1.Effects of ZPSJ Recipe on Insulin Sensitivity,Serum Resistin and Inflammatory Factors in Type 2 Diabetic Patients with Insulin Resistance

Objective To Observe the influence of Zaoshi Jiedu Zhupi Sanjing (ZPSJ) Recipe on insulin sensitivity, serum resistin and inflammatory factors related to the effects of Chinese herbal compound in type 2 diabetic patients with insulin resistance. Methods A single-blind randomized controlled clinical trial was conducted. 80 patients with 2 type diabetic patients with insulin resistance were recruited into a control group and a treatment group. The treatment group was treated with ZPSJ Recipe, and the control group was treated with metformin. Results In each group before and after treatment: blood glucose, serum insulin levels, HOMA-IR were significantly decreased (t=7.14, P<0.01 ) ; between the two groups, FPG, FINS showed no significant difference, while TG TCH, HOMA-LR showed significantly differences (t=2.62, P <0.05 ). Within the two groups before and after the treatment: serum IL-6, TNF-a, hs-CRP were decreased in treatment group, (t= 1.85, P<0.05 ) , while TNF-a, hs-CRP were decreased in the control group (t=2.56, P<0.05 ) , while IL-6 did not change significantly. The serum resistin levels were not statistically changed in both groups before and after treatment. Conclusion ZPSJ Recipe can regulate blood sugar, blood fat and improve insulin sensitivity in type 2 diabetic patients with insulin resistance.
2.Factors for restenosis following stent-assisted angioplasty of intracranial and extracranial arteries

Stent-assisted angioplasty of intracranial and extracranial arteries is a complex pathophysiologic process.Therefore,understanding of related factors is significant for guiding clinical treatment and reducing restenosis rate.Intravascular stent as a foreign body leads to platelet activation and aggregation,cytokine secretion,resulting in thrombosis and leukocyte aggregation at vessel injury site to produce cytokine and inflammatory reaction.In addition,the smooth muscle cells migrate to injury site and proliferate,leading to intimal hyperplasia,vessel wall reconstruction and intra-stent restenosis.Moreover,the restenosis following stenting is related to stent type and medicine supportive treatment.Drug-eluted stent with anti-inflammation and vascular smooth muscle cell proliferation can reduce body inflammation intensity and duration,and postoperative medicine supportive treatment can reduce incidence of intravascular restenosis.
3.Alterations of platelet activation and related factors following carotid artery stenting in rabbits

BACKGROUND: The role of inflammatory reaction and inflammatory factors and the mechanism of pathophysiology of restenosisfollowing the stent implantation remains controversial. OBJECTIVE: To observe the changes of platelet activation markers and correlated factors following carotid artery stenting in rabbits. DESIGN, TIME AND SETTING: Observation experiment was performed at the Institute of Neurobiology of Henan University fromMarch to September 2008. MATERIALS: A total of 26 rabbits, male or female, weighing 2.1-2.9 kg, were included. The alloy stent was made of 90% platinumand 10% iridium with wire diameter of 0.13 mm and monofilament winding spiral structure. The alloy stent was submerged 100 g/Lgelatin solution, and made into protein-coated stent after dried. METHODS: Animals were anesthetized by sodium pentobarbital (30 mg/kg), and the right carotid artery was exposed. The stent was inserted in the proximal end after administration of aspirin and clopidogrel dual antiplatelet therapy. Penicillin was intravenously injected, 800 000 IU per day. MAIN OUTCOME MEASURES: The platelet-activating factor (PAC-1 and CD62P) of peripheral blood, platelet-monocyte aggregates and interleukin (IL-6) immediately before and 0.5 hour, 6 days, 1 month and 6 months after implantation were determined.RESULTS: Twenty-six rabbits were included in the final analysis. Objective To investigate the effect of hepatopoietin Cn(HPPCn) on liver stem cells.Methods In this study, WB-F344 cell line was used, and MTT and flow cytometry assay were conducted to determine cell proliferation and apoptosis.Transwell assay was used to test the migration of WB-F344 cells.A 2AAF-partial hepatectomy(PH) mouse model was used to observe the effect of HPPCn on liver stem cell proliferation in vivo.Results HPPCn enhanced WB-F344 cell proliferation and migration and activated the SphK1, Erk and Stat3 signal pathways.The analysis of the 2AAF-PH mouse model showed that oval cells in the experimental group far outnumbered those in control and the regeneration of the liver was improved post PH.Conclusion HPPCn can increase the liver stem cell proliferation and survival while promoting the regenenation of the liver by augmenting oval cell proliferation.
5.Formulation optimization of metoprolol succinate sustained release pellets using central composite design-response surface methodology

Objective To optimize the formulation of metoprolol succinate ( MS) controlled release pellets by central composite design-response surface methodology .Methods MS sustained-release pellets were prepared using sugar pellet cores as starter beads , ethyl cellulose as coating materials and MS itself as a pore former .The formulation of MS sustained-release pellets was optimized by a central composite design with two factors at five levels .These two factors ( two independ-ent variables) were the pore former level and coating level , and the evaluated indexes ( namely dependent variables ) included the in vitro cumulative release percentages of MS at 1, 4, 8, 12 and 16 h, respectively.Results and Conclusion The results of mathematical equation fitting suggested that the second-order quadratic model was the optimal fitting equa-tion.According to the response surfaces , the optimum values at the pore former level and coating level weve ranged from 16%to 18%and 20% to 25%, respectively .The in vitro cumulative release percentage of MS from the pellets at 1 h reached 9.15%,which consequently eliminated the lag phase in the initial release period and exhibited a good sustained-release effect.Central composite design-response surface methodology can be applied to optimizing the coating formulation for MS sustained release pellets .

ObjectiveTo explore the clinical and epidemiological value of semi-quantitative regional cerebral blood flow(rCBF)imaging in children with school phobia. 7.Effect of whole-period psychiatric rehabilitation on outpatients with schizophrenia

ObjectiveTo investigate the effect of whole-period psychiatric rehabilitation on outpatients with schizophrenia.Methods90 outpatients with schizophrenia were randomly divided into the study group and control group with 45 cases in each group. All cases in two groups received pharmacotherapy, but cases of study group were added with whole-period psychiatric rehabilitation. Assessments were performed before and after study. All subjects were evaluated with the Brief Psychiatric Rating Scale (BPRS) and Social Disability Screening Schedule (SDSS).ResultsThe outcome of the study group was significantly superior to the control group on overall improvement according to the reductions of BPRS score, Anergia factor score, thought disturbance factor score, suspiciousness factor score and SDSS score(P<0.05-0.01).ConclusionThe whole-period psychiatric rehabilitation may play an important role in controlling symptoms and improving social function to outpatients with schizophrenia.
8.New suggestion for clinical downstaging of nasopharyngeal carcinoma in the era of intensity-modulated radiotherapy

Objective To propose a new suggestion for the clinical downstaging of nasopharyngeal carcinoma (NPC) in the era of intensity-modulated radiotherapy (IMRT) without changing the current T,N,and M staging system.Methods We reviewed the records of 536 NPC patients treated in Sun Yat-Sen University Cancer Center from January 2002 to December 2006.The Kaplan-Meier method was used to calculate the disease-specific survival (DSS) rate,and the log-rank test was used for survival difference analysis.The Cox regression model was used to calculate the hazard ratio (HR) of each subset.ResultsAccording to the 7th edition of UICC/AJCC staging system,the 5-year DSS rates of stage Ⅰ-Ⅲ patients (except T3N2M0) were all more than 85%(P>0.05),those of stage ⅣA and ⅣB patients were 71.8% and 46.2%,respectively (P=0.171),and that of stage ⅠVC patients was only 24.0%.In stage Ⅲ,the 5-year DSS rate of non-T3N2M0 patients (91.5%) was significantly higher than that of T3N2M0 patients (78.6%)(P=0.042),but there was no significant difference in DSS between T3N2M0 patients and stage ⅣA and ⅣB patients.Based on the above results,new stage Ⅰ included T1-3N0-1M0 and T1-2N2M0,new stage Ⅱ included T3N2M0,T4N0-2M0,and TxN3M0,and new stage Ⅲ included TxNxM1.The 5-year DSS rates of new stage Ⅰ,Ⅱ,and Ⅲ patients were 93.3%,72.7%,and 24.0%,respectively (P=0.000).Compared with new stage Ⅰ patients,new stage Ⅱ and Ⅲ patients had HRs of 4.01 and 16.76,respectively,for 5-year DSS.Conclusions In the era of IMRT,the new clinical staging system (stages Ⅰ,Ⅱ,and Ⅲ) helps with prognostic evaluation and clinical treatment.
9.Clinical observation of elderly patients with coronary heart disease undergoing coronary artery bypass grafting through descending thoracic aorta

Objective To investigate the clinical effectiveness of coronary artery bypass grafting through descending thoracic aorta in elderly patients with coronary heast disease and to decrease the post-operative complication.Methods Thirteen elderly patients underwent coronary bypass surgery with minimally invasive direct coronary artery bypass (MIDCAB).Age range from 70 to 82 years with a mean of(72.1?6.0)years.Patients suffered from multi vessel disease.Many minimally invasive techniques of“Y”blood vessel graft anastomosis,anastomosis of blood vessel graft to descending aorta,minimally invasive direct,thoracoscope assist were used.Results All patients were survived.The mean duration of intubation was (6.9?0.9) hours.The average ICU stay was (2.5?0.5)days.No patients received blood transfusion.During the short-term follow-up(3 to 14 months) patients had no complaint of angina,Conclusions The technique of“Y”blood vessel graft anastomosis,descending aorta blood vessel graft,minimally invasive direct and thoracoscope assist in combination with coronary artery bypass grafting is a safe and cost-effective new procedure for elderly patients with multi-coronary artery disease.
10.Experimental study on arsenic trioxide-induced apoptosis of human pan creatic cancer cell line

Objective: To study the effect of arsenic trioxide (As2O3) on human pancreatic cancer cell proliferation and apoptosis (mainly early stage) in vitro. Methods: SW1990 cells line were trea ted with As2O3 at different concentration. Cell proliferation was evaluated by MTT and apoptosis by Annexin-Ⅴ-fluostaining, electron-microscopy, flow cytometry and immunocytochemical staining of Bcl-2 and Bax. Results: As2O3 and cisplatin had the same cytotoxity on SW1990. The cytotoxic effe ct on tumor cell was produced by induction of apoptosis. Twelve hours after cult ure with 10 μg/ml As2O3, much more SW1990 cells went into apoptosis than t he control. The apoptosis rate reached 24% after 48 h with the similar concentra tion of As2O3. Immunohistochemical study revealed that the expression of Bcl -2 was decreased after treated with As2O3. Conclusion: As 2O3 can depress the proliferation of SW1990 in vitro, mainly through the i nduction of apoptosis, and it is a potential agent for pancreatic cancer chemoth erapy.
