1.Ongoing improvement of medical services for large-scale activities as guided by patient satisfaction

A satisfaction survey carried out by Beijing Chao-Yang Hospital during the Beijing 2008 Olympic Games, found differences of patient satisfaction between locals and overseas patients in consulting environment, outpatient procedure, quality of service, quality of diagnosis and treatment. The patient satisfaction of overseas patients was lower than locals. Especially noteworthy is the low satisfaction of the former for the hospital coloring, privacy in the sector, registration, vocal communication, patient condition statement, nurse attitude and service quality. Real-time improvement in these aspects of medical services has harvested obvious progress. Results of the satisfaction survey provided valuable reference materials for large-scale activities, and ongoing improvement of medical services in the long run.
2.Clinical analysis of otogenic Mouret abscess: a case report.

Mouret abscess is a rare extracranial complication of suppurative otitis media. It is generally believed to be a deep neck abscess caused by inflammation leading to the rupture of the bony tip of the mastoid tip. The location of Mouret abscess is deep. The symptoms are insidious at the onset, but may eventually spread to the surrounding tissue, and even lead to mediastinal abscess, cavernous sinus thrombosis, meningitis, dyspnea and other serious complications. At present, with the popularization of antibiotics, the occurrence rate of Mouret abscess is very low, and only sporadic cases have been reported.In this paper, a case of Mouret abscess caused by cholesteatoma was analyzed to explore Mouret abscess in terms of the route of infection, clinical manifestations, imaging features, diagnosis and treatment.

3.The influence of diabetes and insulin on the fluctuation of serum potassium of maintenance hemodialysis patients and nursing experience

Objective To observe the influence of diabetes mellitus or insulin on fluctuations of serum potassium in patients with maintenance hemodialysis.Methods 52 cases of hemodialysis patients,including 30 non-diabetes patients,11 patients with diabetes but not using insulin,11 diabetes patients using insulin.At the time of 0,24,48 h during dialysis,we collected serum samples from the 52 patients,serum potassium was compared among the three groups.Results On 0,24,48 h,serum potassium level of the diabetes but not using insulin group had no significant difference with the non-diabetes group.While in diabetes using insulin group,serum potassium level was lower compared with the non-diabetes group or diabetes not using insulin group.Conclusions Diabetes itself had no effect on serum potassium fluctuations of maintenance hemodialysis (MHD) patients,however,insulin can reduce the pre-dialysis serum potassium levels of MHD patients.
4.Development and Application of Clinical Pharmacy Information Management System Software

OBJECTIVE: To develop a set of consummated comprehensive application platform that meets the actual demand so as to promote hospital treatment level and pharmaceutical care quality.METHODS: The protocol and standard meeting the international standard was adopted for system design.The currently popular combination tools(Apache+PHP+MySQL) set was developed and an open information resources management system and multi-structured architecture were established.RESULTS & CONCLUSIONS: This system is practical,advanced and safe and it is composed of 5 modules: drug inquiry system,rational drug use system,management on adverse drug reactions,pharmacy administrative management and network management.The system can not only guarantee the compatibility and the expandability of system,but also meet the needs of the development of hospital pharmacy and effectively enhance the rational drug use.
5.Autophagy of SO-Rb50 cells induced by arsenic trioxide

Background Cellular autophagy is a non-apoptosis death form of tumor tissue.Research determined that arsenie trioxide (As2O3) leads to apoptosis of tumor cells.But whether As2O3 induce autophagy of SO-Rb50 cells or not is unclear.Objective This study was to assess the effects of As2O3 on autophagy of SO-Rb50 cells.Methods As2O3 with the concentration of 0,0.5,1.0,2.0,4.0 μmol/L was used to treat the SO-Rb50 cell line for 48 hours,and the growth and proliferation of SO-Rb50 cells were detected using MTT assay (A570).pGFP-LC3,a marker of autophagy,was constructed to transfer SO-Rb50 cells,and the cells were then divided into RPMI-1640 culture group (untreated group),As2O3 + RPMI-1640 culture group (As2O3 treated group) and rapamycin culture group (positive control group).Autophagy of SO-Rb50 cells was examined by laser confocal microscope and monodansylcadaverine (MDC) influorescence staining,respectively,48 hours following cell culture.Ultrastructural features of autophagy were examined with transmission electron microscope (TEM).The percentage of autophagy positive cells in different concentrations of As2O3 treated groups was calculated with flow cytometer.Results The A570 values of SO-Rb50 cells were 2.194±0.066,1.841 ±0.213,1.035±0.046,0.374±0.042 and 0.167±0.019 in 0,0.5,1.0,2.0,4.0 μmol/L As2O3 treated groups,with a significant difference among these 5 groups(F=547.636,P<0.05),and those of 0.5,1.0,2.0,4.0 μmol/L As2O3 treated groups were significantly reduced in comparison with untreated group (P =0.000).The positive granular spots for GFP-LC3 chimeric protein were seen to aggregate in autophagic vacuoles in the As2O3 treated group and positive control group,but diffuse cytoplasmic signal for GFP-LC3 was found in the untreated group.Normal ultrastructure of SO-Rb50 cells was exhibited in the untreated group,and many double-membrane-like bound vesicles and autlysosomes were documented in the As2O3 treated group and positive control group under the TEM.A lots of MDC fluorescence granule were found in the As2O3 treated group and positive control group rather than the untreated group.Flow cytometry showed that the percentages of SO-Rb50 cells were 0,15.6%,42.7%,57.9%,79.5% and 89.0% in the 0,0.5,1.0,2.0,4.0 μmol/L As2O3 groups and positive control group,respectively,showing a As2O3 concentration-dependent increase.Conclusions As2O3 can induce the autophagy of SO-Rb50 cells and inhibit the proliferation of SO-Rb50 cells.Autophagic response of SO-Rb50 cells appears prior to the nuclear change after exposed to As2O3.The degree of autophagy of SO-Rb50 cells is associated with As2O3 dose.
6.A clinical and pathologic study of meibomian gland carcinoma with reduplicative operations

BackgroundThe meibomian gland carcinoma is an eyelid malignant tumor with a domestic incidence after basal cell carcinoma.Meibomian gland carcinoma is not sensitive to radiation therapy and chemotherapy,and the related factors with its recurrence and metastasis are rarely reported.ObjectiveThis study was to investigate the clinical and pathologic features of meibomian gland carcinoma with multiple operations and the effectiveness of histologically controlled excision.MethodsThe clinical data and the histopathologic sections of 34 cases of the meibomian gland carcinoma diagnosed by pathology were retrospectively analyzed at Zhongshan Ophthalmic Center in September 2003 to April 2011,and the treating effectiveness of histologically controlled excision was evaluated. ResultsIn this group of cases,the appearing rate of the meibomian gland carcinoma was resemble in both lateral eyes.A higher morbidity was on the upper eyelid (26/34,76.5%) and then the lower eyelids (5/34,14.7% ) and both (3/34,8.8%).The average ages of these cases were 57.5 years old.Sixteen of 21 misdiagnosed cases were identified as chalazion at the first visit,and no histopathological examination was performed in 11 cases after initial operation.Twenty-six cases(76.5% )were identified as meibomian gland carcinoma in initial histopathologic diagnosis.Two cases had histologically controlled excision and 16 cases had simple excision while 16 cases had chalazion enucleation in the first operation.All the patients had histologically controlled excision in Zhongshan Ophthalmic Center with 58.8% of the patients having pagetoid invasion.Sixteen cases were followed up for 5 months to 8 years after histologically controlled excision,in which none died of recurrence and metastasis of meibomian gland carcinoma.No significant differences were found in the pathological feature between 16 lost patients and 18 followed-up patients(P > 0.05 ).Conclusions Misdiagnosis of meibomian gland as chalazion is a main cause of repeat operations of meibomian gland carcinoma.Histologically controlled excision is a feasible therapy for the recurrence and metastasis of meibomian gland carcinoma.
7.Remediation of Cr(VI) in solution using vitamin C.

The effectiveness of vitamin C in treating Cr(VI)-contaminated water is being evaluated. Cr(VI) is an identified pollutant of some soils and groundwater. Vitamin C, an important biological reductant in humans and animals, can be used to transform Cr(VI) to essentially nontoxic Cr(III). The removal efficiency was 89% when the mass concentration of vitamin C was 80 mg/L in 60 min, and nearly 100% Cr(VI) was removed when the mass concentration was 100 mg/L. Our data demonstrated that the removal efficiency was affected by vitamin C concentration, the reaction temperature and the dissolved oxygen concentration. The reaction mechanism of Cr(VI) by vitamin C was presented. Our study opens the way to use vitamin C to remediate Cr(VI)-contaminated soils and groundwater.

9.Evaluation on tolerability and safety of long-term administration with shexiang baoxin pill in patients with coronary heart disease of stable angina pectoris.

OBJECTIVETo evaluate the safety and tolerability of long-term (6 months) administration with Shexiang Baoxin Pill (SBP) in patients with coronary heart disease (CHD) of stable angina pectoris.
METHODSAdopting randomized non-blinded and parallel controlled trial, 200 patients with CHD were randomly and equally assigned to the SBP group and the control group. Both received basic therapy for CHD, including anti-platelet, lipid regulating and anti-ischemia with additional SBP 2 pills, taken orally three times per day in the SBP group. They were followed up for 6 months. The drug tolerability and adverse drug reactions occurred in the observation period were recorded, and the laboratory indexes involving blood routine, liver function, renal function, blood glucose and blood lipids were detected before and after treatment.
RESULTSThe trial was completed in 92% of the patients, 5 patients withdrew in the SBP group and 11 patients in the control group; but none for the intolerable therapy. There were 1 case of adverse reaction related to SBP. No obvious change was found in blood glucose and blood lipids in the two groups before and after treatment. No serious adverse reaction and injury of liver and renal function or others happened.
CONCLUSIONLong-term administration of SBP has favorable clinical tolerability and safety for CHD patients.
