1.A case of hibernoma arising in the scalp.

Hibernoma is a rare subcutaneous neoplasm resembling the brown fat of the hibernating animal. It is usually found in the interscapular, cervical or axillary regions. We report a case of this rare tumor which developed on the occeipital scalp of a 42-year-old woman.

2.Plasma Levels of Dehydroepiandrosterone Sulfate ( DHEA - S ) and Total Testosterone in the Patients with Female androgenetic Alopecia.

BACKGROUND: It has been known that androgenetic alopecia in both male and female is due to the role of the androgen hormone in the hair follicle, but the level of the androgen hormone in the plasma is controversial in relation with the development of the alopecia. OBJECTIVE: We have tried the clarify if there is any or no difference in the plasma levels of the hormones between the normal female and the patient with alopecia, because the range of the plasma DHEA-S and total testosterone in the normal female is very wide. METHODS: Plasma levels if the hormones were examined in 22 patients with female androgenetic alopecia and 20 normal females. RESULTS: 1. In DHEA-S, 21(95.5%) aut of 22 female androgenetic alopecia patients were in the normal range while 20(100%) of the 20 normal females were all within the normal range. However the mean value of the hormone(88.89+101.41 g/dl) in the patient group was significantly higher than that(82.18+44.03 g/dl) of the normal group(p<0.05). 2. In total testosterone, 17(94.4%) out of the 18 patents were in the normal range while 20 (100%) of 20 normal females were all within the normal range. However the mean value of the hormone(0.22+0.20ng/ml) in the patient group was significantly higher than that(0.08+0.09ng/ml) of the normal group(p<0.05). 3. As for distribution of DIEA S according to the age, the value of the patient group was higher than that of the normal group in all age groups studied. In the distribution of total testosterone according to age, there was no difference between the normal and the patient in the twenties. After the twenties, the testosterone levels were higher in the patients than the normal females. CONCLUSION: There were significant differences in mean values of plasma DHEA-S and total testosterone between the patients and the normal females although individuals in both groups were largely wit,hin normal range of the hormones.

3.Serum Levels of ICAM-1(In tercelluar Adhesion Molecule-1) in Invasive Cervical Cancer.

ICAM-l(Intercellular adhesion molecule-1) is an important early marker of immune activation and response. ICAM-1 is expressed on varous cell types and observed in a variety of diseases, including patients with asthma, melanoma, prostatic cancer, ovarian and colon cancer. Some authors demonstrated the expression of ICAM-1 protein in high-grade intraepithelial squamous neoplasia of cervix by immunohistochemistry and suggested that the expression was related to human papillomavirus infection. The aim of this study was to determine the serum levels of soluble intercellular adhesion molecule-1(sICAM-1) in patients with squamous cell carcinoma of the cervix, Serum levels of sICAM-1 were measured by enzyme-linked immunosorbent assay(ELISA), We evaluate invasive squamous cell carcinoma of the cervix (40), carcinoma in situ (16) and control (15) patients. Serum levels of sICAM-1 in healthy volunteers, in parients with carcinoma in situ and invasive cervical cancer were 150.1+/-41.3, 182.7+/-105.9, 189.8+/-60.0 ng/ml, respectively. Although the serum levels of sICAM-1 in patients with carcinoma in situ did not increase, serum levels of ICAM-1 in patients with invasive cervical cancer were significantly increased (control vs invasive cervical cancer, p < 0,05). From the above results, sICAM-1 is shed from the cancerous tissue in patients with squamous cell carcinoma of the cervix.

4.A case of atypical fibroxanthoma.

A 43-year-old male patient showed well defined, irregular, dark eythematous, thick plaque and tumor with ulceration on the right chest, right axilla, pubis and scrotum, Histopathological findings rervealed atypical lymphocytes showing epidermotrophism and heavy infiltrations in the deep dermis and subcutaneous tissue. The infiltrative cells were positively stained with leukocyte common antigen and T-cell UCHL-1 antigen, but not with L26 antigen. This clinicohisto-pathological features were consstent with dermblade type of mycosis fungoides. The authors report clinicopathological findings of the case with its thrapeutic responses to systemic chemotherapy and radiation treatment.

5.Supracondylar Osteotomy for Correction of Deformities of the Knee in Poliomyelitis

Supracondylar osteotomy was performed on 78 knees in 65 patients with poliomyelitis for correction of deformities of the knee at the Department of Orthopedic Surgery, Seoui National Unlversity Hospital, during the 16 years from January 1963 to December 1978. Following observations were made from the review of statistics, technical problems, and complications of the operation. 1. The ages of the patients ranged from 4 years to 28 years, the average being 15 years. Two thirds of the patients were in the second decade. 2. 37 knees (47.4%) had flexion deformity, 19 knees (24.4%) flexion-valgus deformity, and 13 knees (16.7%) flexion-valgus-external rotation deformity. The amount of flexion deformity ranged from 7 degrees to 90 degrees, the average being 22 degrees. Valgus deformity ranged from 3 degrees to 25 degrees, the average being 12 degrees. External rotation deformity ranged from 5 degrees to 30 degrees, the average being 18 degrees. 3. The operative techniques employed were categorized into 5 types. The most common type was a closed wedge hinged osteotomy (Technique I), which was employed on 50 knees (64.1%). Satisfactory results were obtained by combining other procedures, when necessary, i.e. pre-operative skeletal traction, Yount fasciotomy, high tibial rotation osteotomy hamstring transfer and epiphysiodesis. 4. The time required for union was the shortests in the Technique 1 group, and longest in the Technique III group, a complete osteotomy with bone shortening. 5. The most common complication was stiff knee, which was encountered in 10 knees. Residual or recurrent deformity was observed in 4 knees, in 3 of which hamstring transfer was recommended but was not consumated. Delayed union was observed in 3 knees in the Technique II group (closed wedge complete osteotomy) and in 1 knee in the Technique III group. Nonunion occured in 1 knee in the Technique II group. The incidence of complications were higher in the Technique II and III groups, and lowest in the Technique I group.

8.Clinical Experience of Transurethral Needle Ablation (TUNA) for the Treatment of Benign Prostatic Hyperplasia with Low Compliance to Medication and High Risk Operative Morbidity and Mortality.

PURPOSE: We investigated the efficacy of transurethral needle ablation (TUNA) of prostate for benign prostatic hyperplasia (BPH) in patients with low compliance to medical therapy and high risk operative morbidity and mortality. MATERIALS AND METHODS: Total 38 patients with BPH and low compliance to medical therapy and high risk operative morbidity and mortality were treated with TUNA under the local anesthesia and evaluated prospectively using the international prostate symtom score (IPSS), Qmax, satisfaction score and postvoid residuals (PVRs), and followed for 3 months after treatment. RESULTS: The mean pretreatment symptom score was 24.82+/-5.76. At 1 and 3 months after treatment, the mean symptom score was decreased to 13.63+/-7.07 and 9.21+/-6.28, respectively (p<0.01). The mean pretreatment satisfaction score was 4.63+/-0.85. It was decreased to 2.84+/-1.26, 1.92+/-1.34 at 1, 3 months (p<0.01). The mean pretreatment Qmax was 5.26+/-3.37mL/s. It was increased to 9.53+/-4.54mL/s, 11.97+/-4.52mL/s at 1, 3 months (p<0.01). The mean pretreatment PVRs were 131.85+/-123.05mL. It was decreased to 49.68+/-38.28mL, 26.77+/-17.92mL at 1, 3 months (p<0.01). CONCLUSIONS: TUNA treatment in the management of BPH improved symptom scores, peak flow rates with lower morbidity. TUNA appears to be a useful alternative treatment for BPH in patients with low compliance to medical therapy and high risk operative morbidity and mortality.

9.A Case of Malignant Transformation of Gastric Tubular Adenoma Proven by 9-year Follow-Up.

The association of gastric tubular adenoma and adenocarcinoma is already well known. Then, are those two pathologic lesions merely incidentally coexisting or does the one evolve to the other? That is a longstanding controversy. Nowadays, as is the case with the colonic tubular adenoma, the hypothesis that gastric tubular adenoma may be a precancerous lesion is generally accepted. However the direct evidences are rare. We report a clear case proving the potential of direct malignant transformation of gastric tubular adenorna through the 9-year close endoscopic follow-up. The developed cancer is well differentiated, surrounded with background tubular adenoma and limited to the submucosal layer.

10.Multicentric Extrahepatic Tumor Involving the Gallbladder and Common Bile Duct A Case Report of Longterm Survival.

A 60-year-old woman has experienced of right upper quadrant abdominal discomfort. This partient took cholecytectomy due to gall stone 15 years ago. At that time microscopic finding the neck of gallbladder showed well differentiated papillary adenocarcinoma confined to superficial part of musele layer. 3 years before admission she took T-tube drainage due to obstructive jaundice. The microscopic finding af distal common bile duct showed well differentiated adenocarcinoma. Recently she complained of pain and discomfort on T-tube site and so we perforrned endoscopic retrograde biliary drainage after removal of T-tube. The computed tomogram showed no mass in liver or lymph node enlargement. 4 months after procedure of ERBD; we changed to the new catheter for the prevention of cloggiag of prosthesis. We report here a case of longterm survival over 15 years after the diagnosis of cancer of extrahepatic bile duct.
