BACKGROUND:For patients with lumbar spondylolisthesis combined with osteoporosis, appropriate fixation system for effective reset and good fixation stability is currently a hot issue of clinical concern. Pedicle screw screw-rod system after bone cement perfusion can achieve the effective fixation between pedicle screw system and the vertebral bone. OBJECTIVE:To observe the therapeutic effect of bone cement-augmented pedicle screw on patients with lumbar spondylolisthesis combined with osteoporosis. METHODS:17 cases of lumbar spondylolisthesis combined with osteoporosis were identified by bone density test. They received the posterior open reduction and internal fixation, and implanted with 68 bone cement-augmented pedicle screws. Their repair effects were observed by short-term fol ow-up. Patients were evaluated using low back pain Visual Analog Scale and lower limb Oswestry Disability Index before treatment, 1 week, 3 months and 1 year after treatment. Vertebral height, intervertebral height, screw loosening and bone cement leakage were observed using imaging.
RESULTS AND CONCLUSION:Compared with pre-treatment, low back pain Visual Analog Scale score and lower limb Oswestry Disability Index were significantly improved at 1 week, 3 months and 1 year after treatment (P<0.05). No significant difference was detected between post-treatment and fol ow-up (P>0.05), which indicated that clinical repair effect could be effectively maintained. At 3 months of fol ow-up, one screw loosening occurred in two patients. During fixation, mild bone cement leakage appeared in seven vertebral bodies with screw fixation, no symptoms or subsequent complications were observed. There were no significant differences in vertebral height and intervertebral height before and after treatment and during fol ow-up (P>0.05). These results suggest that bone cement-augmented pedicle screw for patients with lumbar spondylolisthesis combined with osteoporosis can effectively reset vertebral slippage, effectively provide good anti-pul-out force for a long term, and the effect was stable. Bone cement augmentation can perfectly strengthen fixation, shows good biocompatibility, and avoids osteoporosis around the screw-induced failure fixation.

Objective To probe into the trust of residents on the health centers' service offered in their community in general for providing better family doctor services.Methods A custom-made trust scale for medical service (including 23 indicators) and random sampling were called into play.An imercept survey was made to 224 residents of different gender,age and education in a community in Beijing regarding their overall trust on medical service.Results The different satisfaction over their selection of the 23 indicators among residents of different gender is not significant statistically (P＞0.05); the seven differences of the residents of different ages regarding their trust of the general medical service are significant statistically; the 11 differences of the residents of different education regarding their trust on the community health centers in general are significant statistically.Conclusion The GP-based community health centers are expected to improve the trust of residents on them in general,by means of enhancing government responsibility,government investment and the improvement of services of general practitioners.

Praziquantel ( PQT ) concentrations in plasma after iv 20 mg/kg decayed rapidly with tip of 0.36 h. The absorption of PQT was rapid following the intramuscular doses of 10,20,40mg/kg or intragastic dose of 100mg/kg, but the phase of elimination was much longer than that after iv. Both of MAT1m and MATig were greater than MRTiv. The bioavailability of ig was 13.2%, suggesting a strong first-pass effect. The kinetics of PQT elimination was linear after intramuscular dose of either 10 or 20 mg/kg, but nonlinear process was found when the dose was increased to 40mg/kg.By any route of iv, im and ig administration, the concentrations of PQT in the bile were much lower than the peripheral plasma concentrations and changed in parallel to the later with high levels after iv, medium levels after im and much low levels after ig.

Objective To isolate and identificate a specific new gene highly related to human bladder cancer and to investigate the carcinogenesis mechanism and genetic susceptibility. Methods Normal bladder epithelia and cancer tissue collected from 8 cases of primary bladder transitional cell carcinoma were studied using the high performance?sensitive technique of mRNA differential display.The differential fragments were found by comparing with genetic expression. 6 of them were reamplificated?cloned?sequenced,and performed the autoploidy index in the public database of genbank. Results (1)37 differential fragments have been isolated.36 of which are expressed or highly expressed in cancer tissues,but not so or lowly expressed in normal tissues;while it has been expressed in 1 normal tissue but not in the cancer tissue.(2)Results of indexing:1 is homologous to TBX3 (T box gene),autoploidy 99%.1 is homologous to cl.Ca4 gene (member 4 of Calcium activated chloride channel gene family), autoploidy 99%.1 is homologous to Cu,Zn SOD gene (Cu,Zn superoxide dismutase),99%.The remaining fragments are not homologous to unknown sequences in the public database of genbank,assigned as YHHQ1,XHL,LHX. Conclusions (1)It is verified at genetic level the relation between the conment's increasing of Cu,Zn SOD in bladder cancer derived tissue and the occurrence,advance of bladder cancer.(2)The relation between TBX3,cl.Ca4 gene and human carcinoma has been observed.(3)YHHQ1 may be the new candidate of anti oncogene related to bladder cancer.XHL and LHX may be the new candidates of oncogene related to bladder cancer.

Objective To investigate the clinical pathological factors of papillary thyroid microcarcinoma (PTMC) with clinically node-negative sides of neck stage central lymph node metastasis.Methods Analysis of 136 cases of cNo stage of PTMC of clinical and pathological data,and using the PCR method for the detection of BRAFV600E mutation in the paraffin coated tissue.Results One hundred and thirty-six cases with PTMC lymph node metastasis rate was 38.2％ (52/136),BRAFV600E mutation rate was 44.9％ (61/136).Single factor analysis showed that the lymph node metastasis was related with BRAFV600E mutation and capsular invasion (P＜ 0.05).The size of the tumor was close to significant level (P=0.057).Multiple regression analysis showed that BRAFV600E mutation and capsular invasion were independent factors for affecting the central lymph node metastasis (P ＜ 0.05).Conclusion The routine central lymph node dissection should be performed in patients with BRAFV600E mutation and tumor invasion.

Objective To study the characteristic of in-vivo nasal mucosa absorption of tetramethylpyrazine phosphate(TMPP) liposome in rats. Methods Nasal circulatory perfusion method in rats was used to study the effects of different circulatory rates and TMPP concentrations on nasal mucosa absorption of TMPP liposome. Nasal mucosa absorption rate constants of TMPP liposomes and TMPP solutions at the same concentration were also compared. Results The absorption rate constant of TMPP liposomes was increased with the increase of circulatory rates at the range of 1.2～2.4 mL/min and with the increase of TMPP concentrations at the range of 0.245～0.98 mg/mL. Both the absorption rate constant and absorbed amount of TMPP for TMPP liposomes were higher than those for TMPP solutions at the same concentration. Conclusion The nasal mucosa absorption of TMPP liposome was complete as compared with that of TMPP solution.
