1.The establishment of hemodialysis access with long-term venous indwelling catheter under DSA guidance

Objective To discuss the management, complication and effectiveness of establishing a prolonged hemodialysis access with a long-term venous indwelling catheter. Methods During the penod of July 2007-February 2009, a total of 64 long-term venous indwelling catheters for hemodialysis were placed in 62 patients with terminal chronic renal insufficiency. Results Of the total 64 catheter-placements, 62 were pereformed under DSA guidance and 2 at bed-side. Because of blocked flow, adjustment of the catheter was carried out in 4 cases, replacement of catheter in 2 cases and thrombolytic therapy in one case. In 62 patients, the arterial flow rate after the procedure reached to 230-300ml/min. In 57 cases the indwelling catheter remained in working condition so far. Five cases died within 6 months. Conclusion For establishing a prolonged hemodialysis access. The placement of a long-term venous indwelling catheter is feasible. The main complication is the blocked flow. Which is mainly related to the disposition of the catheter itself or its tip. The right intemal jugular vein is the indwelling site of choice, the tip of double-lumen catheter should be within the superior vein cava. With the help of DSA guidance, the procedure can be performed accurately, safely and easily.
3.Relationship of serum cystatin C level with cytokines and carotid atherosclerosis in mintenance hemodialysis patients

Objective To investigate the serum cystatin C (CysC) level and explore its relationship with cytokines and atherosclerosis (AS) in maintenance hemodialysis (MHD) patients.Methods A total of 110 stable MHD patients undergoing hemodialysis for at least six months and 60 healthy control people were enrolled in the study.Serum levels of CysC and high-sensitivity Creactive protein (hsCRP) were measured by immunoturbidimetry.The serum levels of total homocysteine (tHcy),IL-1β,IL-6 and TNF-α were determined by ELISA.Prevalence of atherosclerosis was detected by carotid ultrasonography.The relationship of CysC level and cardiac geometry incidence in MHD patients was analyzed by Logistic regression model.Results The serum CysC level was significantly higher in MHD patients as compared with healthy controls [(6.19±0.95) mg/L vs (0.76±0.21) mg/L,P<0.01],and the serum levels of hsCRP,tHcy,IL-1β,IL-6,TNF-α were significantly higher in MHD patients than those in healthy control group (P<0.05 or P<0.01).The serum CysC level was higher in MHD patients with carotid artery atherosclerosis compared to patients without carotid artery atherosclerosis (P<0.05).CysC was positively correlated with hsCRP,tHcy,IL-1β,IL-6,TNF-α respectively (P<0.05 or P<O.01),and was positively correlated with carotid intimal medial thickness (IMT) and AS.Besides,a negative correlation was found between the serum CysC level and the serum albumin level (P<0.05),while CysC was positively correlated with dialysis duration,systolic pressure and iPTH (P <0.05).Conclusion Serum CysC level is significantly higher in MHD patients and is correlated with hsCRP,tHcy,IL-1β,IL-6,TNF-α as well as carotid artery atherosclerosis,which indicates that CysC is an independent risk factor of AS in MHD patients.
4.Implementation and evaluation of Sino-French cooperation on medical education

Objective To explore the culturing strategies,curiculum provision,courses conferring methods,teaching effects as well as the associated managerial evaluations on the basis of Sino-French cooperation on medical education with the hope of summarizing helpful suggestions to Sino-Foreign cooperation on medical education. Methods The achievements of our Sino-French cooperation on medical education were analyzed and compared in the teaching models,culturing strategies along with courses conferring processes among seven-year medical students from both English-teaching and French-teaching classes. Results Our Sino-French cooperation on medical education was featured in its distinct culturing purposes and effective teaching model.Its scientifically formulated culturing strategy found its full expression in French-teaching atmosphere.The Sino-French cooperation on medical education was consistently welcomed and favorably recommended by both faculties and students. Conclusion The Sino-French cooperation on medical education has not only gained precious experience in culturing the cutting-edge medical talents with the international visions but also conduced to fulfill the goal to establish a modernized and internationalized medical school.
5.Diversity changes of TCRVbeta gene in AIDS patients with incomplete immune reconstitution and influence of drug.

OBJECTIVETo discuss the drug intervention in diversity changes of TCRVbeta gene in AIDS patients with incomplete immune reconstitution.
METHODPBMCs were isolated from 37 cases of AIDS patients failure to immune reconstitution before and after treatment with Immune 2 and 15 cases of HIV negative healthy donors. The human gene TCRVbeta CDR3 diversity quantitative detection reagent box were used, and mapped the distribution of gene scanning and calculated different CDR3 fragme of each Vbeta family size.
RESULT(1) Gaussian distribution of TCRVbeta families in patients with incomplete immune reconstitution after one year of HAART, had been broken with the occurrence of the offset TCR lineage. After six months of treatment of traditional Chinese medicine combined HAART, the TCR lineage has been partially restored. (2) Evaluated by the D (distance) value calculated by a quantitative analysis software which the kit provides, there were no significant difference in D value change between the two groups, but with traditional Chinese medicine can reduce the data variability. (3) CD4+ T cell counts had a significant correlation (r = -0.772, P = 0.000) with TCRVbeta genetic diversity.
CONCLUSIONStudy of the mechanism showed oligoclonal of TCRVbeta family can get recovery in some degrees after treated by Immune 2 plus HAART, suggesting that the medicine may promote T-cell receptor gene rearrangement, helping immune cells to effectively identify the virus to reduce T-cell apoptosis.

6.The influence of acupuncture on neural stem cell proliferation and differentiation in the hippocampus of the senescence-accelerated mouse prone-8

Objective To investigate the influence of acupuncture therapy on neural stem cell (NCS) proliferation and differentiation in the hippocampus of the senescence-accelerated prone-8 (SAP8) mouse. Methods Twenty-four SAP8 mice were randomly and equally divided into a model group and an acupuncture group. Twelve senescence-accelerated resistant ( SAR1 ) mice served as the control group. Acupuncture was administered at the Baihui (DU20) point to mice in the acupuncture group once daily for 21 consecutive days. Bromodeoxyuridine (BrdU) was used to detect the proliferation and differentiation of NCSs in the hippocampus through double-labeled immunofluoresSAR1 group, the positive cells in the SAP8 group were significantly fewer Compared to the SAP8 group, positive cells markers of neuron or glia cell positive cells appeared. Compared to the SAR1 group, the expression of BrdU/GFAP increased in the SAP8 group, and decreased after acupuncture but not significantly. BrdU/S-100 β cells decreased significantly in the SAP8 group, and increased after acupuncture, but again not significantly. Compared to the SAR1 group BrdU/GalC positive cells increased significantly in the SAP8 group and decreased significantly after acupuncture.Conclusions After acupuncture treatment for 21 days, the differentiation of hippocampal NSCs into oligodendrocytes was inhibited, but there was little effect on their differentiating into neurons and immature astrocytes.
7.Effects of electroacupuncture on IL-2-IFN-NKC immunity immunoloregulation net and IL-2 receptor in rats with exercise stress.

OBJECTIVETo explore the effects of electroacupuncture on exercise-induced immunosuppression in rats and its mechanism.
METHODSSports immunosuppressive model was established successfully by the rats were conducted high intensity swimming training 150 min/day, 6 days/wk for 8 weeks in this study. Forty-three SD rats were randomly divided into a control group (group A, n = 10), a high intensity swimming training group (group B, n = 17), and a high intensity plus electroacupuncture group (group C, n = 16). Group A did not receive any intervention. Group B was conducted 150 min/day, 6 days/wk swimming training for 8 weeks. Group C was treated with electroacupuncture at "Baihui" (GV 20), "Guanyuan" (CV 4) and "Zusanli" (ST 36) after every exercise-time from the second week, once each day for 7 weeks. The changes of the rats' weight, gamma-interferon (gamma-IFN), interleukin-2 (IL-2), solubility IL-2 receptor (SIL-2R) and nature killer cell (NKC) were detected.
RESULTS(1) Compared with group A, gamma-IFN and IL-2 in group B were significantly decreased (P < 0.01, P < 0.05), and NKC in group C was significantly increased (P < 0.01). Meanwhile, gamma-IFN and NKC in group C were both significantly higher than that in group B (P < 0.05, P < 0.01). (2) Compared with group A, the weight of the rats in group B and group C were significantly decreased (both P < 0.01), but SIL-2R in group B was significantly increased (P < 0.05). The weight of the rats in group C was significantly higher than that in group B (P < 0.05) and SIL-2R in group C was significantly lower than that in group B (P < 0.01).
CONCLUSIONLasting gravis exercise stress does decrease the immune function in rats and is even inhibited significantly, but electroacupuncture can up-regulate the exercise-induced immunosuppression.

8.Prokaryotic expression of surface membrane antigen SAG1 gene from Toxoplasma Gondii and the diagnostic value of the recombinant protein

Objective To investigate the diagnostic value of the recombinant surface antigen 1 (rSAG1) in immunodiagnosis of toxoplasmosis. Methods Isopropyl β-D- 1 -thio-galaetopyranoside (IPTG) was used to induce the expression of recombinant plasmid pET28a-SAG1 of Escherich coli(pET28a-SAG1/BL21 ). The expression products (rSAG1) of pET28a-SAG1/BL21 were identified by Western blotting. The serum of mice infected with Toxoplasma gondii tachyzoites, normal mouse serum and the serum from 10 toxoplasma gondii patients were used as primary anti-Toxoplasma gondii antibodies, and the rSAG1 gene products were identified by Western blotting, by which the diagnostic value of rSAG1 in Toxoplasmosis was compared. Results After induction and purification, rSAG1 protein was obtained and its relative molecular mass was 38.5 × 103. The fusion protein could be recognized by the serum of mouse infected with Toxoplasma gondii tachyzoites, rSAG1 of expression products of surface membrane antigen SAG1 gene from Toxoplasma Gondii could be detected in 4 cases from 10 patients by Westem blotting.Conclusion The rSAG1 has a potential value in the immunodiagnosis of Toxoplasmosis.
9.Pathological basis of qi-deficiency and blood stasis syndrome in patients with non-small cell lung cancer viewing from tumor metastasis related factors.

OBJECTIVETo observe the expressions of tumor metastasis related factors (TMRF) in peripheral blood of patients with non-small cell lung cancer (NSCLC) for exploring the molecular mechanism for genesis of qi-deficiency and blood stasis (QDBS) syndrome in patients.
METHODSEighty selected NSCLC patients of stage II B/III were differentiated into the QDBS group and the non-QDBS group according to the Chinese medicine syndrome differentiation criteria, 40 in each group. The serum levels of vascular endothelial growth factor (VEGF), endostatin (ES) and soluble intercellular adhesive molecule-1 (slCAM-1) in patients were detected by ELISA, and the expression of adhesive molecule CD44 in peripheral blood was determined using flow cytometry.
RESULTSSerum levels of VEGF (1002.78 +/- 312.08 ng/L), ES (120.88 +/- 20.00 microg/L), slCAM-1 (531.78 +/- 213.37 microg/L) and CD44 (136.65 +/- 29.60) were significantly higher in patients of QDBS group than in patients of non-QDBS group (653.18 +/- 318.99 ng/L, 98.29 +/- 23.92 microg/L, 409.36 +/- 167.65 microg/L and 98.46 +/- 20.64, respectively, P<0.01).
CONCLUSIONObjective inner links are found between the QDBS syndrome and TMRF in NSCLC patients; serum levels of VEGF, ES, slCAM-1 and CD44 can be served as the microcosmic basis for QDBS syndrome differentiation.

10.A Study of Emergency Measures and Curative Effect of Respiratory Failure Caused by Organophosphorus Pesticide Poisoning

Objective To improve the prognosis among patients with respiratory failure poisoned by organophosphorus pesticide though analyzing and discussing the emergency measures and treatment effects. Methods From January 2014 to December 2016, 86 patients with respiratory failure caused by severe organophosphorus pesticide poisoning were received in the Emergency Department in Nanchong Central Hospital.The patients were numbered according to the order of the treatment and randomly divided into control group and observation group with 43 cases in each group.All patients were treated with routine emergency measures (gastric lavage and endotracheal intubation, rehydration, and symptomatic treatment).The control group was treated with pralidoxime chloride injection and injection of atropine detoxification while the observation group was given pralidoxime chloride combined with Penehyclidine Hydrochloride Injection detoxification.We compared the alleviation of clinical symptoms and the changes of cholinesterase (CHE) and respiratory function (respiratory frequency, Pa02, Pa02/Fi02) between the two groups of patients 10 min and 30 min after administration. The statistics of atropinization time, blood purification treatment rate, tracheotomy rate, hospital mortality,complications and treatment time were recorded. Results No statistic significance was observed among the two groups of patients in gender,age, weight,body mass index (BMI),the severity of poisoning,types of organophosphorus drugs and blood cholinesterase (CHE) at the first visit (P>0.05).Blood CHE was effectively improved among the two groups 10 min and 30 min after the treatment and significantly higher CHE was seen in the observation group compared to the control group (P<0.05) . The overall clinical symptom rate was lower 10min and 30min after the treatment (P< 0.05), and the clinical symptom rate of the observation group was lower than that of the control group (P<0.05) . The respiratory function indexes (respiratory rate, Pa02, Pa02/Fi02) were significantly improved 10 min and 30 min after the treatment in both groups compared with those before the treatment (P< 0.05) .The blood purification treatment rate, tracheotomy rate, complication rate, hospitalization time and atropine time of the observation group were significantly lower than those of the control group (P< 0.05) . No significant difference was found in mortality rate between the two groups (P>0.05).Conclusion After giving effective respiration and circulation support, administration of penehyclidine hydrochloride combined with pralidoxime chloride detoxification can effectively restore the cholinesterase activity among patients with organophosphorus pesticide poisoning and improve respiratory function and prognosis, whichshows a high clinical value.
