1.Research development of APC gene promoter methylation in gynecological cancer

Adenomatous polyposis coli gene is an important anti-oncogene of the Wnt signaling pathway. The promoter hypermethylation of APC gene can effect the expression of APC mRNA and APC proteinum, which can make Wnt signaling pathway anomalous. Recently. the promoter hypermethylation of APC Gene in gynecological tumors is found in many studies, and it is related to the aggravation level of tumor.This paper reviews the influencing factors and progress of promoter methylation of APC gene in Gynecological cancer.
2.EC_(50) of propofol by target-controll infusion,BIS_(50) and AAI_(50) for different levels of sedation during combined spinal-epldural anesthesia

Objective To measure the EC50 of propofol administered by target-controll infusion(TCI) and BIS50 and AAI50 for different levels of sedation during combined spinal-epidural anesthesia (CSEA). EC50 , BIS50 and AAI50 were defined as the effective target effect-site concentration of propofol and BIS and AAI values at which different levels of sedation were achieved in 50% of patients.Methods Forty-five ASA Ⅰ patients (37 males, 8 females) aged 16-60 yr weighing 35-86 kg undergoing lower abdominal or lower limb surgery under CSEA were studied. CSEA was performed at L2-3 or L3-4 interspace. 0.75% bupivacaine 2 ml (15 mg) was injected into CSF. Intermittent boluses of 2% lidocaine were given through the epidural catheter during operation when needed. The average level of spinal block was T8 (T6-10 ) . The patients were sedated with propofol given by TCI. The level of sedation was assessed using observer assessment of the alertness/sedation scale (OAA/S) . BIS, AAI, MAP, ECG, HR, RR and SpO2 were monitored during anesthesia. Binary logistic regression was used to estimate the population values for EC50 BIS50 and AAI50 at different OAA/S scores. Results The EC50 of propofol from the score of 4-1 on the OAA/S scale were 0.6, 1.0, 1.3 and 1.6 ?g?mi-1 ; the BIS50 values 85, 75, 67 and 61 and AAI50 values 72, 59, 39 and 30 respectively. Conclusion EC50 of propofol given by TCI, BIS50 and AAI50 for different levels of sedation were measured during operation under CSEA and are useful for maintaining conscious sedation during CSEA.

Diabetic retinopathy ( DR ) is one of the main blinding eye diseases for people over the age of 50, and diabetic macular edema ( DME) is the leading cause of vision loss is DR patients. The early diagnosis and early treatment is important. As OCT and FFA, mfERG, especially the retinal thickness, volume, retinal edema index quantitative indicators such as objective evaluation of macular edema, embodies the new progress of retinal imaging technology in recent years. OCT is a non -contact clinical application in recent years, noninvasive, high resolution of ophthalmic imaging examination, can do it on retinal ultrastructure observation and quantitative analysis, and the technology is relatively mature, become a routine inspection diagnosis of macular edema. Laser photocoagulation, intravitreous injection with Ranibizumab and vitrectomy is nowadays the important means for the treatment of intractable macular edema.
4.Intervention with drug resistance related to metabolism of cancer cells:advances in research

The metabolic properties of cancer cells diverge significantly from those of normal cells. Energy production in cancer cells is abnormally dependent on aerobic glycolysis. In addition,cancer cells have other metabolic characteristics,such as increasing fatty acid synthesis and glutamine metabolism. Emerging evidences show that many key enzymes in dysregulated Warburg-like glucose metabolism,fatty acid synthesis and glutaminolysis are linked to drug resistance in cancer treatment. For example, lactate dehydrogenase A contributes to paclitaxel/trastuzuma resistance in breast cancer, fatty acid synthase is linked to docetaxel/trastuzumab/adriamycin resistance in breast cancer, and glutaminolysis is linked to cisplatin resistance in gastric cancer. Therefore,targeting cellular metabolism may improve the response to cancer therapeutics,and the combination of chemotherapeutic drugs with cellular metabolism inhibitors may overcome drug resistance in cancer therapy. This review discussed the relationship between dysregulated cellular metabolism and chemotherapy resistance, and the way in which targeting of metabolic enzymes can help overcome the resistance to cancer therapy or enhance the efficacy of common therapeutic agents.
5.Rapamycin aggravates the renal damage in rats with protein overload nephropathy and the protection of losartan

Objective To investigate the effects of rapamycin on renal tissue and function of rats with protein overload nephropathy and to explore the protective mechanism of losartan. Methods Experimental rat models with protein overload nephropathy, induced by intraperiotoneal injection of BSA (2. 0 g/d)into female Wistar rats, were divided into three groups: control group, rapamycin group(injected intraperitoneally with rapamycin) and losartan group(injected intraperitoneally with rapamycin and given orally with losartan). At different time points, the quantity of 24-hour urine protein and renal function were measured, and the morphologic changes of renal tissues were evaluated by HE staining and electron microscope. Results Both at day 7 and day 14, rats received BSA developed intense proteinuria. At day 7, compared with control group, 24-hour proteinuria increased markedly in rapamyein group (P<0.05). Nevertheless,proteinufia was notably alleviated in losartan group (P<0.05). At day 14, 24-hour-urine protein of rapamycin group was also significantly higher than that of the losartan group (P<0.05), but therewas no significant difference between control group and losartan group (P>0.05). Proteinuria and intratubular albumin cast formation were alleviated notably in losartan group. The fusion of focal podocytes in rapamycin group was obvious in comparison with control group. Conclusions Rapamycin can agrravate proteinuria in rats with protein overload nephropathy through changing the barrier of glomerular filtration by damaging podocytes. Furthermore, losartan can alleviate severe proteinuria induced by rapamycin.

8.Analysis of clinical effects of shuxuening injectionon renal function based on hospital information system data.

The safety of drugs has been the important issue that has caught the mutual attention of clinicians and patients. Kidney is one of the important organs in drug metabolism. The study is based on hospital information system (HIS) data from 20 nationwide general hospitals, indicated by Scr and BUN. Generalized boosted models (GBM) based on propensity score method were used to balance confounding factors. The study contrasts changes on renal function between used Shuxuening injection patients and the unused to explore whether Shuxuening injection affects renal function in clinical. The results showed that Shuxuening injection is likely to affect Scr in logistic regression (P value is 0.035); after the logistic regression based on propensity score method (P value became 0.513, 0.590), no differences were found between the two groups; before and after the logistic regression based on propensity score method (P value was less than 0.05), Shuxuening injection possibly affects BUN. Nevertheless, this study is a retrospective analysis. Logistic regression based on propensity score method can control the known confounding factors very well, while there may be the unknown ones. Therefore, the further exploration is required to monitor whether Shuxuening injection affects really BUN, which will provide clinical guidance for Shuxuening injection safe application.
