1.Clinical investigation of chemotherapy with intraperitoneal hyperthermal perfusion for malignant ascites

Objective To observe the clinical effects of chemotherapy with intraperitoneal hyper-thermal perfusion for malignant ascites. Methods 40 patients with malignant ascites were randomly divided into two groups. In the treated group, 21 patients underwent intraperitoneal hyperthermal perfusion and local thermotherapy. 19 patients in the control group received commonly intraperitoneal perfusion. Results The CR, PR patients were 6,10 and 0,7 in the treated group and control group, respectively. The efficacy rate in the treated group was significantly higher than that in the control group. 2 patients oc-cured bellyache in the treated group and 1 patient in the control group, there was no statistically significant difference between the two groups. Conclusion Chemotherapy with intraperitoneal hyperthermal perfusion can significantly improve the therapeutic effects for malignant ascites, and has no more side effects.

3.Application of intelligent simulator (SimMan) in comprehensive skill training of doctors and nurses

Objective To examine the application effect of intelligent simulator(SimMan) in comprehensive skill training among doctors and nurses.Methods Totally 168 doctors and nurses were chosen as the training object,6 people (4 doctors and 2 nurses) formed a work team.Comprehensive training course was in accordance with 2010 American Heart Association guidelines for cardiopulmonary resuscitation and emergency cardiovascular care science.Real clinical situation was designed by using the third generation SimMan of Norway laerdal company.Trainees were administered two tests before and after training,in which there were two parts containing specialized operation ability and non-technical ability.Overall qualification rate,average mark and treatment time of qualified teams were compared before and after the training.SPSS 16.0 software was used to analyze the data and P <0.05 signifies statistically significance.Results Overall qualification rate,average mark and treatment time of qualified teams were 71.43%,(84.70 ± 3.15),and (449.25 ± 77.0) s respectively before training while 100%,(93.55 ± 3.86) and (323.71 ± 42.26) s after training.There were significant differences in results of two groups before and after training (P =0.002,0.000,0.000).Conclusions Application of the third generation intelligent simulator (SimMan) in comprehensive training will obviously improve training effectiveness and cultivate the non-technical ability.
4.Experimental study on H2-Ab1 gene expression in the nasal mucosa of mice with allergic rhinitis.

OBJECTIVE:
To investigate the level of H2-Ab1 in the nasal mucosa of mice with allergic rhinitis.
METHOD:
Twenty-four female 129/sv mice were divided into 2 groups: ovalbumin (OVA) group and control. The allergic rhinitis models were induced by classical method with OVA. After the last challenge, the pathological differences between the two groups were surveyed. The levels of H2-AB1 were measured by ELISA and quantitative real time PCR.
RESULT:
The expression of H2-Ab1 is higher in subjects with AR than that in controls (P < 0.05).
CONCLUSION
The levels of H2-Ab1 were highly increased in allergic rhinitis group, which might be associated with the pathogenesis of allergic rhinitis.

7.Clinical Observation of Argatroban in the Treatment of Progressive Stroke of Different Ischemic Ranges

OBJECTIVE:To study clinical effect of argatroban for progressive stroke patients of different cerebral ischemic ranges. METHODS:A total of 116 progressive stroke patients selected from neurology department of our hospital during Feb. 2015-May 2016 were divided into anterior circulation (ischemia) group (n=60),posterior circulation (ischemia) group (n=50) and lacunar(cerebral infarction)group(n=6)according to cerebral ischemic ranges. They all received routine treatment combined with argatroban,and given continuous intravenous infusion of argatroban 60 mg/d on the day and 2nd day of disease aggravation, and then continuous intravenous infusion of argatroban 5th day after relieving,3 h/time,bid,7 d as a treatment course. NIHSS scores,modified RANKIN (mRS) scores,APTT and ADR were compared among 3 groups. RESULTS:Forteen days after treat-ment,NIHSS scores and mRS scores of 3 groups were decreased significantly compared to before treatment,with statistical signifi-cance (P<0.05). There was statistical significance between anterior circulation group and posterior circulation group (P<0.05). Two hours after treatment,APTT of 3 groups were prolonged significantly,especially in lacunar group,with statistical significance (P<0.05). Forty hours after treatment,APTT of 3 groups were recover to normal,without statistical significance (P>0.05). No obvious ADR was found in 3 groups. CONCLUSIONS:Argatroban shows significant therapeutic efficacy for progressive stroke of different ischemic ranges with good safety;especially for the patients with anterior circulation ischemics stroke,the effect is quick and anticoagulant effect is significant.
8.The relieving effect of cyclooxygenase inhibitors and prostaglandin receptor blocker on hyperalgesia of diabetic rats

Objective To observe the effects of cyclooxygenase (COX) inhibitors and prostaglandin (PG) receptor blocker on hyperalgesia of diabetic rats. Methods Nociceptive paw-withdrawal threshold (NPWT) and thermal latency (TL) of diabetic hyperalgesic rats were tested after subcutaneous injections of COX inhibitors indomethacin and meloxicam, and EP 1 PG receptor blocker SC-19220 into the dorsum of the hindpaw of the rats. Results Indomethacin, a classic inhibitor of COX, produced a significant and long-lasting relief of mechanical hyperalgesia in diabetic hyperalgesic rats. Meloxicam, a selective cyclooxygenase 2 (COX-2) inhibitor, alleviated mechanical and thermal hyperalgesia, but with a shorter duration. SC-19220, a selective EP 1 PG receptor blocker, produced significant relief of mechanical and thermal hyperalgesia. Conclusion COX inhibitors and EP 1 PG receptor blocker relieve hyperalgesia of diabetic rats, which suggests that PGs play an important role in inducing and maintaining hyperalgesia in diabatic rats.
