MRI) plain scan,enhanced scan and DWI.The minimum ADC (ADCmin) values of different tumors were measured,and the receiver operating characteristic (ROC) curve was delineated.The threshold value of ADCmin,sensitivity,specificity and the accuracy for differential diagnosis of 3 tumors were obtained.Results The ADCmin value of medulloblastoma was the lowest [(0.482±0.290)×10-3 mm2/s],and the ADCmin value of pilocytic astrocytoma was the highest [(1.592±0.320)×10-3 mm2/s],while that of ependymoma was in the middle [(0.826±0.390)×10-3mm2/s].There was a significant difference in ADCmin value among 3 tumors(F=48.415,P=0.000).The threshold value of ADCmin to distinguish medulloblastoma from ependymoma was 0.672×10-3 mm2/s,the sensitivity was 97.0%,the specificity was 100.0%,and the diagnostic accuracy rate was 97.8%.The threshold value ADCmin to distinguish ependymoma from pilocytic astrocytoma was 1.058×10-3 mm2/s,the sensitivity was 95.7%,the specificity was 97.9%,and the diagnostic accuracy rate was 94.7%.Conclusions The minimum ADC value can be used as a supplementary means for conventional MRI,which is helpful for the differential diagnosis of pediatric posterior fossa tumors.
5.Pattern recognition of the fingerprint of Polygonum orientale in Guizhou Province

AIM: To study pattern recognition of the fingerprint of Polygonum orientale from Guizhou Province. METHODS: 20 batches of samples,Polygonum orientale,come from different producing areas and harvest time were carried out to gain fingerprints by HPLC-DAD,as compared with retention time and ultraviolet spectra of standard substances.The pattern recognition of the characteristic fingerprint of Polygonum orientale,known as cluster analysis and principal component analysis,formed. RESULTS: The fingerprint of 20 batches of Polygonum orientale showed 12 characteristic peaks,in which 9 common peaks were confirmed.the samples were grouped into 3 types of harvest time. CONCLUSION: The quality of Polygonum orientale in Guizhou Province is stable,the pattern recognition of the fingerprint provides the experimental basis for manufacture and quality control of Polygonum orientale.
6.Comparison of different diagnostic tests for antinuclear antibodies

Objective To compare and evaluate the usefulness of diagnostic tests of IFA with HEp-2 cell substrate and ELISA coated with purified nuclear antigens for ANA in SLE.Methods Sera derived from 226 SLE cases and 183 healthy controls were tested for ANA and all parameters were compared such as sensi- tivity,specificity,accuracy,positive predictive value,negative predictive value,positive likelihood ratio,nega- tive likelihood ratio,result consistency,rank correlation coefficient and kappa of ANA detected by IFA and ELISA.Accuracy was evaluated by ROC for two methods.All 36 samples with different results from two meth- ods were detected for ENA.The correlation of titer to A ratio of different patterns as studied.Results The sensitivity of IFA and ELISA was 91.15% and 92.04% respectively for SLE patients,specificity was 96.17% and 92.90%,accuracy was 93.40% and 92.42%,positive predictive value was 96.71% and 94.12%,negative predictive value was 89.80% and 90.43%,no significant difference was found between the two methods (P>0.05).No significant difference was found in accuracy of both methods by ROC (P=0.409).Good agreement was found between two methods with rank correlation coefficient (R=0.823) and kappa (k=0.825).All of 36 samples with different ANA results from two methods were detected for ENA.In 14 cases with IFA positive and ELISA negative,the titer of one case was up to 1:1000 and the pattern was Golgi by IFA,the titers of the rest were about cutoff level and the pattern were granular and nucleolus mostly.In 22 cases with IFA nega- tive and ELISA positive,11 cases of them had the A ratio ranged from 2.67~30.5.Positive rate of ENA was 14.29% in 14 cases with IFA positive and ELISA negative,68.18% in 22 eases with IFA negative and ELISA positive and the difference was significant (P<0.01).Poor correlation of titer to A ratio for granular pattern samples (R=0.083),but good correlation for homogeneous pattern was found (R=0.595).Conclusion IFA as the recommended detecting method for ANA is intuitive and can provide more information by different pattem than ELISA but it needs fluorescence microscope and experienced technician.While ELISA is very simple and the concentration of ANA can be evaluated by A ratio value.ELISA can be a substitute method for ANA be- cause both IFA and ELISA have high sensitivity,specificity,accuracy,agreement rate,kappa and rank correla- tion coefficient.In addition,ELISA is more accessable for screen test because of low rate of false negative re- sult.Result of ELISA is more accurate if new and uncommon antigens are coated such as Golgi and nucleolus. The new work flow in which ELISA is used to screen out the positive ANA samples and IFA is used then to detect the nuclear pattern of ANA can save time,cost,and in turn improve work efficiency.
7.Simultaneous determination of nine chemical markers of bletillae rhizoma by ultra performance liquid chromatography.

A UPLC method has been developed in the current investigation for simultaneous determination of nine chemical markers of Bletilla striata, 4-hydroxymethylphenyl beta-D-glucoside, blestroside, dactylorhin A, militarine, dihydrophenanthrene 5, gymnoside V, dihydrophenanthrene 1, benzylphenanthrene 3 and gymnosides IX. Separation was performed at 45 degrees C on an ACQUITY UPLC BEH C18 column (2.1 mm x 150 mm, 1.7 microm) with a gradient solvent system of acetonitrile-water as the mobile phase. The flow rate was 0.3 mL x min(-1), the detection wavelength was 280 nm. The results showed that the nine chemical markers could be well resolved and that in the selected linear range, all calibration curves of the nine chemical markers showed good linearity (r > or = 0.999 3). The recoveries (n = 6) were in the range of 98.15% - 102.2% and RSDs were between 2.1% - 3.6%. The data suggested that the developed UPLC-UV method had good reproducibility, robustness, and accuracy, which was suitable for the quality control of Bletilla striata. Applications of the method showed that the nine chemical markers had higher contents in the wild B. striata than in the cultivated ones.

8.Analysis of DNA fingerprint of Mycobacterium tuberculosis enterbacterial repetitive intergenic consensus-polymerase chain reaction

Objective To analyze the epidemiological characteristics of Mycobacterium tuberculosis by enterbaeterial repetitive intergenic consensus-polymerase chain reaction(ERIC-PCR)DNA fingerprint. Methods Mycobacterium tuberculosis positive sputum samples between September 2003 to May 2006 were collected and cultured.Chromosomal DNA were extracted and ERIC-PCR DNA fingerprinting was analyzed by software,such as RAPD PHYLIP and Treeview.Results A total of 42 different fingerprints were detected.Phylogenetic analysis showed that they could be classified into three clusters,the clustering rate was 72.6%.The characteristics of ERIC-PCR fingerprint patterns were related to age,drug resistance,and type of resistance.Conclusions ERIC-PCR DNA fingerprinting technique used in this study is good for epidemiological studies with its strong discrimination,simplicity and rapidness.A high level of recent transmission is found in our city.
9.Characteristics and survival of connective tissue disease associated pulmonary arterial hypertension patients with and without interstitial lung disease

Objective To explore the characteristics and survival of connective tissue disease (CTD) patients with both of pulmonary arterial hypertension (PAH) arnd interstitial lung disease (ILD),and to compare with CTD patients with isolated PAH.Methods All adult CTD patients who visited one of the three referral centers in China with a diagnosis of PAH confirmed by right heart catheterization from July 2006 to May 2011 were enrolled.They were then divided into two groups (ILD with and without-ILD group) based on chest CT and then the comparison of baseline characteristics and survival at the endpoint of follow up were made between the two groups.T test,Mann-Whitney U test,x2 test,Kaplan-Meier survival analysis and Cox regression analysis were used for statistical analyses.Results One hundred and twenty-six patients were recruited into the study.Patients with ILD (n=27) were older than those without ILD (n=99).Lung function results including FVC [(75±18)% vs (83±13)%,t=2.212,P=0.037] and DLCO [(54±22)% vs (68±20)%,t=2.392,P=0.019] in ILD group were significantly wose than those without-ILD group.Although some important hemodynamic parameters such as mean pulmonary arterial pressure and pulmonary vascular resistance were better in the ILD group than the without-ILD group,Kaplan-Meier analysis showed that the short term survival of ILD group was significantly worse than that of the without-ILD group (72.7% versus 94.7% at 1 year and 63.6% versus 81.1% at 3 year,P=0.047).In ILD group,Cox regression analysis showed that SvO2 was the only independent factor for the short term survival [HR=0.19,95%CI (0.04,0.83),P=0.027],and Kaplan-Meier analysis showed patients with SvO2<60% had significantly lower short term survival than patients with SvO2 ≥60% (1 and 2 year survival were 60.0% and 40.0% versus 92.9% and 77.4% respectively,P=0.002).Conclusion Patients with both PAH and ILD is a special subtype in CTD.Although with the superiority of hemodynamics,these patients have significantly worse survival than CTD patients with isolated PAH.Low SvO2 is the independent risk factor for the short term mortality in patients of CTD complicated by both PAH and ILD.More attention should be paid to these patients and the management strategy should be investigated further.
10.Prokaryotic expression of surface membrane antigen SAG1 gene from Toxoplasma Gondii and the diagnostic value of the recombinant protein

Objective To investigate the diagnostic value of the recombinant surface antigen 1 (rSAG1) in immunodiagnosis of toxoplasmosis. Methods Isopropyl β-D- 1 -thio-galaetopyranoside (IPTG) was used to induce the expression of recombinant plasmid pET28a-SAG1 of Escherich coli(pET28a-SAG1/BL21 ). The expression products (rSAG1) of pET28a-SAG1/BL21 were identified by Western blotting. The serum of mice infected with Toxoplasma gondii tachyzoites, normal mouse serum and the serum from 10 toxoplasma gondii patients were used as primary anti-Toxoplasma gondii antibodies, and the rSAG1 gene products were identified by Western blotting, by which the diagnostic value of rSAG1 in Toxoplasmosis was compared. Results After induction and purification, rSAG1 protein was obtained and its relative molecular mass was 38.5 × 103. The fusion protein could be recognized by the serum of mouse infected with Toxoplasma gondii tachyzoites, rSAG1 of expression products of surface membrane antigen SAG1 gene from Toxoplasma Gondii could be detected in 4 cases from 10 patients by Westem blotting.Conclusion The rSAG1 has a potential value in the immunodiagnosis of Toxoplasmosis.
