1.Experimental study on protective role of propofol in hepatic ischemia-reperfusion injury

ve To assess the effect of propofol on hepatic ischemia-reperfusion (I/R) injury and the mechanism. Methods Sixty healthy rabbits of either sex, weighing 2.0-3.4 kg were randomly divided into three groups of twenty animals each: control group (C), propofol group (P) and etomidate group (E) . The rabbits were anesthetized with 3.0% pentobarbital 1.0 ml?kg-1 iv. Internal jugular artery was cannulated for continuous MAP monitoring and internal jugular vein was cannulated for intravenous fluid and drug administration. In group C normal saline was infused at a rate of 2 ml?kg? h-1; in group E etomidate 0.1 mg ? kg-1? h-1 and in group P propofol 20 mg? kg-1? h-1 was infused during the experiment until the end of surgery. Hepatic ischemia was produced by clamping hepatic hilum for 20 min and reperfusion was allowed for 30 min after release of the clamp before the animals were sacrificed. Blood samples were taken from internal jugular artery before hepatic ischemia immediately, 15 and 30 min after I/ R for determination of AST, ALT and SOD concentration. Liver tissue 0.5g was taken from right lobe and kept in liquid nitrogen for determination of MDA content and left lobe of the liver was taken for electron microscopic examination. Results The serum levels of AST and ALT increased after reperfusion in all three groups, but were significantly lower in group P than in group C and E. The serum SOD level decreased in all three groups after reperfusion, but the decrease in SOD was significantly less in group P than in group C and E. The MDA content of liver increased in all three groups after reperfusion but the increase in MDA was significantly less in group P than in group C and E. Electron microscopic examination revealed that mitochondria swelled obviously, the ridge disappeared, ribosome was disarranged and endoplasmic reticulum was expanded and vacuolated in group C and E; while in group P mitochondria only slightly swelled, the ridge was seen clearly, the arrangement of endoplasmic reticulum was trim and there was no exfoliated ribosome. Conclusions Propofol has protective effect in liver I/R.
2.Limb-shaking transient ischemic attack responsive to nimodipine: A case report

Limb-shaking transient ischemic attack (TIA), a rare manifestation, is commonly caused by severe
stenosis or occlusion of an extracranial internal carotid artery. Such patients are usually treated with
surgical revascularization or anti-platelet therapy. We present a 56-year-old woman with 6 months’
episodic attacks starting with mouth skewed to the right and a sensation of ‘weakness’ involving
predominantly her left arm, and at times, also involved the left leg. This was immediately followed by
rhythmic jerky movements of the left arm and at times, also involved the left leg. Magnetic resonance
angiography revealed severe stenosis of M1 segment of the right middle cerebral artery. The patient’s
symptoms were signifi cantly improved by treatment with anti-platelet drugs and nimodipine.
3.Clinical Observation of Salbutamol Sulfate by Atomized Inhalation in the Treatment of Pediatric Asthmatic Disease

OBJECTIVE:To observe the efficacy and safety of salbutamol sulfate by atomized inhalation in the treatment of pe-diatric asthmatic disease. METHODS:156 children with asthmatic disease were randomly divided into control group (78 cases) and observation group (78 cases). Control group was given oxygen inhalation, anti-infection, relieving cough and reducing phlegm,aminophylline and other conventional treatment;observation group was additionally given 0.03 ml/kg salbutamol sulfate, adding into 2 ml 0.9% Sodium chloride injection,putting into mask atomizer for nebulization,5-10 min every time,twice a day. They were treated for 5 d. Clinical efficacy,asthma remission time,pulmonary rales disappearing time,average hospitalization days and incidence of adverse reactions in 2 groups were observed. RESULTS:The total effective rate in observation group was sig-nificantly higher than control group,asthma remission time,pulmonary rales disappearing time and average hospitalization days were significantly shorter than control group,the differences were statistically significant(P<0.05). There were obvious adverse re-actions in 2 groups during treatment. CONCLUSIONS:Based on conventional treatment,salbutamol sulfate by atomized inhalation has obvious efficacy in the treatment of pediatric asthmatic disease,with good safety.

5.Application Status of Evaluation Methodology of Electronic Medical Record: Evaluation of Bibliometric Analysis.

In order to provide a reference and theoretical guidance of the evaluation of electronic medical record (EMR) and establishment of evaluation system in China, we applied a bibliometric analysis to assess the application of methodologies used at home and abroad, as well as to summarize the advantages and disadvantages of them. We systematically searched international medical databases of Ovid-MEDLINE, EBSCOhost, EI, EMBASE, PubMed, IEEE, and China's medical databases of CBM and CNKI between Jan. 1997 and Dec. 2012. We also reviewed the reference lists of articles for relevant articles. We selected some qualified papers according to the pre-established inclusion and exclusion criteria, and did information extraction and analysis to the papers. Eventually, 1 736 papers were obtained from online database and other 16 articles from manual retrieval. Thirty-five articles met the inclusion and exclusion criteria and were retrieved and assessed. In the evaluation of EMR, US counted for 54.28% in the leading place, and Canada and Japan stood side by side and ranked second with 8.58%, respectively. For the application of evaluation methodology, Information System Success Model, Technology Acceptance Model (TAM), Innovation Diffusion Model and Cost-Benefit Access Model were widely applied with 25%, 20%, 12.5% and 10%, respectively. In this paper, we summarize our study on the application of methodologies of EMR evaluation, which can provide a reference to EMR evaluation in China.

6.Study on Association between Interleukin-1 Beta Gene Polymorphism and Childhood Asthma

Objective To evaluae possible association between interleukin-1 beta (IL-1?) gene exon 5 polymorphism and childhood asthma.Methods The study was conducted in two different groups: asthmatic children(n=55) and healthy children(n=35). The IL-1? gene exon 5 polymorphism was genotyped by polymerase chain reaction-restriction fragment length polymorphism analysis (PCR-RFLP). Results Frequencies of CC,CT and TT genotypes were 92.7%,7.3%,0, and frequencies of C,T allele were 96.4%,3.6% in asthmatic group. However, frequencies of CC,CA and AA genotypes were 85.7%,14.3%,0, and frequencies of C,T allele were 92.9% ,7.1% in healthy group. There were no significant difference in distribution of genotypes and allele frequencies between two groups.Conclusion IL-1? gene exon 5 polymorphism may not be associated with childhood asthma.

Objective To investigate the polymorphism of glutathione S-transferease P1 gene(GSTP1) and the association between the mutation and susceptibility in childhood asthma.Methods The distribute frequency of Ile105/Ile105,Ile105/Val105 and Val105/Val105 ge-notype in GSTP1 of 51 children with asthmatic and 40 normal children were studied with polymerase chain reaction-restriction tragment length polymorphism(PCR-RFLP).Results The frequencies of Ile/Ile Ile/Val,Val/Val genotype were 66.7%,27.4% and 5.9%,the frequencies of Ile,Val allele were 80.4% and 19.6% in the asthmatic group.But the frequencies of Ile/Ile,Ile/Val,Val/Val genotype were 90.0%,7.5% and 2.5%,the frequencies of Ile,Val allele were 93.8%,6.2% in control group.The frequencies Ile/Val,Val/Val genotype and Val allele in asthmatic group were more than that in control group.A significant difference was found in the frequency distribution of GSTP1 genotypes between two groups(?2=6.947 P
10.MR Imaging Features of Pyocephalus

Objective To analyze the MR imaging features of pyocephalus. Methods MR imaging features!of pyocephalus in 5 cases with clinically proved were analyzed retrospectively. All patients were undergone no-contrast MR imaging. 3 patients received diffusion weighted-imaging(DWI). Results In all five patients,the debris and pus with pus-fluid level were shown inside the lateral ventricle.The debris or pus was slightly hyperintense on T1WI, slightly hypointense on T2WI relative to cerebrospinal fulid(CSF). On DWI, the debris was hyperintense in 1 case and isointense in 2 cases.Conclusion The features of MR imaging of pyocephalus is specific. MR imaging is valuable in the diagnosis of pyocephalus.
