Objective To investigate the clinical efficacy of atorvastafin intensive treatment before percutaneous coronary intervention (PCI) in patients with acute coronary syndrome (ACS) and its affects on serum interleukin (IL)-6 and IL-8.Methods Sixty ACS patients were divided into observation group and control group according to different treatment with 30 cases each.Control group received conventional therapy,while observation group was added with atorvastafin intensive treatment before PCI on the basis of conventional therapy.The serum IL-6 and IL-8 levels were detected before PCI at 24 h,3 d and 6 months after surgery,and the occurrence of cardiac adverse events were observed within 30 days after surgery.Results The serum IL-6 and IL-8 levels between observation group and control group had no statistical differences before surgery (P＞ 0.05).The serum IL-6 and IL-8 levels of observation group were significantly lower than those of control group at 24 h,3 d and 6 months after surgery,respeclively [ IL-6:( 11.56 ± 4.95 )ng/L vs.(20.24 ± 8.67) ng/L,(8.51 ± 3.65) ng/L vs.( 18.83 ± 8.07 ) ng/L,(5.60 ± 2.40) ng/L vs.( 10.02 ±4.29) ng/L;IL-8:(50.34 ± 21.57) ng/L vs.(74.50 ± 31.93) ng/L,(30.80 ± 13.20) ng/L vs.(53.30 ± 22.84)ng/L,( 26.00 ± 11.14 ) ng/L vs.( 40.61 ± 17.40 ) ng/L ] ( P ＜ 0.05 ).There was no cardiac adverse event in observation group within 30 days after surgery.There was 1 case who occurred angina pectoris at 16 days after PCI in control group,and the syrnptoms disappeared after emergency treatment.Conclusions The atorvastatin intensive treatment before PCI can effectively decrease the levels of IL-6 and IL-8 in ACS patients and has the effects of anti-inflammatory and protecting myocardium from damage.

Objective To explore the correlations between measurements of Doppler velocity waveform parameters of fetal main pulmonary artery and gestation age (GA) in normal fetus. Methods This is a cross-sectional study, in which totally 252 normal singleton fetuses between 23 and 40 weeks of gestation were divided into 10 groups. The following velocity waveform parameters were measured in the main trunk of fetal pulmonary artery by Doppler ultrasound: acceleration time(AT), ejection time(ET), acceleration time/ejection time(AT/ET), peak systolic velocity(PSV), end diastolic velocity(EDV), mean velocity(MV), pulsatility index(PI) and resistance index(RI). The normal reference values of Doppler velocity waveform parameters at various GA were established. A scatter diagram of each parameter against GA was plotted out. The optimal formula was selected out according to the correlation and regression analysis. Results Among 252 normal fetuses, satisfactory Doppler velocity waveform of fetal main pulmonary were achieved in 249 (98.8%, 249/252). Among these parameters, AT, AT/ET, PSV, EDV and MV increased with increasing GA(F=343.23, 96.012, 8.649, 2.158, 10.169, all P<0.05), while PI decreased slightly(F=2.346, P<0.05) and ET, RI had no signiifcant changing(F=0.883, 1.534, both P>0.05). Signiifcant and positive linear correlations were found between parameters of AT, AT/ET, PSV, EDV, MV and GA(r= 0.898, 0.868, 0.446, 0.203, 0.507, all P<0.01);while PI was inversely linear correlated with GA(r=-0.184, P<0.01) and ET, RI had no correlation with GA(r=-0.103, 0.011, both P>0.05). Conclusions The fetal main pulmonary artery could be a reliable sampling point to obtain pulmonary velocity waveform by Doppler ultrasound. Doppler velocity waveform parameters have correlations with GA in normal fetus;among which AT and AT/ET were the best.

Objective The study aimed to observe the clinical effects and the adverse reactions of recombinant mutant human tumor necrosis factor-alpha ( rhu-TNF) on the treatment of malignant pleural effusion ( MPE) induced by lung adenocarcinoma . Methods 70 patients with MPE caused by lung adenocarcinoma hospitalized in our department were chosen as the research objects . After conventional drainage of pleural effusion , the patients were divided into two groups according to the weight , respectively receiving intra pleural injection of 2 000 000 units and 3 000 000 units of rhu-TNF, followed by the observation of clinical effects and adverse reac-tions.A further retrospective analysis were made on the effects of dexamethasone injected before operation on clinical effects and ad -verse events. Results The effective rates of 2 000 000 unit group and 3 000 000 unit group were respectively 75.68%and 87.88%(P=0.23), with no statistical difference.The adverse reactions in the group of patients being injected dexamethasone before operation significantly reduced(P=0.021) and the use of dexamethasone had no influence on the efficacy of rhu-TNF ( P=0 .486 ) . Conclusion Rhu-TNF is a safe drug with high efficiency in the treatment of MPE induced by lung adenocarcinoma , and the efficacy and safety of re-peated application in clinic still need more supporting data .

Objective To evaluate the value of digital subtraction computed tomography angiography(DSCTA)in the diagnosis of intracranial small aneurysms.Methods 92 patients with intracranial aneurysms confirmed by digital subtraction angiography(DSA) or surgery were selected as the subjects of this study.Their clinical and CT imaging data were analyzed retrospectively.The diagnos-tic accuracy of DSCTA and conventional CTA for intracranial aneurysms was compared.Results A total of 102 aneurysms were con-firmed in the 92 patients,100 of them were detected by DSCTA and 89 by conventional CTA (χ2 =8.707,P =0.003).Two small aneurysms of less than 5.0 mm located respectively at the cavernous sinus and beneath the clinoid segments were missed by DSCTA, while 13 by conventional CTA.When aneurysms of less than 3.0 mm and 3.0-5.0 mm were combined with calculation,the sensi-tivity of DSCTA in detecting aneurysms of less than 5.0 mm was significantly higher than that of conventional CTA (χ2 =8.393,P =0.004).Conclusion DSCTA has more advantages than conventional CTA in the diagnosis of intracranial aneurysms,especially for the small aneurysms adjacent to the skull base.DSCTA can be used as a preferred technique in the screening and diagnosis of in-tracranial aneurysms.

Objective To analyze the MRI manifestations of meningeal carcinomatosis, and to investigate the value of MRI in diagnosis of meningeal carcinomatosis.Methods The MRI data of 28 cases with proven meningeal carcinomatosis were reviewed retrospectively.Results 11, 8 and 25 cases were detected respectively on pre-contrast T 2WI and T 1WI of SE sequence , and post-contrast T 1WI.According to the manifestations of post-contrast MRI, meningeal carcinomatosis were divided into two types: dura-arachnoid enhancement(7 cases) and pia-subarachnoid space enhancement(18 cases) in accordance with the standard of Meltzer.Conclusion MRI, especially the post-contrast MRI is a reliable modality in diagnosis of meningeal carcinomatosis. Double dosage of contrast medium and thin slice thickness can provide more informations.
