1.Early efficacy comparison between independent exercises under the doctors guidance and passive exercises by using CPM in rehabilitation after TKA
The Journal of Practical Medicine 2015;(7):1120-1122
Objective To compare the early efficacy of two different ways of rehabilitation exercises in total knee arthroplasty (TKA) patients. Method From January 2011 to January 2014,136 patients having undergone TKA were randomly divided into two groups. In group A , patients exercised under the guidance with doctor. In group B, patients exercised by using continuous passive movement (CPM) machine. Compared the data from the two groups including range of motion (ROM) of the knee joint, knee function score (KFS) and circumference of the knee joint, t test of independent sample was utilized in the research. Results All patientswere followed up in 4 weeks. The ROM and KFS of Group A was significantly better than Group B (P < 0.05). The knee circumference had no statistically significant difference between two groups (P> 0.05).Conclusion Group A and B are effective methods in TKA patients in early rehabilitation training. But training under the guidance of doctor has greater advantages than CPM.

Objective:To explore the mechanism of total saponins of panax notogineseny (PNS) on secretion of TNF-α by macrophages and optimal dosage of PNS in vitro, through observation of the effects of different PNS dosages on nuclear factor-κB (NF-κB) activity and TNF-α mRNA expression of murine peritoneal macrophages (PMΦs) after severe scald. Methods: The experimental model of 15% TBSA full-thickness scalded mice with vapor was used and PMΦ collected. NF-κB activity was measured by EMSA and TNF-α mRNA by RT-PCR. Results: There was a significant increase of the NF-κB activity and TNF-α mRNA expression in the PMΦs following scald, which was inhibited by application of PNS. It was also found that the effects of PNS was dosage-dependent within a certain range of concentrations, with the inhibition effect most obvious at 0.8 mg*mL-1. Conclusion:PNS probably decreases TNF-α mRNA expression by inhibiting the NF-κB activity of PMΦs.

Objective To assess the dagnostic value of computed tomography(CT) for recurrent carcinoma of larynx after operation. Methods 45 CT cases of recurrent carcinoma of larynx were analysed retrospective and were compared with pathology results. Results CT was capable of demonstrating the existence, border and range of the recurred tumor and showing the information about inetastasis in lymph node of neck, the involved throat space and the invaded blood vcsscls in cervical. Conclusion CT is an cxact, rcliable and convenient method to evaluate the recurrence of larynx carcinoma, and is helpful to make therapy planning.

Patients with irritable bowel syndrome(IBS,n=41)subjected to gastroscopy,patholo- gy,esophageal manometry and 24h pH monitoring. The records were compared with that of normal in- dividuals(n=30)to clarify the relation of esophageal motor function with esophageal complaints in cas- es with IBS.Results showed that 21/41 cases with IBS presented esophageal symptoms.Rate of abnor- mal endoscopic and pathologic findings in 6 of which was statistically higher than the control's; esophageal lower sphincter pressure in IBS with symptoms declined significantly than that without and the control;esophageal peristaltic velocity and duration increased with significance;no pressure differ- ence was detected in esophageal upper sphincter;total pH score ,fraction-time pH

Limb-shaking transient ischemic attack (TIA), a rare manifestation, is commonly caused by severe
stenosis or occlusion of an extracranial internal carotid artery. Such patients are usually treated with
surgical revascularization or anti-platelet therapy. We present a 56-year-old woman with 6 months’
episodic attacks starting with mouth skewed to the right and a sensation of ‘weakness’ involving
predominantly her left arm, and at times, also involved the left leg. This was immediately followed by
rhythmic jerky movements of the left arm and at times, also involved the left leg. Magnetic resonance
angiography revealed severe stenosis of M1 segment of the right middle cerebral artery. The patient’s
symptoms were signifi cantly improved by treatment with anti-platelet drugs and nimodipine.

Objective To observe the effects of drainage with vacuum and closure (VAC) on acute wounds, and explore the mechanism of drainage with VAC in promoting wound healing. Methods Twenty-four acute wounds were inflicted on the backs of 12 New Zealand white rabbits (each rabbit two wounds), and the rabbits were divided into a drainage with VAC group and a control group randomly. The drainage with VAC group was treated with drainage with VAC. The control group was treated with wet saline gauze. The wounds were observed 3 and 7 days after treatment. Patho-morphological changes in tissues from the compressed area were observed by HE staining. The expression level of Cx 43 mRNA was detected using a RT-PCR.Results At the 3rd and 7th day after treatment, the wounds of the drainage with VAC group were clean, fresh and had less edema compared with those of the control group. Pathomorphological tissue changes were more obvious in the drainage with VAC group. The expression of Cx 43 mRNA in the drainage with VAC group had declined significantly compared with the control group. Conclusion Drainage with VAC can promote inflammatory cell infiltration, down-regulate the expression of Cx 43 mRNA, and promote wound healing.

AIM: To study pattern recognition of the fingerprint of Polygonum orientale from Guizhou Province. METHODS: 20 batches of samples,Polygonum orientale, come from different producing areas and harvest time were carried out to gain fingerprints by HPLC-DAD,as compared with retention time and ultraviolet spectra of standard substances. The pattern recognition of the characteristic fingerprint of Polygonum orientale, known as cluster analysis and principal component analysis, formed. RESULTS: The fingerprint of 20 batches of Polygonum orientale showed 12 characteristic peaks,in which 9 common peaks were confirmed.the samples were grouped into 3 types of harvest time. CONCLUSION: The quality of Polygonum orientale in Guizhou Province is stable, the pattern recognition of the fingerprint provides the experimental basis for manufacture and quality control of Polygonum orientale.

The regulation mechanism of arecoline on rat hepatic CYP2E1 was studied in vivo. After oral administration of arecoline hydrobromide (AH; 4, 20 and 100 mg x kg(-1) x d(-1)) to rats for one week, the hepatic CYP2E1 mRNA level remained unchanged, but the hepatic CYP2E1 protein content was dose-dependently increased. Additionally, although the hepatic CYP2E1 activity was induced by AH treatment, the induction was attenuated with the increase in dosage. The results indicate that the effect of arecoline on rat hepaticdoes not involve transcriptional activation of the gene, but largely involves the stabilization of CYP2E1 protein against degradation or increased efficiency of CYP2E1 mRNA translation, and additionally involve the post- ranslational modification of CYP2E1 protein. Furthermore, the CYP2E1 response is fairly equal among the different species, the induction of rat hepatic CYP2E1 by arecoline suggests that there is a risk of metabolic interaction among the substrate drugs of CYP2E1 in betel-quid use human.

Objective To explore the effect of exposure of the recurrent laryngcal nerve(RLN) in prevention of RLN injury during thyorid surgery. Methods The clinical data of 810 cases of thyroid operation in which the RLN was exposed in 252 cases(group A) and was unexposed in 558 cases(group B) were analyzed. Results Although the extent of thyroid resection was greater in group A than in group B,the rate of RLN injury in group A(1.19 %) was significantly lower than that of group B(3.05 %,P
