1.Screening and model construction of the clinical diagnostic indictors for Kashin-Beck disease in adolescents

Objective To understand the relationship between clinical manifestations and Kashin-Beck disease(KBD) and their contribution to diagnosis of KBD and to construct the diagnosis model for KBD in adolescents.Methods A total of 2248 subjects under the age of 18 were collected from 6 KBD endemic and 1 non-KBD areas of the Shaanxi province in China.Analysis of 32 indicators,including gender,age,and KBD clinical indicators.Indicators of the distribution of measurement data between the two groups using t test and analysis of variance,x2 test with count data,multi-category ordered response variables Logistic regression analysis for model building.Results It showed the KBD prevalence rate in adolescent had an increasing tendency with age.Analysis of indicators between the two groups,in addition to the age factor(P < 0.05),the difference of ankle pain,knee pain,wrist movement disorder and other 5 indicators(P < 0.05) and the last bend,elbow movement disorder,syndactyly and other 9 indicators(P < 0.01 ) were statistically significant.Sixteen clinical and radiographic features in the clinical manifestations were significantly related with the clinical severity grading with KBD(P < 0.01 ).Four models on the diagnostic indictors were constructed by cumulative logit model for adolescent KBD (-21ogL,Score,Wald x2 test,P < 0.01 ).Conclusions The establishment of the diagnostic model based on their contribution of the joint involvement in systemic performance-related indicators has an important role for clinical diagnosis of KBD.
2.Relationship between whole blood chemical dements and coal-burning borne endemic fluorosis

ObjectiveTo investigate the differences of whole blood chemical elements and urinary fluorine between patients with endemic fluomsis and patients without endemic fluorosis,and to find out the elements associated with endemic fluorosis and further lay a theoretical basis for clarify the pathogenesis of the disease.MethodsUsing case-control study,100 children aged 8 - 12 with dental fluorosis in Wushan and Fengjie counties of Chongqing from December 2010 to February 2011,and 30 adults with skeletal fluorosis were enrolled as case group; 100 children aged 8 - 12 without dental fluorosis and 30 adults without skeletal fluorosis were enrolled as internal control group; and 50 children without dental fluorosis and 30 healthy adults were selected as external control group in non-epidemic areas in Yubei district.Whole blood copper,zinc,calcium,magnesium,iron and urinary fluorine of all subjects were determined,and differences of these indexes were compared between groups.ResultsThe levels of copper,zinc,calcium,magnesium,iron and urinary fluorine of children in the case group were (30.08 ± 2.83),(74.04 ± 9.75)μmol/L,(1.65 ± 0.29),(1.37 ± 0.17),(6.79 ± 1.27)mmol/L,and (0.73 ±0.37)mg/L,respectively; the levels of these elements of children in internal control group were (28.65 ± 3.96),(72.83 ± 11.35)μmol/L,(1.62 ± 0.27),(1.36 ± 0.18),(6.73 ± 1.22)mmol/L,and (0.48 ± 0.21)mg/L,respectively; in external control group were (32.03 ± 2.99),(77.78 ± 10.85)μmol/L,(1.41 ± 0.11),(1.43 ± 0.13),(7.66 ±0.55)mmol/L,and (0.49 ± 0.26)mg/L,respectively(all P< 0.05),the comparison between any two groups indicated the levels of copper,zinc,magnesium,iron of the case group were lower than that of external control group,urinary fluorine was higher than that of internal and external control groups(all P < 0.05).The levels of copper,zinc,calcium,magnesium,iron and urinary fluorine of adult case were (26.93 ± 4.37),(95.89 ± 12.45)μmol/L,(1.50 ± 1.76),(1.56 ± 1.96),(8.15 ± 1.00)mmol/L,and (2.17 ± 0.99)mg/L; internal control group were (26.26 ±4.96),(94.86 ± 12.18)μmol/L,(1.57 ± 0.12),(1.46 ± 0.16),(7.64 ± 1.00)mmol/L,and (1.44 ± 1.22)mg/L;external control group were (26.20 ± 2.96),(96.52 ± 11.11)μmol/L,(1.48 ± 0.14),(1.45 ± 0.16),(7.81 ±0.91 )mmol/L,and (0.55 ± 0.21 )mg/L,respectively.The levels of magnesium,iron and urinary fluorine of case group were higher than that of internal control group,magnesium and urinary fluorine were higher than that of external control group(all P < 0.05).ConclusionsIn vivo anti-fluorine elements are deficient in the areas with endemic fluorosis.Other chemical elements,the environment and genetic factors may be related to the pathogenesisof the disease,which needs a further comprehensive analysis.
3.Logistic regression analysis on postoperative pulmonary infection in hospitalized patients undergoing spinal surgery

Objective To understand the risk factors for postoperative pulmonary infection in patients undergoing spinal surgery,and put forward the intervention measures.Methods Patients who underwent spinal surgery in a hospital from May 2008 to June 2016 were analyzed retrospectively,they were divided into non-pulmonary infection group and pulmonary infection group according to whether they had postoperative pulmonary infection,clinical data of two groups were compared.Results A total of 612 patients who underwent spinal surgery were monitored,43 had postoperative pulmonary infection,incidence of postoperative pulmonary infection was 7.03%.Univariate analysis showed that 14 risk factors for pulmonary infection in patients after spinal surgery were as follows:length of hospital stay≥30 days,long-term smoking,chronic pulmonary disease,diabetes,number of surgical level≥2,general anesthesia,duration of operation≥4 hours,bleeding≥500mL,time of bed rest≥7 days,use of glucocorticoid,indwelling urinary catheter,mechanical ventilation,serum albumin<30 g/L,blood glucose≥1 1mmol/L,and hemoglobin<90 g/L(P<0.05);while atomization inhalation was a protective factor(P<0.05).Multivariate logistic regression analysis showed that 6 independent risk factors for pulmonary infection in patients after spinal surgery were as follows:length of hospital stay≥30 days,long-term smoking,chronic pulmonary disease,general anesthesia,time of bed rest≥7 days,and use of glucocorticoid(all P<0.05),while atomization inhalation was a independent protective factor(P<0.05).Conclusion Patients with pulmonary infection after spinal surgery is related to multiple factors,comprehensive and effective preventive measures should be taken according to the risk factors of postoperative pulmonary infection,so as to reduce the incidence of postoperative pulmonary infection in spinal surgery patients.
4.The effects of rational emotive behavior therapy in the rehabilitation ot patients with spinal cord injury

Objective To study the effects of rational emotive behavior therapy (REBT) in the rehabilitation of patients with spinal cord injury (SCI).MethodsA total of 300 SCI patients from six institutions were divided into a research group and a control group. Systematic rehabilitation was given to the patients of the control group, while systematic rehabilitation and REBT were given to the patients of the research group. All patients were followed up for 1 year. Psychological state, activities of daily living (ADL), and quality of life (QOL) were evaluated with a symptom check list (SCL-90), a modified version of the Barthel index (MBI) and the World Health Organization's quality of life assessment (WHOQOL-100).ResultsOne year after treatment, the improvement in QOL of patients in the research group was better, on average, than that in the control group. The differences were primarily in mental items. The ADL ability of patients in both groups improved, and any differences were not significant. The psychological state of patients in the research group had improved significantly 1 year after treatment, while the improvements in the control group were not significant on average, except in terms of interpersonal sensitivity.ConclusionREBT had little effect on the ADL ability of SCI patients, however, it improved their psychological state significantly, and thus improved their QOL.
5.The effect of intermittent deferoxamine therapy on relieving iron overload in children with ? thalassemia

Objective To evaluate the effect of the intermittent deferomamine(DF) therapy on relieving iron overload caused by transfusion in children with ? thalassemia.Methods Sixteen children who were finally diagnosed as ? thalassemia major were treated with deferomamine for 124 times totally to low the iron overload. The serum iron(SI), serum ferritin(SF) and urine ferritin were detected each time with radio-immunity technique and difference was compared before and after treatment. Meanwhile, weather DF involved children′s liver and renal function was observed in whole procedure.Results Iron overload exists in 16 cases of ? thalassemia major children by a long- term hypertransfusion therapy, with average level SI 33.69?6.72 mmol/L,SF 441.19? 54.70 ?g/L,urine ferritin 8.64?6.79 ?g/L. The difference was significant (paired-samples t test,t =6.173 P 0.05).Conclusion The study suggest that intermittent low-dose DF therapy is effective for iron overload caused by transfusion in ? thalassemia children, without apparent side effects.
6.Advances in researches on lactate dehydrogenase-C4 isozyme.

Lactate dehydrogenase-C4 isozyme has been widely studied as the sperm-specific antigen in the research of the contraceptive vaccine. Developmental and testis-specific expression of the LDH-C gene requires mechanisms for activation in germ cells and repression in somatic cells. LDH-C promoter, several transcription factors and methylation of CpG dinucleotides have been proved of governing action on LDH-C gene transcription. The strategy is to induce antibodies in the female reproductive tract against LDH-C antigens at a sufficient level to block fertilization. Chemically modified LDH-C4 offers a potential application in the induction of infertility of homologous species in marked contrast to native LDH-C4. In addition, advances are being made in researches on the male contraceptive vaccine and synthetic peptide immunocontraceptive.

8.Effect of Tongluo Xingnao effervescent tablets on learning and memory dysfunction in rats with chronic cerebral ischemia.

OBJECTIVETo study the effect of Tongluo Xingnao effervescent tablets on learning and memory capacity and expression of Na(+)-K(+)-ATPase in hippocampus of rats with chronic cerebral ischemia-induced learning and memory dysfunction model.
METHODThe 2-VO method was used to establish sd rat model learning and memory dysfunction induced by chronic cerebral ischemia. The 50 rats in the successfully established model were randomly divided into the model control group, the Dihydroergotoxine Mesylate tablets group (0.7 mg x kg(-1), Tongluo Xingnao effervescent tablets high dose (7.56 g x kg(-1)), middle dose (3.78 g x kg(-1)) and low dose (1.59 g x kg(-1)) groups and the sham operation group (n = 10) as the control group. The groups were orally given 10 ml x kg(-1) x d(-1) drugs for consecutively 90 days. On the 86th day, Morris water maze was adopted for them. On the 90th day, a leaning and memory capacity test was held. The brain tissues were fixed with 10% formaldehyde and observed for pathomorphism after routine slide preparation and staining. The expression of hippocampal Na(+)-K(+)-ATPase was detected with immunohistochemistry and image quantitative analysis.
RESULTCompared with the model group, all of Tongluo Xingnao effervescent tablets groups showed significant decrease in the escape latency at the 5th day in the Morris water maze, and notable increase in the frequency of the first quadrant dwell, the frequency passing the escape platform and the frequency entering effective area (p < 0.05). According to the pathomorphological detection, the control group showed a significantly higher pathological score than the sham operation group (p < 0.01), the middle dose group showed a significantly lower pathological score than the model group (p < 0.05). According to the immunohistochemistical detection, the model control group showed a remarkably lower mean OD value of Na(+)-K(+)-ATPase than the sham operation group (p < 0.05), high and middle dose groups showed a significantly higher mean od value than the model control group (p < 0.01).
CONCLUSIONTongluo Xingnao effervescent tablets can improve the learning and memory capacity, reduce pathological changes of hippocampal tissues of rats with chronic cerebral ischemia-induced learning and memory dysfunction model, and promote the expression of Na(+)-K(+)-ATPase in hippocampus.

9.Clinical observation on effect of fuzheng yiliu granule on cell cycle and nuclear transcription factor-kappa B in tissue of esophageal-gastric carcinoma.

OBJECTIVETo observe the effect of Fuzheng Yiliu Granule (FZYLG) on cell cycle and nuclear transcription factor-kappa B (NF-kappa B) in tissue of esophageal-gastric carcinoma.
METHODSSeventy-six patients with esophageal gastric carcinoma were randomly divided into two groups, the FZYLG group and the control group. FZYLG was given to the former for 15 days. The tumor tissue in both groups was resected and cell cycle and apoptosis rate as well as NF-kappa B were determined by flowcytometry.
RESULTSLevel of NF-kappa B in the treated group was significantly higher than that in the control group (P < 0.05). In the treated group, the percentage of G0/G1 stage cells were significantly increased and that of S stage significantly decreased (both P < 0.05). At the same time, obvious cell apoptosis was found in the treated group, the apoptosis rate of which was significantly higher than that in the control group (P < 0.01).
CONCLUSIONFZYLG can increase the NF-kappa B expression, block the proliferation to promote the apoptosis of tumor cells.

10.Cefotaxime-agar medium for laboratory detection of extended-spectrum and AmpC ?-lactamases in Enterobacteriaceae

Objective To develop an easy,rapid and reproducible cefotaxime-agar medium(CTX- AM)for phenotypic detection of extended-spectrum ?-lactamases(ESBLs)and AmpC ?-lactamases (AmpCs)in Enterobacteriaceae.Methods The surface of a cefotaxime(CTX,0.5 ?g/ml)-Mueller- Hinton agar and ceftizoxime(CAZ,1 ?g/ml)-Mueller-Hinton agar plate was inoculated with a lawn of E. coli ATCC 25922 according to the standard disk diffusion method,respectively.Immediately prior to use.blank and clavulanic acid(10 ?g),cloxacillin(300 ?g),clavulanic acid/cloxacillin(10/300 ?g) disk were rehydrated with 10 ?l of saline and several colonies of each test organism were applied to disks. Then the results of CTX-AM method to interpret based on a zone of growth around the periphery of disks.A total 58 of ESBL and AmpC producing and non-producing isolates of Enterobacteriaceae,as identified by the double-disk enhancement test(DDET)and the three-dimensional extract method(TDEM).were used to evaluate the CTX-AM method.Positive control(E.cloacae 029M,K.pneumoniae ATCC 700603)and negative control(E.coli ATCC 25922)strains were included.Results The results of CTX-AM method were similar to the DDET and TDEM method for detecting ESBLs and AmpC production in Enterobacteriaceae,respectively.But inhibitor-resistant ?-lactamase(IR-BLs)and other ?-lactamases were not detected by DDET method.Conclusions The new method described here allows for testing of ESBL and AmpCs on a single plate.It is easy to perform and interpret,and also cost-effective,clinical laboratories may use this technique routinely to detect the oresence of ESBL and AmoCs.
