3.An experiment on focal cerebral ischemia reperfusion model in mice established with suture method

BACKGROUND: Techniques in the focal cerebral ischemia model in mice have been well developed in some foreign countries. However, rats are commonly used in cerebral ischemia studies due to resource availability in China.OBJECTIVE: To establish reliable and reproducible focal cerebral ischemia reperfusion model in mouse to explore the molecular mechanism of cerebral ischemia reperfusion process in genetic level.DESIGN: Randomized controlled study.SETTING: Fundamental Medical Research Institute of Chengde Medical College. MATERIALS: The experiment was carried out in Fundamental Medical Research Institute of Chengde Medical College from September 2002 to May 2003. Totally 30 Kunming mice (25 to 30 g), provided by Experimental Animal Center, Chengde Medical College, were randomly grouped as follows: control group, 6, 3-hour occlusion and 18-hour, 24-hour reperfusion, with 6 mice in each group respectively.INTERVENTIONS: Nylon thread (qb0.128 mm) was soaked in paraffin wax and inserted into internal carotid artery in mouse to reversibly occlude the middle cerebral at the right side after 3 hours, then was removed, and reperfusion was performed in 18, 24 hours, respectively.MAIN OUTCOME MEASURES: The infarct volume was observed and neurological deficit scored was analysis.of mice in operation group was shown as head turning to left, adduction of left forelimb, internal rotation of shoulder, decrease of muscular tension of left forelimb, unable to straighten completely during tail lifted. At independent activity, mice turned to left as the left hidlegs as the center of a circle; under the abdomen, left forelimb was crossed with right forelimb as scissor shape. Palsy of left limbs was observed obviously, especially forelimb.tion was shown clearly with TTC. The infarct volumes were (13±2.3) mm3 and (16±4.5) mm3 for 18 hours and 24 hours reperfusion and 3-hour ischemia, respectively. Pathological changes were observed under light microscope after ischemia. The infarct volume increased as the increasing of reperfusion time, including 3-hour blocking and 18-hour and 24-hour reperfusion.CONCLUSION: Using suture method to establish focal cerebral ischemia reperfusion model in mice, it is less invasion and trauma without requiring skull opening. Ischemia position is relatively fixed, thus, ischemia reperfusion time can be controlled precisely. It is also an ideal animal model to study pathological changes in cerebral vascular diseases and an efficient tool to study the therapeutic effect of medicine after surgery.
4.A new selaginellin derivative from Selaginella pulvinata.

Abstract: A new selaginellin derivative named as selaginellin S (1) was isolated from the whole plants of Selaginella pulvinata (Hook. et Grev.) Maxim. (Selaginellaceae), together with a known one (selaginellin M, 2). Compounds 1 and 2 were separated and purified by silica gel and Sephadex LH-20 column chromatography. Their structures were determined on the basis of extensive spectroscopic analysis including IR, MS, 1D and 2D NMR experiments, as well as ECD calculations. Compound 1 is a key intermidiant in the biosynthesis pathway of selaginellins. Compound 2 is first reported in this plant.

5.Questionnaire investigation on cost-effectiveness analysis of acupuncture for migraine.

OBJECTIVEBy using questionnaire method to understand the evaluation regarding cost-effectiveness of acupuncture for migraine in both doctors and patients.
METHODSBy using questionnaire method, questionnaires for doctors and patients were made respectively; by using network platform, two sets of questionnaires were publicly released, which were filled in online. The results were real-time background collected and then analyzed.
RESULTSTotally 696 effective questionnaires for patients and 114 questionnaires for doctors were collected. The results indicated that (1)the direct cost (including treatment fee and material fee) for preventive treatment of migraine was 109 yuan per time in regular acupuncture, 152 yuan per time in regular acupuncture + electroacupuncture (EA) and 238 yuan per time in acupoint catgut embedding. The travelling expense was 42 yuan per time within the city and 193 yuan per time between cities. The indirect cost included cost for loss of working time (208 yuan per time), cost for treatment time (acupoint catgut embedding: 10 to 30 min per time, once one to three weeks; the remaining two treatments: 20 to 50 min per time, once to 5 times every week), time cost for arriving and departing hospital (1 to 3 hours per time within the city and 4 hours per time between cities). From the angle of treatment course, acupoint catgut embedding reduced the treatment frequency, leading to a lower total cost. (2)A mong the reasons to select different acupuncture methods to treat migraine, the results of patient questionnaire indicated that the focus in the patients who selected acupoint catgut embedding was different from that in the patients who selected two other treatments. The main advantage of acupoint catgut embedding was superior and lasting efficacy with low cost. The results of doctor questionnaire indicated the main reason to select acupoint catgut embedding was "lasting efficacy after single treatment" (87. 5%) and "lower frequency of treatment" (75. 0%). (3) The main reason to obstruct the popularization of acupoint catgut embedding was this method was not widely known.
CONCLUSIONThe acupoint catgut embedding is one ideal preventive treatment for migraine, which has higher cost-effectiveness, however, currently it is not widely applied. In future advertisement and training program should be strengthened to perform targeted popularization of acupoint catgut embedding for migraine.

6.Application of nasolabial flaps in repair of defects after midfacial skin tumor removal

Objective To investigate the methods and effects of nasolabial flaps for repair of defects after skin tumor removal on midface.Methods From Augest 2012 to March 2015,the different flaps from nasolabial fold were utilized to repair 22 skin defects due to surgical removal of skin tumors on midface.There were 12 flaps with subcutaneous pedicle and 10 flaps with skin pedicle in the group.The size of the defects ranged from0.5cm × 1.0cmto4.0cm × 5.0cm.Results After operation,all flaps survived with primary healing,cosmetic results were satisfactory with conceal scar and no facial organs distortion after 1 month to 2 years following-up.No tumor recurrence occurred.Conclusions Thenasolabial flaps with appropriate pedicles could obtain the satisfactory results for the reconstruction of medium-sized skin defects on midface,especially suitable for middle-aged and elderly patients.
7.Surgical Treatment of Chronic Pancreatitis with Mass in Head

Objective To summarize and discuss the diagnostic and treating experiences of chronic pancreatitis with mass in the head. Methods Eight patients of chronic pancreatitis with mass in the head who were misdiagnosised as carcinoma of head of pancreas were analyzed retrospectively in the past 10 years. Results All the patients exhibited abdominal pain,5 of whom were with jaundice and 3 with anorexia. All the patients were misdiagnosised as carcinoma of head of pancreas before the operation,but the pathology after operation indicated chronic pancreatitis. The pancreaticoduodenectomy was performed in 5 patients,the choledochojejunostomy in 2 patients,while the exploratory laparotomy in 1 patient. After the operations,the abdominal pain was relieved in 7 patients, while 2 patients who accepted pancreatoduodenectomy suffered from pancreatic fistula,1 of whom died in the end. Conclusion It’s hard to differentiate the chronic pancreatitis with mass in the head from the carcinoma of head of pancreas before operation. If the carcinoma of head of pancreas can’t be excluded during the operation,the pancreatoduodenectomy should be performed,while the duodenum-preserving total resection of the head of the pancreas or any intra-drainage operations should be done if chronic inflammation is found in the whole pancreas with a negative result of the biopsy of the pancreas through the needle aspiration.
8.Predictive effect of central lymph node intensity on lateral lymph node metastasis for papillary thyroid carcinoma

Objective:To investigate the risk factors and predictive effect of lateral cervical lymph node metastasis of papillary thyroid carcinoma (PTC) by applying the concept of central lymph node metastasis intensity.Methods:This study retrospectively analyzed integrated clinic data of 106 cases with PTC undergoing treatment of cervical lymph node dissection in Department of Thyroid and Breast Surgery of the Affiliated Hospital of Inner Mongolia Medical University from Dec. 2009 to Jan. 2014. Based on whether lateral cervical lymph nodes had metastasis, patents were classified into lymph node metastasis positive group ( n=75 cases) , lymph node metastasis negative group ( n=31 cases) . This study explored metastasis-associated risk factors of age, gender, triiodothyronine (T3) , thyroxine (T4) , free triiodothyronine (FT3) , free thyroxine (FT4) , thyroid stimulating hormone (TSH) , thyroglobulin antibody (TGAb) , thyroid peroxidase antibody (TPOAb) , whether combined with Hashimoto’s disease, tumor location, infringing the membrane, mulifocality, tumor glands distribution, tumor diameter, number of central lymph node metastases, central lymph node metastasis ratio, and analyzed the effects of central lymph node metastasis intensity on lateral cervical lymph node metastasis. SPSS 21.0 software was used for data analysis, the metering data of normal distribution was expressed as ± s, and t test was used for comparison between groups. Count data was expressed as a rate (composition ratio) , and comparisons between groups were performed by χ2 test or Fisher exact probability method. Results:Univariate analysis found that whether combined with Hashimoto’s disease ( P=0.087) , tumor location ( P=0.249) , tumor glands distribution ( P=0.219) and tumor diameter ( P=0.224) had no correlation with lateral cervical lymph node metastasis, which showed no statistical significant differences ( P>0.05) . Infringing the membrane ( P=0.030) , mulifocality ( P=0.031) , number of central lymph node metastases ( P=0.022) and central lymph node metastasis ratio ( P=0.001) had correlation with lateral cervical lymph node metastasis, which showed statistical significant differences ( P<0.05) . The number of central lymph node metastases and the increase of central lymph node metastasis ratio had positive correlation with the occurrence of lateral cervical lymph node metastasis; when the number of central lymph node metastases was ≥4 or (and) the central lymph node metastasis ratio was ≥20%, the incidence of lateral cervical lymph node metastases increased significantly, and the difference was statistically significant ( P<0.05) . Conclusion:Infringing the membrane and mulifocality are risk factors for lateral cervical lymph node metastasis. When central lymph node metastasis intensity: number of metastases ≥4 or (and) metastasis ratio ≥20%, lateral cervical lymph node dissection is recommended.
9.Clinical application of blepharoplasty by synchronously incising two eyelids and comparatively suturing sym-metrical points

Objective To explore the effective methods of increasing the symmetry of blepharoplas-ty. Methods The two eyelids were synchronously incised, and equal amount of musculi dormitator and fat tissues removed. Firstly, suture at the golden point of intersection for one eyelid, then suture the same point on the other one, and carefully compare the symmetry of both eyelids. Results All the 31 patients, except 1, were satisfied with the results of the symmetry with same height and length. Conclusion Syn-chronously incising the two eyelids and suturing at the symmetrical points by comparison can evidently in-crease the symmetry of blepharoplasty.
10.Determination of 10 mycotoxin contaminants in Panax notoginseng by ultra performance liquid chromatography-tandem mass spectrometry.

To ensure the quality and safety of Panax notoginseng, a method for the simultaneous determination of 10 mycotoxins in Panax notoginseng was developed using ultra performance liquid chromatography tandem mass spectrometry (UPLC-MS/MS). The sample was extracted with acetonitrile and purified by HLB multifunction cleanup column. The separation was performed on a Phenomenex Kinetex XB-C18 column by gradient elution using methanol and 5 mmol·L(-1) ammonium acetate as mobile phase. The targeted compounds were detected in MRM mode by mass spectrometry with electrospray ionization (ESI) source operated in both positive and negative ionization modes. The linear relationships of the 10 mycotoxins were good in their respective linear ranges. The correlation coefficients (r) ranged from 0.9981 to 1.0000. The LOQs of the 10 mycotoxins were between 0.15 and 8.6 μg·kg(-1). The average recoveries ranged from 73.8% to 107.0% with relative standard deviations (RSDs) of 0.10%-10.9%. The results demonstrated that the proposed method was sensitive and accurate, and suitable for the mycotoxins quantification in Panax notoginseng.
