Objective To summarize our experience in the diagnosis and management of popliteal artery entrapment syndrome (PAES). Methods This is a retrospective study on 10 patients (13 limbs)who were admitted for symptoms of claudication and the diagnosis of popliteal entrapment was established either with angiography, computed tomographic angiography, magnetic resonance angiogram or during the operation in recent 7 years (2002-2009).All patients were treated surgically. Results The mean age at the time of presentation was (25 ±7) years old (range,17-41 years).Claudication was the most frequent presenting symptom (12 limbs). The surgical procedures consisted of simple musculotendinous dissociation in 1 limb, thrombectomy with balloon angioplasty in 1 limb, musculotendinous dissociation plus thromboendarterectomy with autogenous saphenous vein (ASV)patch angioplasty in 2 limbs,ASV graft interposition or bypass in 6 limbs and graft interposition or bypass in 3 limbs. At a median follow-up of (35 ±27) months (range,2 months-7 years),there were no intraoperative or long-term postoperative complications and all the patients were cured. Conclusions PAES is an unusual but important cause of peripheral vascular insufficiency especially in young patients.A combined approach is necessary for diagnosis.Popliteal artery release alone or with vein bypass or reconstruction is the treatment of choice.

Objective To evaluate the effects of external substance P (SP) on scalding wound healing and neovascularization. Methods Eighty-four Wistar rats were induced into diabetic models by intraperitoneal injection of streptozotocin (STZ), and deep partial thickness scalding wound on the back with diameter of 2 cm was prepared. Rats were randomly divided into experiment group (n=42, local injection of SP) and control group (n=42, local injection of PBS). The process of wound healing was observed, and the percentages of wound closure were calculated on D0, D1, D3, D7, D10, D14, D21 post scald. The expression of CD31, surface marker of neovascular endothelial cells, was detected within the wound sites by immunohistochemical staining, and the microvessel density was calculated. Results The percentage of wound closure in experiment group was significantly higher than that in control group on D7 post scald [(42.69±3.26) % vs (30.24±1.17)%, P<0.01]. Immunohistochemical detection revealed that the expression of CD31 and the microvessel density in experiment group were significantly higher than those in control group from D7 post scald (P<0.01). ConclusionExternal SP may promote scalding wound healing in diabetic rats, the mechanism of which may be associated with upregulation of expression of CD31 and acceleration of neovascularization.

As CD40 transduces activation signals involved in inflammatory and immune disorders, we explored the expression and response to CD40 engagement in human glioma cell lines in this study. The CD40 expression in BT-325 and U251 cells was flow cytometrically detected. The cells were incubated with srhCD40L for 72 h to assess its effects on cell growth in vitro. TNF-α expression was quantified by real-time PCR, and protein expression was analyzed by ELISA. The I-κb mRNA was detected by RT-PCR. I-κB expression decreased after stimulation with 1 μg/mL srhCD40L, but it was upregulated after the cells were pretreated with CD40 antibody. srhCD40L significantly inhibited the proliferation of the CD40+ human glioma cells. The stimulation of CD40+ glioma cells with soluble CD40L (CD154) up-regulated the expression of TNF-α at both mRNA and protein levels. We are led to conclude that CD40L/CD40 could inhibit human glioma cells through I-κb signaling pathway. Interferon-γ can augment CD40 expression and the inhibitory effect of CD40 ligand on cell growth in vitro. These results suggest that srhCD40L may benefit the therapy strategy of glioma.

Background With the changes of diet and living style, the diabetes has become the major diseases affecting human health.Diabetic cataract is a common complication of diabetes. Objective The present study was to investigate the difference of lens proteomics between diabetic cataract and age related cataract using two dimensional electrophoresis (2-DE) and mass spectrometry in order to postpone happening of diabetic cataract and offer the effective approach to the prevention and therapy of diabetic cataract. Methods The lenses were obtained from 8 diabetic patients and 12 age-related cataract patients during the surgery to extract the protein by lysis and centrifugation. The lens proteins were separated using immobilized pH gradients 2-DE.Image analysis was carried out using PDQuest Advanced-8.0.1 software package.Significant difference of the crystallines was identified by matrixassisted laser adsorption/ionization time of-flight-mass spectrometry (MALDI-TOF-MS) and peptide mass fingerprint combined with protein database. Results The maps of 2-DE showed that lens proteins of diabetic cataract and age related cataract were in the section of pH 5-9 with the relative molecular weight 14000-97000;while relative molecular weight of more abundant crystalline was localized at 20000-31000.About 3 differential protein spots were detected by image analysis software. Two crystallines,αB and βB1 crystallin, were identified using MALDI-TOF-MS.Conclusions Proteomic analysis of lens can be accomplished and the proteins can be well separated, moreover, differential proteins can be analyzed using 2-DE and mass spectrometry between diabetic cataract and age related cataract. These results indicate that αB and βB1 crystallin proteins accelerate the development of diabetic cataract. This technique offers a new avenue for clarity of lens proteins of diabetic cataract other than age related cataract.

Objective To investigate how much the patient setup accuracy for irradiation of head and neck cancer can be improved by online setup verification and offline setup verification using cone-beam computed tomography(CBCT), and the feasibility of image-guided adaptive correction procedure to reduce the PIN margin. Methods 16 patients of head-and-neck cancer treated with three-dimensional conformal radiotherapy (3D-CRT)or intensity modulated radiotherapy(IMRT)were investigated. The first online kV CBCT scan, rigid image registration, setup correction were performed before radiotherapy. The second kV CBCT scan were acquired immediately after treatment and analysis was performed as above. CBCT scans were acquired at two or three fractions weekly during the entire course of radiotherapy and CBCT online-guided correction data were recorded. The data was used to calculate the population-based CTV-PTV margins under the condition of non-correction, correction every fraction and compensation of the systematic setup error respectively. The number of initial images required to predict systematic setup error was evaluated with the permission of 0.5 mm residue error. Results Total of 320 sets of CBCT images were analyzed for 16 patients. Under the condition of non-correction, the margins required to account for total error are 5.7 mm,5.6 mm,and 7.3 mm in the left-right(X axis),cranio-eaudal(Y axis), and anterior-posterior (Z axis)directions respectively, when the tumor was corrected every fraction, the margins required to account for intrafraetion error are 1.7 mm,1.7 mm,and 2.3 mm in X, Y,and Z axis.To correct the systematic setup error,8 sets of CBCT images are adequate. After compensation for the effect of the systematic setup error, 2.7 mm,2.5 mm, and 3.6 mm PTV margins are necessary in X, Y, and Z axis respectively. Conclusions There exists some extent of setup error in head and neck 3D-CRT or IMRT.The on-line CBCT correction and the approach based on off-line adaptive correction both can be used to reduce the impact of setup error obviously, the required margins for the PTV was reduced accordingly.
