1.Thinking of therapeutic mechanism of small knife needle in treating closed myofascitis.

The authors investigated and discussed therapeutic mechanism of small knife needle in treating closed myofascitis on the basis of pathomechanism of modern medicine and acupuncture theory of TCM among numbers of clinical cases and experimental data. Therapeutic mechanism lies in 6 aspects: (1) Relieve the energy crisis of tenderness point on muscular fasciae; (2) Affect nervous system and reduce induction of harmful stimulating signal; (3) Inhibit aseptic inflammatory reaction on muscular fasciae; (4) Regulate dynamic equilibrium of soft tissue by cutting scar and releasing the conglutination; (5) Increase patients' regional threshold of feeling; (6) Reduce tension and pressure of soft tissue of tenderness point so as to relieve extrusion of nervus cutaneous.

2.The Relationship between Orexin A and Respiratory Drive in Patients with Chronic Obstructive Pulmonary Disease

Obejective To explore the correlation of Orexin A and respiratory drive in chronic obstructive pulmo-nary disease (COPD) patients. Methods Pulmonary function was tested in 30 stable COPD patients and 20 normal healthy adults. Plasma orexin A level was measured with a radioimmunoassay kit. The correlation of Orexin A with BMI, age, BDI, FEV1, FVC, FEV1/FVC, MEP, MIP, P0.1 in COPD patients was analyzed. Results Plasma Orexin A levels in patients with COPD group(1.87±0.43)ng/L was higher than those in the control group(1.49±0.19)ng/L, P<0.01. Plasma Orexin A lev-els in patients with COPD correlated negatively with FEV1(r=-0.389,P < 0.05),and correlated positively with P0.1(r=0.728,P<0.01). Conclusion Plasma orexin A in COPD patients increased which may be caused by smoking and hyper-capnia. Orexin A may play a crucial role in regulating respiratory drive in COPD patients.
5.Efficacy of stereotactic radiotherapy combined with chemotherapy for local recurrent non-small-cell lung cancer after radiotherapy

Objective:To study the efficacy of stereotactic radiotherapy (SRT) combined with gemcitabine plus cisplatin (GP)-based chemotherapy on local recurrent non-small-cell lung cancer (NSCLC) after radiotherapy. Methods:Thirty-eight patients with local recurrent NSCLC were treated with SRT(3.00-4.85 Gy per fraction, five times every week, totally 8-12 fractions, total course of 2-3 weeks). Before SRT the patients were given two cycles of GP-based chemotherapy (gemcitabine 1 000 mg/m~2 d 1 and d 8;cisplatin 30 mg/m~2 d 1-3) and at the end of SRT they were given 4 cycles of GP-based chemotherapy. Results:Of the 38 patients,7 patients had complete response, and 23 patients achieved partial response. The overall immediate response rate was 78.95% (30/38). The 6-month and 12-month total survival rates were 71.05% and 10.53%,respectively. The median survival time was 7.8 months. Conclusion:SRT combined with GP-based chemotherapy had better short-term efficacy and the patients were tolerable. No severe short-term radiation-induced injury was observed. The long-term radiation-induced injury and long-term efficacy need further investigation.
6.Expression of Serum Anti PLA2 R Antibodies in Idiopathic Membranous Nephropathy

Objective To analyze the expression of serum anti M phospholipase A2 receptor (PLA2R)antibody in idiopathic membranous nephropathy (IMN),and to investigate its value in the diagnosis and evaluation of idiopathic membranous ne-phropathy.Methods One hundred and eighteen patients with biopsy-proved glomerular diseases were involved in this study, including 97 cases with IMN,21 cases with IgA nephropathy (IgAN)and 19 healthy people.ELISA was used to detect ser-um anti-PLA2R antibodies.Correlations of anti-PLA2R antibody level with laboratory parameters,including serum albumin, 24-hour urine protein of IMN patients were evaluated.Results The median of anti PLA2R antibody in IMN group,IgAN group and healthy group was 45.2(3.6~705.9)RU/ml,5.9(2.3~10.6)RU/ml and 1.2(0.6~9.3)RU/ml.The levels of serum anti PLA2R antibody in IMN group were higher than those in IgA nephropathy group and healthy control group.The difference was statistically significant (t=-5.027,-3.077;P=0.05).Among 97 cases with IMN,76 cases showed posi-tive anti-PLA2R antibodies (positive rate 78.35%).There was none patient showed positive anti-PLA2R antibody respec-tively in IgAN and healthy people.Furthermore,anti-PLA2R antibody level was negatively correlated with serum albumin (r=-0.453,P=0.000)and positively correlated with CREA,TC,ESR,24 hour urine protein (r=0.233,0.234,0.363, 0.586;P=0.004,0.217,0.021,0.000)in IMN patients.Conclusion Serum anti PLA2R antibody may be used as a IMN specific marker for the diagnosis of IMN,and it has important reference value for evaluating the severity of IMN.
7.Construction and expression of prokaryotic expression vector for Rana catesbeiana ribonuclease gene

Objective:To clone the RC-RNase gene and prepare its recombinant prokaryotic construct, and then to express RC-RNase protein using Escherichia coli system. Methods: RC-RNase cDNA was obtained by RT-PCR from liver of Rana catesbeiana, and cloned into pUCm-T plasmid for nucleotide sequencing. Its expression construct was prepared using the 6?His vector pRSET-A, and induced to express by IPTG in Escherichia coli BL21(DE3). Western blotting identified the expression product. Results: A 380 bp long cDNA was obtained from liver of Rana catesbeiana, restriction sites and sequence being consistent to those reported for RC-RNase. After introducing the gene into Escherichia coli and through the induction by IPTG, it was observed a new peptide at the expected position (Mr 16000) on SDS-PAGE gel. This product was proved to be the target protein via Western blotting. It existed in a form of inclusion body and its efficiency reached 12.5% of total bacterial proteins. Conclusion: RC-RNase gene was cloned and expressed in Escherichia coli. The protein could be used for characterizing the biological activities and function of RC-RNase.
8.Nosocomial Escherichia coli Isolates:Their Extended Spectrum ?-Lactamases,AmpC Enzyme and Resistance Analysis 2005-2007

OBJECTIVE To analyze the situation of extended spectrum ?-lactamases(ESBLs)and AmpC enzyme produced by nosocomial Escherichia coli isolates in 2005-2007.METHODS ESBLs were detected by double disk synergy test and disk diffusion confirmatory test.AmpC enzyme was detected by the three dimensional assay.Chi square test was used to test the significance.The application of different kinds of antimicrobials before the results of etiology be presented and the resistence rate of the ESBLs both producing and no producing were compared respectively.RESULTS The detectable rate of ESBLs in E.coli isolates of nosocomial and community infection was 55.1% and 21.3% and the detectable rate of AmpC enzyme nosocomial E.coli isolates was 17.4%.All strains were 100% susceptible to meropenem and imipenem but resistant to 15 other antimicrobials in different degree.The sensitivity to Piperacillin/tazobactam,cefoperazone/sulbactam and amikacin were relatively high.CONCLUSIONS The carrying rate of ESBLs from nosocomial E.coli isolates is high and AmpC enzyme and other resistance genes,which lead to multiple drug resistance.Standardized management of antimicrobials application should be strengthened and the consciousness of rational antimicrobials utilization should be raised.
9.Investigation of Somatic Diseases of Inpatients with Psychosis and Hospitlized a Long-term

ObjectiveTo investigate the diagnoses of somatic diseases of inpatients with psychosis and hospitalized more then 2 years.MethodsThe material of all 350 inpatients were investigated and analyzed.ResultsThe first three psychiatric diagnoses were schizophrenia (61.32%), Alzheimer's disease (15.26%), mood disorder (4.74%). The first three diagnoses of accompanying somatic diseases were heart disease (8.68%), diabetes mellitus (8.42%), and hypertension (4.47%).ConclusionWith the changes of social environment, the disease pedigree of inpatients with psychosis changes, senile patients become more and more, and the incidence of concurrent somatic diseases gradually increases.
10.Intragastric and subcutaneous administration of yolk antibodies against adipocyte plasma membrane proteins on growth and fat deposition in rats.

OBJECTIVETo study the effects of different administrations of antibodies against adipocyte plasma membrane proteins on growth and fat deposition in rats.
METHODNinety six female SD rats that weighed approximately 140 g were allotted randomly into four groups which were given negitive control yolk and positive yolk containing antibody (IgY) against adipocyte plasma membrane (APM) proteins by intragastric administration (i.g.) and subcutaneous injection (s.c.) respectively. Rats were given 1 ml of yolk for every three days in i.g. groups. Rats were given 1 ml of yolk for 4 consecutive days, and the procedure was repeated after one month. The trial lasted for 75 days after which rats were slaughtered for carcass analysis and sampling.
RESULTSThe body weight gain and food intake of rats were not different between treatment and control groups. In i.g. goups, positive yolk decreased mesemteric fat index, paramertrial fat index and perirenal fat index (P < 0.05), reduced serum triglycerides (P < 0.05) and increased serum free fatty acids (FFA) (P < 0.01), and also decreased serum leptin, insulin and TNF-alpha levels ( P < 0.01 or P < 0.05), but did not affect gastrocnemius muscle index and serum cholesterol. In s.c. groups, positive yolk increased gastrocnemius muscle index (P < 0.05), reduced serum triglycerides and serum leptin (P < 0.01), increased serum TNF-alpha (P < 0.05), but did not affect adipose tissue depots, serum FFA, cholesterol and insulin.
CONCLUSIONAdministration of yolk antibody against APM proteins could effectively improve body composition of rats, and the treatment by intragastric administration could give better effect than by subcutaneous injection.
