1.Clinical study of bone imaging in 117 cases with primary hyperparathyroidism

Objective To analyze the characteristics of bone scintigraphy in 117 cases with primary hyperparathyroidism (PHPT).Methods Of these 117 cases (50 males and 67 females),there were 116 parathyroid adenomas and 1 parathyroid cancer.Mean age was 61.1(12-86) years old.All had ~(99)Tc~m-methylene diphosphonate (MDP) bone scintigraphy.The bone images could be classified into 4 categories.Category Ⅰ:normal;category Ⅱ:localized abnormal,which could be subcategorized as Ⅱ A with skull and mandible involvement,and Ⅱ B with Ⅱ A characteristics plus metabolic derangement;category Ⅲ:systemic,whole-body incmased tracer uptake;category Ⅳ:systemic plus localized metabolic derangement.Data were analyzed statistically with X~2 and t-test (isolated samples).Results According to the scintigraphic findings,there were 47 cases(40.17%)of category Ⅰ,35 cases(29.91%) category Ⅱ (21/35cases Ⅱ A and 14/35 cases Ⅱ B),30 cases (25.64%) category Ⅲ,and 5 cases (4.27%) category Ⅳ.Combining categories Ⅱ、Ⅲ and Ⅳ together,there were 70 abnormal cases.These patients had history of abnormal bone images such as bone fracture (39 cases,55.71%),calculus (8 cases,11.43%),bone fracture plus calculus(7 cases,10.00%),osteoporosis (51 cases,72.86%) or ostalgia(26 cases,37.14%);however,in the 47 cases of category Ⅰ,only 1 (2.13%),0,0,10(21.28%)and 10 cases (21.28%),respectively,were found.Therefore.these case history characteristics were statistically significant (X~2=11.152,P=0.01).The tumor size,parathyroid hormone (PTH),blood calcium,blood phosphorus in the patients of abnormal PHPT categories Ⅱ to Ⅲ were(14.52±13.72)cm~3,(731.67±618.40)ng/L,(3.05±0.29) mmol/L and (0.71±0.14) mmol/L,respectively.with statistically significant difference compared to category Ⅰ:(0.78±1.33) cm~3,(112.04±62.98)ng/L,(2.56±0.42) mmol/L and (1.03±0.36)mmol/L(t=-5.724,-5.741,-7.274 and -6.451;all P<0.01).Conclusions (1)Bone scintigraphy was normal in 40% of PHPT patients.(2)The bone images of PHPT could be classified into 4 categories and each could reflect the duration and severity of the disease status on bone.(3)The bone imaging characteristic could be useful for differential diagnostic purposes.
2.Three-column plate internal fixation for the treatment of complex tibial plateau fracture through antero-midline and postero-medial approaches.

OBJECTIVETo study technique and clinical therapeutic effects of internal fixation with three-column plates for the treatment of complex tibial plateau fractures through antero-midline and postero-medial approaches.
METHODSFrom January 2010 to December 2012, 28 patients with complex tibial plateau fractures were treated with internal fixation using three-column plates through antero-midline and postero-medial approaches. There were 17 males and 11 females, with an average age of 45.3 years old (ranged, 28 to 64 years old). Twelve patients had injuries in the left side and 16 patients had injuries in the right side. According to Schatzker classification, 12 patients were type V, 16 patients were type VI. According to three-column classification, all the patients had injuries of lateral, medial and posterior columns. The mean interval from injury to operation was 9.4 days (ranged, 6 to 15 days). The main clinical symptoms were knee joint swelling, deformity and limitation of motion before operation. The X-ray and CT showed all patients had complex tibial plateau fractures, which involved in the lateral, medial and posterior columns. The therapeutic effects were evaluated by fracture healing time, hospital for special surgery knee score (HSS) at one year after operation. The indexes such as tibial plateau-tibial shaft angle (TPA), posterior slope angle (PA) and femoral-tibial angle (FfA) were compared between immediate postoperation and one year after operation.
RESULTSAll incisions primarily healed without postoperative complications such as infection and cutaneous necrosis. All the patients were followed up, and the duration ranged from 12 to 24 months, with a mean of 18.1 months. The bone union time ranged from 5 to 10 months (mean, 7.8 months) after operation. Knee joint swelling and pain disappeared after bony union, and joint function completely recovered. The results of hospital for special surgery knee score (HSS) was 27.81 ± 2.17 in pain, 19.52 ± 2.05 in function,15.82 ± 1.73 in passive range of motion, 8.51 ± 1.32 in muscle strength, 8.33 ± 1.08 in flexion deformity, 9.36 ± 0.52 in joint stability, and the total mean score was 89.35 ± 3.19. According to results of HSS, 20 patients got an excellent result, 5 good,2 fair and 1 poor. There were no significant differences in tibial plateau-tibial shaft angle (TPA), posterior slope angle (PA) and femoral-tibial angle (FTA) between immediate postoperation and one year after operation.
CONCLUSIONThree-column plate internal fixation for the treatment of complex tibial plateau fractures through antero-midline and posteromedial approaches is effective to achieve anatomic reduction,rigid internal fixation and early functional exercise.

3.Removal of the template molecules from vinblastine-imprinted polymer.

OBJECTIVETo investigate the optimal condition for complete removal of the template molecules from vinblastine (VLB)-imprinted polymer.
METHODThe prepared polymers were packed into the cartridges of solid-phase extraction column and washed by methanol-glacial acetic acid mixture with different proportions. The contents and recoveries of VLB in the effluents were determined.
RESULTSPolymer extraction with methanol-glacial acetic acid (9:1, V/V) resulted in VLB recovery of 91.73%, but template bleeding was observed because of incomplete VLB removal. Using methanol-glacial acetic acid (6:4, V/V) as the extraction solvent, the recovery of VLB reached 98.03% with less solvents and extract times. The polymers could selectively adsorb VLB through non-covalent interactions and still exhibited strong affinity for the template molecule but not for the structural analogue vincristine after extraction with methanol-glacial acetic acid (6:4, V/V).
CONCLUSIONMethanol-glacial acetic acid (6:4, V/V) is an ideal extract solvent for complete template molecule removal from the polymers, and the processed polymers possess stable capacity of specific recognition and selectivity to the template.

5.Distribution of blood vessels and relationship of angiogenesis with hypoxia-inducible factor-1α and vascular endothelial growth factor in meningiomas

Objective To investigate the distribution of blood vessels and the relationship of angiogenesis with hypoxia-inducible factor-1α (HIF-1α) and vascular endothelial growth factor (VEGF) in meningiomas.Methods Fifteen specimens of meningiomas,collected in our hospital during the resection and conformed by pathology from May 2010 to March 2011,were chosen; 4 different tissues from each specimen were taken,including the tail sign,base center,tumor' s center and tumor' s margin; three successive slices were made from each tissue part.Envision immunohistochemistry was employed to detect the expressions of CD34,VEGF and HIF-1α and their different distributions were compared.Results CD34 mainly disturbed in the cell membrane and cytoplasm,showing yellow granules in the dyeing; VEGF located at the cytoplasm of the tumor cells,while HIF-1α mainly at the nucleus.The expressions of CD34,VEGF and HIF-1α in the tail sign and tumor's margin were significantly higher than those in the MVD in the base center and tumor's center (P<0.05).Positive correlations between expressions of VEGF and MVD,expressions of HIF-1α and MVD,as well as expressions of HIF-1α and VEGF were noted (r=0.960,P=0.040; r=0.964,P=0.036; r=0.998,P=0.002).Conclusion The angiogenesis in the periphery ofmeningiomas is much richer than that in the center; H1F-1α and VEGF may induce and promote the angiogenesis of meningiomas.
6.An investigation on AIDS knowledge of Chinese peacekeepers in Liberia and effects of relevant health education.

OBJECTIVETo investigate and analyze AIDS knowledge of the Chinese peacekeepers in Liberia and to evaluate the effects of AIDS education in order to conduct more effective health education of AIDS and increase AIDS knowledge among the Chinese peacekeepers in the areas with a high incidence of AIDS.
METHODSA questionnaire was designed on the basis of that recommended by WHO to investigate the knowledge of AIDS among the 528 Chinese peacekeepers in Liberia respectively before and after relevant health education, and the results were compared.
RESULTSThere was an obvious deficiency in AIDS knowledge among the subjects before the education and an evident increase after it, and the difference was significant (P <0.01).
CONCLUSIONThe Chinese peacekeepers are deficient in AIDS knowledge, and the deficiency can be effectively improved by relevant health education, which is essential for the health of the Chinese peacekeepers in the areas with a high incidence of AIDS.

7.Investigation of satisfactory rate in patients with body dysmorphic disorder after cosmetic surgery.

OBJECTIVETo explore the satisfactory rate in patients with body dysmorphic disorder (BDD) after cosmetic surgery.
METHODSWe designed a questionnaire to investigate the postoperative satisfactory rate in patients with BDD and without BDD.
RESULTSThere was significant difference in postoperative satisfactory rate between patients with BDD and without BDD.
CONCLUSIONThe patients with BDD have a lower satisfactory rate, which is even worse after repeated surgery.

8.Micro-CT system based on the flat panel detector for small animal imaging.

A high resolution Micro-CT system for small animal imaging is introduced in this paper. Micro-focus X-ray tube with focal diameter of 100 microm and flat panel detector with imaging area of 13cm x 13cm are adopted in this system. The data acquired in rotation scanning is reconstructed with cone beam algorithm. The resolving power of the detector is measured to be 31 lp/cm at 10% of the MTF. The resolution of the Micro-CT system could achieve 185 m when the magnification factor is 1.94. Thighbone of a rabbit is used as sample imaging with the system. The trabecular bone could be imaged clearly. And the ability of small animal imaging of the system has been demonstrated.

9.Role of connective tissue growth factor (CTGF) in proliferation and migration of pancreatic cancer cells.

OBJECTIVETo explore the expression of connective tissue growth factor (CTGF) in pancreatic cancer and its influence on the proliferation and migration of cancer cells.
METHODSThe expression of CTGF in pancreatic cell line PANC-1 cells was analyzed by real-time PCR and in pancreatic carcinoma (50 cases) tissues by immunohistochemistry. The ability of proliferation and migration in vitro of PANC-1 cells was tested by MTT assay, scratch test and Boyden chamber test after the CTGF gene was overexpressed by Ad5-CTGF or silenced with Ad5-siCTGF transfection.
RESULTSCTGF was overexpressed in both pancreatic cancer cells and tissues. Overxpression of CTGF leads to increased proliferation and migration of PANC-1 cells. The CTGF-transfected PANC-1 cells showed apparent stronger proliferation ability and scratch-repair ability than that of empty vector controls. The results of Boyden chamber test showed that there were 34 cells/field (200× magnificantion) of the CTGF-transfected overexpressing cells, much more than the 11 cells/field of the empty vector control cells; and 6 cells/microscopic field of the Ad5-siCTGF-transfected silenced cells, much less than the 15 cells/field of the control cells.
CONCLUSIONSCTGF is overexpressed in both pancreatic cancer cells in vitro and in vivo, indicating that it may play an important role in the cell proliferation and migration in pancreatic cancer.

10.Two-stage revision for treatment of periprosthetic infection following hip arthroplasty.

OBJECTIVETo evaluate the efficacy and optimal re-implantation time of two-stage revision for management of periprosthetic infection following hip arthroplasty.
METHODSWe retrospectively analyzed the clinical data of 15 patients (15 hip joints) undergoing two-stage ipsilateral total hip arthroplasty (THA) revision from January, 2006 to January, 2010. In the first stage, after surgical debridement and thorough removal of all the implants, a self-made Vancomycin-loaded cement spacer was implanted. The second stage operation was performed 3-6 months later for debridement and removal of the antibiotic-loaded spacer, followed by re-implantation of Vancomycin-loaded bone cement prosthesis in 9 cases and cementless prosthesis in 6 cases. The patients were followed up for 9-46 months (mean 25 months) after the operation.
RESULTSNo reinfection or prosthesis loosening/displacement was found in these cases after the operation. The Harris score increased from 40.3 before the operation to 54.0 after the first-stage operation, and to 88.2 at the last follow-up.
CONCLUSIONTwo-stage revision is effective for treatment of periprosthetic infection following hip arthroplasty, and 3-6 months can be the optimal interval between the two the first-stage and second-stage operation for re-implantation.
