Cervical spondylotic myelopathy (CSM) is a common cause of spinal cord dysfunction clinical disease. Surgery is the main therapeutic tool for CSM. However, there are obvious differences in clinical functional recovery after operation. For the past few years, the influence factors of prognosis in cervical spondylosis myelopathic has been widely concerned. Age, nerve function, course of desease, imaging findings, surgical method and related factors became the investigative point for prognosis of cervical spondylotic myelopathy. Present viewpoint showed that the older patient, preoperative worse nerve function, longer the course of disease would result in worse outcomes. Imaging examination maybe can indicate the prognosis, but the correlation is unclear. Selection of surgical method and approach should be based on the principles of sufficient decompression, stabilize the alignment of the cervical spine, keeping backward extension of cervical spine, maintain effective decompression, preventing complications. Therefore, the treatment of cervical spondylotic myelopathy should be on the basis of pathogenic condition and imaging examination at early stage and a suitable usrgical procedure should be performed to obtain a better prognosis.

Objective To study the dosimetric impact of different angle fields in intensitymodulated radiotherapy (IMRT) and the feasibility of beam angle optimization (BAO) for multiple intracranial metastases. Methods In total,11 patients with multiple intracranial metastases were included in these analyses. Two treatment techniques were designed for each patient:the 7 equal spaced fields (BAF group) IMRT,and 7 fields by beam angle optimization (BAO group) IMRT.The dose distribution in the target, the dose to the organs at risk and normal brain tissues, and total MU in two groups were compared to explore the dosimetric differences. Results In comparison to the BAF group, the BAO group reduced the maximum dose to left and right lenses by an average of 45%,37% (t =-5.707,-4.438,P < 0.05);the mean dose to the left and right eyes were reduced by an average of 42.6%,44.5% (t =-4.380,-5.638,P <0.05);the maximum dose to the right eyes were reduced by an average of 32.5% (t =-2.518,P < 0.05).The maximum dose of the right optic nerve and the mean dose of normal brain tissue were reduced by an average of 23% and 3% (t =-3.105,-3.437,P <0.05),respectively.For the target dose, conformity and homogeneity in PTV,no statistical differences were observed between the two groups (P > 0.05).The BAO group reduced the maximum dose of the brainstem and the optic chiasm,as well as the number of MU,however, the differences were not statistically significant (P > 0.05).Conclusions In comparison to the BAF group, the BAO group shows a similar target dose and reduces the dose for the organs at risk. For multiple intracranial metastases,IMRT protocols with BAO are feasible and beneficial.

Objective To evaluate the molecular mechanism of 11 beta-hydroxysteroid dehydrogenase type 1 (11 β-HSD1)in tumor necrosis factor-α (TNF-α) induced insulin resistance. Methods The optimal concentration and incubation time of TNF-α for treating HepG2 cells were selected.Real-time polymerase chain reaction(PCR)and Western blot analysis were used to explore the effect of TNFα on the mRNA and protein expression levels of 11β-HSD1,glucocorticoid receptor (GR),phosphoenolpyruvate carboxylase(PEPCK)and glucose 6-phosphatase(G6Pase).Glucose(GO) Assay Kit was used to determine the level of gluconeogenesis.Liver/Muscle glycogen assay kit,PAS/Glycogen Stain kit were used to detect glycogen synthesis. The expression of AKT/GSK pathway was analyzed by Western blot analysis. In addition,HepG2 cells were pre-treated with BVT2733,a specific inhibitor of 11β-HSD1,before exposure to TNFα The expression of AKT/GSK pathway, the level of gluconeogenesis and glycogen synthesis were analyzed. Results The expression of 11β-HSD1 was increased along with the increasing concentration of TNF-α,showing a dose-dependent effect. When HepG2 cells treated with the optimal dose of TNF-α 10 μg/L for 24 hours, the levels of GR,gluconeogenesis and gluconeogenesis-related genes PEPCK and G6Pase were increased, while conversely, the levels of phosphorylation of AKT and GSK,and glycogen synthesis were decreased. However, all the above effects induced by TNF-α in HepG2 cells were greatly reversed by BVT2733 pre treatment. Conclusions 11 β-HSD1 participates in TNF-α-induced insulin resistance in HepG2 cells, and BVT2733 could significantly improve insulin sensitivity.

BACKGROUND:There are stil some controversies about lateral compression rotational y unstable pelvic ring injuries and fixed mode. OBJECTIVE:To evaluate the pelvic stability of locking plate fixation pelvic reconstruction repairing lateral compression rotational y unstable pelvic ring injuries. METHODS:We retrospectively analyzed 15 patients with unstable lateral compression rotational y unstable pelvic ring injuries treated with reconstruction locking plates fixation at Suzhou Xiangcheng People’s Hospital between November 2011 and November 2014. The Matta standard was used to evaluate the quality of fracture reduction. Clinical efficacy was evaluated by Majeed score. RESULTS AND CONCLUSION:Fifteen patients were fol owed up for 12-44 months (mean 24 months). According to the standard of Matta, 11 cases were excel ent, 2 cases were good, 2 cases were average, 0 case poor, and the excel ent and good rate was 87%. According to Majeed functional assessment, clinical function outcomes were graded as 9 cases excel ent, 3 cases good, 1 case average, 1 case poor;excel ent and good rate was 80%. There were no patient loss of reduction and internal fixation failure among these 15 patients in the final fol ow-up. No iatrogenic neurovascular injury occurred. Incision superficial infection was detected in 2 patients with a Morel-Laval ee lesion after subjecting to double-tube continuous negative pressure drainage, and healed after wound management. A large area of infection and skin necrosis did not occur. These results confirm that pelvic universal reconstruction locking plates in repair of lateral compression rotational y unstable pelvic ring injuries can maintain a strong and effective fixation. We should pay much attention to the cases of pelvic fractures combined with soft tissue injury around the pelvis.
